    Well-being of physicians: protective factors from work stress in a sample of Italian oncologists

    Background: Recently a great deal of attention has been focused on doctors' work related stress and the possibility of improving their quality of life. Several studies report that healthcare professionals, especially oncologists, are overloaded physically, emotionally and psychologically. They develop a confidential relationship with their patients through close contact and interpersonal communication. Previous studies in the field of healthcare have shown that communication skills training improves the sense of satisfaction and well-being of physicians and patients. Aims: Our research investigates the relationship between Work Stress, Work Engagement and Personal Well-being in a sample of doctors working in Italian hospitals. Specifically, the study investigates some protective factors involved in this relationship. Materials and Methods: The research included 176 physicians working in Italian healthcare units. Doctors filled out self-report questionnaires to evaluate Work Stress and Coping Strategies (Health Professions Stress and Coping Scale), Personal Well-being (General Health Questionnaire), Work Engagement (Utrecht Work Engagement Scale) and two purpose-built scales to measure the degree of perceived Organizational Support and the level of specific training on social and relational skills. Result: The data seem to confirm our hypothesis. Positive and significant correlations were found between variables observed. Moreover, physicians who obtained higher levels of specific training on social and relational skills reported lower levels of stress. Finally, we examined the differences between oncologists and other specialists: oncologists experienced greater stress (particularly in problematic relationships with patients) than other physicians in terms of maladaptive coping (such as emotional distress), and they lack additional training. Conclusions: The results presented seem to confirm that the well-being of physicians is mediated by typical aspects of the profession, such as social skills in relationships with patients. The possible implication of research will also be discussed: the training needs of physicians and planned learning opportunities

    Plant Extracts for Sleep Disturbances: A Systematic Review

    Background. Sleep complaints are common health issues in the general population. These conditions are associated with poorer physical and psychological activity, and they may have important social, economic, and personal consequences. In the last years, several food supplements with different plant extracts have been developed and are currently taken for improving sleep. Study Objectives. The aim of this study is to systematically review recent literature on oral plant extracts acting on sleep disorders distinguishing their action on the different symptoms of sleep complaints: difficulty in initiating or maintaining sleep, waking up too early, and quality of sleep. Methods. We searched the PubMed database up to 05/03/2020 based on data from randomized, double-blind, placebo-controlled trials, noncontrolled trials, and cohort studies conducted in children and adult subjects. The search words used contained the following terms: oral food supplement and sleep disorders and the like. The most studied compounds were further analyzed with a second search using the following terms: name of the compound and sleep disorders. We selected 7 emerging compounds and 38 relevant reports. Results. Although nutraceutical natural products have been used for sleep empirically, there is a scarcity of evidence on the efficacy of each product in clinical studies. Valerian and lavender were the most frequently studied plant extracts, and their use has been associated (with conflicting results) with anxiolytic effects and improvements in quality and duration of sleep. Conclusions. Sleep aids based on plant extracts are generally safe and well tolerated by the population. More high-quality research is needed to confirm the effectiveness of food supplements containing plant extracts in sleep complaints; in particular, it would be interesting to evaluate the association between plant extracts and sleep hygiene guidelines and to identify the optimal products to be used in a specific symptom of sleep complaint, giving more appropriate tools to the medical doctor

    Social media are powerful means of communication that can also have an important role in the healthcare sector. They are sometimes seen with diffidence in the healthcare setting, partly because they risk blurring professional boundaries. This issue is particularly relevant to relations between caregivers and adolescent patients. The Italian Pediatric Hematology and Oncology Association created a multidisciplinary working group to develop some shared recommendations on this issue. After reviewing the literature, the working group prepared a consensus statement in an effort to suggest an analytical approach rather than restrictive rules

    A low-cost portable chamber based on Arduino micro-controller for measuring cover crops water use

    Cover crop adoption is growing in sustainable vineyard management to replace tillage and limit its ecosystem disservices. However, water use of such crops must be known and low-cost, yet fast reading, portable and accurate equipment is therefore needed for multipoint measurements in the field. Using an Internet of Things (IoT) approach, in this study we provide details for setup, calibration and operational data of a very low cost (~210 $), small closed type chamber. Chamber calibration was performed either as instantaneous evaporation (E) rates under laboratory condition (25 runs, 2 min each) and daytime cumulative evapotranspiration (ET) rates performed outside in small pots sown with different cover crops or managed with light tillage. In both cases chamber's derived water loss rates were validated against a gravimetric method. A very close linear relationship between gravimetric vs chamber values was found for lab and outdoor calibration runs (R2 = 0.96 and 0.99, respectively) within ranges of 0–0.8 mm h−1 (lab) and 0–23 mm d-1 (outdoor). Ideal measurement time window was estimated at 60 s after “time zero” set at 15 s upon chamber positioning. Under any condition, chamber heating never exceeded 2 °C above air temperature. In the warmest hours of the day (i.e. from 11:00 to 16:00) Festuca arundinacea hourly ET was 0.56 mm while Lotus corniculatus and wet soil tillage registered 0.71 and 0.69 mm h−1, respectively. Dry soil tillage showed ET values between 0.2 and 0.3 mm h−1. The proposed device represents an effective IoT application as total cost of the needed components does not reach a total amount of 200 euros and its size as well as flexibility of use makes it an ideal tool for fast multipoint readings of soil and grass water losses in the field

Le foglie, grazie alla traspirazione, sono gli organi della pianta pi\uf9 efficienti nel regolare la propria temperatura. Se per\uf2 i tassi traspirativi sono ridotti drasticamente a causa di una siccit\ue0 prolungata, le foglie perdono tale capacit\ue0 e la loro temperatura pu\uf2 salire anche oltre i 40 \ub0C. In tali situazioni \ue8 frequente la comparsa di ingiallimenti a carico delle foglie pi\uf9 esposte, le quali perdono la loro capacit\ue0 di recuperare i pieni tassi fotosintetici anche al successivo ripristino di condizioni non limitanti.     OBJECTIVE: Depression is a comorbidity affecting quality of life (QoL) in patients with Parkinson's disease (PD) and requires appropriate treatment. This study evaluated the tolerability, safety, and efficacy of duloxetine 60 mg once daily for 12 weeks in PD patients with major depressive disorder (MDD). RESEARCH AND DESIGN METHODS: Non-comparative, open-label, multi-center study. MAIN OUTCOME MEASURES: Tolerability was evaluated by discontinuation rate (acceptable if ≤ 19%) due to treatment-emergent adverse events (TEAEs) and motor symptoms (UPDRS). Safety measures were TEAEs, the UKU side effect rating scale, vital signs, weight, laboratory tests, and ECG. Efficacy measures included HAMD-17, BDI, CGI-S, PGI-I, and pain VAS. QoL was measured by PDQ-39. RESULTS: Of the 151 patients enrolled, 8.6% (95% upper CI: 13.3%) discontinued the study due to TEAEs. Worsening in PD-related tremor and rigidity was not observed, activities of daily living significantly improved and UKU subscales progressively decreased. Clinically significant abnormalities in laboratory findings were limited to four cases of hypercholesterolemia and one increase of total bilirubin, CPK, and fasting glucose. Blood pressure, weight, and ECG did not change from baseline. HAMD-17 and PDQ-39 total score and individual domains, BDI, CGI-S, and PGI-I total scores significantly improved. CONCLUSIONS: Duloxetine seems well tolerated and likely effective in the treatment of depression associated with PD, with no detrimental effects in PD signs and symptoms
