    Investigating cognitive effort and its role in control over Pavlovian bias

    Effort is a key determinant of cognitive performance, particularly for processes involving cognitive control – without it, performance may be slow, inaccurate or biased. Related to this, decreased ability to exert effort has been implicated in the symptoms of conditions including depression and anxiety. In this thesis I investigate the role of effort in the specific case of control over Pavlovian biases. In the first two experimental chapters I examine whether Pavlovian biases are in principle modifiable, a necessary precondition for demonstrating that they are also controllable. Following a simple programme of behavioural training, participants showed reduced influence of Pavlovian biases on behaviour, a result which is consistent with increased cognitive control. In the third experimental chapter, I present a new task for measuring cognitive effort sensitivity, suitable in particular for individual differences research. Subsequently, in the final experimental chapter, I use this task to test directly the hypothesis that the strength of Pavlovian bias is influenced by effortful cognitive control. I present initial evidence that indeed willingness to exert effort and the strength of Pavlovian biases seem to be negatively correlated, while effort also seems to be negatively associated with both depression and anxiety symptoms. Finally, in a standalone theoretical chapter, I discuss the rationale for effort costs, which currently are not well understood; I introduce and extend two existing ideas from outside of neuroscience which I think may be informative in this regard. Overall this thesis extends our understanding of the link between effort and control, suggesting in particular that the expression of Pavlovian biases can be framed in terms of effort-based decision-making. Additionally, by introducing fresh ideas about the basis of cognitive effort costs, it is hoped that this thesis will provide stronger foundations on which experimental research on cognitive effort can be built in the future

    Measuring cognitive effort without difficulty

    An important finding in the cognitive effort literature has been that sensitivity to the costs of effort varies between individuals, suggesting that some people find effort more aversive than others. It has been suggested this may explain individual differences in other aspects of cognition; in particular that greater effort sensitivity may underlie some of the symptoms of conditions such as depression and schizophrenia. In this paper, we highlight a major problem with existing measures of cognitive effort that hampers this line of research, specifically the confounding of effort and difficulty. This means that behaviour thought to reveal effort costs could equally be explained by cognitive capacity, which influences the frequency of success and thereby the chance of obtaining reward. To address this shortcoming, we introduce a new test, the Number Switching Task (NST), specially designed such that difficulty will be unaffected by the effort manipulation and can easily be standardised across participants. In a large, online sample, we show that these criteria are met successfully and reproduce classic effort discounting results with the NST. We also demonstrate the use of Bayesian modelling with this task, producing behavioural parameters which can be associated with other measures, and report a preliminary association with the Need for Cognition scale

    Cost-effectiveness of scaling up mass drug administration for the control of soil-transmitted helminths : a comparison of cost function and constant costs analyses

    Background: The coverage of mass drug administration (MDA) for neglected tropical diseases, such as the soil-transmitted helminths (STHs), needs to rapidly expand to meet WHO's 2020 targets. We aimed to compare use of a cost function to take into account economies of scale to the standard method of assuming a constant cost per treatment when investigating the cost and cost-effectiveness of scaling up a STH MDA programme targeting Ascaris lumbricoides. Methods: We fitted a cost function describing how the costs of MDA change with scale to empirical cost data and incorporated it into a STH transmission model. Using this cost function, we investigated the consequences of taking into account economies of scale on the projected cost-effectiveness of STH control, by comparison with the standard method of assuming a constant cost per treatment. The cost function was fitted to economic cost data collected as part of a school-based deworming programme in Uganda using maximum likelihood methods. We used the model to investigate the total reduction in the overall worm burden, the total number of prevalent infection case-years averted, and the total number of heavy infection case-years averted. For each year, we calculated the effectiveness as the difference between the worm burden or number of cases and the number in absence of treatment. Findings: When using the cost function, the cost-effectiveness of STH control markedly increased as the programme was scaled up. By contrast, the standard method (constant cost per treatment) undervalued this and generated misleading conclusions. For example, when scaling up control in the projected district from 10% to 75% coverage of at-risk school-age children, the cost-effectiveness in terms of prevention of heavy burden infections was projected to increase by over 70% when using the cost function, but decrease by 18% when assuming a constant cost per treatment. Interpretation: The current exclusion of economies of scale in most economic analyses must be addressed if the most cost-effective policies for the control of neglected tropical diseases are to be formulated. These findings are also relevant to other large-scale disease interventions

    Economic considerations for moving beyond the Kato-Katz technique for diagnosing intestinal parasites as we move towards elimination

    While the need for more sensitive diagnostics for intestinal helminths is well known, the cost of developing and implementing new tests is considered relatively high compared to the Kato-Katz technique. Here, we review the reported costs of performing the Kato-Katz technique. We also outline several economic arguments we believe highlight the need for further investment in alternative diagnostics, and considerations that should be made when comparing their costs. In our opinion, we highlight that, without new diagnostic methods, it will be difficult for policy makers to make the most cost-effective decisions and that the potentially higher unit costs of new methods can be outweighed by the long-term programmatic benefits they have (such as the ability to detect the interruption of transmission)

    A phenome-wide comparative analysis of genetic discordance between obesity and type 2 diabetes

    Obesity and type 2 diabetes are causally related, yet there is considerable heterogeneity in the consequences of both conditions and the mechanisms of action are poorly defined. Here we show a genetic-driven approach defining two obesity profiles that convey highly concordant and discordant diabetogenic effects. We annotate and then compare association signals for these profiles across clinical and molecular phenotypic layers. Key differences are identified in a wide range of traits, including cardiovascular mortality, fat distribution, liver metabolism, blood pressure, specific lipid fractions and blood levels of proteins involved in extracellular matrix remodelling. We find marginal differences in abundance of Bacteroidetes and Firmicutes bacteria in the gut. Instrumental analyses reveal prominent causal roles for waist-to-hip ratio, blood pressure and cholesterol content of high-density lipoprotein particles in the development of diabetes in obesity. We prioritize 17 genes from the discordant signature that convey protection against type 2 diabetes in obesity, which may represent logical targets for precision medicine approaches.</p

    Felodipine induces autophagy in mouse brains with pharmacokinetics amenable to repurposing.

    Neurodegenerative diseases like Alzheimer's disease, Parkinson's disease and Huntington's disease manifest with the neuronal accumulation of toxic proteins. Since autophagy upregulation enhances the clearance of such proteins and ameliorates their toxicities in animal models, we and others have sought to re-position/re-profile existing compounds used in humans to identify those that may induce autophagy in the brain. A key challenge with this approach is to assess if any hits identified can induce neuronal autophagy at concentrations that would be seen in humans taking the drug for its conventional indication. Here we report that felodipine, an L-type calcium channel blocker and anti-hypertensive drug, induces autophagy and clears diverse aggregate-prone, neurodegenerative disease-associated proteins. Felodipine can clear mutant α-synuclein in mouse brains at plasma concentrations similar to those that would be seen in humans taking the drug. This is associated with neuroprotection in mice, suggesting the promise of this compound for use in neurodegeneration

    Knowing your own heart: distinguishing interoceptive accuracy from interoceptive awareness

    Interoception refers to the sensing of internal bodily changes. Interoception interacts with cognition and emotion, making measurement of individual differences in interoceptive ability broadly relevant to neuropsychology. However, inconsistency in how interoception is defined and quantified led to a three-dimensional model. Here, we provide empirical support for dissociation between dimensions of: (1) interoceptive accuracy (performance on objective behavioural tests of heartbeat detection), (2) interoceptive sensibility (self-evaluated assessment of subjective interoception, gauged using interviews/questionnaires) and (3) interoceptive awareness (metacognitive awareness of interoceptive accuracy, e.g. confidence-accuracy correspondence). In a normative sample (N = 80), all three dimensions were distinct and dissociable. Interoceptive accuracy was only partly predicted by interoceptive awareness and interoceptive sensibility. Significant correspondence between dimensions emerged only within the sub-group of individuals with greatest interoceptive accuracy. These findings set the context for defining how the relative balance of accuracy, sensibility and awareness dimensions explain cognitive, emotional and clinical associations of interoceptive ability
