Reducing alcohol-exposed pregnancies: a report of the National Task Force on Fetal Alcohol Syndrome and Fetal Alcohol Effect
"This report is a collaborative effort of the National Task Force on Fetal Alcohol Syndrome and Fetal Alcohol Effect (NTFFASFAE), the Centers for Disease Control and Prevention's (CDC) National Center on Birth Defects and Developmental Disabilities (NCBDDD) FAS Prevention Team, National Center for Health Marketing Community Guide Branch, and Research Triangle Institute International (RTI). Evidence for this report began with a systematic search of the literature to identify community-level FASD interventions and policies that can prevent AEPs and reduce the prevalence of physical, mental, behavioral, and learning disabilities due to prenatal alcohol exposure. This evidence, along with the findings and recommendations of the U.S. Preventive Services Task Force on behavioral counseling interventions for alcohol misuse, helped lay the groundwork for the information presented in this report. The report reviews the current evidence on prevention strategies to reduce alcohol use and AEPs, provides recommendations on promoting and improving these strategies, and offers future research directions in the field of FASD prevention. The INTERGROWTH-21st project neurodevelopment package:a novel method for the multi-dimensional assessment of neurodevelopment in pre-school age children
BACKGROUND: The International Fetal and Newborn Growth Consortium for the 21st Century (INTERGROWTH-21st) Project is a population-based, longitudinal study describing early growth and development in an optimally healthy cohort of 4607 mothers and newborns. At 24 months, children are assessed for neurodevelopmental outcomes with the INTERGROWTH-21st Neurodevelopment Package. This paper describes neurodevelopment tools for preschoolers and the systematic approach leading to the development of the Package. METHODS: An advisory panel shortlisted project-specific criteria (such as multi-dimensional assessments and suitability for international populations) to be fulfilled by a neurodevelopment instrument. A literature review of well-established tools for preschoolers revealed 47 candidates, none of which fulfilled all the project's criteria. A multi-dimensional assessment was, therefore, compiled using a package-based approach by: (i) categorizing desired outcomes into domains, (ii) devising domain-specific criteria for tool selection, and (iii) selecting the most appropriate measure for each domain. RESULTS: The Package measures vision (Cardiff tests); cortical auditory processing (auditory evoked potentials to a novelty oddball paradigm); and cognition, language skills, behavior, motor skills and attention (the INTERGROWTH-21st Neurodevelopment Assessment) in 35-45 minutes. Sleep-wake patterns (actigraphy) are also assessed. Tablet-based applications with integrated quality checks and automated, wireless electroencephalography make the Package easy to administer in the field by non-specialist staff. The Package is in use in Brazil, India, Italy, Kenya and the United Kingdom. CONCLUSIONS: The INTERGROWTH-21st Neurodevelopment Package is a multi-dimensional instrument measuring early child development (ECD). Its developmental approach may be useful to those involved in large-scale ECD research and surveillance efforts

Nos últimos anos o agravamento da crise económica convergiu profundas alterações o setor farmacêutico, assistindo-se por isso a uma transferência de inventário das farmácias para os distribuidores, passando estes também a desempenhar a função de armazenistas. Estas preocupações manifestaram-se noutros setores de atividade em primeiro lugar, tendo algumas soluções sido inspiradas para a resolução de problemas no setor farmacêutico.
Este trabalho de dissertação visa desenvolver ferramentas com conhecimento técnico na avaliação de melhores soluções de um caso real. Estas ferramentas centram-se em primeiro lugar no processo de previsão na procura e, posteriormente, de forma complementar, um estudo de heurÃsticas matemáticas no contexto retalhista farmacêutico.
Tendo por base alguns trabalhos desenvolvidos neste âmbito pretende-se desenvolver ferramentas e retirar conclusões orientadoras na gestão e planeamento estratégico da indústria farmacêutica.
O objetivo global do trabalho passa por comparar diferentes modelos matemáticos com os dados reais e retirar algumas conclusões sobre a qualidade da previsão de procura, e comportamento das heurÃsticas de cálculo de encomendas. Em duas partes distintas, validar-se-á métodos de matemáticos que possam ser usados na previsão, e com os mesmos dados da procura cálculos de quantidades de encomenda reais para os mesmos perÃodos. Em ambos os casos serão sempre sempre feitas comparações com a realidade e retiradas conclusões.
As principais elações retiradas de um trabalho no âmbito da gestão de inventários ilustram uma área pouco criteriosa e rudimentar em termos práticos, mesmo em setores de atividade avançados e menos prováveis como é o farmacêutico. No caso de estudo deste trabalho em concreto, a gestão destes bens é muito condicionada pela interpretação do operador e intuição de vendas do gestor. Nestas condições existe, ainda antes de implementar novas filosofias de trabalho e gestão, algum trabalho de sensibilização e preparação do trabalho. Este trabalho preliminar permite uma menor resistência ao processo: organização de dados concretos e necessários aos modelos; sensibilização da necessidade de otimização nos trabalhadores. Nas questões técnicas das previsões existe inúmeros fatores na escolha das mesmas: universo temporal, dados disponÃveis e caracterÃsticas dos produtos entre outros, por exemplo. Contudo o tempo disponÃvel e disponibilidade financeira para realizar as mesmas não devem ser menosprezados. As previsões diárias proporcionam um menor erro quando comparadas com as previsões periódicas. Contudo, a complexidade e morosidade das primeiras é bastante mais acentuada, podendo não compensar o seu menor erro (previsão-procura).
Na avaliação em Lot-sizing as caracterÃsticas dos medicamentos (procura e preço) condicionam os inputs. Estas variações de inputs podem tornar as respostas das heurÃsticas um pouco imprevisÃveis, tendo por referência a referência bibliográfica. De forma concreta foi possÃvel avaliar neste trabalho prático a heurÃstica Wagner-Within com os melhores resultados, mas obtendo alguns resultados não tão favoráveis. Estes fatores já podem ser encontrados na literatura anteriormente: procura pouco intensa e stock inicial diferente de zero. Para além disso foi possÃvel verificar que um custo demasiado baixo (custo de posse, neste caso) pode criar alguns problemas de sensibilidade nos ciclos/perÃodos de cálculo.
As restantes heurÃsticas oscilaram com os inputs introduzidos, sendo as heurÃsticas CMT e QEP com comportamentos bastante bons, comparativamente

Este facto justifica-se porque a investigação revela ser árduo, tendo a OMS alertado para poderem vir a ser um grupo de risco, se não se intervier.
O propósito deste estudo foi identificar as necessidades dos cuidadores familiares idosos, com enfoque para a necessidade de descanso e lazer. É um estudo exploratório, transversal e descritivo efetuado a 30 cuidadores familiares com 65 ou mais anos, a cuidarem de pessoas idosas há mais de 3 meses, e inscritos no Agrupamento de Centros de Saúde de Oeiras e alvo de Cuidados Continuados Integrados das Unidades de Cuidados na Comunidade.
Os resultados em termos sociodemográficos foram similares aos de outros estudos. Entre as principais conclusões, a avaliação multidimensional revelou estes serem independentes nas atividades de vida diárias, sendo alguns dependentes na alimentação. Apresentavam queixas emocionais e o estado cognitivo era de nÃvel bom. Os cuidadores familiares têm perceção da sobrecarga a que estão submetidos, tendo-se verificado associação significativa entre o tempo de cuidados diários e a opinião do cuidador familiar, relativamente ao seu desempenho. As necessidades mais afectadas foram a comunicação com os outros e o apoio económico. Todos recebiam visita domiciliária de enfermagem, médica e assistente social.
As atividades de descanso e lazer utilizadas eram variadas e diminuÃram substancialmente na assunção do papel de cuidador sendo especialmente afetadas as realizadas fora de casa. Os cuidadores familiares desejam ter vida própria e para tal, necessitam separar-se de forma definitiva ou temporária do familiar, desde que o cuidado seja assegurado sob determinadas condições. O cuidador substituto pode assumir somente o papel de companhia, ou a totalidade do papel; em qualquer dos casos o cuidador familiar só consegue descansar se confiar totalmente no substituto
Advancing essential services and research on fetal alcohol spectrum disorders
"In 2004, the National Task Force on Fetal Alcohol Syndrome and Fetal Alcohol Effect, coordinated by the Centers for Disease Control and Prevention's National Center on Birth Defects and Developmental Disabilities, established a working group committed to addressing the needs of individuals living with fetal alcohol spectrum disorders (FASDs) and their families. The culmination of this working group's discussions and Task Force deliberations is reflected in this Call to Action report. The document highlights ten recommendations to improve and expand efforts regarding early identification, diagnostic services, and quality research on interventions for individuals with FASDs and their families. Additional background information is provided to support these recommendations and to further educate readers on the topic of FASDs, progress to date, and what still needs to be done to support individuals with FASDs. Statistical methodology for constructing gestational age-related charts using cross-sectional and longitudinal data: The INTERGROWTH-21st project as a case study.
Most studies aiming to construct reference or standard charts use a cross-sectional design, collecting one measurement per participant. Reference or standard charts can also be constructed using a longitudinal design, collecting multiple measurements per participant. The choice of appropriate statistical methodology is important as inaccurate centiles resulting from inferior methods can lead to incorrect judgements about fetal or newborn size, resulting in suboptimal clinical care. Reference or standard centiles should ideally provide the best fit to the data, change smoothly with age (eg, gestational age), use as simple a statistical model as possible without compromising model fit, and allow the computation of Z-scores from centiles to simplify assessment of individuals and enable comparison with different populations. Significance testing and goodness-of-fit statistics are usually used to discriminate between models. However, these methods tend not to be useful when examining large data sets as very small differences are statistically significant even if the models are indistinguishable on actual centile plots. Choosing the best model from amongst many is therefore not trivial. Model choice should not be based on statistical considerations (or tests) alone as sometimes the best model may not necessarily offer the best fit to the raw data across gestational age. In this paper, we describe the most commonly applied methodologies available for the construction of age-specific reference or standard centiles for cross-sectional and longitudinal data: Fractional polynomial regression, LMS, LMST, LMSP, and multilevel regression methods. For illustration, we used data from the INTERGROWTH-21st Project, ie, newborn weight (cross-sectional) and fetal head circumference (longitudinal) data as examples
Quality control of GEM detectors using scintillation techniques
Non destructive quality control of microstructures at the manufacturing stage
is an important issue in the foreseen use of huge numbers of such gaseous
detectors in the future high luminosity colliders. In this work we report on
the use of the scintillation light emitted by the avalanches in GEM channels
for checking defects in the foils. The test system is described and data on the
relative efficiency of several gaseous mixtures are presented. The foil images
obtained with a low-noise CCD system are analysed and compared with the optical
images obtained with an industrial inspection system of high magnification. The
validity of this test method is established and possible extensions of its use

Design and other methodological considerations for the construction of human fetal and neonatal size and growth charts.
This paper discusses the features of study design and methodological considerations for constructing reference centile charts for attained size, growth, and velocity charts with a focus on human growth charts used during pregnancy. Recent systematic reviews of pregnancy dating, fetal size, and newborn size charts showed that many studies aimed at constructing charts are still conducted poorly. Important design features such as inclusion and exclusion criteria, ultrasound quality control measures, sample size determination, anthropometric evaluation, gestational age estimation, assessment of outliers, and chart presentation are seldom well addressed, considered, or reported. Many of these charts are in clinical use today and directly affect the identification of at-risk newborns that require treatment and nutritional strategies. This paper therefore reiterates some of the concepts previously identified as important for growth studies, focusing on considerations and concepts related to study design, sample size, and methodological considerations with an aim of obtaining valid reference or standard centile charts. We discuss some of the key issues and provide more details and practical examples based on our experiences from the INTERGROWTH-21st Project. We discuss the statistical methodology and analyses for cross-sectional studies and longitudinal studies in a separate article in this issue
Diversificação e segurança energética europeia : gás natural e o corredor meridional
A União Europeia enquanto uma das maiores economias globais é
dependente do gás natural fornecido por terceiros. Considerando que os mercados
deste recurso são regionais, derivado da maioria do seu transporte ser efectuado via
gasoduto, o contexto geopolÃtico europeu depara-se com um conjunto de riscos,
alguns de natureza polÃtica, que condicionam o estado de segurança energética
relativa ao seu abastecimento de gás natural. Por conseguinte, a União Europeia
implementou diversas estratégias que visam a criação de um mercado e espaço único
relativo ao consumo de gás natural, tal como, tem procurado dinamizar as suas
relações com outros exportadores, paÃses-trânsito e consumidores. A respeito dessa
dinamização de relações, a Comissão Europeia priorizou a criação de uma nova rota
de abastecimento, o Corredor Meridional, composto por um sistema de gasodutos
com o propósito de interligar potenciais produtores alternativos com os seus
mercados consumidores. O objectivo deste projecto serve o aumento da sua
diversificação, como uma das soluções fundamentais para o reforço da sua segurança
energética. Todavia, as regiões alternativas incluÃdas nessa estratégia apresentam um
conjunto de desafios com diversas origens que poderão condicionar o
desenvolvimento do Corredor. Analisar e compreender todos estes contextos e em
última instância, confirmar a importância da contribuição do Corredor Meridional
para o aumento da segurança energética europeia, constituem os objectivos deste
trabalho.As one of the major global economies, the European Union is dependent on
external supplies of natural gas. Considering that its markets have a regional
dimension, as main form of transportation of natural gas is effected through
pipelines, the European geopolitical context faces a set of risks, some of which are
political in nature, that undermine its energy security status regarding natural gas
supplies. Hence, the European Union has implemented several strategies that aim to
create a common market and space correlated to natural gas consumption; as it has
been trying to boost its relations with other exporter, transit and consumer countries.
Concerning such improvement in relations, the European Commission prioritized the
creation of a new supply route, the Southern Corridor, composed by a pipeline
system with the purpose of connecting potential alternative suppliers with its
consumer markets. The main goal of this project serves the increase of
diversification, as one of the fundamental solutions to reinforce European energy
security. However, the alternative regions included in this strategy pose a number of
challenges of diverse naturewhich may compromise the Corridor’s development. To
analyse and understand all these contexts and, ultimately, to confirm the importance
of the Southern Corridor’s contribution to the increase of the European energy
security, are the objectives of this work
