The Cure: Making a game of gene selection for breast cancer survival prediction
Motivation: Molecular signatures for predicting breast cancer prognosis could
greatly improve care through personalization of treatment. Computational
analyses of genome-wide expression datasets have identified such signatures,
but these signatures leave much to be desired in terms of accuracy,
reproducibility and biological interpretability. Methods that take advantage of
structured prior knowledge (e.g. protein interaction networks) show promise in
helping to define better signatures but most knowledge remains unstructured.
Crowdsourcing via scientific discovery games is an emerging methodology that
has the potential to tap into human intelligence at scales and in modes
previously unheard of. Here, we developed and evaluated a game called The Cure
on the task of gene selection for breast cancer survival prediction. Our
central hypothesis was that knowledge linking expression patterns of specific
genes to breast cancer outcomes could be captured from game players. We
envisioned capturing knowledge both from the players prior experience and from
their ability to interpret text related to candidate genes presented to them in
the context of the game.
Results: Between its launch in Sept. 2012 and Sept. 2013, The Cure attracted
more than 1,000 registered players who collectively played nearly 10,000 games.
Gene sets assembled through aggregation of the collected data clearly
demonstrated the accumulation of relevant expert knowledge. In terms of
predictive accuracy, these gene sets provided comparable performance to gene
sets generated using other methods including those used in commercial tests.

An investigation of aerodynamic effects of body morphing for passenger cars in close-proximity
The potential energy-saving benefit for vehicles when travelling in a ‘platoon’ formation results from the reduction in total aerodynamic drag which may result from the interaction of bluff bodies in close-proximity. Early investigations of platooning, prompted by problems of congestion, had shown the potential for drag reduction but was not pursued. More recently, technologies developed for connected-autonomous vehicle control have provided a renewed interest in platooning particularly within the commercial vehicle industry. To date, most aerodynamics-based considerations of platooning have been conducted to assess the sensitivity of drag-saving to vehicle spacing and were based on formations of identically shaped constituents. In this study, the interest was the sensitivity of drag-saving to the shape of the individual platoon constituents. A new reference car, the Resnick model, was specially designed to include front and rear-end add-on sections to make distinct changes in profile form and simulate large-scale body morphing. The results of wind tunnel tests on small-scale models suggested that current trends in low-drag styling may not provide the ideal shape for platoon constituent members and that optimised forms are likely to be dependent upon position in the platoon

A structural MRI study in monozygotic twins concordant or discordant for attention/hyperactivity problems: Evidence for genetic and environmental heterogeneity in the developing brain.
Several structural brain abnormalities have been reported in patients with Attention Deficit Hyperactivity Disorder (ADHD). However, the etiology of these brain changes is still unclear. To investigate genetic and environmental influences on ADHD related neurobiological changes, we performed Voxel-Based Morphometry on MRI scans from monozygotic (MZ) twins selected from a large longitudinal population database to be highly concordant or highly discordant for ratings on the Child Behavior Checklist Attention Problem scale (CBCL-AP). Children scoring low on the CBCL-AP are at low risk for ADHD, whereas children scoring high on this scale are at high-risk for ADHD. Brain differences between concordant high-risk twin pairs and concordant low-risk twin pairs likely reflect the genetic risk for ADHD; brain differences between the low-risk and high-risk twins from discordant MZ twin pairs reflect the environmental risk for ADHD. A major difference between comparisons of high and low-risk twins from concordant pairs and high/low twins from discordant pairs was found for the prefrontal lobes. The concordant high-risk pairs showed volume loss in orbitofrontal subdivisions. High-risk members from the discordant twin pairs exhibited volume reduction in the right inferior dorsolateral prefontal cortex. In addition, the posterior corpus callosum was compromised in concordant high-risk pairs, only. Our findings indicate that inattention and hyperactivity symptoms are associated with anatomical abnormalities of a distributed action-attentional network. Different brain areas of this network appear to be affected in inattention/hyperactivity caused by genetic (i.e., high concordant MZ pairs) vs. environmental (i.e., high-low discordant MZ pairs) risk factors. Somatic diversification of variable lymphocyte receptors in the agnathan sea lamprey
Although jawless vertebrates are apparently capable of adaptive immune responses, they have not been found to possess the recombinatorial antigen receptors shared by all jawed vertebrates. Our search for the phylogenetic roots of adaptive immunity in the lamprey has instead identified a new type of variable lymphocyte receptors (VLRs) composed of highly diverse leucine-rich repeats (LRR) sandwiched between amino- and carboxy-terminal LRRs. An invariant stalk region tethers the VLRs to the cell surface by means of a glycosyl-phosphatidyl-inositol anchor. To generate rearranged VLR genes of the diversity necessary for an anticipatory immune system, the single lamprey VLR locus contains a large bank of diverse LRR cassettes, available for insertion into an incomplete germline VLR gene. Individual lymphocytes express a uniquely rearranged VLR gene in monoallelic fashion. Different evolutionary strategies were thus used to generate highly diverse lymphocyte receptors through rearrangement of LRR modules in agnathans ( jawless fish) and of immunoglobulin gene segments in gnathostomes ( jawed vertebrates).Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/62870/1/nature02740.pd
The use of opioids at the end of life: knowledge level of pharmacists and cooperation with physicians

96464.pdf (publisher's version ) (Open Access)PURPOSE: What is the level of knowledge of pharmacists concerning pain management and the use of opioids at the end of life, and how do they cooperate with physicians? METHODS: A written questionnaire was sent to a sample of community and hospital pharmacists in the Netherlands. The questionnaire was completed by 182 pharmacists (response rate 45%). RESULTS: Pharmacists were aware of the most basic knowledge about opioids. Among the respondents, 29% erroneously thought that life-threatening respiratory depression was a danger with pain control, and 38% erroneously believed that opioids were the preferred drug for palliative sedation. One in three responding pharmacists did not think his/her theoretical knowledge was sufficient to provide advice on pain control. Most pharmacists had working agreements with physicians on euthanasia (81%), but fewer had working agreements on palliative sedation (46%) or opioid therapy (25%). Based on the experience of most of responding pharmacists (93%), physicians were open to unsolicited advice on opioid prescriptions. The majority of community pharmacists (94%) checked opioid prescriptions most often only after dispensing, while it was not a common practice among the majority of hospital pharmacists (68%) to check prescriptions at all. CONCLUSIONS: Although the basic knowledge of most pharmacists was adequate, based on the responses to the questionnaire, there seems to be a lack of knowledge in several areas, which may hamper pharmacists in improving the quality of care when giving advice to physicians and preventing or correcting mistakes if necessary. If education is improved, a more active role of the pharmacist may improve the quality of end-of-life pharmacotherapy
