    Tournaisian and Viséan Lophophyllum of Gorskiy (1932) from the Kirghiz Steppe and a possible ancestor of a new Bashkirian rugose coral genus from the Donets Basin (Ukraine)

    All specimens assigned by Gorskiy (1932) to the genus Lophophyllum Milne Edwards and Haime, 1850 are revised, redescribed and reillustrated. The corallite identified by him as a second, specifically indeterminate species of Lophophyllum is here questionably included in Amygdalophyllum Dun and Benson, 1920. For the reminding specimens two new, unnamed genera are suggested. ”Lophophyllum” subtortuosum Gorskiy, 1932 belongs to a new, non-dissepimented genus of an unknown family. A possible relationship between gen. nov. 1, sp. nov. 1 and the new Bashkirian genus from the Donets Basin (Ukraine) is proposed

    Late Carboniferous colonial corals from the Moscovian Saginaw Bay Formation and the underlying Bashkirian crinoidal limestone exposed on northeastern Kuiu Island and a nearby islet, part of the Alexander terrane in southeastern Alaska, are described and illustrated for the first time, and are supplemented by revision, redescription and reillustration of most Atokan specimens from Brooks Range, northern Alaska, first described by Armstrong (1972). New taxa from the Kuiu Island area include the new species Paraheritschioides katvalae and the new genus and species Arctistrotion variabilis, as well as the new Subfamily Arctistrotioninae. The corals Corwenia jagoensis and Lithostrotionella wahooensis of Armstrong (1972) also are redefined and redescribed. Paraheritschioides jagoensis is based on the holotype of ‘C’. jagoensis. P. compositus sp. nov. is based on a “paratype” of ‘C.’ jagoensis. In addition to a redefinition and redescription of ‘L.’ wahooensis as Arctistrotion wahooense, one “paratype” of that species is described as A. simplex sp. nov. The phylogeny and suspected relationships of some fasciculate Carboniferous Rugosa also are discussed. Based on relationships and similarities within the Late Carboniferous colonial Rugosa from the Brooks Range, Kuiu Island and the eastern Klamath terrane, we conclude that all three areas were geographically close enough at that time so that larvae were occasionally dispersed by oceanic currents. Distances between these areas, however, may have been so great and/or isolation so long that individual speciation occurred in all of the various locations.     New Late Carboniferous Heritschioidinae (Rugosa) from the Kuiu Island area and Brooks Range, Alaska

    Three new species of the genus Heritschioides, i.e., H. alaskensis sp. nov., H. kuiuensis sp. nov., and H. splendidus sp. nov., and Kekuphyllum sandoense gen. et sp. nov. from the northeastern Kuiu Island area and nearby islets, part of Alexander terrane in southeastern Alaska, and Heritschioides separatus sp. nov. from the Brooks Range, Alaska, are described and illustrated. The three new fasciculate colonial coral species from the Kuiu Island area, collected from the Moscovian Saginaw Bay Formation, are phylogenetically related to those of probable Bashkirian age in the Brooks Range in northern Alaska as shown by the presence of morphologically similar species of Heritschioides. These corals from both areas also are related to one species in the Quesnel terrane in western Canada. Kekuphyllum sandoense from the Saginaw Bay Formation of the Kuiu Island area is the only cerioid-aphroid species within the Subfamily Heritschioidinae described so far. The complete early ontogeny of a protocorallite is for the first time described here on a basis of H. kuiuensis sp. nov. and compared to the hystero-ontogeny in order to show similarities and differences in those processes

Przegląd dorobku literatury prowadzi autora do wniosku, że potrzebne są bardziej zaawansowane badania tych zagadnień w polskiej gospodarce, ze względu na znaczenie, jakie metody finansowania zaradnego mogą mieć dla powodzenia rozwoju firm w początkowej fazie ich istnienia

    Classical and delayed orthostatic hypotension in patients with unexplained syncope and severe orthostatic intolerance

    Background: Orthostatic hypotension (OH) is a major sign of cardiovascular autonomic failure leading to orthostatic intolerance and syncope. Orthostatic hypotension is traditionally divided into classical OH (cOH) and delayed OH (dOH), but the differences between the two variants are not well-studied. We performed a systematic clinical and neuroendocrine characterization of OH patients in a tertiary syncope unit. Methods: Among 2,167 consecutive patients (1,316 women, 60.7%; age, 52.6 ± 21.0 years) evaluated for unexplained syncope and severe orthostatic intolerance with standardized cardiovascular autonomic tests including head-up tilt (HUT), we identified those with a definitive diagnosis of cOH and dOH. We analyzed patients' history, clinical characteristics, hemodynamic variables, and plasma levels of epinephrine, norepinephrine, C-terminal-pro-arginine-vasopressin (CT-proAVP), C-terminal-endothelin-1, mid-regional-fragment of pro-atrial-natriuretic-peptide and pro-adrenomedullin in the supine position and at 3-min HUT. Results: We identified 248 cOH and 336 dOH patients (27% of the entire cohort); 111 cOH and 152 dOH had blood samples collected in the supine position and at 3-min HUT. Compared with dOH, cOH patients were older (68 vs. 60 years, p < 0.001), more often male (56.9 vs. 39.6%, p < 0.001), had higher systolic blood pressure (141 vs. 137 mmHg, p = 0.05), had lower estimated glomerular filtration rate (73 vs. 80 ml/min/1.73 m2, p = 0.003), more often pathologic Valsalva maneuver (86 vs. 49 patients, p < 0.001), pacemaker-treated arrhythmia (5 vs. 2%, p = 0.04), Parkinson's disease (5 vs. 1%, p = 0.008) and reported less palpitations before syncope (16 vs. 29%, p = 0.001). Supine and standing levels of CT-proAVP were higher in cOH (p = 0.022 and p < 0.001, respectively), whereas standing norepinephrine was higher in dOH (p = 0.001). After 3-min HUT, increases in epinephrine (p < 0.001) and CT-proAVP (p = 0.001) were greater in cOH, whereas norepinephrine increased more in dOH (p = 0.045). Conclusions: One-quarter of patients with unexplained syncope and severe orthostatic intolerance present orthostatic hypotension. Classical OH patients are older, more often have supine hypertension, pathologic Valsalva maneuver, Parkinson's disease, pacemaker-treated arrhythmia, and lower glomerular filtration rate. Classical OH is associated with increased vasopressin and epinephrine during HUT, but blunted increase in norepinephrine

    Association of incident fragility fractures in patients hospitalised due to unexplained syncope and orthostatic hypotension

    Background Fragility fractures are caused by low-energy insults such as falls from standing height or less and pose a growing health challenge as their incidence rises with increasing age. Impaired orthostatic blood pressure response and a number of cardiovascular biomarkers have been previously identified as risk factors for fractures. It is likely that severe episodes of syncope and orthostatic hypotension increase the risk of subsequent fragility fractures, however this relationship has not been thoroughly examined.PurposeTo investigate the relationship of hospital admissions due to unexplained syncope and OH with incident fragility fractures in a middle-aged population.MethodsWe analysed a large population-based prospective cohort of 30,446 middle-aged individuals (age, 57.5 ± 7.6; men, 39.8%). We included patients hospitalised due to unexplained syncope and OH. Cox regression analysis adjusted for age, sex, prevalent fractures, body mass index (BMI) were applied to assess the impact of unexplained syncope/OH hospitalisations on subsequent incident fragility fractures. Prevalent fractures occurring before syncope/OH hospitalisation were excluded (n = 39) as well as cases with no follow-up time after the event of syncope/OH (n= 8).ResultsThe mean follow-up from baseline to first incident fracture or end of follow-up was 17.8 + 6.5 years, and 8201 (27%) suffered incident fracture. The mean age of patients with unexplained syncope (n = 493) and OH patients (n = 406) at baseline was 61.5 ± 7.1 years (50.1%, male) and 62.6 ± 6.6 years (49.8% male), respectively. The mean time between baseline and first admission for syncope and OH was 12.3 ± 4.5 years, and the mean age at first hospitalisation was 74.4 ± 7.6 years. In the multivariable-adjusted Cox regression, the risk of subsequent incident fractures was increased among patients hospitalised due to unexplained syncope (HR: 1.20; 95% CI 1.03–1.40; p < 0.02) and OH (HR: 1.40; 95% CI 1.20–1.64; p < 0.001), respectively (Kaplan-Meier curves; Figure 1).ConclusionsPatients hospitalised due to unexplained syncope and OH demonstrate increased risk of subsequent fragility fractures. We suggest that patients who are hospitalised for unexplained syncope and OH should be clinically assessed for true syncope aetiology, systematically treated against fall risk, and evaluated for additional risk factors for fragility fractures

    History of syncope predicts loss of consciousness after head trauma: Retrospective study

    Background: Head trauma may present as transient loss of consciousness (TLOC) currently classified as traumatic in origin, in contrast to non-traumatic forms, such as syncope. Whether past history of syncope predisposes to loss of consciousness after head injury has been poorly studied. Methods: A retrospective analysis of data obtained from 818 consecutive patients admitted to Emergency Departments was conducted. Face-to-face semi-structured interviews were performed, where patients’ past history of syncope and head injury were explored. Head injury events were stratified as high- or low-energy trauma. Data regarding past syncopal events were explored in regard to number, age at the first occurrence, and syncope circumstances. Multivariate logistic regression model was applied to assess the relationship between loss of consciousness during head injury and past history of syncope. Results: Both past history of non-traumatic TLOC (odds ratio [OR] 3.78; 95% confidence interval [CI] 2.13–6.68, p &lt; 0.001) and high-energy mechanism (OR 3.84; 95% CI 2.35–6.28, p &lt; 0.001) predicted TLOC after head trauma. This relationship was even stronger when past episodes of TLOC were limited to those typical for reflex syncope (OR 4.34; 95% CI 2.34–7.89, p &lt; 0.001). Further, the number of non-traumatic TLOC episodes in the patient’s history was also predictive of TLOC after head injury (OR per 1 episode: 1.24; 95% CI 1.04–1.48, p = 0.015). Conclusions: Syncope in a patient’s history predicts loss of consciousness after head injury. The clinical importance of this finding merits further investigation
