    Lightning Safety: How Is It Conducted at NIAAA Member High Schools?

    The purposes of this study were to explore the current scenario of interscholastic athletics in regards to the existence and enforcement of lightning safety policies applied to athletic outdoor activities, and to identify the common practices related to lightning safety currently utilized. The results showed that 87.2% of the respondents (N=804) have lightning safety policies. However, only 90.3% of the respondents who have lightning safety policies actually enforce them. It seems that during practices coaches are most commonly responsible for making the decision to stop/resume activity, and that during games athletic directors are most commonly making the call. However, almost one third of the respondents (N=804) do not have a clear designation as to who makes the decision. A less than desirable percentage of respondents reported frequent use of a lightning detection system, availability of shelters for spectators, and posted lightning policy in facilities. Only 7.8% of the respondents indicated that all athletic coaches and staff receive lightning safety training. The results also showed that more experienced administrators were more likely to have and enforce lightning safety policies, and employ lightning safety best practices. This study provides high school athletic administrators and principals with relevant information that can be used to support their decision to adopt and enforce lightning safety policies for interscholastic athletic activities

    Insights into early Earth from the Pt-Re-Os isotope and highly siderophile element abundance systematics of Barberton komatiites

    Highly siderophile element (HSE: Os, Ir, Ru, Pt, Pd, and Re) abundance and Pt-Re-Os isotopic data are reported for well-preserved komatiites from the Komati and Weltevreden Formations of the Barberton Greenstone Belt in South Africa. The Re-Os data for whole-rock samples and olivine and chromite separates define isochrons with ages of 3484 +/- 38 and 3263 +/- 12 Ma for the Komati and Weltevreden systems, respectively. The respective initial Os-187/Os-188 = 0.10335 +/- 15 (gamma Os-187 = +0.34 +/- 0.15) and 0.10442 +/- 4 (gamma Os-187 = -0.14 +/- 0.04) are well within the range defined by chondritic meteorites. When considered together with the Re-Os data for late Archean komatiite systems, these data indicate that the mantle sources of most Archean komatiites evolved with essentially uniform long-term Re/Os that is well within the chondritic range. By contrast, the initial Os-186/Os-188 = 0.1198283 +/- 9 (epsilon Os-186 = -0.12 +/- 0.08) and 0.1198330 +/- 8 (epsilon Os-186 = +0.22 +/- 0.07) for the Komati and Weltevreden systems, respectively, are outside of known chondritic evolution paths, indicating that the mantle sources of these two komatiite systems evolved with fractionated time-integrated Pt/Os. The new 186,187 Os isotopic data for these early Archean komatiite systems, combined with published Nd-142,Nd-143 and Hf-176 isotopic data for these systems, are consistent with formation and long-term isolation of deep-seated mantle domains with fractionated time-integrated Sm/Nd, Lu/Hf, and Pt/Os ratios, at ca. 4400 Ma. These domains may have been generated as a result of late-stage crystallization of a primordial magma ocean involving Mg-perovskite, Ca-perovskite and Pt-alloys acting as the fractionating phases. The inferred fractionated mantle domains were sampled by the early Archean komatiites, but were largely mixed away by 2.7 Ga, as evidenced by uniform time-integrated Sm/Nd, Lu/Hf, and Pt/Os ratios inferred for the sources of most late Archean komatiite systems. The calculated total Pt + Pd abundances present in the sources of the early Archean komatiite systems fall only 7-14% short of those present in estimates for the modern primitive mantle. These are also within the range of the total Pt + Pd abundances present in the sources of late Archean komatiite systems, indicating little change in the HSE abundances in the Archean mantle between 3.5 and 2.7 Ga. The new HSE data for the early Archean komatiite systems may implicate late accretion of HSE to the mantle prior to completion of crystallization of a final terrestrial magma ocean, followed by sluggish mixing of diverse, post-magma ocean domains characterized by variably fractionated lithophile element and HSE abundances.     Ontogeny of myosin isoform expression and prehensile function in the tail of the gray short-tailed opossum ( Monodelphis domestica)

    Terrestrial opossums use their semiprehensile tail for grasping nesting materials as opposed to arboreal maneuvering. We relate the development of this adaptive behavior with ontogenetic changes in myosin heavy chain (MHC) isoform expression from 21 days to adulthood. Monodelphis domestica is expected to demonstrate a progressive ability to flex the distal tail up to age 7 mo, when it should exhibit routine nest construction. We hypothesize that juvenile stages (3-7 mo) will be characterized by retention of the neonatal isoform (MHC-Neo), along with predominant expression of fast MHC-2X and -2B, which will transition into greater MHC-1ÎČ and -2A isoform content as development progresses. This hypothesis was tested using Q-PCR to quantify and compare gene expression of each isoform with its protein content determined by gel electrophoresis and densitometry. These data were correlated with nesting activity in an age-matched sample of each age group studied. Shifts in regulation of MHC gene transcripts matched well with isoform expression. Notably, mRNA for MHC-Neo and -2B decrease, resulting in little-to-no isoform translation after age 7 mo, whereas mRNA for MHC-1ÎČ and -2A increase, and this corresponds with subtle increases in content for these isoforms into late adulthood. Despite the tail remaining intrinsically fast-contracting, a critical growth period for isoform transition is observed between 7 and 13 mo, correlating primarily with use of the tail during nesting activities. Functional transitions in MHC isoforms and fiber type properties may be associated with muscle tuning repetitive nest remodeling tasks requiring sustained contractions of the caudal flexors. NEW & NOTEWORTHY Little is understood about skeletal muscle development as it pertains to tail prehensility in mammals. This study uses an integrative approach of relating both MHC gene and protein expression with behavioral and morphometric changes to reveal a predominant fast MHC expression with subtle isoform transitions in caudal muscle across ontogeny. The functional shifts observed are most notably correlated with increased tail grasping for nesting activities

    Maintained physical activity and physiotherapy in the management of distal upper limb pain – a protocol for a randomised controlled trial (the arm pain trial)

    <b>Background</b><p></p> Distal upper limb pain (pain affecting the elbow, forearm, wrist, or hand) can be non-specific, or can arise from specific musculoskeletal disorders. It is clinically important and costly, the best approach to clinical management is unclear. Physiotherapy is the standard treatment and, while awaiting treatment, advice is often given to rest and avoid strenuous activities, but there is no evidence base to support these strategies. This paper describes the protocol of a randomised controlled trial to determine, among patients awaiting physiotherapy for distal arm pain, (a) whether advice to remain active and maintain usual activities results in a long-term reduction in arm pain and disability, compared with advice to rest; and (b) whether immediate physiotherapy results in a long-term reduction in arm pain and disability, compared with physiotherapy delivered after a seven week waiting list period.<p></p> <b>Methods/Design</b><p></p> Between January 2012 and January 2014, new referrals to 14 out-patient physiotherapy departments were screened for potential eligibility. Eligible and consenting patients were randomly allocated to one of the following three groups in equal numbers: 1) advice to remain active, 2) advice to rest, 3) immediate physiotherapy. Patients were and followed up at 6, 13, and 26 weeks post-randomisation by self-complete postal questionnaire and, at six weeks, patients who had not received physiotherapy were offered it at this time. The primary outcome is the proportion of patients free of disability at 26 weeks, as determined by the modified DASH (Disabilities of the Arm, Shoulder and Hand) questionnaire.<p></p> We hypothesise (a) that advice to maintain usual activities while awaiting physiotherapy will be superior than advice to rest the arm; and (b) that fast-track physiotherapy will be superior to normal (waiting list) physiotherapy. These hypotheses will be examined using an intention-to-treat analysis.<p></p> <b>Discussion</b><p></p> Results from this trial will contribute to the evidence base underpinning the clinical management of patients with distal upper limb pain, and in particular, will provide guidance on whether they should be advised to rest the arm or remain active within the limits imposed by their symptoms

    Co-Administration of Injected and Oral Vaccine Candidates Elicits Improved Immune Responses over Either Route Alone

    Infectious diseases continue to be a significant cause of morbidity and mortality, and although efficacious vaccines are available for many diseases, some parenteral vaccines elicit little or no mucosal antibodies which can be a significant problem since mucosal tissue is the point of entry for 90% of pathogens. In order to provide protection for both serum and mucosal areas, we have tested a combinatorial approach of both parenteral and oral administration of antigens for diseases caused by a viral pathogen, Hepatitis B, and a fungal pathogen, Coccidioides. We demonstrate that co-administration by the parenteral and oral routes is a useful tool to increase the overall immune response. This can include achieving an immune response in tissues that are not elicited when using only one route of administration, providing a higher level of response that can lead to fewer required doses or possibly providing a better response for individuals that are considered poor or non-responders

    Comparison of weight changes following unilateral and staged bilateral STN DBS for advanced PD

    Unilateral and bilateral subthalamic nucleus deep brain stimulation (STN DBS) in Parkinson's disease (PD) result in weight gain in the initial postoperative months, but little is known about the changes in weight following unilateral and staged bilateral STN DBS over longer time intervals. A case–control comparison evaluated weight changes over 2 years in 43 consecutive unilateral STN DBS patients, among whom 25 elected to undergo staged bilateral STN DBS, and 21 age-matched and disease severity matched PD controls without DBS. Regression analyses incorporating age, gender, and baseline weight in case or control were conducted to assess weight changes 2 years after the initial unilateral surgery. Unilateral STN DBS and staged bilateral STN DBS patients gained 3.9 ± 2.0 kg and 5.6 ± 2.1 kg versus their preoperative baseline weight (P < 0.001, respectively) while PD controls without DBS lost 0.8 ± 1.1 kg. Although bilateral STN DBS patients gained 1.7 kg more than unilateral STN DBS patients at 2 years, this difference was not statistically significant (P = 0.885). Although there was a trend toward greater weight gain in staged bilateral STN DBS patients versus unilateral patients, we found no evidence for an equivalent or synergistic increase in body weight following placement of the second DBS electrode

    Acetaminophen Protects Against Iron-Induced Cardiac Damage in Gerbils

    There are few effective agents that safely remove excess iron from iron-overloaded individuals. Our goal was to evaluate the iron-removing effectiveness of acetaminophen given ip or orally in the gerbil iron-overload model. Male gerbils were divided into 5 groups: saline controls, iron-overloaded controls, iron-overloaded treated with ip acetaminophen, iron-overloaded treated with oral acetaminophen, and iron-overloaded treated with ipdeferoxamine. Iron dextran was injected iptwice/wk for 8 wk. Acetaminophen and deferoxamine treatments were given on Mondays, Wednesdays, and Fridays during the same 8 wk and continued for 4 wk after completion of iron-overloading. Echocardiograms were performed after completion of the iron-overloading and drug treatments. Liver and cardiac iron contents were determined by inductively coupled plasma atomic emission spectrometry (ICP-AES). Iron-overloaded controls had 232-fold and 16-fold increases in liver and cardiac iron content, respectively, compared to saline controls. In iron-overloaded controls, echocardiography showed cardiac hypertrophy, right and left ventricular distension, significant reduction in left ventricular ejection fraction (−22%), and fractional shortening (−31%) during systole. Treatments with acetaminophen (ip or oral) or deferoxamine (ip) were equally effective in reducing cardiac iron content and in preventing cardiac structural and functional changes. Both agents also significantly reduced excess hepatic iron content, although acetaminophen was less effective than deferoxamine. The results suggest that acetaminophen may be useful for treatment of iron-induced pathology

    Age-Associated Changes in Hearts of Male Fischer 344/Brown Norway F1 Rats

    Aging is associated with left ventricular hypertrophy, dilatation, and fibrosis of the heart. The Fischer 344/Brown Norway F1 (F344/BNF1) rat is recommended for age-related studies by the National Institutes on Aging because this hybrid rat lives longer and has a lower rate of pathological conditions than inbred rats. However, little is known about age-associated changes in cardiac and aortic function and structure in this model. This study evaluated age-related cardiac changes in male F344/BNF1 rats using ECHO, gross, and microscopic examinations. Rats aged 6-, 30-, and 36-mo were anesthetized and two-dimensional ECHO measurements, two-dimensional guided M-mode, Doppler M-mode, and other recordings from parasternal long- and short-axis views were obtained using a Phillips 5500 ECHO system with a 12 megahertz transducer. Hearts and aortas from sacrificed rats were evaluated grossly and microscopically. The ECHO studies revealed persistent cardiac arrhythmias (chiefly PVCs) in 72% (13/18) of 36-mo rats, 10% (1/10) of 30-mo rats, and none in 6-mo rats (0/16). Gross and microscopic studies showed left ventricular (LV) dilatation, borderline to mild hypertrophy, and areas of fibrosis that were common in 36-mo rats, less evident in 30-mo rats, and absent in 6-mo rats. Aging was associated with mild to moderate decreases of LV diastolic and systolic function. Thus, male F344/BN F1 rats demonstrated progressive age-related (a) decline in cardiac function (diastolic and systolic indices), (b) LV structural changes (chamber dimensions, volumes, and wall thicknesses), and (c) persistent arrhythmias. These changes are consistent with those in humans. The noninvasive ECHO technique offers a means to monitor serial age-related cardiac failure and therapeutic responses in the same rats over designated time intervals
