    Repos y operaciones simultáneas : estudio de la normativa

    Durante los ultimos años, el volumen de operaciones dobles con deuda publica en España ha crecido significativamente. Estas operaciones han proporcionado un alto grado de liquidez al mercado secundario de valores publicos. En nuestro pais coexisten dos tipos de operaciones dobles con caracteristicas semejantes, no siempre facilmente delimitables: las operaciones simultaneas y las operaciones repo. Analiza la normativa que regula ambas modalidades, tanto desde la optica de su regimen juridico como de sus implicaciones contables y fiscales. Su comparacion permite concluir que existe una casi total identidad juridica, fiscal y contable entre ambas operaciones. Las diferencias existentes surgen del distinto tratamiento por parte de la Central de Anotaciones y de las cautelas introducidas en la regulacion del segmento de negocio con terceros. El trabajo está dedicado a la memoria de David Moisés Macedo, becario del proyecto y que desafortunadamente, falleciera durante el proceso de investigación, descansa en paz.En la época actual, donde el acto de alimentarse remite en gran parte a una ausencia de sentido de lo nutricio, se hace emergente mirar de nueva cuenta la importancia de resignificar tan relevante acontecimiento, desde los aspectos socioculturales más profundos y engastados en determinantes antropológicos y filosófico-morales que dotan a las prácticas alimentarias y sus diversos imaginarios de un sinfín de paradojas y correspondencias. En esta intención, planteamos una revisión teórica que aperture de nueva cuenta, la relación sustancial entre la corporeidad, la alimentación y sus imaginarios sociales en un contexto social cada vez más inestable. Se propone una comprensión de la problemática a partir de la teoría del reconocimiento recíproco y las patologías sociales desarrollada por Axel Honneth, para definir la dirección de las posibles intervenciones alimentarias. Abstract: Currently, where the act of feeding is based largely in a lack of nurturing sense, it is necessary to look back to the importance of giving a new perspective to such event, since the deepest and sociocultural aspects set into anthropological and moral Philosophy determinants which provide to the feeding practices and its several social imaginaries, endless paradoxes and correspondence. In this intention, we propose a theoretical review which apertures once again, the substantial relationship between embodiment, feeding and its social imaginaries in an increasingly unstable social context.     A new antibiotic-loaded sol-gel can prevent bacterial intravenous catheter-related infections

    The aim of this study was to evaluate the effectiveness of a moxifloxacin-loaded organic-inorganic sol-gel (A50) by locally preventing the catheter-related bloodstream infection (CRBSI) provoked by Staphylococcus epidermidis (S. epidermidis) and the effect resulting from its hydrolytic degradation on coagulation by using a rabbit in-vivo model. A50 coating can completely inhibit growth and would locally prevent CRBSI provoked by S. epidermidis. None of the coagulation blood parameters showed a significant difference constant over time between the control catheter group and the A50-coated catheter group, despite the visible silica release resulting from physiological A50 sol-gel degradation detected in serum at least during the first week. At pathological level, foreign body reaction was present in both of types of catheter, and it was characterized by the presence of macrophages and foreign body giant cell. However, this reaction was different in each group: the A50-coated catheter group showed a higher inflammation with histiocytes, which were forming granuloma-like aggregates with an amorphous crystalline material inside, accompanied by other inflammatory cells such as plasma cells, lymphocytes and mast cells. In conclusion, A50 coating a venous catheter showed excellent bactericidal anti-biofilm response since it completely inhibited S. epidermidis biofilm development and, far from showing procoagulant effects, showed slightly anticoagulant effects.This research received financial support from the Mutua Madrileña Foundation (04078/001).     Knowledge about contraceptive methods and sexual behavior in young university students.

    Introducción: En México los embarazos no planificados se presentan entre 30% y 60% en adolescentes. Esta problemática permea a todos los grupos de la sociedad observándose aún en las universidades. La Universidad Autónoma del Estado de México ha realizado acciones preventivas; sin embargo, en cinco años, el número de embarazos en las estudiantes incrementó en un plantel de la zona oriente, por lo que se propuso evaluar el conocimiento sobre métodos de planificación familiar y conducta sexual que tienen los estudiantes universitarios. Metodología: Estudio cuantitativo transversal realizado en 2018. La muestra no probabilística estuvo integrada por 224 alumnos de ciencias sociales del nivel superior de la Universidad Autónoma del Estado de México, quienes dieron su consentimiento informado. Se les aplicó un cuestionario de 32 ítems a través del cual se evaluó conocimiento sobre métodos de planificación familiar y conductas sexuales que tenían. Resultados: 51% informaron recibir información sobre métodos de planificación familiar entre 13 y 17 años, proviniendo de su profesor(a). 69% de los estudiantes reportaron haber tenido relaciones sexuales; de los cuales 97% respondieron que antes de ingresar al nivel superior recibieron la información sobre métodos anticonceptivos. Actualmente 45% no utiliza algún método anticonceptivo, 49% declararon haber tenido sexo oral y 18% anal. 9% reportó tener relaciones sexuales bajo influencia de drogas. Conclusiones: Se realizó un acercamiento al conocimiento sobre métodos de planificación familiar y conducta sexual de los estudiantes, para diseñar e implementar un programa de salud sexual que prevenga infecciones de transmisión sexual y embarazos no planificados.Unplanned pregnancies in Mexico occur between 30 and 60 percent of the cases in adolescents. This problem permeates all groups in society being observed even in universities. Universidad Autónoma del Estado de Mexico has developed preventive actions, but in five years the number of pregnancies in students increased in one of its campuses in the western area, reason why it was proposed to evaluate the knowledge about family planning methods and sexual behavior that university students have. Methodology: Cross-sectional quantitative study conducted in 2018. The non-probability sample consisted of 224 students from the upper level of Universidad Autónoma del Estado de Mexico who gave their informed consent. A questionnaire of 32 items was applied, through which knowledge about family planning methods and sexual behaviors they had were evaluated. Results: 51% reported receiving information about family planning methods when they were 13 and 17 years old, coming from their teachers. 69% of the students reported having had sexual intercourse from whom 97% answered that they had received information about contraceptive methods before entering the upper level. Currently 45% do not use any contraception method, 49% reported having oral sex and 18% anal sex; 9% reported having sex under the influence of drugs. Conclusions: An approach was made to knowledge about family planning methods and sexual behavior of students, to design and implement a sexual health program that prevents sexually transmitted infections and unplanned pregnancies

    Los dos volumenes de que consta este trabajo ofrecen una detallada descripcion de los distintos aspectos organizativos del mercado español de deuda publica. Su reciente creacion, en 1987, significo la introduccion de las estructuras mas convenientes para financiar los deficit publicos con recurso al mercado de valores. La obra analiza el diseño y evolucion del mercado primario, en el que el gobierno capta financiacion emitiendo bonos de acuerdo con criterios de mercadoestudia como se produce su transmision en el mercado secundario, identificandose los miembros, los tipos de operaciones y el perfil general de actividaddescribe los procedimientos de liquidacion y registro de operaciones en el marco del sistema de anotaciones en cuenta (SACDE) gestionado por el Banco de España. Asimismo, examina la organizacion y funcionamiento del mercado español de derivados sobre deuda publica, dada su importante funcion estabilizadora.     Long-term anticoagulation in secondary ischemic stroke prevention: The prospective multicenter RESTAIC registry

    Background and Objective: Oral anticoagulation (OAC) for secondary stroke prevention is recommended in atrial fibrillation (AF) and other sources of cardioembolic stroke. Our objectives were to explore the differences in ischemic and hemorrhagic events when using OAC for secondary stroke prevention according to the type of anticoagulant treatment and to analyze the number and reasons for OAC switches during long-term follow-up. Methods: Ischemic stroke (IS) patients who were discharged on OAC for secondary stroke prevention from January 2014 to October 2017 were recruited in a prospective, multicenter, hospital-based registry. Follow-up at 3 months was scheduled at the outpatient clinic with subsequent annual phone interviews for 3 years. Patients were classified into three study groups according to OAC at discharge: Vitamin K antagonist (VKA), Factor Xa inhibitor (FXa), or direct thrombin inhibitor (DTI). We compared stroke recurrences, intracranial hemorrhage, major bleeding, and all-cause mortality during the follow-up. We recorded any switches in OAC and the main reasons for the change. Results: A total of 241 patients were included. An anticoagulant was indicated in the presence of a source of cardioembolic stroke in 240 patients (99.6%) and lupus plus antiphospholipid syndrome in one patient. Up to 86 patients (35.6%) were on OAC before the index stroke; in 71 (82.5%) of them, this was VKA. At hospital discharge, 105 were treated with FXa (43.8%), 96 with VKA (39.6%), and 40 with DTI (16.6%). The cumulative incidences at 3 years were 17% for stroke recurrence, 1.6% for intracranial hemorrhage, 4.9% for major hemorrhage, and 22.8% for all-cause mortality, with no differences among the OAC groups in any outcomes. During the follow-up, 40 OAC switches were recorded (63% of them to FXa), mostly due to stroke recurrence. Conclusion: Long-term OAC in secondary stroke prevention is associated with a lower frequency of bleeding complications than stroke recurrences. No differences between anticoagulant drugs were found in any of the analyzed outcomes.     Assessing Pandemic Preparedness, Response, and Lessons Learned From the Covid-19 Pandemic in Four South American Countries: Agenda for the Future

    INTRODUCTION: The COVID-19 pandemic emerged in a context that lacked adequate prevention, preparedness, and response (PPR) activities, and global, regional, and national leadership. South American countries were among world\u27s hardest hit by the pandemic, accounting for 10.1% of total cases and 20.1% of global deaths. METHODS: This study explores how pandemic PPR were affected by political, socioeconomic, and health system contexts as well as how PPR may have shaped pandemic outcomes in Argentina, Brazil, Colombia, and Peru. We then identify lessons learned and advance an agenda for improving PPR capacity at regional and national levels. We do this through a mixed-methods sequential explanatory study in four South American countries based on structured interviews and focus groups with elite policy makers. RESULTS: The results of our study demonstrate that structural and contextual barriers limited PPR activities at political, social, and economic levels in each country, as well as through the structure of the health care system. Respondents believe that top-level government officials had insufficient political will for prioritizing pandemic PPR and post-COVID-19 recovery programs within their countries\u27 health agendas. DISCUSSION: We recommend a regional COVID-19 task force, post-pandemic recovery, social and economic protection for vulnerable groups, improved primary health care and surveillance systems, risk communication strategies, and community engagement to place pandemic PPR on Argentina, Brazil, Colombia, and Peru and other South American countries\u27 national public health agendas

    Effect of sex in systemic psoriasis therapy: Differences in prescription, effectiveness and safety in the BIOBADADERM prospective cohort

    The effect of sex on systemic therapy for psoriasis has not been well studied. The aim of this study was to analyse a large multicentre Spanish cohort of 2,881 patients with psoriasis (58.3% males), followed from January 2008 to November 2018, to determine whether sex influences prescription, effectiveness of therapy, and the risk of adverse events. The results show that women are more likely than men to be pre-scribed biologics. There were no differences between men and women in effectiveness of therapy, measur-ed in terms of drug survival. Women were more likely to develop adverse events, but the difference in risk was small and does not justify different management.     Endothelial dysfunction is an early indicator of sepsis and neutrophil degranulation of septic shock in surgical patients

    Producción CientíficaBackground: Stratification of the severity of infection is currently based on the Sequential Organ Failure Assessment (SOFA) score, which is difficult to calculate outside the ICU. Biomarkers could help to stratify the severity of infection in surgical patients. Methods: Levels of ten biomarkers indicating endothelial dysfunction, 22 indicating emergency granulopoiesis, and six denoting neutrophil degranulation were compared in three groups of patients in the first 12 h after diagnosis at three Spanish hospitals. Results: There were 100 patients with infection, 95 with sepsis and 57 with septic shock. Seven biomarkers indicating endothelial dysfunction (mid-regional proadrenomedullin (MR-ProADM), syndecan 1, thrombomodulin, angiopoietin 2, endothelial cell-specific molecule 1, vascular cell adhesion molecule 1 and E-selectin) had stronger associations with sepsis than infection alone. MR-ProADM had the highest odds ratio (OR) in multivariable analysis (OR 11·53, 95 per cent c.i. 4·15 to 32·08; P = 0·006) and the best area under the curve (AUC) for detecting sepsis (0·86, 95 per cent c.i. 0·80 to 0·91; P < 0·001). In a comparison of sepsis with septic shock, two biomarkers of neutrophil degranulation, proteinase 3 (OR 8·09, 1·34 to 48·91; P = 0·028) and lipocalin 2 (OR 6·62, 2·47 to 17·77; P = 0·002), had the strongest association with septic shock, but lipocalin 2 exhibited the highest AUC (0·81, 0·73 to 0·90; P < 0·001). Conclusion: MR-ProADM and lipocalin 2 could be alternatives to the SOFA score in the detection of sepsis and septic shock respectively in surgical patients with infection.Instituto de Salud Carlos III (grants PI15/01959, PI15/01451 and PI16/01156
