Science, clients, and the state : a study of scientific knowledge production and use
This study addresses two main issues that refer to aspects of the relationship between producers and users of research. The first half of the study is focused on the stage of the research process where research problems are selected. The role that potential users of research play in the selection of research problems is investigated, and the extent to which potential applications of research results are defined as important in problem choice. Second, the study analyses the ways in which scientific information is used by government agencies and examines the factors that affect the use of such information in bureaucratic decision-making, and in other activities of government agencies. The two main issues are addressed with a core focus on the possible effects of the organisational context within which research processes and bureaucratic utilisation of research are conducted. The empirical setting of this study is the agricultural and fishery sectors in Norway. Norwegian fisheries and agriculture are sectors where nature, science and public management are intertwined. The two sectors have organisational arrangements that underline the strong ties between science and public administration
Internasjonal IKT-utdanning − Formål, organisering og innhold
I forbindelse med handlingsplanen for IKT i norsk utdanning (2000-2003) fremkom det et behov for en større utredning om IKT som fag. Utdannings- og forskningsdepartementet har bedt Norgesnettrådet iverksette en slik utredning i løpet av 2002. Som et ledd i denne utredningen har Norgesnettrådets sekretariat bedt NIFU om å kartlegge og vurdere hvordan IKT utdanningene er organisert i en del utvalgte land, slik at en nærmere sammenligning med norsk IKT utdanning kan gjøres. Dette notatet er resultatet av dette arbeidet
University Autonomy and Organizational Change Dynamics
In this paper university autonomy is discussed from four different analytical
perspectives. First, a discussion is presented of autonomy as conceptualized in
the academic literature covering public sector governance in general. Second,
the concept of autonomy is deconstructed through discussing its underlying
assumptions and by examining the relationship between state authorities and
universities. In so doing the paper proposes an institutional approach to the
study of autonomy. Third, the way in which autonomy affects organizational
design according to centralization, formalization, standardization,
legitimization and flexibility is addressed. Fourth, relating to our
interpretation of the living autonomy we will discuss how reforms that are
aimed at enhancing university autonomy have affected the internal
governance structure. The empirical setting consists of a study on flagship
universities in eight continental European countries. First findings show
tensions as a consequence of the ways in which enhanced institutional
autonomy is interpreted, operationalized and used within flagship
universities. These tensions are manifested by the nature of the interactions
between the traditional academic domain and the emerging executive
structure inside these institutions
European Parliament staff: who are they and do their backgrounds influence decision-making?
The European Parliament and the political groups within it employ a number of staff members to help co-ordinate various aspects of the Parliament’s legislative work. But who are these staff members and do their individual backgrounds influence their decisions? Morten Egeberg, Åse Gornitzka and Jarle Trondal present the results of a survey of European Parliament officials. They write that while staff come from a variety of different backgrounds, factors such as their nationality and previous experiences have only a modest effect on their decision-making behaviour. Nevertheless, these factors may have a more symbolic impact, for instance in terms of the trust placed in the Parliament by citizens
A Multi-level Approach to Differentiated Integration: Distributive Policy, National Heterogeneity and Actors in the European Research Area
This paper argues that differentiated integration can be understood more
thoroughly by using a multi-level approach that conceives of the nation state
as an aggregate of partly autonomous subunits and actors. The participation of
such components in European integration is influenced by a combination of
type of policy through which integration is pursued, national heterogeneity,
their loosely coupledness within national systems. By examining the case of
the European Research Area, we document patterns of differentiated
integration across governance levels and discuss how the following factors
shape these patterns: the competitive nature of the European distributive
instruments, stark variation in national and sub-national material conditions
and in the governance of national research systems, as well as the normative
and cognitive factors specific to the research policy sector. The conceptual and
policy-based implications to the debate on European differentiated integration
are discussed
Klinisk presentasjon av infeksjon og sepsis hos pasienter eldre enn 75 år. En kvalitetssikringsstudie for UNN Tromsø med fokus på SIRS, qSOFA, delir og antibiotikabruk
Bakgrunn og formål: Infeksjon og sepsis opptrer hyppig hos de eldste pasientene og sepsisrelatert mortalitet øker. Formålet med oppgaven var å kartlegge det kliniske bildet hos de eldste pasientene som innlegges med infeksjon og/eller sepsis, og se om sepsis-syndromet fanges opp ved hjelp av dagens tilgjengelige scoringsverktøy. Vi ønsket å studere om de eldste pasientene hadde en atypisk presentasjon og/eller forsinket fysiologisk respons på infeksjon. Vi studerte bruk av antibiotika og registrerte grad av journalføring og dokumentasjon blant helsepersonell.
Metode: Oppgaven er en kvalitetssikringsstudie for UNN Tromsø, og kartleggingen ble gjennomført ved journalgjennomgang av pasienter ≥ 75 år som ble innlagt med mistenkt pneumoni eller urinveisinfeksjon i perioden 01.01.17-31.12.17. Vi registrerte variabler ved innleggelse (dag 0) og påfølgende dag (dag 1).
Resultater: 139 pasienter ble inkludert i studien. Ved innleggelse ble det av behandlende lege mistenkt sepsis hos 52 (37,7%) pasienter og 78 (56,9%) og 23 (16,6%) pasienter hadde henholdsvis ≥ 2 SIRS- og ≥ 2 qSOFA-kriterier. Leukocytose og økt respirasjonsfrekvens var hyppigste presentasjon. Kliniske observasjoner gjort dag 1 viste ikke forverring sammenlignet med observasjoner utført dag 0. Ved innleggelse ble 38,3% av pasientene registrert som ikke orienterte og 9,4% ble oppfattet å ha mistenkt delir eller akutt forvirring. Det var 36 (36,3%) pasienter som fikk antibiotika innen 1 time etter ankomst. Blant pasienter med ≥ 2/4 SIRS- eller ≥ 2/3 qSOFA-kriterier fikk henholdsvis 43,1% og 70,5% antibiotika innen en time. Samlet sett fikk 70,5% av pasientene antibiotika i henhold til nasjonale retningslinjer, og Cefotaxim var det hyppigste foretrukne startvalg. Dokumentasjon av SIRS- og qSOFA-kriteriene ved innleggelse var gjennomgående langt bedre blant sykepleiere sammenlignet med leger. Dokumentasjon av mental status var gjennomgående mangelfull.
Konklusjon: Vi kunne ikke observere en atypisk eller forsinket patofysiologisk respons på infeksjon ettersom kliniske og parakliniske målinger dag 1 var gjennomgående bedre enn det som ble registrert i mottak. Blant pasienter der behandlende lege mistenkte sepsis, var det langt flere som hadde≥ 2/4 SIRS-kriterier og lang færre som hadde ≥ 2/3 qSOFA-kriterier. Tilsvarende forskjeller er funnet i andre lignende studier, og det er reist tvil om qSOFA fanger opp de dårligste pasientene. Det bør innarbeides bedre rutiner for både å identifisere og dokumentere delir og akutt forvirring. Det bør jobbes systematisk for å redusere tid fra ankomst til antibiotika blir gitt, og en må i større grad følge nasjonale retningslinjer for antibiotikabruk i håp om å redusere bruken av Cefotaxim
