    The Extracellular Vesicles of the Helminth Pathogen, Fasciola hepatica: Biogenesis Pathways and Cargo Molecules Involved in Parasite Pathogenesis

    Extracellular vesicles (EVs) released by parasites have important roles in establishing and maintaining infection. Analysis of the soluble and vesicular secretions of adult Fasciola hepatica has established a definitive characterization of the total secretome of this zoonotic parasite. Fasciola secretes at least two subpopulations of EVs that differ according to size, cargo molecules and site of release from the parasite. The larger EVs are released from the specialized cells that line the parasite gastrodermus and contain the zymogen of the 37 kDa cathepsin L peptidase that performs a digestive function. The smaller exosome-like vesicle population originate from multivesicular bodies within the tegumental syncytium and carry many previously described immunomodulatory molecules that could be delivered into host cells. By integrating our proteomics data with recently available transcriptomic data sets we have detailed the pathways involved with EV biogenesis in F. hepatica and propose that the small exosome biogenesis occurs via ESCRT-dependent MVB formation in the tegumental syncytium before being shed from the apical plasma membrane. Furthermore, we found that the molecular "machinery" required for EV biogenesis is constitutively expressed across the intramammalian development stages of the parasite. By contrast, the cargo molecules packaged within the EVs are developmentally regulated, most likely to facilitate the parasites migration through host tissue and to counteract host immune attack

    Ultra-narrow photonic nanojets through a glass cuboid embedded in a dielectric cylinder

    A glass cuboid, embedded inside a dielectric cylinder is studied when illuminated with a monochromatic plane wave. A photonic nanojet (PNJ) with a full-width at half-maximum (FWHM) waist of around 0.25λ0 is obtained outside the external surface of the cuboid. The influence of the parameters of a square section cuboid is studied. Three particular phenomena can be obtained and are discussed: an ultra-narrow PNJ on the external surface of the cuboid, a long photonic jet and the excitation of whispering gallery modes (WGMs). A parametric study, over the width and the height of a rectangular section cuboid, shows that these parameters can be used to control the photonic jet properties. We also study several other geometries of the insert, which shows that the key parameter is the refractive index of the inserted material. Finally, we show that by changing the incident angle we can obtain a curved photonic jet

    Infection-mediated priming of phagocytes protects against lethal secondary Aspergillus fumigatus challenge

    Phagocytes restrict the germination of Aspergillus fumigatus conidia and prevent the establishment of invasive pulmonary aspergillosis in immunecompetent mice. Here we report that immunecompetent mice recovering from a primary A. fumigatus challenge are protected against a secondary lethal challenge. Using RAGÎłc knock-out mice we show that this protection is independent of T, B and NK cells. In protected mice, lung phagocytes are recruited more rapidly and are more efficient in conidial phagocytosis and killing. Protection was also associated with an enhanced expression of CXCR2 and Dectin-1 on bone marrow phagocytes. We also show that protective lung cytokine and chemokine responses are induced more rapidly and with enhanced dynamics in protected mice. Our findings support the hypothesis that following a first encounter with a non-lethal dose of A. fumigatus conidia, the innate immune system is primed and can mediate protection against a secondary lethal infection

    Galactokinase deficiency:lessons from the GalNet registry

    PURPOSE Galactokinase (GALK1) deficiency is a rare hereditary galactose metabolism disorder. Beyond cataract, the phenotypic spectrum is questionable. Data from affected patients included in the Galactosemias Network registry were collected to better characterize the phenotype. METHODS Observational study collecting medical data of 53 not previously reported GALK1 deficient patients from 17 centers in 11 countries from December 2014 to April 2020. RESULTS Neonatal or childhood cataract was reported in 15 and 4 patients respectively. The occurrence of neonatal hypoglycemia and infection were comparable with the general population, whereas bleeding diathesis (8.1% versus 2.17-5.9%) and encephalopathy (3.9% versus 0.3%) were reported more often. Elevated transaminases were seen in 25.5%. Cognitive delay was reported in 5 patients. Urinary galactitol was elevated in all patients at diagnosis; five showed unexpected Gal-1-P increase. Most patients showed enzyme activities ≤1%. Eleven different genotypes were described, including six unpublished variants. The majority was homozygous for NM_000154.1:c.82C>A (p.Pro28Thr). Thirty-five patients were diagnosed following newborn screening, which was clearly beneficial. CONCLUSION The phenotype of GALK1 deficiency may include neonatal elevation of transaminases, bleeding diathesis, and encephalopathy in addition to cataract. Potential complications beyond the neonatal period are not systematically surveyed and a better delineation is needed

    Phase II Evaluation of Sensitivity and Specificity of PCR and NASBA Followed by Oligochromatography for Diagnosis of Human African Trypanosomiasis in Clinical Samples from D.R. Congo and Uganda

    Diagnosis plays a central role in the control of human African trypanosomiasis (HAT) whose mainstay in disease control is chemotherapy. However, accurate diagnosis is hampered by the absence of sensitive techniques for parasite detection. Without concentrating the blood, detection thresholds can be as high as 10,000 trypanosomes per milliliter of blood. The polymerase chain reaction (PCR) and nucleic acid sequence-based amplification (NASBA) are promising molecular diagnostics that generally yield high sensitivity and could improve case detection. Recently, these two tests were coupled to oligochromatography (OC) for simplified and standardized detection of amplified products, eliminating the need for electrophoresis. In this study, we evaluated the diagnostic accuracy of these two novel tests on blood specimens from HAT patients and healthy endemic controls from D.R. Congo and Uganda. Both tests exhibited good sensitivity and specificity compared to the current diagnostic tests and may be valuable tools for sensitive and specific parasite detection in clinical specimens. These standardized molecular test formats open avenues for improved case detection, particularly in epidemiological studies and in disease diagnosis at reference centres

    Determinants of diagnostic investigation sensitivities across the clinical spectrum of sporadic Creutzfeldt-Jakob disease

    To validate the provisional findings of a number of smaller studies and explore additional determinants of characteristic diagnostic investigation results across the entire clinical spectrum of sporadic Creutzfeldt-Jakob disease (CJD), an international collaborative study was undertaken comprising 2451 pathologically confirmed (definite) patients. We assessed the influence of age at disease onset, illness duration, prion protein gene (PRNP) codon 129 polymorphism (either methionine or valine) and molecular sub-type on the diagnostic sensitivity of EEG, cerebral MRI and the CSF 14-3-3 immunoassay. For EEG and CSF 14-3-3 protein detection, we also assessed the influence of the time point in a patient's illness at which the investigation was performed on the likelihood of a typical or positive result. Analysis included a large subset of patients (n = 743) in whom molecular sub-typing had been performed using a combination of the PRNP codon 129 polymorphism and the form of protease resistant prion protein [type 1 or 2 according to Parchi et al. (Parchi P, Giese A, Capellari S, Brown P, Schulz-Schaeffer W, Windl O, Zerr I, Budka H, Kopp N, Piccardo P, Poser S, Rojiani A, Streichemberger N, Julien J, Vital C, Ghetti B, Gambetti P, Kretzschmar H. Classification of sporadic Creutzfeldt-Jakob disease based on molecular and phenotypic analysis of 300 subjects. Ann Neurol 1999; 46: 224-233.)] present in the brain. Findings for the whole group paralleled the subset with molecular sub-typing data available, showing that age at disease onset and disease duration were independent determinants of typical changes on EEG, while illness duration significantly influenced positive CSF 14-3-3 protein detection; changes on brain MRI were not influenced by either of these clinical parameters, but overall, imaging data were less complete and consequently conclusions are more tentative. In addition to age at disease onset and illness duration, molecular sub-type was re-affirmed as an important independent determinant of investigation results. In multivariate analyses that included molecular sub-type, time point of the investigation during a patient's illness was found not to influence the occurrence of a typical or positive EEG or CSF 14-3-3 protein result. A typical EEG was most often seen in MM1 patients and was significantly less likely in the MV1, MV2 and VV2 sub-types, whereas VV2 patients had an increased likelihood of a typical brain MRI. Overall, the CSF 14-3-3 immunoassay was the most frequently positive investigation (88.1%) but performed significantly less well in the very uncommon MV2 and MM2 sub-types. Our findings confirm a number of determinants of principal investigation results in sporadic CJD and underscore the importance of recognizing these pre-test limitations before accepting the diagnosis excluded or confirmed. Combinations of investigations offer the best chance of detection, especially for the less common molecular sub-types such as MV2 and MM2

    Carbon dynamics and CO2 and CH4 exchange in the mangrove dominated Guayas river delta, Ecuador

    Although estuaries are considered important pathways in the global carbon cycle, carbon dynamics in tropical estuaries is relatively understudied. Here, the tidal, seasonal and spatial variability of particulate organic carbon (POC), dissolved inorganic carbon (DIC), carbon dioxide (CO2) and methane (CH4), among other biogeochemical variables related to carbon cycling, were studied in the Guayas river delta (Ecuador) to document the sources, processing and fluxes of these carbon forms. All variables were studied during a semi-diurnal (13 hour) tidal cycle and along river transects at low and high tides, all carried out during one dry and rainy season. POC and total suspended matter (TSM) strongly covaried and peaked at high tidal flow velocities during a tidal cycle and at high river discharge during the rainy season, suggesting that resuspension of bottom sediments and/or surface erosion in the river catchment were a dominant source of particulate matter in the water column. The δ13C of POC, (from ~-22‰ to ~-27‰) showed an increasing contribution of marine phytoplankton to the POC pool as moving downstream along the delta during the dry season. Upstream DIC concentrations (~1200 μmol L-1) were high in the Guayas river delta as compared to other tropical estuarine systems, and the δ13C of DIC revealed a shift from a more phytoplankton dominated source in the dry season and downstream (~-4‰) to a relatively more terrestrial source in the rainy season and upstream (~-12.5‰). Both DIC and its δ13C showed slight but consistent deviations from conservative mixing that hint at inputs of 13C depleted DIC from mineralization along the delta. High values of the partial pressure of CO2 (pCO2) observed upstream and in the rainy season (~5250 μatm), associated with O2 undersaturation (~60%) and low δ13CDIC, suggest a strongly heterotrophic system, and resulted in high CO2 efflux to the atmosphere. CH4 concentrations were also higher during the rainy than dry season (93.5±62.5 vs. 61.3±39.5 nmol L-1), but unlike pCO2, showed tidal variations similar to TSM and POC, thus alluding to potential CH4 release from sediments during resuspension events at high tidal flow velocities. This explorative survey revealed complex drivers and biogeochemical processes acting upon various spatio-temporal scales which are necessary to consider for a complete understanding of the carbon biogeochemistry in estuarine systems. Similar surveys on estuarine carbon in data scarce regions are encouraged to constrain uncertainties in coastal zone carbon budgets
