Post-operative Aspergillus mediastinitis in a man who was immunocompetent: a case report
<p>Abstract</p> <p>Introduction</p> <p><it>Aspergillus </it>spp. infections mainly affect patients who are immunocompromised, and are extremely rare in immunocompetent individuals.</p> <p>Case presentation</p> <p><it>Aspergillus </it>post-operative mediastinitis is considered to be a devastating infection, usually affecting patients undergoing cardiothoracic surgery with specific predisposing factors. We describe the case of an immunocompetent 68-year-old Caucasian man with severe chronic thromboembolic pulmonary hypertension, who underwent pulmonary thromboendarterectomy and developed post-operative mediastinitis due to <it>Aspergillus flavus</it>. The environmental control did not reveal the source of <it>A. flavus </it>infection and, despite combined antifungal therapy, our patient died as a result of septic shock and multiple organ failure.</p> <p>Conclusion</p> <p><it>Aspergillus </it>mediastinitis mainly affects patients after cardiosurgery operations with predisposing factors, and it is unusual in patients who are immunocompetent. The identification of the <it>Aspergillus </it>spp. source is often difficult, and there are no guidelines for the administration of pre-emptive therapy in this population of at-risk patients.</p
Diagnostic and prognostic value of procalcitonin among febrile critically ill patients with prolonged ICU stay
<p>Abstract</p> <p>Background</p> <p>Procalcitonin (PCT) has been proposed as a diagnostic and prognostic sepsis marker, but has never been validated in febrile patients with prolonged ICU stay.</p> <p>Methods</p> <p>Patients were included in the study provided they were hospitalised in the ICU for > 10 days, were free of infection and presented a new episode of SIRS, with fever >38°C being obligatory. Fifty patients fulfilled the above criteria. PCT was measured daily during the ICU stay. The primary outcome was proven infection.</p> <p>Results</p> <p>Twenty-seven out of 50 patients were diagnosed with infection. Median PCT on the day of fever was 1.18 and 0.17 ng/ml for patients with and without proven infections (p < 0.001). The area under the curve for PCT was 0.85 (95% CI; 0.71-0.93), for CRP 0.65 (0.46-0.78) and for WBC 0.68 (0.49-0.81). A PCT level of 1 ng/mL yielded a negative predictive value of 72% for the presence of infection, while a PCT of 1.16 had a specificity of 100%. A two-fold increase of PCT between fever onset and the previous day was associated with proven infection (p 0.001) (OR = 8.55; 2.4-31.1), whereas a four-fold increase of PCT of any of the 6 preceding days was associated with a positive predictive value exceeding 69.65%. A PCT value less than 0.5 ng/ml on the third day after the advent of fever was associated with favorable survival (p 0.01).</p> <p>Conclusion</p> <p>The reported data support that serial serum PCT may be a valuable diagnostic and prognostic marker in febrile chronic critically ill patients.</p
A MSFD complementary approach for the assessment of pressures, knowledge and data gaps in Southern European Seas : the PERSEUS experience
PERSEUS project aims to identify the most relevant pressures exerted on the ecosystems of the Southern
European Seas (SES), highlighting knowledge and data gaps that endanger the achievement of SES Good
Environmental Status (GES) as mandated by the Marine Strategy Framework Directive (MSFD). A complementary
approach has been adopted, by a meta-analysis of existing literature on pressure/impact/knowledge
gaps summarized in tables related to the MSFD descriptors, discriminating open waters from coastal
areas. A comparative assessment of the Initial Assessments (IAs) for five SES countries has been also
independently performed. The comparison between meta-analysis results and IAs shows similarities
for coastal areas only. Major knowledge gaps have been detected for the biodiversity, marine food
web, marine litter and underwater noise descriptors. The meta-analysis also allowed the identification
of additional research themes targeting research topics that are requested to the achievement of GES.

Prediction of SARS-CoV-2 Omicron Variant Immunogenicity, Immune Escape and Pathogenicity, through the Analysis of Spike Protein-Specific Core Unique Peptides
The recently discovered Omicron variant of the SARS-CoV-2 coronavirus has raised a new, global, awareness. In this study, we identified the Core Unique Peptides (CrUPs) that reside exclusively in the Omicron variant of Spike protein and are absent from the human proteome, creating a new dataset of peptides named as SARS-CoV-2 CrUPs against the human proteome (C/H-CrUPs), and we analyzed their locations in comparison to the Alpha and Delta variants. In Omicron, 115 C/H-CrUPs were generated and 119 C/H-CrUPs were lost, almost four times as many compared to the other two variants. At the Receptor Binding Motif (RBM), 8 mutations were detected, resulting in the construction of 28 novel C/H-CrUPs. Most importantly, in the Omicron variant, new C/H-CrUPs carrying two or three mutant amino acids were produced, as a consequence of the accumulation of multiple mutations in the RBM. These C/H-CrUPs could not be recognized in any other viral Spike variant. Our findings indicated that the virus binding to the ACE2 receptor is facilitated by the herein identified C/H-CrUPs in contact point mutations and Spike cleavage sites, while the immunoregulatory NF9 peptide is not detectably affected. Thus, the Omicron variant could escape immune-system attack, while the strong viral binding to the ACE2 receptor leads to the highly efficient fusion of the virus to the target cell. However, the intact NF9 peptide suggests that Omicron exhibits reduced pathogenicity compared to the Delta variant
Protein biomarkers distinguish between high- and low-risk pediatric acute lymphoblastic leukemia in a tissue specific manner
The current study evaluated the differential expression detected in the
proteomic profiles of low risk- and high risk- ALL pediatric patients to
characterize candidate biomarkers related to diagnosis, prognosis and
patient targeted therapy. Bone marrow and peripheral blood plasma and
cell lysates samples were obtained from pediatric patients with low-
(LR) and high-risk (HR) ALL at diagnosis. As controls, non-leukemic
pediatric patients were studied. Cytogenetic analysis was carried out by
G- banding and interphase fluorescent in situ hybridization.
Differential proteomic analysis was performed using two-dimensional gel

expression of certain proteins was confirmed by Western blot analysis.
The obtained data revealed that CLUS, CERU, APOE, APOA4, APOA1, GELS,
S10A9, AMBP, ACTB, CATA and AFAM proteins play a significant role in
leukemia prognosis, potentially serving as distinctive biomarkers for
leukemia aggressiveness, or as suppressor proteins in HR-ALL cases. In
addition, vitronectin and plasminogen probably contributed to
leukemogenesis, whilst bicaudal D-related protein 1 could afford a
significant biomarker for pediatric ALL therapeutics
