    Difference of late potentials detected by signal-averaged ECG in patients with spontaneous or drug-induced type 1 electrocardiogram pattern of Brugada syndrome

All rights reserved. © The Author 2017.Introduction: Brugada syndrome (BrS) patients (pts) with spontaneous type 1 electrocardiographic (ECG) pattern (sT1ECGp) have a greater arrhythmic risk compared to those with fleicainide-induced type 1 ECG pattern (iT1ECGp). However, when the analysis is restricted to asymptomatic pts, the type 1 spontaneous pattern loses its independent prognostic value. Late potentials (LP) obtained by signal-averaged ECG (SA-ECG) are associated with regions of delayed myocardial depolarization and consequent abnormal electrical conduction. There is a higher prevalence of LP obtained by SA-ECG in pts with BrS and their detection showed a strong prognostic predictor value in several studies. Objective: To evaluate the presence of LP by SA-ECG in pts with BrS and assess differences between patients with spontaneous or drug-induced type 1 ECG pattern. Methods: This was a single-center prospective study of consecutive pts diagnosed with BrS, including sT1ECGp and iT1ECGp. The patients were submitted to SA-ECG study to detect LP, with determination of the duration of filtered QRS (fQRS), root-mean-square voltage of the terminal 40ms of the filtered QRS (RMS40) and the duration of low-amplitude signal (<40 μV) in the terminal part of the filtered QRS complex (LAS40), using conventional and right modified leads. The presence of LP was considered positive when ≥2 of the following were present: fQRS ≥114 ms, RMS40 <20 μV or LAS40 ≥38ms. The results were displayed using medians and interquartile ranges, obtained using the Mann-Whitney test. Results: The presence of LP by SA-ECG was studied in 29 pts (75.9% male, mean age 44±12 years), 18 with sT1ECGp and 11 iT1ECGp. Only 3 pts (10.3%) had symptoms related with BrS (unexplained syncope) and none had documented malignant ventricular arrhythmias. Known or potential pathogenic mutations were identified in 5 pts (17.2%). The presence or absence of LP showed no statistically significant difference according to clinical, electrocardiographic or genetic characteristics of the pts. However, in conventional leads, pts with sT1ECGp showed significantly higher values of fQRS and lower values of RMS40 [fQRS 108 (103–112) vs. 97 (89–103), p=0.016; RMS40 19 (10–22) vs. 22 (16–40), p=0.028]. In addition, in modified right leads, pts with sT1ECGp had significantly higher values of fQRS, lower RMS40 and longer LAS40 [fQRS 108 (101 -111) vs. 98 (89–102), p=0.0005; RMS40 15 (11–21) vs. 25 (18–33), p=0.007; LAS40 41 (34–49) vs. 31 (28- 39), p=0.007]. Conclusion: Patients with the spontaneous type 1 electrocardiographic (ECG) pattern revealed a higher detection of late potentials, which may partially explain the higher arrhythmogenic risk classically described in this subgroup of BrS patients.info:eu-repo/semantics/publishedVersio

    On the correlation between electronic intramolecular delocalization and Au-S bonding strength of ruthenium tetraammine SAMs

    Trans-[Ru(L)(NH3)4(L’)](PF6)n type complexes, where L = 4-cyanopyridine (CNpy), NCS-, CN-, and L’ = CNpy, 1,4-dithiane (1,4-dt), 4-mercaptopyridine (pyS) and thionicotinamide (tna), were synthesized and characterized. SAMs on gold of the complexes containing sulfur were studied by reductive desorption and SERS spectroscopy. Depending on the nature of L’, the withdrawing capability of the CNpy ligand is strong enough to partially oxidize the ruthenium atom and, as a consequence, delocalize the s electronic density from the trans located ligand. The reductive desorption results showed that the stability of the SAMs is directly related to this effect

    A análise do papel dos engenheiros militares perpassa as discussões sobre regiões específicas, mesmo quando eles, aparentemente, estão ausentes delas. Sua presença ou ausência sempre foi sinônimo de interesse e de projeto colonizador mais ou menos dirigido. Este ensaio tem como objeto de estudo a cartografia dos engenheiros militares atuantes na Capitania de São Paulo, território comumente visto como economicamente periférico e mais autônomo face aos desígnios da Coroa portuguesa. Com foco no corpus cartográfico de alguns profissionais que aqui atuaram, trata indiretamente da multifacetada atuação desses engenheiros, bem como do processo de feitura das cartas geográficas, corográficas e topográficas, do borrão às aguadas. Investigar a cartografia de alguns desses profissionais permite não apenas explorar a riqueza de suas biografias, como, também, acompanhar o processo de produção técnica dos mapas e os desígnios políticos envolvidos no devassamento e representação das terras descobertas no Novo Mundo.Analyzing the role of military engineers means venturing into discussions about specific regions, even those from which they are apparently absent. Their presence or absence has always been synonymous with interest and a more or less directed colonization project. This essay examines the maps produced by the military engineers working in the Captaincy of São Paulo, a territory often looked upon as one of little economic import and a rather autonomous area in light of the plans of the Portuguese Crown. By focusing on the cartographic corpus bequeathed by certain professionals serving in this region, we deal indirectly with the multifaceted role of these engineers as well as with the process of making geographic, chorographic and topographic charts - all the way from the sketch to watercolored maps. Perusing the charts constructed by some of these professionals enabled us not only to delve into the rich details of their biographies but also to observe the technical mapmaking process and to unravel the political designs behind the exploration and representation of the lands discovered in the New World

    Efficacy of rifabutin-based triple therapy as second-line treatment to eradicate helicobacter pylori infection

    <p>Abstract</p> <p>Background</p> <p>Rifabutin has been found to be effective in multi-resistant patients after various treatment cycles for Helicobacter pylori (HP) infection, but it has not been analysed as a second-line treatment. Therefore, we seek to compare the effectiveness of a treatment regimen including rifabutin versus conventional quadruple therapy (QT).</p> <p>Methods</p> <p>Open clinical trial, randomised and multi-centre, of two treatment protocols: A) Conventional regime -QT- (omeprazole 20 mg bid, bismuth citrate 120 mg qid, tetracycline 500 mg qid and metronidazole 500 mg tid); B) Experimental one -OAR- (omeprazole 20 mg bid, amoxicillin 1 gr bid, and rifabutin 150 mg bid), both taken orally for 7 days, in patients with HP infection for whom first-line treatment had failed. Eradication was determined by Urea Breath Test (UBT). Safety was determined by the adverse events.</p> <p>Results</p> <p>99 patients were randomised, QT, n = 54; OAR, n = 45. The two groups were homogeneous. In 8 cases, treatment was suspended (6 in QT and 2 in OAR). The eradication achieved, analysed by ITT, was for QT, 38 cases (70.4%), and for OAR, 20 cases (44.4%); p = 0.009, OR = 1.58. Of the cases analysed PP, QT were 77.1%; OAR, 46.5%; p = 0.002. Adverse effects were described in 64% of the QT patients and in 44% of the OAR patients (p = 0.04).</p> <p>Conclusion</p> <p>A 7-day rifabutin-based triple therapy associated to amoxicillin and omeprazole at standard dose was not found to be effective as a second-line rescue therapy. The problem with quadruple therapy lies in the adverse side effects it provokes. We believe the search should continue for alternatives that are more comfortably administered and that are at least as effective, but with fewer adverse side effects.</p> <p>Trial Registration</p> <p>Current Controlled Trials ISRCTN81058036</p
