    Prevention method of toric intraocular lens rotation after phacoemulsification of cataract using a standard capsule ring

    At the same time, phacoemulsification of cataracts remains the most popular surgery. But the problem among phacosurgeons is the correction of existing corneal astigmatism. According to studies, about 30% of the world's population have corneal astigmatism with a strength of 0.75D and above, which leads to a decrease in visual acuity after phacoemulsification of cataracts in the absence of its correction.The most commonly used method to correct corneal astigmatism is the implantation of toric intraocular lenses (TIOL). The main condition for obtaining high visual acuity after TIOL implantation is the location of the astigmatic component of the artificial lens in accordance with the strong meridian of the cornea. The aim of this study was to evaluate the effectiveness of the proposed method of preventing the toric intraocular lens rotation after phacoemulsification of cataracts in the eyes with corneal astigmatism by implantation of a standart capsule ring.Material and methods. The article presents the results of a comprehensive examination of 43 patients (59 eyes) who underwent surgery by phacoemulsification of cataracts with implantation of toric IOL to correct concomitant corneal astigmatism. Corneal astigmatism according to keratometry ranged from 0.5D to 4.25D (average 1.96 ± 0.1 D). Patients were divided into 2 groups. The first group consisted of 26 patients (31 eyes). Patients in this group underwent phacoemulsification with toric IOL implantation according to standard techniques without the use of techniques that prevent rotation of the artificial lens in the postoperative period. The second group included 18 patients (28 eyes) who were operated on according to our proposed prevention method of TIOL rotation. As a result of the study, it was found that rotation of TIOL was detected 6 months after surgery in the I group without the use of developed method in 22 eyes (70.9%), in the II group, where the capsule ring was implanted, rotation occurred in 12 eyes (42.85%). In the II group in all cases, the deviation of the TIOL axis did not exceed 6 degrees. The proposed method of prevention of rotation of TIOL allows to increase the efficiency of phacoemulsification of cataracts.Conclusions. Developed a method of preventing the rotation of the toric intraocular lens using a standard capsule ring, which is implanted and located on top of the haptic support elements of the artificial lens can increase the efficiency of phacoemulsification of cataracts in the eyes with corneal astigmatism, contributes to the rotational stability of the toric lens and provides long-term results of visual functions, reduces the need for repeated surgery due to reposition of the rotated lens and can be recommended for implementation in the practice of ophthalmic surgeons.The use of implantation of a standard capsule ring according to the proposed in this study method prevents the rotation of the toric intraocular lens by more than 6 degrees in all cases, which does not significantly reduce visual acuity and does not require reposition of the artificial lens. This method of preventing rotation of the toric intraocular lens allows to achieve visual acuity of 0.7 and above in 85.6% of cases

    Binary Archetype in Ben Elton’s Novel “Two Brothers”

    The novel of the modern British writer Ben Elton “Two Brothers” (2012) is analyzed in terms of the functioning of the binary archetype in it. It is noted that contemporary cultural-historical thinking is characterized by a rejection of the classical rational type of thinking based on binary oppositions. The restoration of a certain clear opposition in B. Elton’s novel is interpreted as the author’s desire to preserve the opposition of humanitarian ideology to fascism at the present stage of socio-cultural development. It is established that the artistic structure of the novel clearly correlates with the tradition of twin myths. The author of the article draws attention to the fact that the plot of the novel is connected with the fate of two boys from a Jewish family, one of whom was his own son, and the second, a German by nationality, was adopted. It is emphasized that according to the laws of the twin myth, they conflict among themselves, but this is an imaginary confrontation, because in fact the brothers belong to the world of Germany, which opposed the onslaught of Nazi ideology. It is shown that at the deep level of his work, Ben Elton builds a distinct binary opposition of Good and Evil, in which there can be no justification for chauvinism as an extreme manifestation of fascist doctrine

    Impact of corneal astigmatism on refractive outcomes after phacoemulsification with implantation of a spherical IOL

    Background: To date, particular emphasis is being put to correction of preoperative corneal astigmatism in phacoemulsification, since approximately 30% of the world’s population has astigmatism of at least 0.75D which results in decreased visual acuity after cataract surgery. Purpose: To assess the effect of corneal astigmatism on refractive outcomes of phacoemulsification with implantation of a spheric intraocular lens (IOL). Material and Methods: We retrospectively analyzed the outpatient medical records of 39 patients (50 eyes) who received phacoemulsification with a spherical IOL and had corneal astigmatism of 0.5-3.75D (as assessed by keratometry). Eyes were divided into four groups based on the degree of corneal astigmatism. We assessed changes in visual acuity and corneal astigmatism and refractive outcomes of cylindrical correction at one month after surgery. Results: The greater the presurgical astigmatism, the lower was uncorrected visual acuity at one month after surgery. There was no significant difference (р > 0.05) in change in corneal astigmatism values after phacoemulsification. In group 1 (preoperative astigmatism of 0.75 D or less) and group 2 (preoperative astigmatism of 1.0 to 1.5 D), the mean increase in visual acuity after cylindrical refractive correction was 10% or less, and had no significant impact on the quality of vision. In group 3 (preoperative astigmatism of 1.75 to 2.5 D) and group 4 (preoperative astigmatism of 2.75 D or more), the mean increase in visual acuity was 15% and 25%, respectively. Conclusion: Surgically induced astigmatism after phacoemulsification had no substantial impact on refractive outcomes. When planning refractive outcomes for eyes with astigmatism after cataract surgery, it should be taken into account that implanting a spherical IOL is acceptable only for eyes with an amount of preoperative astigmatism of 0.75 D or less and vertical axis of astigmatism. Cataract patients with preoperative corneal astigmatism of >0.75 D will require implantation of a toric IOL or a plan for astigmatism correction with another method

    Prediction and evaluation of nonlinear site response with potentially liquefiable layers in the area of Nafplion (Peloponnesus, Greece) for a repeat of historical earthquakes

    We examine the possible non-linear behaviour of potentially liquefiable layers at selected sites located within the expansion area of the town of Nafplion, East Peloponnese, Greece. Input motion is computed for three scenario earthquakes, selected on the basis of historical seismicity data, using a stochastic strong ground motion simulation technique, which takes into account the finite dimensions of the earthquake sources. Site-specific ground acceleration synthetics and soil profiles are then used to evaluate the liquefaction potential at the sites of interest. The activation scenario of the Iria fault, which is the closest one to Nafplion (<i>M</i>=6.4), is found to be the most hazardous in terms of liquefaction initiation. In this scenario almost all the examined sites exhibit liquefaction features at depths of 6–12 m. For scenario earthquakes at two more distant seismic sources (Epidaurus fault – <i>M</i>6.3; Xylokastro fault – <i>M</i>6.7) strong ground motion amplification phenomena by the shallow soft soil layer are expected to be observed

    Tecnologias e dispositivos de microondas que separam a pele da pele de coelho

    The purpose of this work is the development of microwave technology and de-vices separating fur from rabbit skins in a continuous mode at reduced operating costs. They developed the method for the action of the ultrahigh frequency electro-magnetic field (UFEF), which provides the reduction of fur fiber holding power in the meat fabric of rabbit skins with simultaneous combing and gathering. They calcu-lated the mathematical dependences describing the dynamics of multicomponent raw material heating with the change of dielectric and physicomechanical parameters in the process of UFEF. They calculated and visualized the electromagnetic field distri-butions in the developed resonators, on the basis of which they substantiated the ef-fective parameters of the electrodynamic system: the maximum property, the quality of resonators and the electric field strength. They substantiated the complex of struc-tural and technological parameters of devices with new structural design of working chambers by a multi-criteria evaluation of the technological process for the separation of fur from rabbit skin. They manufactured and tested the microwave device under production conditions for the separation of fur from rabbit skin, and adjusted the op-erating modes, which allow to reduce operating costs. They evaluated the technical and economic efficiency of microwave technology introduction for the processing of fur and the devices for rabbit farms; they developed practical recommendations for the operation of microwave units to separate fur from rabbit skins.El propósito de este trabajo es el desarrollo de la tecnología de microondas y los dispositivos que separan la piel de los cueros de conejo en un modo continuo a costos operativos reducidos. Desarrollaron el método para la acción del campo electromagnético de frecuencia ultraalta (UFEF), que proporciona la reducción del poder de retención de la fibra de la piel en el tejido de carne de la piel de conejo con el peinado y la recolección simultáneos. Calcularon las dependencias matemáticas que describen la dinámica del calentamiento de materias primas de componentes múltiples con el cambio de los parámetros dieléctricos y fisicomecánicos en el proceso de la UFEF. Calcularon y visualizaron las distribuciones del campo electromagnético en los resonadores desarrollados, sobre la base de los cuales fundamentaron los parámetros efectivos del sistema electrodinámico: la propiedad máxima, la calidad de los resonadores y la intensidad del campo eléctrico. Ellos sustentaron el complejo de parámetros estructurales y tecnológicos de los dispositivos con un nuevo diseño estructural de las cámaras de trabajo mediante una evaluación de múltiples criterios del proceso tecnológico para la separación de la piel del cuero del conejo.  Fabricaron y probaron el dispositivo de microondas en condiciones de producción para la separación de la piel de la piel de conejo, y ajustaron los modos de operación, lo que permite reducir los costos operativos. Evaluaron la eficiencia técnica y económica de la introducción de la tecnología de microondas para el procesamiento de pieles y los dispositivos para granjas de conejos; desarrollaron recomendaciones prácticas para el uncionamiento de las unidades de microondas para separar las pieles de los cueros de conejo.O objetivo deste trabalho é o desenvolvimento de tecnologia e dispositivos que separam a pele de peles de coelho em um modo contínuo a custos operacionais reduzidos microondas. Eles desenvolveram o método para a acção do campo electromagnético de alta frequência ultra-(UFEF), que fornece pele de coelho carne tecido de pele para retenção de fibras de energia reduzido com cabelo e recolha simultânea. Calcularam as dependências matemático para descrever a dinâmica do aquecimento materiais multicomponentes com a mudança de dieltricos e parâmetros fisicomecánicos no processo de UFEF. Eles calculado e visualizado as distribuições de campo eletromagnético em ressonadores desenvolvidos, com base na qual fundamentar os parâmetros reais do sistema electrodynamic: a propriedade máximo, a qualidade dos ressonadores e intensidade do campo elétrico. Eles sustentadas parâmetros estruturais e tecnológicas complexas dos dispositivos com um novo design estrutural das câmaras de trabalho através de uma avaliação multi-critérios do processo tecnológico para a separação de couro pele de coelho.
