    On density effects and large structure in turbulent mixing layers

    Plane turbulent mixing between two streams of different gases (especially nitrogen and helium) was studied in a novel apparatus. Spark shadow pictures showed that, for all ratios of densities in the two streams, the mixing layer is dominated by large coherent structures. High-speed movies showed that these convect at nearly constant speed, and increase their size and spacing discontinuously by amalgamation with neighbouring ones. The pictures and measurements of density fluctuations suggest that turbulent mixing and entrainment is a process of entanglement on the scale of the large structures; some statistical properties of the latter are used to obtain an estimate of entrainment rates. Large changes of the density ratio across the mixing layer were found to have a relatively small effect on the spreading angle; it is concluded that the strong effects, which are observed when one stream is supersonic, are due to compressibility effects, not density effects, as has been generally supposed

    Large Structure Dynamics and Entrainment in the Mixing Layer at High Reynolds Number

    Observations were made on a turbulent mixing layer in a water channel at Reynolds numbers up to 3 x 10^6 . Flow visualization with dyes revealed (once more) large coherent structures and showed their role in the entrainment process; observations of the reaction of a base and an acid indicator injected on the two sides of the layer, respectively, gave some indication of where molecular mixing occurs. Autocorrelations of streamwise velocity fluctuations, using an LDV, revealed a fundamental periodicity associated with the large structures. The surprisingly long correlation times suggest time scales much longer than had been supposed; it is argued that the mixing layer dynamics at any point is coupled to the large structure further downstream, and some possible consequences about the effects of initial conditions and,. of the influence of apparatus geometry are discussed

    Stable and Unstable Accretion Flows with Angular Momentum near a Point Mass

    The properties of axisymmetric accretion flows of cold adiabatic gas with zero total energy in the vicinity of a Newtonian point mass are characterized by a single dimensionless parameter, the thickness of incoming flow. In the limit of thin accretion flows with vanishing thickness, we show that the governing equations become self-similar, involving no free parameters. We study numerically thin accretion flows with finite thickness as well as those with vanishing thickness. Mass elements of the incoming flow enter the computational regime as thin rings. In the case with finite thickness, after a transient period of initial adjustment, an almost steady-state accretion shock with a small oscillation amplitude forms, confirming the previous work by Molteni, Lanzafame, \& Chakrabarti (1994). The gas in the region of vorticity between the funnel wall and the accretion shock follows closed streamlines, forming a torus. This torus, in turn, behaves as an effective barrier to the incoming flow and supports the accretion shock which reflects the incoming gas away from the equatorial plane. The postshock flow, which is further accelerated by the pressure gradient behind the shock, goes through a second shock which then reflects the flow away from the symmetry axis to form a conical outgoing wind. As the thickness of the inflowing layer decreases (or if the ratio of the half thickness to the distance to the funnel wall along the equatorial plan is smaller than 0.1\sim0.1), the flow becomes unstable. In the case with vanishing thickness, the accretion shock formed to stop the incoming flow behind the funnel wall oscillates quasi-periodically with an amplitude comparable to the thickness.     A framework for improved predictions of the climate impacts on potential yields of UK winter wheat and its applicability to other UK crops

    •Changes in the frequency of extreme weather events related to climate change potentially pose significant challenges to UK agricultural production. There is a need for improved climate change risk assessments to support adaptation strategies and to ensure security of food production in future. •We describe an innovative and practical framework for spatially explicit modelling of climate change impacts on crop yields, based on the UKCP18 climate projections. Our approach allows the integration of relatively simple crop growth models with high spatial and temporal resolution Earth Observation datasets, describing changes in crop growth parameters within year and over the longer term. We focus on modelling winter wheat, a commercially important crop. We evaluate the results of the model against precision yield data collected from 719 fields. We show that the assimilation of leaf area index data from Sentinel-2 satellite observations improves the agreement of the modelled yields with those observed. Our national-scale results indicate that wheat production initially becomes more favourable under climate change across much of the UK with the projected increase in temperature. From 2050 onwards, yields increase northwards, whilst they decline in South East England as the decrease in precipitation offsets the benefits of rising temperature. •Our framework can readily accommodate growth models for other crops and LAI retrievals from other satellite sensors. The ability to explore impacts of crop yields at fine spatial resolutions is an important part of assessing the potential risks of climate change to UK agriculture and of designing more climate resilient agricultural systems

    How do workers make wage comparisons? Both an experimental study and an analysis of 16,000 British employees are reported. Satisfaction and well-being levels are shown to depend on more than simple relative pay. They depend upon the ordinal rank of an individual's wage within a comparison group. “Rank” itself thus seems to matter to human beings. Moreover, consistent with psychological theory, quits in a workplace are correlated with pay distribution skewness

    Neuropathologic Characterization of Pontocerebellar Hypoplasia Type 6 Associated With Cardiomyopathy and Hydrops Fetalis and Severe Multisystem Respiratory Chain Deficiency due to Novel RARS2 Mutations

    Autosomal recessive mutations in the RARS2 gene encoding the mitochondrial arginyl-transfer RNA synthetase cause infantile-onset myoencephalopathy pontocerebellar hypoplasia type 6 (PCH6). We describe 2 sisters with novel compound heterozygous RARS2 mutations who presented perinatally with neurologic features typical of PCH6 but with additional features including cardiomyopathy, hydrops, and pulmonary hypoplasia and who died at 1 day and 14 days of age. Magnetic resonance imaging findings included marked cerebellar hypoplasia, gyral immaturity, punctate lesions in cerebral white matter, and unfused deep cerebral grey matter. Enzyme histochemistry of postmortem tissues revealed a near-global cytochrome c oxidase-deficiency; assessment of respiratory chain enzyme activities confirmed severe deficiencies involving complexes I, III, and IV. Molecular genetic studies revealed 2 RARS2 gene mutations: a c.1A>G, p.? variant predicted to abolish the initiator methionine, and a deep intronic c.613-3927C>T variant causing skipping of exons 6–8 in the mature RARS2 transcript. Neuropathologic investigation included low brain weights, small brainstem and cerebellum, deep cerebral white matter pathology, pontine nucleus neuron loss (in 1 sibling), and peripheral nerve pathology. Mitochondrial respiratory chain immunohistochemistry in brain tissues confirmed an absence of complexes I and IV immunoreactivity with sparing of mitochondrial numbers. These cases expand the clinical spectrum of RARS2 mutations, including antenatal features and widespread mitochondrial respiratory chain deficiencies in postmortem brain tissues

    Improving the management of behaviour that challenges associated with dementia in care homes: protocol for pharmacy–health psychology intervention feasibility study

    Introduction The inappropriate use of antipsychotics in people with dementia for behaviour that challenges is associated with an estimated 1800 deaths annually. However, solely focusing on antipsychotics may transfer prescribing to other equally dangerous psychotropics. Little is known about the role of pharmacists in the management of psychotropics used to treat behaviours that challenge. This research aims to determine whether it is feasible to implement and measure the effectiveness of a combined pharmacy–health psychology intervention incorporating a medication review and staff training package to limit the prescription of psychotropics to manage behaviour that challenges in care home residents with dementia.Methods/analysis 6 care homes within the West Midlands will be recruited. People with dementia receiving medication for behaviour that challenges, or their personal consultee, will be approached regarding participation. Medication used to treat behaviour that challenges will be reviewed by the pharmacist, in collaboration with the general practitioner (GP), person with dementia and carer. The behavioural intervention consists of a training package for care home staff and GPs promoting person-centred care and treating behaviours that challenge as an expression of unmet need. The primary outcome measure is the Neuropsychiatric Inventory-Nursing Home version (NPI-NH). Other outcomes include quality of life (EQ-5D and DEMQoL), cognition (sMMSE), health economic (CSRI) and prescribed medication including whether recommendations were implemented. Outcome data will be collected at 6 weeks, and 3 and 6 months. Pretraining and post-training interviews will explore stakeholders’ expectations and experiences of the intervention. Data will be used to estimate the sample size for a definitive study.Ethics/dissemination The project has received a favourable opinion from the East Midlands REC (15/EM/3014). If potential participants lack capacity, a personal consultee will be consulted regarding participation in line with the Mental Capacity Act. Results will be published in peer-reviewed journals and presented at conferences

    Smoking cessation in severe mental ill health : what works? an updated systematic review and meta-analysis

    BACKGROUND: People with severe mental ill health are more likely to smoke than those in the general population. It is therefore important that effective smoking cessation strategies are used to help people with severe mental ill health to stop smoking. This study aims to assess the effectiveness and cost -effectiveness of smoking cessation and reduction strategies in adults with severe mental ill health in both inpatient and outpatient settings. METHODS: This is an update of a previous systematic review. Electronic databases were searched during September 2016 for randomised controlled trials comparing smoking cessation interventions to each other, usual care, or placebo. Data was extracted on biochemically-verified, self-reported smoking cessation (primary outcome), as well as on smoking reduction, body weight, psychiatric symptom, and adverse events (secondary outcomes). RESULTS: We included 26 trials of pharmacological and/or behavioural interventions. Eight trials comparing bupropion to placebo were pooled showing that bupropion improved quit rates significantly in the medium and long term but not the short term (short term RR = 6.42 95% CI 0.82-50.07; medium term RR = 2.93 95% CI 1.61-5.34; long term RR = 3.04 95% CI 1.10-8.42). Five trials comparing varenicline to placebo showed that that the addition of varenicline improved quit rates significantly in the medium term (RR = 4.13 95% CI 1.36-12.53). The results from five trials of specialised smoking cessation programmes were pooled and showed no evidence of benefit in the medium (RR = 1.32 95% CI 0.85-2.06) or long term (RR = 1.33 95% CI 0.85-2.08). There was insufficient data to allowing pooling for all time points for varenicline and trials of specialist smoking cessation programmes. Trials suggest few adverse events although safety data were not always reported. Only one pilot study reported cost effectiveness data. CONCLUSIONS: Bupropion and varenicline, which have been shown to be effective in the general population, also work for people with severe mental ill health and their use in patients with stable psychiatric conditions. Despite good evidence for the effectiveness of smoking cessation interventions for people with severe mental ill health, the percentage of people with severe mental ill health who smoke remains higher than that for the general population
