    Early (n170/m170) face-sensitivity despite right lateral occipital brain damage in acquired prosopagnosia.

    Compared to objects, pictures of faces elicit a larger early electromagnetic response at occipito-temporal sites on the human scalp, with an onset of 130 ms and a peak at about 170 ms. This N170 face effect is larger in the right than the left hemisphere and has been associated with the early categorization of the stimulus as a face. Here we tested whether this effect can be observed in the absence of some of the visual areas showing a preferential response to faces as typically identified in neuroimaging. Event-related potentials were recorded in response to faces, cars, and their phase-scrambled versions in a well-known brain-damaged case of prosopagnosia (PS). Despite the patient's right inferior occipital gyrus lesion encompassing the most posterior cortical area showing preferential response to faces ("occipital face area"), we identified an early face-sensitive component over the right occipito-temporal hemisphere of the patient that was identified as the N170. A second experiment supported this conclusion, showing the typical N170 increase of latency and amplitude in response to inverted faces. In contrast, there was no N170 in the left hemisphere, where PS has a lesion to the middle fusiform gyrus and shows no evidence of face-preferential response in neuroimaging (no left "fusiform face area"). These results were replicated by a magnetoencephalographic investigation of the patient, disclosing a M170 component only in the right hemisphere. These observations indicate that face-preferential activation in the inferior occipital cortex is not necessary to elicit early visual responses associated with face perception (N170/M170) on the human scalp. These results further suggest that when the right inferior occipital cortex is damaged, the integrity of the middle fusiform gyrus and/or the superior temporal sulcus - two areas showing face-preferential responses in the patient's right hemisphere - might be necessary to generate the N170 effect

    Treatment of polymyalgia rheumatica

    Polymyalgia rheumatica (PMR) is an inflammatory disease characterized by bilateral pain involving predominantly the shoulders and proximal aspects of the arms and less commonly the neck and the pelvic girdle. This review discusses briefly the main epidemiological data and clinical features of this condition. Especial attention is paid in the management of the disease. For this reason, both the classic management and the impact of new therapies are discussed in depth. In general, patients with PMR experience a rapid response to 12.5-25?mg/prednisone/day in less than a week. Patients with poor response to glucocorticoids or with relapsing disease require other therapies aimed mainly to spare glucocorticoids. Among them, methotrexate is the most commonly used. Nevertheless, different studies indicate that this agent yields only a modest effect. Biologic therapies against the main cytokines involved in the pathogenesis of the disease have been used in refractory patients. However, randomized controlled trials do not support the use of anti-tumor necrosis factor agents in PMR. In contrast, several case series and retrospective studies highlight the efficacy of the anti-interleukin-6 receptor tocilizumab in PMR. Nonetheless, controlled trials are needed to fully establish the beneficial effect of this agent.     Trabajar con metodologías participativas en la formación universitaria, todo un desafío

    Este artículo tiene como objeto de estudio el proceso de innovación con prácticas participativas realizado por un equipo multidisciplinar de profesoras de la Universidad del País Vasco. Desde las pautas de la metodología de la investigación-acción, los instrumentos utilizados para generar los datos han sido: relatos personales, entrevistas, tertulias pedagógico-dialógicas y técnicas activas. Partiendo de un análisis categorial emergente, se formulan unos resultados en términos de desafíos o retos, que evidencian los aspectos claves a considerar en la puesta en marcha y el seguimiento de estas novedosas prácticas formativas universitarias, dado su efecto en el aprendizaje del alumnado; a saber: transitar del individualismo a la colaboración, tener en cuenta los procesos de socialización previa del alumnado y del profesorado, compartir el poder de decisión, atender al cuidado de las personas, dar un nuevo significado a la participación y avanzar en el cambio de contextos de trabajo. Finalmente, el estudio alerta acerca de la responsabilidad que nos compete sobre el uso de estas metodologías en el ámbito universitario y su extensión a otros campos de la educación.This essay centres on the subject of advancing innovative procedures for participative practices at university level, as they have been undertaken by a multidisciplinary team of teachers from the University of the Basque Country. Following the guidelines of the action-research methodology, the instruments used to generate the information have been: personal experiences reports, interviews, pedagogic-dialogic discussions, and active techniques. Starting from an emergent categorical analysis, the results are formulated in terms of challenges, trying to provide answers to the needs of new formative university practices; for example, facing the transition between individualism and collaboration, considering the process of socialisation of the students and teachers, sharing the taking of decisions, paying attention to the care of people, providing a broader meaning to the concept of participation and progressing in changing the work environment and contexts. Finally, this study warns us of the responsibility that rests on the use of these methodologies at university and its extension to other areas of education

    The Age-Related Cryptosporidium Species Distribution in Asymptomatic Cattle from North-Western Spain

    An age-related distribution of Cryptosporidium species has been reported in cattle, with C. parvum being predominant in suckling calves, C. bovis and C. ryanae being predominant in post-weaned calves and C. andersoni being predominant in adults. However, variants to this pattern have recently been reported. Thus, fecal samples (n = 594) from asymptomatic cattle were collected in north-western Spain. Animals were classified as 2 years (G5). Cryptosporidium detection and species identification were performed by SSU rRNA PCR. Individual Cryptosporidium prevalence was 16.7%; it significantly decreased with age. Cryptosporidium parvum was predominant in G1 and C. bovis was predominant in the rest of the age classes; C. bovis and C. ryanae were especially prevalent in G2 and G3. Cryptosporidium occultus was not found in suckling calves. Finally, C. andersoni and C. xiaoi were occasionally detected in G5. The presence of C. parvum in all age classes implies significant animal and public health concerns. The predominance of C. bovis in cattle older than 1 month supports the idea that the age-related pattern of Cryptosporidium species described in cattle is not fully consistent, and thus further studies are still needed to identify those factors determining the species distributionThis research was funded by Ministerio de Ciencia e Innovación (Spain), grant number AGL2016-76034-P and by Xunta de Galicia (Spain), grant number 2017-PG117S

    El agua, como motor de desarrollo y fuente de riqueza, ha constituido uno de los pilares fundamentales para el progreso del hombre. La ordenación y gestión de los recursos hídricos, que ha sido desde siempre un objetivo prioritario para cualquier sociedad, se ha realizado históricamente bajo directrices orientadas a satisfacer la demanda en cantidades suficientes, bajo una perspectiva de política de oferta. El incremento de la oferta de agua como herramienta para el impulso económico, el mayor nivel de contaminación, irremisiblemente asociado a un mayor nivel de desarrollo, algunas características naturales (sequías prolongadas, inundaciones) y en definitiva una sobreexplotación de los recursos hídricos, han conducido a un deterioro importante de los mismos. El desarrollo de la Directiva Europea y Nacional respecto a control de vertido responsabiliza al Ayuntamiento directamente de los vertidos del municipio al exterior, y consecuentemente al Alcalde Presidente de cada Municipio. Por ello la Diputación de Sevilla, cumpliendo su labor de auxilio técnico al municipio ha encargado al Grupo TAR este manual municipal de control de vertidos para asesorar continuamente a todos los pueblos de la provincia de Sevilla, con la vocación de que sea una herramienta permanente de consulta que ayude a conseguir que la provincia lidere el cumplimiento de las normas y el desarrollo sostenible de todo su entorno. Real Decreto Legislativo 1/2001, de 20 de julio, por el que se aprueba el texto refundido de la Ley de Aguas de 1.985, junto con la nueva Directiva Marco europea para la política de agua suponen un cambio importante en los conceptos y criterios utilizados en la planificación hidrológica e introducen la calidad de las aguas y la protección de los recursos hídricos como puntos fundamentales para estructurar dicha planificación. El estado ecológico de un agua sería una expresión de la calidad de la estructura y del funcionamiento del ecosistema y cuyos criterios de clasificación (muy bueno, bueno y aceptable), en función del tipo de ecosistema acuático de que se trate, se recogen en los anexos de la directiva

    Mucin 1/Krebs von den Lungen-6 (KL-6) is proposed as a serum biomarker of several interstitial lung diseases (ILDs), including connective tissue disorders associated with ILD. However, it has not been studied in a large cohort of Caucasian antisynthetase syndrome (ASSD) patients. Consequently, we assessed the role of MUC1 rs4072037 and serum KL-6 levels as a potential biomarker of ASSD susceptibility and for the differential diagnosis between patients with ILD associated with ASSD (ASSD-ILD?+) and idiopathic pulmonary fibrosis (IPF). 168 ASSD patients (149 ASSD-ILD?+), 174 IPF patients and 523 healthy controls were genotyped for MUC1 rs4072037 T?>?C. Serum KL-6 levels were determined in a subgroup of individuals. A significant increase of MUC1 rs4072037 CC genotype and C allele frequencies was observed in ASSD patients compared to healthy controls. Likewise, MUC1 rs4072037 TC and CC genotypes and C allele frequencies were significantly different between ASSD-ILD+ and IPF patients. Additionally, serum KL-6 levels were significantly higher in ASSD patients compared to healthy controls. Nevertheless, no differences in serum KL-6 levels were found between ASSD-ILD+ and IPF patients. Our results suggest that the presence of MUC1 rs4072037 C allele increases the risk of ASSD and it could be a useful genetic biomarker for the differential diagnosis between ASSD-ILD+ and IPF patients

    Biologic Therapy in Refractory Non-Multiple Sclerosis Optic Neuritis Isolated or Associated to Immune Mediated Inflammatory Diseases. A Multicenter Study

    We aimed to assess the e cacy of biologic therapy in refractory non-Multiple Sclerosis (MS) Optic Neuritis (ON), a condition more infrequent, chronic and severe than MS ON. This was an open-label multicenter study of patients with non-MS ON refractory to systemic corticosteroids and at least one conventional immunosuppressive drug. The main outcomes were Best Corrected Visual Acuity (BCVA) and both Macular Thickness (MT) and Retinal Nerve Fiber Layer (RNFL) using Optical Coherence Tomography (OCT). These outcome variables were assessed at baseline, 1 week, and 1, 3, 6 and 12 months after biologic therapy initiation. Remission was defined as the absence of ON symptoms and signs that lasted longer than 24 h, with or without an associated new lesion on magnetic resonance imaging with gadolinium contrast agents for at least 3 months. We studied 19 patients (11 women/8 men; mean age, 34.8 13.9 years). The underlying diseases were Bechet?s disease (n = 5), neuromyelitis optica (n = 3), systemic lupus erythematosus (n = 2), sarcoidosis (n = 1), relapsing polychondritis (n = 1) and anti-neutrophil cytoplasmic antibody -associated vasculitis (n = 1). It was idiopathic in 6 patients. The first biologic agent used in each patient was: adalimumab (n = 6), rituximab (n = 6), infliximab (n = 5) and tocilizumab (n = 2). A second immunosuppressive drug was simultaneously used in 11 patients: methotrexate (n = 11), azathioprine (n = 2), mycophenolate mofetil (n = 1) and hydroxychloroquine (n = 1). Improvement of the main outcomes was observed after 1 year of therapy when compared with baseline data: mean SD BCVA (0.8 0.3 LogMAR vs. 0.6 0.3 LogMAR; p = 0.03), mean SD RNFL (190.5 175.4 m vs. 183.4 139.5 m; p = 0.02), mean SD MT (270.7 23.2 m vs. 369.6 137.4 m; p = 0.03). Besides, the median (IQR) prednisone-dose was also reduced from 40 (10?61.5) mg/day at baseline to. 2.5 (0?5) mg/day after one year of follow-up; p = 0.001. After a mean SD follow-up of 35 months, 15 patients (78.9%) achieved ocular remission, and 2 (10.5%) experienced severe adverse events.     Sleep Problems Are Related to a Worse Quality of Life and a Greater Non-Motor Symptoms Burden in Parkinson’s Disease

    COPPADIS Study Group.[Introduction] The aim of the present study was to examine the frequency of self-reported sleep problems and their associated factors in a large cohort of PD patients.[Methods] PD patients and controls, recruited from 35 centers of Spain from the COPPADIS cohort were included in this cross-sectional study. Sleep problems were assessed by the Spanish version of the Parkinson’s disease Sleep Scale version 1 (PDSS-1). An overall score below 82 or a score below 5 on at least 1 item was defined as sleep problems.[Results] The frequency of sleep problems was nearly double in PD patients compared to controls: 65.8% (448/681) vs 33.5% (65/206) (p < 0.0001). Mean total PDSS score was lower in PD patients than controls: 114.9 ± 28.8 vs 132.8 ± 16.3 (p < 0.0001). Quality of life (QoL) was worse in PD patients with sleep problems compared to those without: PDQ-39SI, 19.3 ± 14 vs 13 ± 11.6 (p < 0.0001); EUROHIS-QoL8, 3.7 ± 0.5 vs 3.9 ± 0.5 (p < 0.0001). Non-motor symptoms burden (NMSS; OR = 1.029; 95%CI 1.015–1.043; p < 0.0001) and impulse control behaviors (QUIP-RS; OR = 1.054; 95%CI 1.009–1.101; p = 0.018) were associated with sleep problems after adjustment for age, gender, disease duration, daily equivalent levodopa dose, H&Y, UPDRS-III, UPDRS-IV, PD-CRS, BDI-II, NPI, VAS-Pain, VAFS, FOGQ, and total number of non-antiparkinsonian treatments.[Conclusion] Sleep problems were frequent in PD patients and were related to both a worse QoL and a greater non-motor symptoms burden in PD. These findings call for increased awareness of sleep problems in PD patients.Peer reviewe
