    La calidad de vida en adultos con diabetes mellitus tipo 2 en centros de salud de Guadalajara, Jalisco (México)

    Objetivo: Evaluar la percepción de Calidad de Vida Relacionada con la Salud (CVRS) de Objetivos: Evaluar la relación de la diabetes sobre la calidad de vida, utilizando el cuestionario Diabetes 39 en pacientes diagnosticados como diabéticos que acuden a visita ambulatoria a centros de salud. Materiales y métodos: Con un diseño transversal en una muestra no probabilística determinada por un periodo de tiempo en cuatro centros de salud de la Secretaría de Salud Jalisco (SSJ) en la Zona Metropolitana de Guadalajara, en pacientes que acuden a control de diabetes. Se estudiaron tres variables: calidad de vida, socio-demográficas y específicas de la enfermedad. Resultados: En la muestra de 198 pacientes ambulatorios con diabetes tipo 2 y con un tiempo de evolución de 9.51 ± 8 años, un 76.7% sin complicaciones relacionadas con la diabetes, una mitad (47.9%) fueron tratados con dieta. Con base en los dominios medidos con el Diabetes 39, las puntuaciones más altas se dan en la "calidad de vida" con un dete- rioro en la misma seguida de la percepción que se tiene sobre la "severidad de la diabetes", de los cinco dominios los de "energía y movilidad" y el de "funcionamiento sexual" fueron los que tienen una mayor media del total de la muestra. Las diferencias significativas por género se dan en el dominio de "funcionamiento sexual" para los hombres. Conclusiones: La calidad de vida se ve afectada en pacientes con diabetes, especialmen- te en el dominio de energía y movilidad y en el funcionamiento sexual, lo que indica la necesidad de un programa de intervención en actividades física-recreativas y en asesoría educativa-psicológica. Abstract Objectives: To evaluate the relationships of the diabetes on the quality of life, being used questionnaire Diabetes 39 to patients diagnosed like diabetics who go ambulatory visit to health centers. Materials and methods: With a transversal design in a nonrandom sampling determi- ned by a time period in four centers of health of the salubrity secretary was made (SSA) in the Metropolitan Zone of Guadalajara, in patients attending diabetic control. Three variables were studied: quality of life, socio-demographics and the disease specific. Results: In the sample of 198 ambulatory patients with diabetes type 2 iand with a du- ration of 9.51 ± 8 years, 76,7% without diabetes-related complications, one half (47.95) were treated with diet. Based on the dominions measured with Diabetes 39, the highest scores occur in the dominion of "quality of life" with a followed deterioration in their lives and in their perception about the "severity of the diabetes", of the five dominions those of "energy and mobility" and the one of "sexual operation" was those that they have a greater average of the total of the sample. The significant differences by gender differences exist in the domain of "sexual performance" for men. Conclusions: The quality of life is affected in patients with diabetes, especially in the do- minion of energy and mobility and in the sexual operation, which indicates the necessity of a program of intervention in physical-recreational activities and educative-psychological consultant's office. The greater negative impact of the diabetes in the quality of life was associated with man, low entrance to the average and with presence of complications by the diabetes.     Preserved Ex Vivo Inflammatory Status in Decidual Cells from Women with Preterm Labor and Subclinical Intrauterine Infection

    Objective To compare the inflammatory response preserved ex vivo by decidual cells isolated from women who experienced preterm labor with and without subclinical intrauterine infection. Methods Fetal membranes were obtained after cesarean section from 35 women who delivered before 37 weeks of gestation following spontaneous preterm labor, with no clinical evidence of intrauterine infection. Decidua was microbiologically tested and cultured. Concentrations of anti-inflammatory cytokines (IL-2, IL-4, IL-10), pro-inflammatory cytokines (IL-6, IL-8, IL-1β and TNF-α), and matrix metalloproteinases (MMP-1, MMP-2, MMP-3, MMP-7, MMP-8, MMP-9) were measured in the supernatants using Bio-Plex, and prostaglandin E2 (PGE2) was measured by enzyme immunoassay. Results Subclinical infection was confirmed in 10 women (28.5%). Microorganisms isolated wereUreaplasma urealyticum (4), group B streptococci (3), Gardnerella vaginalis (1), andEscherichia coli (2). We found a significant increase of pro-inflammatory cytokines and a significant decrease of anti-inflammatory cytokines in supernatants from decidual cells obtained from women with preterm labor and subclinical intrauterine infection compared to women without infection. Secretion of MMP-1, MMP-8, MMP-9 and PGE2 was significantly higher in infected women. Secretion of IL-8 by decidual cells from infected women persisted upon repeated in vitro culture passages. Conclusions Almost 30% of idiopathic preterm labor cases were associated with subclinical intrauterine infection, and decidual cells isolated from these cases preserved an ex vivo inflammatory status after in vivo bacterial exposure

    Combined Boyden-Flow Cytometry Assay Improves Quantification and Provides Phenotypification of Leukocyte Chemotaxis

    Chemotaxis has been studied by classical methods that measure chemotactic and random motility responses in vitro, but these methods do not evaluate the total number and phenotype of migrating leukocytes simultaneously. Our objective was to develop and validate a novel assay, combined Boyden-flow cytometry chemotaxis assay (CBFCA), for simultaneous quantification and phenotypification of migrating leukocytes. CBFCA exhibited several important advantages in comparison to the classic Boyden chemotaxis assay (CBCA): 1) improved precision (intra-assay coefficients of variation (CVs): CBFCA-4.7 and 4.8% vs. CBCA-30.1 and 17.3%; inter-observer CVs: CBFCA-3.6% vs. CBCA 30.1%); 2) increased recovery of cells, which increased assay to provide increased sensitivity; 3) high specificity for determining the phenotype of migrating/attracted leukocytes; and 4) reduced performance time (CBFCA 120 min vs. CBCA 265 min). Other advantages of CBFCA are: 5) robustness, 6) linearity, 7) eliminated requirement for albumin and, importantly, 8) enabled recovery of migrating leukocytes for subsequent studies. This latter feature is of great benefit in the study of migrating leukocyte subsets. We conclude that the CBFCA is a novel and improved technique for experiments focused on understanding leukocyte trafficking during the inflammatory response

    Maternal haemoglobin levels in pregnancy and child DNA methylation : a study in the pregnancy and childhood epigenetics consortium

    Altered maternal haemoglobin levels during pregnancy are associated with pre-clinical and clinical conditions affecting the fetus. Evidence from animal models suggests that these associations may be partially explained by differential DNA methylation in the newborn with possible long-term consequences. To test this in humans, we meta-analyzed the epigenome-wide associations of maternal haemoglobin levels during pregnancy with offspring DNA methylation in 3,967 newborn cord blood and 1,534 children and 1,962 adolescent whole-blood samples derived from 10 cohorts. DNA methylation was measured using Illumina Infinium Methylation 450K or MethylationEPIC arrays covering 450,000 and 850,000 methylation sites, respectively. There was no statistical support for the association of maternal haemoglobin levels with offspring DNA methylation either at individual methylation sites or clustered in regions. For most participants, maternal haemoglobin levels were within the normal range in the current study, whereas adverse perinatal outcomes often arise at the extremes. Thus, this study does not rule out the possibility that associations with offspring DNA methylation might be seen in studies with more extreme maternal haemoglobin levels.Peer reviewe

    Seroepidemiology of Toxoplasma gondii infection in pregnant women in a public hospital in northern Mexico

    BACKGROUND: Toxoplasma gondii (T. gondii) infection in pregnant women represents a risk for congenital disease. There is scarce information about the epidemiology of T. gondii infection in pregnant women in Mexico. Therefore, we sought to determine the prevalence of T. gondii infection and associated socio-demographic, clinical and behavioural characteristics in a population of pregnant women of Durango City, Mexico. METHODS: Three hundred and forty three women seeking prenatal care in a public hospital of Durango City in Mexico were examined for T. gondii infection. All women were tested for anti-T. gondii IgM and IgG antibodies by using IMx Toxo IgM and IMx Toxo IgG 2.0 kits (Abbott Laboratories, Abbott Park, IL, USA), respectively. Socio-demographic, clinical and behavioural characteristics from each participant were also obtained. RESULTS: Twenty one out of the 343 (6.1%) women had IgG anti-T. gondii antibodies. None of the 343 women had IgM anti-T. gondii antibodies. Multivariate analysis using logic regression showed that T. gondii infection was associated with living in a house with soil floor (adjusted OR = 7.16; 95% CI: 1.39–36.84), residing outside of Durango State (adjusted OR = 4.25; 95% CI: 1.72–10.49), and turkey meat consumption (adjusted OR = 3.85; 95% CI: 1.30–11.44). Other characteristics as cat contact, gardening, and food preferences did not show any association with T. gondii infection. CONCLUSION: The prevalence of T. gondii infection in pregnant women of Durango City is low as compared with those reported in other regions of Mexico and the majority of other countries. Poor housing conditions as soil floors, residing in other Mexican States, and turkey meat consumption might contribute to acquire T. gondii infection

Publisher Copyright: © 2023, The Author(s).Preterm birth (PTB) is the leading cause of infant mortality worldwide. Changes in PTB rates, ranging from −90% to +30%, were reported in many countries following early COVID-19 pandemic response measures (‘lockdowns’). It is unclear whether this variation reflects real differences in lockdown impacts, or perhaps differences in stillbirth rates and/or study designs. Here we present interrupted time series and meta-analyses using harmonized data from 52 million births in 26 countries, 18 of which had representative population-based data, with overall PTB rates ranging from 6% to 12% and stillbirth ranging from 2.5 to 10.5 per 1,000 births. We show small reductions in PTB in the first (odds ratio 0.96, 95% confidence interval 0.95–0.98, P value <0.0001), second (0.96, 0.92–0.99, 0.03) and third (0.97, 0.94–1.00, 0.09) months of lockdown, but not in the fourth month of lockdown (0.99, 0.96–1.01, 0.34), although there were some between-country differences after the first month. For high-income countries in this study, we did not observe an association between lockdown and stillbirths in the second (1.00, 0.88–1.14, 0.98), third (0.99, 0.88–1.12, 0.89) and fourth (1.01, 0.87–1.18, 0.86) months of lockdown, although we have imprecise estimates due to stillbirths being a relatively rare event. We did, however, find evidence of increased risk of stillbirth in the first month of lockdown in high-income countries (1.14, 1.02–1.29, 0.02) and, in Brazil, we found evidence for an association between lockdown and stillbirth in the second (1.09, 1.03–1.15, 0.002), third (1.10, 1.03–1.17, 0.003) and fourth (1.12, 1.05–1.19, <0.001) months of lockdown. With an estimated 14.8 million PTB annually worldwide, the modest reductions observed during early pandemic lockdowns translate into large numbers of PTB averted globally and warrant further research into causal pathways.Peer reviewe
