Resultados preliminares de tenorrafia de tendones flexores por lesión en zona II con técnica Kessler-Tsuge de nudo escondido. Serie de casos
Introduction : Flexor tendon injuries have an annual incidence of 33 x 100,000. Zone II lacerations account for the
most difficult to repair. There is no true consensus regarding a technique or the best suture material to be used. A strong
suture with a high tensile strength avoiding avascularity changes, provide the ability to expedite the rehabilitation
protocols. The goal of this study is to evaluate the results of the Kessler-Tsuge knotless technique.
Methods: Thirty patients with Zone II flexor tendon lacerations were included from 2017 to 2020 in this prospective
multicenter series of cases study. The Kessler-Tsuge knotless repair technique with 4-0 FiberLoop was used on all the
tendon lacerations.
Results: A total of 30 surgical procedures were performed with the described technique. Sixteen patients were lost
to final follow-up. The average age was 26 with a median of 20. The small and ring fingers were the most commonly
injured (57% and 35% respectively). Duran’s flexor tendon rehabilitation protocol was used. At the first month, the average
range of motion (ROM) was 65° PIP and 43° for the DIP joint and ROM of 92° PIP and 72.5° DIP joint at 3 third
month. The Visual Analogue Scale (VAS) for pain was 2 at 3 months. There were no any re-ruptures at final follow-up.
Discussion: kessler-Tsuge technique is a safe and reproducible technique that allows for a hidden knot at the A2
pulley level with a configuration of 4 core strands in two passes. Furthermore, it does not cause shortening of the
ends, provides a homogeneous and smooth bed for tendon gliding, allows for an expedite post-operative protocol, and
displayed good functional results at 3 months post- operative.Introducción. Las lesiones en los tendones flexores tienen una incidencia de 33 casos por cada 100.000 habitantes. Las lesiones en zona II son las que presentan la mayor dificultad al momento de ser intervenidas, pues no existe un estándar de oro en la técnica de tratamiento ni de sutura; sin embargo se ha establecido que una sutura resistente, con fuerza tensil alta y que no genere vascularidad permitirá una movilización y rehabilitación temprana.
Objetivo. Evaluar los resultados de la técnica de tenorrafia tipo Kessler-Tsuge.
Materiales y métodos. Estudio descriptivo multicéntrico de tipo serie de casos realizado entre los años 2017 y 2020 en 30 pacientes con lesión en la zona II de los tendones flexores. Los participantes fueron intervenidos mediante la técnica de tenorrafia tipo Kessler-Tsuge con fiberloop 4.0.
Resultados. La media de edad de los pacientes fue de 26 años, con una mediana de 20 años. El dedo meñique y el anular fueron los más afectados (57% y 35%, respectivamente). Se realizó rehabilitación con protocolo Duran. Las medianas de movilidad articular de las articulaciones interfalángica proximal e interfalángica distal fueron de 65° y 43°, respectivamente, luego de un mes de la cirugía y de 92° y 72.5°, respectivamente, al tercer mes postoperatorio. La media en la escala visual análoga del dolor fue de 2 al tercer mes y no se reportaron rerupturas.
Conclusión. La técnica de tenorrafia tipo Kessler-Tsuge es segura y reproducible, pues esta permite realizar un nudo escondido a nivel de la polea A2 con una configuración de cuatro haces en dos pasadas sin acortamiento de los cabos, da una excursión homogénea y favorece la movilidad temprana con buenos resultados funcionales a los tres meses posoperatorios

Análisis de la importancia de las segundas especialidades en ortopedia en la formación de nuevos profesionales en Colombia
La pregunta alrededor de por qué es relevante la implementación de la segunda especialización en Cirugía de Mano y Miembro Superior en Ortopedia para la formación de profesionales idóneos en Colombia, cobra importancia en tanto que en el momento actual se desconocen aspectos asociados a la oferta y la demanda de estos servicios y, especialmente, en la medida en que políticas actuales de salud contemplan la necesidad de acabar con las segundas especialidades que tengan una duración menor a un año que puedan significar un incremento en el gasto médico nacional.
En este contexto, el presente estudio se concentra en analizar la relevancia de mantener este tipo de programas de formación, identificando los criterios que le confieren pertinencia, valorando la normativa nacional actual, así como estrategias para determinar la cobertura y presencia nacional de cirujanos de mano y considerar la oferta y la demanda laboral de estos especialistas.
Con estos objetivos se plantea un estudio descriptivo, cuyo diseño metodológico incluye la formulación de un instrumento virtual para la recolección de información por intermediación de las sociedades científicas colombianas que agremian a estos especialistas.

El análisis de los resultados permitiría la creación de la primera base de datos nacional de ortopedistas y cirujanos de mano con segunda especialidad; brindará un panorama actualizado de la oferta y la demanda de estos servicios; y permitirá prospectar análisis “costo-beneficio” en el sistema de salud frente a la necesidad de la formación de profesionales con esta segunda especialidad médica.The question about why the implementation of the second specialization in Hand
and upper extremity surgery for orthopedists surgeons is relevant for the training of
suitable professionals in Colombia, becomes important because nowadays aspects
associated with the offer are unknown. and the demand for these services and, especially, to
the extent that current health policies contemplate the need to end second specialties that
last less than a year that could mean an increase in national medical spending.
In this context, the present study concentrates on analyzing the relevance of
maintaining this type of special training programs, identifying the criteria that make it
relevant, evaluating current national regulations, as well as strategies to determine the
coverage and national presence of hand surgeons. and consider the supply and labor
demand of these specialists.
With these objectives, a descriptive study is proposed, whose methodological design
includes the formulation of a rigorous instrument for the collection of information through
the Colombian scientific societies that gather these specialists.
The analysis of the results would allow the creation of the first national database of
orthopedists and plastic surgeons with second specialty; It will provide an updated
overview of the offer and demand for these services; and it will allow prospecting “costbenefit” analyzes in the health system against the need for the training of professionals with
this second medical specialty, taking into account their competences and expertise, versus

the emergence of medical complications associated with pathologies that could be
prevented by the expert intervention of these professionals
Catalytic cleavage of HEAT and subsequent covalent binding of the tetralone moiety by the SARS-CoV-2 main protease
Here we present the crystal structure of SARS-CoV-2 main protease (Mpro) covalently bound to 2-methyl-1-tetralone. This complex was obtained by co-crystallization of Mpro with HEAT (2-(((4-hydroxyphenethyl)amino)methyl)-3,4-dihydronaphthalen-1(2H)-one) in the framework of a large X-ray crystallographic screening project of Mpro against a drug repurposing library, consisting of 5632 approved drugs or compounds in clinical phase trials. Further investigations showed that HEAT is cleaved by Mpro in an E1cB-like reaction mechanism into 2-methylene-1-tetralone and tyramine. The catalytic Cys145 subsequently binds covalently in a Michael addition to the methylene carbon atom of 2-methylene-1-tetralone. According to this postulated model HEAT is acting in a pro-drug-like fashion. It is metabolized by Mpro, followed by covalent binding of one metabolite to the active site. The structure of the covalent adduct elucidated in this study opens up a new path for developing non-peptidic inhibitors
