Diet and Sleep Physiology: Public Health and Clinical Implications

This mini-review examines the complex relationship between diet and sleep and explores the clinical and public health implications of the current evidence. Dietary quality and intake of specific nutrients can impact regulatory hormonal pathways to alter sleep quantity and quality. Sleep, in turn, affects the intake of total energy, as well as of specific foods and nutrients, through biological and behavioral mechanisms. Initial research in this field focused primarily on the effects of short sleep duration on nutritional quality. However, more recent studies have explored the dynamic relationship between long sleep duration and diet. Current evidence suggests that extremes of sleep duration alter sleep patterns, hormonal levels, and circadian rhythms, which contribute to weight-related outcomes and obesity, and other risk factors for the development of chronic disease such as type 2 diabetes and cardiovascular disease. These patterns may begin as early as childhood and have impacts throughout the life course. Given that non-communicable diseases are among the leading causes of death globally, deeper understanding of the interactions between sleep and nutrition has implications for both public health and clinical practice

Francisella tularensis Transmission by Solid Organ Transplantation, 20171.

In July 2017, fever and sepsis developed in 3 recipients of solid organs (1 heart and 2 kidneys) from a common donor in the United States; 1 of the kidney recipients died. Tularemia was suspected only after blood cultures from the surviving kidney recipient grew Francisella species. The organ donor, a middle-aged man from the southwestern United States, had been hospitalized for acute alcohol withdrawal syndrome, pneumonia, and multiorgan failure. F. tularensis subsp. tularensis (clade A2) was cultured from archived spleen tissue from the donor and blood from both kidney recipients. Whole-genome multilocus sequence typing indicated that the isolated strains were indistinguishable. The heart recipient remained seronegative with negative blood cultures but had been receiving antimicrobial drugs for a medical device infection before transplant. Two lagomorph carcasses collected near the donor's residence were positive by PCR for F. tularensis subsp. tularensis (clade A2). O pós-parto e seu impacto sobre a qualidade de vida e satisfação sexual feminina após 6 a 12 meses em uma UBS do interior de RO: The postpartum period and its impact on quality of life and female sexual satisfaction after 6 to 12 months in a UBS in the countryside of RO

A sexualidade é considerada um importante pilar dentro da qualidade de vida pela Organização Mundial da Saúde (OMS) por ser um aspecto que acompanha toda a vida do ser humano. Qualquer disfunção que interfira no conforto e na vivência da sexualidade colabora para uma maior dificuldade e transtorno no âmbito afetivo, emocional e social do indivíduo. O objetivo do estudo foi avaliar o nível de satisfação sexual, qualidade vida geral e aspectos sociodemográficos de mulheres no pós-parto. A pesquisa foi aprovada pelo Comitê de Ética em Pesquisa, protocolo (Nº 3.635.266). Trata-se de um estudo transversal observacional realizado na Unidade Básica de Saúde (UBS) Pastor Jonas, em Pimenta Bueno/RO. Amostra composta por 39 mulheres com idade média de 24 anos (± 4,05), no pós-parto de 6 a 12 meses. Foram utilizados três questionários: o sociodemográfico, Quociente Sexual Feminino (QSF) para avaliar a satisfação sexual feminina, e o questionário SF-36 (qualidade de vida). Em relação ao nível de satisfação sexual observou-se que apenas 28,20% das mulheres apresentaram um score de bom a excelente, sendo 2,56% da amostra apresentou um score ruim; 5,12% desconfortável; 28,20% regular e 35,89% bom. Detectou-se ainda a prevalência de dor durante relação sexual, sendo que 21% das mulheres já apresentavam esses sintomas antes da gestação, aumentando para 36% no período gestacional e 64% no período pós-parto. Notou-se impacto negativo na qualidade de vida e satisfação sexual. Pode-se considerar fatores associados como aspectos culturais e emocionais (religião, autoestima, vergonha, falta de diálogo com o parceiro), falta de autoconhecimento corporal, amamentação, uso de anticoncepcionais, não prática de atividade física e sobrecarga de trabalho

Genetic Signatures in the Envelope Glycoproteins of HIV-1 that Associate with Broadly Neutralizing Antibodies

A steady increase in knowledge of the molecular and antigenic structure of the gp120 and gp41 HIV-1 envelope glycoproteins (Env) is yielding important new insights for vaccine design, but it has been difficult to translate this information to an immunogen that elicits broadly neutralizing antibodies. To help bridge this gap, we used phylogenetically corrected statistical methods to identify amino acid signature patterns in Envs derived from people who have made potently neutralizing antibodies, with the hypothesis that these Envs may share common features that would be useful for incorporation in a vaccine immunogen. Before attempting this, essentially as a control, we explored the utility of our computational methods for defining signatures of complex neutralization phenotypes by analyzing Env sequences from 251 clonal viruses that were differentially sensitive to neutralization by the well-characterized gp120-specific monoclonal antibody, b12. We identified ten b12-neutralization signatures, including seven either in the b12-binding surface of gp120 or in the V2 region of gp120 that have been previously shown to impact b12 sensitivity. A simple algorithm based on the b12 signature pattern was predictive of b12 sensitivity/resistance in an additional blinded panel of 57 viruses. Upon obtaining these reassuring outcomes, we went on to apply these same computational methods to define signature patterns in Env from HIV-1 infected individuals who had potent, broadly neutralizing responses. We analyzed a checkerboard-style neutralization dataset with sera from 69 HIV-1-infected individuals tested against a panel of 25 different Envs. Distinct clusters of sera with high and low neutralization potencies were identified. Six signature positions in Env sequences obtained from the 69 samples were found to be strongly associated with either the high or low potency responses. Five sites were in the CD4-induced coreceptor binding site of gp120, suggesting an important role for this region in the elicitation of broadly neutralizing antibody responses against HIV-1

An international effort towards developing standards for best practices in analysis, interpretation and reporting of clinical genome sequencing results in the CLARITY Challenge

There is tremendous potential for genome sequencing to improve clinical diagnosis and care once it becomes routinely accessible, but this will require formalizing research methods into clinical best practices in the areas of sequence data generation, analysis, interpretation and reporting. The CLARITY Challenge was designed to spur convergence in methods for diagnosing genetic disease starting from clinical case history and genome sequencing data. DNA samples were obtained from three families with heritable genetic disorders and genomic sequence data were donated by sequencing platform vendors. The challenge was to analyze and interpret these data with the goals of identifying disease-causing variants and reporting the findings in a clinically useful format. Participating contestant groups were solicited broadly, and an independent panel of judges evaluated their performance. RESULTS: A total of 30 international groups were engaged. The entries reveal a general convergence of practices on most elements of the analysis and interpretation process. However, even given this commonality of approach, only two groups identified the consensus candidate variants in all disease cases, demonstrating a need for consistent fine-tuning of the generally accepted methods. There was greater diversity of the final clinical report content and in the patient consenting process, demonstrating that these areas require additional exploration and standardization. CONCLUSIONS: The CLARITY Challenge provides a comprehensive assessment of current practices for using genome sequencing to diagnose and report genetic diseases. There is remarkable convergence in bioinformatic techniques, but medical interpretation and reporting are areas that require further development by many groups
