    Health-Related Risk Factors for Women Who Have Sex with Women

    Women, bisexual women, certain heterosexual women, and people of the same sex in a setting are all examples of women who have sex with women (WSW). Extensive research on the behavioral traits and health status of the WSW community has revealed that they have a worse overall health status, a greater risk of sexually transmitted disease infection, and a worse mental health status than heterosexuals. By doing a study of the WSW population’s current characteristics, sexually transmitted disease infection status, mental and mental health state, and health-related risk factors, this paper establishes a foundation for health education and behavioral interventions with this population

    Semantic frame activation and contextual aptness of metaphorical expressions

    The study aims to explore the possibility of semantic frame activation and interaction in metaphorical expressions, and the conditions of contextual aptness of metaphorical expressions. To ensure the appropriate level of ecological validity, experimental stimuli have been selected from a corpus of newspaper articles, and included in the norming procedures. The theoretical framework includes the investigations into context and semantic frames, semantic priming, discourse processing, conceptual metaphor theory, and psycholinguistic approaches to metaphor. The first four experiments were designed to test the activation and interaction of the organizing frames of source and target inputs in metaphorical expressions from the conceptual keys of CONFLICT and MOTION. This included an online priming paradigm with a categorization task, and the main dependent variable of interest was response time (RT). Stimuli were presented in (1) congruent metaphorical (metaphorical sentences), (2) congruent literal (literal sentences), and (3) incongruent conditions (unrelated sentences). Targets were individual words from the relevant frames. The results showed higher degrees of activation of the organizing frames of target inputs for both metaphor groups. There were no significant differences between the two congruent conditions, while RTs in the incongruent condition were significantly longer. The data seem to offer support for the interaction view of metaphor processing. The final two experiments tested the level of contextual aptness of target metaphorical expressions from the same two conceptual keys, in (1) congruent metaphorical (metaphor clusters), (2) congruent literal (literal paragraphs), and (3) incongruent priming conditions (unrelated paragraphs). Targets were metaphorical sentences from the two conceptual keys. The obtained results did not reveal any differences between the two congruent conditions, while the recorded RTs were significantly shorter in the incongruent condition. Overall, the study provides empirical insight into the phenomena of (metaphorical) framing, frame activation and interaction, and contextualization, and their import in online meaning construction

    EMOTIONAL APPEAL OF CONCEPTUAL METAPHORS OF CONFLICT IN THE POLITICAL DISCOURSE OF DAILY NEWSPAPERS

    The paper explores the ability of conceptual metaphors of CONFLICT to provoke an emotional response in the context of the political discourse of daily newspapers. Furthermore, it is argued that such emotional involvement can facilitate the construction of political reality. The emotional appeal of the most salient CONFLICT metaphors from the corpus that consists of newspaper articles extracted from the on-line editions of The New York Times is tested via appropriate questionnaires. The theoretical framework of the first part of the paper is based on Conceptual Blending Theory, which is expected to provide a detailed account of the conceptual basis that underlies metaphor usage, and the way the emotional appeal of metaphors influences the online process of meaning construction. The questionnaires used to determine the emotional response to metaphors are based on the Conceptual Act Theory of Emotion, and they were designed to test the reaction in terms of affect and more specific emotion concepts.     The strategy of cost leadership, although always current, acquires special importance in the time of crisis. Cost reduction, savings program, savings measures, etc. are used as synonyms for this strategy; finally, all of these express the need to reduce costs for an enterprise. Enterprises make the cost reduction program which includes mandatory stages of reduction, reduction tools and the most appropriate strategies. The choice of tools depends on the chosen strategy, but the stages cannot be skipped or omitted within the strategies.     GMP (Good Manufacturing Practices) adalah cara berproduksi yang baik dan benar untuk menghasilkan produk yang memenuhi persyaratan mutu dan keamanaan. Good Manufacturing Practices juga menjadi persyaratan minimum sanitasi dan pengolahannya yang diperlukan untuk memastikan diproduksinya pangan yang aman dan sehat. GMP juga menjadi salah satu program persyaratan dasar dalam penerapan sistem HACCP, yang menjamin praktek pencegahan terhadap kontaminasi yang menyebabkan produk menjadi tidak aman. Penerapan GMP yang baik dapat memberikan mutu dan keamanan pangan yang baik. Untuk itu, dilakukan Praktek Kerja Lapang yang bertujuan untuk mempelajari sistem GMP yang diterapkan pada produk Octopus Cut Boiled. Praktek Kerja Lapang ini dilaksanakan di PT. Istana Cipta Sembada, Dusun Krajan, Kecamatan Rogojampi, Kabupaten Banyuwangi, Provinsi Jawa Timur pada tanggal 18 Desember 2017 sampai 18 Januari 2018.     Ex utero intrapartum surgery (EXIT): Combined spinal epidural anesthesia (CSE) and general anesthesia with remifentanil. Case Report

    Congenital high airway obstruction syndrome (CHAOS) is a life-threatening condition with a poorly understood natural history[1]. EXIT is a controlled technique that is designed to allow partial fetal delivery via cesarean section with subsequent establishment of a safe fetal airway, requires maintenance of uterine relaxation to continue placental perfusion and prevent placental separation[2]. A healthy 28 year old primigravida, 33 weeks of gestation, with a fetus known to have CHAOS due to tracheal atresia and ascites, scheduled for EXIT. Phase 1: ascites drainage by punction and fetus external version (breech presentation) before EXIT. CSE was chosen for this phase (5mg hyperbaric bupivacaine and 25ug fentanyl, intrathecally), with light conscious sedation (midazolan 5mg). Epidural catheter was placed. Phase 2: Fetus had a tracheal atresia. Laryngotracheoplasty was needed. Based on estimated fetal weight, an intramuscular injection of fentanyl 10ug/kg, vecuronium 0.2mg/kg and atropine 10ug/kg was given to the fetus[3]. After propofol induction, sevoflurane (2 MAC) and intravenous nitroglycerin, to ensure a fully relaxed uterus, and remifentanyl infusion (cardiovascular stability), was given to the mother. Fetopalcental support last for 20 min, fetus laryngotracheoplasty was done. Apgar was 3/4. CSE should be considered for EXIT. Nitroglycerin infusion along with deep volatile anesthesia provides full uterine relaxation preventing placental separation while preserving placental perfusion. Remifentanyl should be considered in high-risk obstetric surgery[4]

    Background: Spontaneous spinal epidural hematoma (SSEH) in a pregnant patient is an extremely rare clinical condition and requires emergency surgical evacuation, while termination of pregnancy depends on fetal viability status1 . Etiology includes hemorrhagic diathesis, autoimmune inflammatory vasculitis, anticoagulant therapy, vascular malformations, and tumors. Pregnancy is considered a risk factor for SSEH. Below is a patient with SSEH and acquired FXIII deficiency (FXIII D) as a sole finding. Case report: A 37 year old, gravida 3 para 2, 28 weeks of gestation was transferred to our hospital with symptoms of progressive paraplegia over 72 hours. MRI showed spinal lesion due to spontaneous epidural hematoma, emergency decompression followed with a T1-T5 laminectomy. A hemorrhagic episode required transfusions up to 4 RBCs and 2 FFPs during the laminectomy, neither FXIII nor Tranexamic acid (TXA) was given on that occasion. The neurological examination showed flaccid paraplegia, with sensitive level at T3. Test results showed normal values except in FXIII-A concentration 28% (60-160%). No other major bleeding cause could be determined aside from the FXIII D. Two months later, she had a c-section performed at 36 weeks of gestation. Intravenous plasma-derived FXIII (pdFXIII) concentrate 2500 IU was given preoperatively along with TXA 1g. General anesthesia using propofol for induction and sevorane for maintenance was administered, no muscle relaxants or opioids were necessary. Videolaryngoscopy was used for orotracheal intubation. Bleeding was between normal ranges. There was no need for RBC transfusions. A healthy female baby was born with a 8/9 Apgar score and 2,840 grams. Post-operative FXIII-A level was 69%. Discussion: There are merely a few dozens of pregnant SSEH cases reported in the literature. Regional anesthesia for paraplegic condition is advised as the best technique to prevent autonomic dysreflexia; although general anesthesia seems more appropriate to prevent further spinal complications in patients with hemorrhagic diathesis2 . Learning points: General anesthesia is an adequate anesthesia technique for paraplegic patients with FXIIID. FXIII should be tested even if coagulation test results are normal, and preoperative pdFXIII administration is probably the best way to prevent severe bleeding as well as TXA in these patients
