    Design of Wireless Sensors for IoT with Energy Storage and Communication Channel Heterogeneity

    Autonomous Wireless Sensors (AWSs) are at the core of every Wireless Sensor Network (WSN). Current AWS technology allows the development of many IoT-based applications, ranging from military to bioengineering and from industry to education. The energy optimization of AWSs depends mainly on: Structural, functional, and application specifications. The holistic design methodology addresses all the factors mentioned above. In this sense, we propose an original solution based on a novel architecture that duplicates the transceivers and also the power source using a hybrid storage system. By identifying the consumption needs of the transceivers, an appropriate methodology for sizing and controlling the power flow for the power source is proposed. The paper emphasizes the fusion between information, communication, and energy consumption of the AWS in terms of spectrum information through a set of transceiver testing scenarios, identifying the main factors that influence the sensor node design and their inter-dependencies. Optimization of the system considers all these factors obtaining an energy efficient AWS, paving the way towards autonomous sensors by adding an energy harvesting element to them

As machine learning models become more sophisticated, and bio-metric data becomes more readily available through new non-invasive technologies, it becomes increasingly possible to gain access to interesting biometric data that could revolutionize Human-Computer Interaction. In this research, we propose a method to assess and quantify human attention levels and their effects on learning. In our study, we employ a brain computer interface (BCI) capable of detecting brain wave activity and displaying the corresponding electroencephalograms (EEG). We train recurrent neural networks (RNNS) to identify the type of activity an individual is performing

    Body mass index in nursing home residents during the first year after admission

    La desnutrición, que comprende tanto la desnutrición como el sobrepeso, debe ser abordada en el seguimiento médico de los adultos mayores debido a las consecuencias negativas para el estado funcional y la salud general. Aún así, se sabe poco sobre el estado nutricional de los residentes de hogares de ancianos (NH), especialmente con respecto al aumento o la pérdida de peso después de la admisión a NH. Por lo tanto, este estudio tiene como objetivo evaluar los cambios en el índice de masa corporal (IMC) durante el primer año posterior al ingreso a NH y explorar las características demográficas y clínicas relacionadas con los cambios en el IMC. Métodos Se combinaron los datos de dos estudios prospectivos que reclutaron participantes al ingreso a NH. Las características demográficas y clínicas, incluido el IMC, se evaluaron al inicio y después de un año. Se estimó un modelo de regresión lineal para explorar el impacto de las características demográficas y clínicas en el cambio del IMC. Resultados La cohorte del estudio constaba de 1044 participantes con una edad media de 84,3 años (DE 7,6) al inicio del estudio; El 64,2% eran mujeres. Al inicio del estudio, el 33% de los residentes de NH tenían desnutrición severa a moderada, mientras que el 10% eran obesos. Durante el primer año de su estadía en NH, los residentes con desnutrición severa a moderada tuvieron un aumento promedio en el IMC de 1,3 kg/m2 (DE 2,2; p < 0,001), mientras que los cambios de peso fueron muy pequeños o no significativos en los otros grupos de IMC . Las características relacionadas con el aumento de peso fueron una edad más joven y menos agitación. Conclusión La desnutrición es un desafío de salud común al momento de la admisión a NH, con un tercio de los residentes de NH con un peso bajo moderado a severo y un 10% con obesidad. Sin embargo, durante el primer año de estancia en NH, hubo una evolución favorable para los residentes de NH con bajo peso, ya que aumentaron su IMC, y el 43,6% cambió a una clasificación de mayor peso, mientras que no observamos cambios en el IMC en los residentes con obesidad. Still, little is known about the nutritional state of nursing home (NH) residents, especially with respect to weight gain or weight loss after NH admission. Therefore, this study aims to evaluate changes in the body mass index (BMI) during the first year following NH admission, and to explore demographic and clinical characteristics related to BMI changes. Methods Data from two prospective studies that recruited participants at NH admission were combined. Demographic and clinical characteristics including the BMI were assessed at baseline and after one year. A linear regression model was estimated to explore the impact of demographic and clinical characteristics on the change in BMI. Results The study cohort consisted of 1,044 participants with a mean age of 84.3 years (SD7.6) at baseline; 64.2% were female. At baseline, 33% of the NH residents had severe to moderate undernutrition, while 10% were obese. During the first year of their NH stay, residents with severe to moderate undernutrition had an average increase in BMI of 1.3 kg/m2 (SD 2.2; p < 0.001), while weight changes were either very small or not significant in the other BMI groups. Characteristics related to weight gain were younger age and less agitation. Conclusion Malnutrition is a common health challenge at NH admission, with one third of NH residents being moderately to severely underweight and 10% being obese. However, during the first year of NH stay, there was a favourable development for underweight NH residents, as they increased their BMI, and 43.6% changed to a higher weight classification, while we observed no changes in the BMI in residents with obesity.     All-oral metronomic DEVEC schedule in elderly patients with peripheral T cell lymphoma

    Purpose: Peripheral T cell lymphomas (PTCLs) have an overall poor prognosis. Indeed, registry data in elderly patients show that the median progression-free survival (mPFS) following first- and second-line therapies are only 6.7 and 3.1&nbsp;months, respectively. The aim of the study is to show the activity of metronomic chemotherapy, a regular administration of low chemotherapeutic drug doses allowing a favourable toxicity profile, on elderly PTCL patients. Methods: We report a series of 17 PTCL patients, treated with the all-oral metronomic schedule DEVEC (prednisolone–etoposide–vinorelbine–cyclophosphamide) in four Italian centres. Patients 5/17 (29.4%) were treatment-naïve (naïve) and 12/17 (70.6%) were relapsed-refractory (RR), respectively. The median age was 83&nbsp;years (range 71–87) and 71.5&nbsp;years (range 56–85) for naïve and RR, respectively. In vitro activity of metronomic vinorelbine (VNR), etoposide (ETO) and their concomitant combination on HH, a PTCL cell line, was also assessed. Results: Histology: PTCL-not-otherwise-specified = 12; angioimmunoblastic = 2; NK/T nasal type = 1; adult-type leukaemia lymphoma = 1, transformed Mycosis Fungoides = 1. The overall response rate was 80 and 58% in naïve and RR, respectively; whereas the PFS was 20 in naïve (95% CI 0–43) and 11&nbsp;months (95% CI 4.2–17.8) in RR. The occurrence of relevant adverse events was 23.5%, which was managed with ETO dose reduction. In vitro experiments showed that both metronomic VNR and ETO caused a significant inhibitory activity on HH cells and a strong synergism when administered concomitantly. Conclusion: All-oral DEVEC showed an encouraging activity and acceptable toxicity. This schedule deserves further studies in elderly PTCL also for assessing combinations with targeted drugs

    Integrins and discoidin domain receptors (DDRs) 1 and 2 promote cell adhesion and migration on both fibrillar and non fibrillar collagens. Collagen I contains DDR and integrin selective binding motifs; however, the relative contribution of these two receptors in regulating cell migration is unclear. DDR1 has five isoforms (DDR1a-e), with most cells expressing the DDR1a and DDR1b isoforms. We show that human embryonic kidney 293 cells expressing DDR1b migrate more than DDR1a expressing cells on DDR selective substrata as well as on collagen I in vitro. In addition, DDR1b expressing cells show increased lung colonization after tail vein injection in nude mice. DDR1a and DDR1b differ from each other by an extra 37 amino acids in the DDR1b cytoplasmic domain. Interestingly, these 37 amino acids contain an NPxY motif which is a central control module within the cytoplasmic domain of β integrins and acts by binding scaffold proteins, including talin. Using purified recombinant DDR1 cytoplasmic tail proteins, we show that DDR1b directly binds talin with higher affinity than DDR1a. In cells, DDR1b, but not DDR1a, colocalizes with talin and integrin β1 to focal adhesions and enhances integrin β1-mediated cell migration. Moreover, we show that DDR1b promotes cell migration by enhancing Rac1 activation. Mechanistically DDR1b interacts with the GTPase-activating protein (GAP) Breakpoint cluster region protein (BCR) thus reducing its GAP activity and enhancing Rac activation. Our study identifies DDR1b as a major driver of cell migration and talin and BCR as key players in the interplay between integrins and DDR1b in regulating cell migration

    Cerebral involvement in a patient with Goodpasture's disease due to shortened induction therapy: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Goodpasture's disease is a rare immunological disease with formation of pathognomonic antibodies against renal and pulmonary basement membranes. Cerebral involvement has been reported in several cases in the literature, yet the pathogenetic mechanism is not entirely clear.</p> <p>Case presentation</p> <p>A 21-year-old Caucasian man with Goodpasture's disease and end-stage renal disease presented with two generalized seizures after a period of mild cognitive disturbance. Blood pressure and routine laboratory tests did not exceed the patient's usual values, and examination of cerebrospinal fluid was unremarkable. Cerebral magnetic resonance imaging (MRI) revealed multiple cortical and subcortical lesions on fluid-attenuated inversion recovery sequences. Since antiglomerular basement membrane antibodies were found to be positive with high titers, plasmapheresis was started. In addition, cyclophosphamide pulse therapy was given on day 13. Encephalopathy and MRI lesions disappeared during this therapy, and antiglomerular basement membrane antibodies were significantly reduced. Previous immunosuppressive therapy was performed without corticosteroids and terminated early after 3 months.</p> <p>The differential diagnostic considerations were cerebral vasculitis and posterior reversible encephalopathy syndrome. Vasculitis could be seen as an extrarenal manifestation of the underlying disease. Posterior reversible encephalopathy syndrome, on the other hand, can be triggered by immunosuppressive therapy and may appear without a hypertensive crisis.</p> <p>Conclusion</p> <p>A combination of central nervous system symptoms with a positive antiglomerular basement membrane test in a patient with Goodpasture's disease should immediately be treated as an acute exacerbation of the disease with likely cross-reactivity of antibodies with the choroid plexus. In our patient, a discontinuous strategy of immunosuppressive therapy may have favored recurrence of Goodpasture's disease.</p
