    Intravitreous injection of bevacizumab and C3F8 gas for the treatment of submacular hemorrhage due to age-related macular degeneration: case reports

    O objetivo desta série de casos foi demonstrar se a aplicação de bevacizumab e gás perfluoropropano (C3F8) intravítreos beneficiariam o deslocamento da hemorragia sub-retiniana dos pacientes com degeneração macular relacionada à idade. Foi realizada uma série retrospectiva de 5 olhos que tinham recebido injeção intravítrea simultânea de bevacizumab e C3F8. Os resultados foram medidos pelo grau de deslocamento de sangue sob a fóvea, pela acuidade visual final e pelas complicações intraoperatórias. Na apresentação inicial, a idade média dos pacientes foi de 72,6 ± 8,9 anos e a duração média dos sintomas foi de 13 ± 9,7 dias. Dos 5 pacientes do estudo, 3 (60%) eram homens e 2 (40%) mulheres. O sucesso do deslocamento da hemorragia submacular foi alcançado em 4 pacientes. A média de acuidade visual pré-operatória foi de 1,12 ± 0,34 logMAR e pós-operatório foi de 0,92 ± 0,4 logMAR. Não foram observados nenhum caso de descolamento da retina, endoftalmite, hemorragia vítrea, uveíte, catarata e hipertensão ocular. A injeção intravítrea bevacizumab e C3F8, juntamente com a posição pronada pode ser uma valiosa opção terapêutica nos olhos com degeneração macular relacionada à idade neovascular e hemorragia sub-retiniana a fim de deslocar o sangue para fora da área foveal.The purpose of this case series is to describe if the intravitreal use of bevacizumab and perfluoropropane gas (C3F8) would be beneficial to the displacement of subretinal hemorrhage in patients with age-related macular degeneration (AMD). A retrospective study of 5 eyes that received concurrent intravitreal injection of bevacizumab and C3F8 was performed. The results were graded according to blood displacement under the fovea, best final visual acuity and intraoperative complications. At the initial presentation, mean age of patients was 72.6 ± 8.9 years-old and duration of symptoms was 13 ± 9.7 days. From the 5 patients, 3 (60%) were male and 2 (40%) female. The success of submacular hemorrhage full displacement was achieved in 4 patients. The mean preoperative visual acuity (VA) was 1.12 ± 0.34 logMAR and the mean postoperative VA was 0.92 ± 0.4 logMAR. No cases of retinal detachment, endophthalmitis, vitreous hemorrhage, uveitis, cataracts and increased intraocular pressure were noted during the follow-up period. Intravitreal bevacizumab and C3F8 injection, associated to prone position can be a valuable therapeutic option for eyes with neovascular age-related macular degeneration and subretinal hemorrhage to the blood displacement out of the foveal area

    Extensão desenvolvida na oficina de prototipação com arduino, uma introdução geral e simplificada sobre este hardware livre e seus periféricos, além de conceitos de eletrônica básica e atividades práticas. O Arduino é uma plataforma de prototipagem eletrônica de hardware livre, desenvolvida para ser uma simples ferramenta didática de baixo custo, flexível e fácil de ser usado por principiantes e profissionais. Tópicos abordados funcionamento da placa de prototipagem modelo arduino Mega2560, conceitos sobre corrtente elétrica, tensão e resistencia, protoboard, entradas e saidas analógicas, digitais e portas seriais da placa. Software, utilizado a IDE nativa do ambiente de programação do arduino.     Optical coherence tomography imaging in commotio retinae: case report

    O objetivo deste relato é correlacionar achados à tomografia de coerência óptica e prognóstico visual de pacientes com commotio retina e de gravidades diferentes. Dois pacientes do sexo masculino, de 20 e 23 anos com baixa visual unilateral após trauma ocular contuso atribuível a edema de Berlin foram avaliados pela retinografia e tomografia de coerência óptica. A acuidade visual no olho afetado era de 20/25 no primeiro paciente e conta dedos a 2 metros no segundo. O exame oftalmológico revelou uveíte traumática e, na fundoscopia, evidenciaram-se opacificação retiniana moderada no primeiro caso e grave no segundo. A tomografia de coerência óptica confirmou discreta diminuição da depressão foveal no primeiro caso e desorganização das camadas retinianas no segundo. Houve resolução anatômica e funcional completa a tomografia de coerência óptica no primeiro paciente, enquanto o segundo evoluiu com baixa visual permanente e desorganização da arquitetura retiniana. A tomografia de coerência óptica é um exame complementar útil na avaliação do trauma retiniano, ajudando a entender sua fisiopatologia e predizer prognóstico a partir da análise anatômica da região acometida.The purpose of this case report was to correlate optical coherence tomography findings and visual outcomes of patients with different degrees of commotio retinae. A 20-year-old male and a 23-year-old male that presented with decreased vision due to Berlin's edema after blunt ocular trauma were evaluated by optical coherence tomography and retinography. The visual acuity in the affected eye was 20/25 in the first patient and counting fingers in the second one. The ophthalmic examination showed traumatic uveitis and fundoscopy revealed mild retinal opacification in the first case and severe opacification in the latter. The optical coherence tomography confirmed the reduction of foveal depression on the first case and the disarrangement of all retinal layers on the second. There has been complete functional and anatomical resolution by optical coherence tomography in the first patient, while the second evolved to permanent visual loss. Optical coherence tomography is a useful method in the evaluation of retinal trauma, helping to understand its physiopathology and to predict its prognosis through the anatomical analysis of the affected region

    Proposal of a new slit-lamp shield for ophthalmic examination and assessment of its effectiveness using computational simulations

    PURPOSE: This study aimed to use computational models for simulating the movement of respiratory droplets when assessing the efficacy of standard slit-lamp shield versus a new shield designed for increased clinician comfort as well as adequate protection. METHODS: Simulations were performed using the commercial software Star-CCM+. Respiratory droplets were assumed to be 100% water in volume fraction with particle diameter distribution represented by a geometric mean of 74.4 (±1.5 standard deviation) μm over a 4-min duration. The total mass of respiratory droplets expelled from patients' mouths and droplet accumulation on the manikin were measured under the following three conditions: with no slit-lamp shield, using the standard slit-lamp shield, and using our new proposed shield. RESULTS: The total accumulated water droplet mass (kilogram) and percentage of expelled mass accumulated on the shield under the three aforementioned conditions were as follows: 5.84e-10 kg (28% of the total weight of particle emitted that settled on the manikin), 9.14e-13 kg (0.045%), and 3.19e-13 (0.015%), respectively. The standard shield could shield off 99.83% of the particles that would otherwise be deposited on the manikin, which is comparable to 99.95% for the proposed design. Conclusion: Slit-lamp shields are effective infection control tools against respiratory droplets. The proposed shield showed comparable effectiveness compared with conventional slit-lamp shields, but with potentially enhanced ergonomics for ophthalmologists during slit-lamp examinations

    MOIRÉ OPTICAL TECHNIQUE FOR EVALUATION OF CASHEW NUTS (Anacardium Occidentale, L.) ISOSTRAIN

    A figure corresponding to 96% of the Brazilian production of cashew nuts is forwared to industrial processing yielding only 50 to 60% of whole kernels. An appropriated knowledge of nut mechanical behavior would be important to support the development of equipment designed to minimize losses during processing.  The nut shell is composed of three layers identified as the epicarp, mesocarp and endocarp, which exhibit distinct behavior beyond mechanical excitations. An important class of mechanical loading is recognized to be the contact stress which is generated from the pressure exerted among two elastic bodies in contact. However the structural as well as the geometrical complexity of the nut deserves a suitable methodology to investigate stress distribution in cashew nuts. Shadow moiré technique was identified to support a suitable stress analysis method for the present case. Ten individuals were submitted to the trials, being 05 scarified nuts and 05 non scarified. A conventional optical setup was prepared to support a shadow moiré test. It was observed close correlation between external and internal load distribution.     The impact of the COVID-19 pandemic on dental trauma attendance: a systematic review and meta-analysis

    This systematic review and meta-analysis aimed to analyze the impact of the COVID-19 pandemic on dental trauma patient attendance. The study was registered in the PROSPERO system, using the CRD42021288398 protocol. Searching was performed in PubMed, Scopus, Web of Science, Embase, Lilacs, and OpenGrey databases, using the following keywords: “Tooth injuries,” “Dental trauma,” “Traumatic Dental injury,” and “COVID-19". We included observational studies evaluating dental trauma in the context of the COVID-19 pandemic. Quality assessment was performed using the Joanna Briggs Institute Critical Appraisal Checklist for Cross-Sectional Studies. Meta-analysis was performed in RevMan 5.4 software with Odds Ratios as a pooled measure of effect, with a 95% confidence interval, and using random-effects modeling. After applying the eligibility criteria, 32 studies were included for qualitative analysis, in which 10 were used to assess the frequency of dental trauma diagnoses in dental emergencies. Despite the decrease of visits during COVID-19, the analysis revealed no difference between the pandemic and pre-pandemic periods. Regarding the type of dental trauma, two of the studies revealed no differences for the periods before and during the pandemic. This study revealed that the COVID-19 pandemic has not impacted the frequency or type of dental trauma compared to previous periods

    A UTILIZAÇÃO DO LAUDO PERICIAL ELABORADO PELO PERITO CONTADOR: UM ESTUDO DESCRITIVO NAS VARAS CÍVEIS ESTADUAIS DA CIDADE DE MACEIÓ – ALAGOAS

    Este trabalho é resultado de um estudo teórico-empírico na área de Perícia Contábil, que teve como objetivo analisar a utilização dos laudos periciais contábeis em função de possuírem linguagem simples, clara, concisa e objetiva, na visão dos magistrados das varas cíveis estaduais da cidade de Maceió – Alagoas. Sua execução teve como ponto de partida a revisão da literatura e documentos legais que fundamentaram um estudo empírico em treze varas cíveis da esfera estadual, localizadas na Cidade de Maceió/AL, com a preocupação de orientar um trabalho pericial de qualidade desde a nomeação do perito até a apresentação do Laudo Pericial, além de destacar o perfil profissional exigido para aqueles que atuam nessa área. Para o propósito do presente estudo, visando uma maior confiabilidade, a amostra foi formada por Juízes que fossem titulares de Varas Cíveis. A entrevista foi feita por meio de questionário estruturado, baseando-se na literatura que norteia a pesquisa em tela, além de observações dos próprios pesquisadores. O tratamento dos dados deu-se através da estatística descritiva. O estudo se limitou a uma única esfera judiciária, a Justiça Estadual de Alagoas – Comarca de Maceió, sendo assim, um estudo de um único caso. Dessa forma, todas as conclusões encontradas são exclusivas da entidade estudada, não podendo ser generalizadas para outras instâncias. A pesquisa mostra que os juízes têm uma percepção satisfatória dos laudos periciais contábeis elaborados pelos perito-contadores por eles nomeados. Entretanto foram identificados pontos controversos que têm influenciado ou no tempo para deslinde do litígio, ou na decisão judicial

    Idiopathic polypoidal choroidal vasculopathy masquerading as choroidal tumors: one year follow-up of a peripheral lesion

    ABSTRACT This case report describes peripheral idiopathic polypoidal choroidal vasculopathy (IPCV) with a collection of small aneurysmal dilations that masqueraded as choroidal tumors in an elderly patient. A 68-year-old African American woman was referred to us with a suspected diagnosis of asymptomatic vascular choroidal tumor and choroidal capillary hemangioma, affecting the temporal peripheral fundus. Upon examination, optical coherence tomography (OCT) revealed two large hemorrhagic pigment epithelium detachments (PED), and indocyanine green angiography (ICG) confirmed the diagnosis of IPCV. One year later, there was reduction in the hemorrhagic pigment epithelium detachments and the lesion took on a different appearance, resembling a choroidal osteoma. No treatment was necessary despite the presence of multiple polyps. IPCV is a rare condition that can resemble other choroidal diseases depending on the stage of presentation. OCT is the best tool to determine the characteristics of the lesions, and indocyanine green angiography should be used to confirm the diagnosis. Not all cases require treatment
