    Patient education and disease activity: A study among rheumatoid arthritis patients

    Objective: To determine whether patients experiencing high disease activity derive more benefit from patient education than those experiencing low disease activity. - \ud Methods: Data from a randomized study on the effects of a program of patient education were analyzed retrospectively. Four subgroups were studied: the high disease activity subgroup of patients who had participated in the educational program, the complementary low disease activity subgroup, the high disease activity subgroup of controls, and its low disease activity complement. Patients with erythrocyte sedimentation rate >28 mm/first hour were classified as having high disease activity. Effects on frequency of physical exercises, endurance exercises, and relaxation exercises and effects on health status (Modified Health Assessment Questionnaire, Dutch Arthritis Impact Measurement Scales [AIMS]) were measured. - \ud Results: There were no significant differences between the adherence parameters of the various pairs of groups. Four months after the educational program began, anxiety and depression scores on the Dutch-AIMS had increased among participating patients who were experiencing high disease activity and decreased among those who were experiencing low disease activity. - \ud Conclusions: Patients experiencing high disease activity did not derive more benefit from patient education than those experiencing low disease activity. On the contrary, an increase of anxiety and depression is found in these patients. Further study is needed to confirm our findings

    Group Education for patients with rheumatoid arthritis

    Patients with rheumatoid arthritis must learn to adjust their exercise, rest and medication to the varying activity of the disease. Patient education can help patients in making the right decisions about adjustments in their treatment regimen and in attaining ¿self-management¿ behaviors. We developed a group education program based on social learning theory and the `Arthritis Self Management Course¿ developed in the USA by Lorig. Goal of the program is the strengthening of self-efficacy, outcome expectations and self-management behaviors of RA patients which may lead to better health status. The program has been evaluated in an experimental design. We established significant positive effects of the group training on functional disability, joint tenderness, practice of relaxation and physical exercises, self-management behavior, outcome expectations, self-efficacy function and knowledge. After 14 months we still found effects on practice of physical exercises, self-efficacy function and knowledge

    On the Detection of Supermassive Primordial Stars. II. Blue Supergiants

    Supermassive primordial stars in hot, atomically-cooling haloes at z∼z \sim 15 - 20 may have given birth to the first quasars in the universe. Most simulations of these rapidly accreting stars suggest that they are red, cool hypergiants, but more recent models indicate that some may have been bluer and hotter, with surface temperatures of 20,000 - 40,000 K. These stars have spectral features that are quite distinct from those of cooler stars and may have different detection limits in the near infrared (NIR) today. Here, we present spectra and AB magnitudes for hot, blue supermassive primordial stars calculated with the TLUSTY and CLOUDY codes. We find that photometric detections of these stars by the James Webb Space Telescope (JWST) will be limited to z≲z \lesssim 10 - 12, lower redshifts than those at which red stars can be found, because of quenching by their accretion envelopes. With moderate gravitational lensing, Euclid and the Wide-Field Infrared Space Telescope (WFIRST) could detect blue supermassive stars out to similar redshifts in wide-field surveys.Comment: 9 pages, 5 figures, accepted by MNRA

    Invariant versus classical quartet inference when evolution is heterogeneous across sites and lineages

    One reason why classical phylogenetic reconstruction methods fail to correctly infer the underlying topology is because they assume oversimplified models. In this paper we propose a topology reconstruction method consistent with the most general Markov model of nucleotide substitution, which can also deal with data coming from mixtures on the same topology. It is based on an idea of Eriksson on using phylogenetic invariants and provides a system of weights that can be used as input of quartet-based methods. We study its performance on real data and on a wide range of simulated 4-taxon data (both time-homogeneous and nonhomogeneous, with or without among-site rate heterogeneity, and with different branch length settings). We compare it to the classical methods of neighbor-joining (with paralinear distance), maximum likelihood (with different underlying models), and maximum parsimony. Our results show that this method is accurate and robust, has a similar performance to ML when data satisfies the assumptions of both methods, and outperforms all methods when these are based on inappropriate substitution models or when both long and short branches are present. If alignments are long enough, then it also outperforms other methods when some of its assumptions are violated.Comment: 32 pages; 9 figure

    The Connecticut Department of Energy and Environmental Protection (CT DEEP) seeks to better classify their streams into thermal regimes (cold, cold transitional, warm transitional, and warm water). A prediction model was created based upon physical characteristics such that CT DEEP could classify streams into thermal regimes based upon the parameters described in Lyons et al. 2009 and compare them to their own classification system. Accurately classifying these thermal regimes determines the environmental protection provided to a stream as well as the potential for establishing fisheries

    Measure, Topology and Probabilistic Reasoning in Cosmology

    I explain the difficulty of making various concepts of and relating to probability precise, rigorous and physically significant when attempting to apply them in reasoning about objects (e.g., spacetimes) living in infinite-dimensional spaces, working through many examples from cosmology. I focus on the relation of topological to measure-theoretic notions of and relating to probability, how they diverge in unpleasant ways in the infinite-dimensional case, and are difficult to work with on their own as well in that context. Even in cases where an appropriate family of spacetimes is finite-dimensional, however, and so admits a measure of the relevant sort, it is always the case that the family is not a compact topological space, and so does not admit a physically significant, well behaved probability measure. Problems of a different but still deeply troubling sort plague arguments about likelihood in that context, which I also discuss. I conclude that most standard forms of argument used in cosmology to estimate the likelihood of the occurrence of various properties or behaviors of spacetimes have serious mathematical, physical and conceptual problems.Comment: 26 page
