    The gut microbiome as a biomarker of differential susceptibility to SARS-CoV-2

    Coronavirus disease 2019 (COVID-19) continues to exact a devastating global toll. Ascertaining the factors underlying differential susceptibility and prognosis following viral exposure is critical to improving public health responses. We propose that gut microbes may contribute to variation in COVID-19 outcomes. We synthesise evidence for gut microbial contributions to immunity and inflammation, and associations with demographic factors affecting disease severity. We suggest mechanisms potentially underlying microbially mediated differential susceptibility to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). These include gut microbiome-mediated priming of host inflammatory responses and regulation of endocrine signalling, with consequences for the cellular features exploited by SARS-CoV-2 virions. We argue that considering gut microbiome-mediated mechanisms may offer a lens for appreciating differential susceptibility to SARS-CoV-2, potentially contributing to clinical and epidemiological approaches to understanding and managing COVID-19

    The O I] 1641A line as a probe of symbiotic star winds

    The neutral oxygen resonance 1302A line can, if the optical depth is sufficiently high, de-excite by an intercombination transition at 1641A to a metastable state. This has been noted in a number of previous studies but never systematically investigated as a diagnostic of the neutral red giant wind in symbiotic stars and symbiotic-like recurrent novae. We used archival IUEIUE high resolution, and GHRS and STIS medium and high resolution, spectra to study a sample of symbiotic stars. The integrated fluxes were measured, where possible, for the O I 1302A and O I] 1641A lines. The intercombination 1641A line is detected in a substantial number of symbiotic stars with optical depths that give column densities comparable with direct eclipse measures (EG And) and the evolution of the recurrent nova RS Oph 1985 in outburst. In four systems (EG And, Z And, V1016 Cyg, and RR Tel), we find that the O I] variations are strongly correlated with the optical light curve and outburst activity. This transition can also be important for the study of a wide variety of sources in which an ionization-bounded H II region is imbedded in an extensive neutral medium, including active galactic nuclei, and not only for evaluations of extinction.Comment: accepted for publication in Astronomy and Astrophysics (2010 Feb. 23), in press, NASA-GSFC-Code 66

    Correlation of Adherence to the 2012 Infectious Diseases Society of America Practice Guidelines with Patient Outcomes in the Treatment of Diabetic Foot Infections in an Outpatient Parenteral Antimicrobial Programme

    Aim To evaluate adherence to the 2012 Infectious Diseases Society of America practice guidelines for the management of patients with diabetic foot infections and to determine an association between adherence and clinical outcome. Methods A retrospective chart review was performed to evaluate the management and clinical outcomes of patients with diabetic foot infections treated with outpatient parenteral antimicrobial therapy between 1 January 2011 and 30 June 2012 at Wishard Health Services/Eskenazi Health. Adherence to individual Infectious Diseases Society of America diabetic foot infection treatment guideline recommendations was measured, and then assessed in relation to clinical outcome. Results A total of 57 patients (61% male, mean age 54 years) with moderate to severe diabetic foot infection met the inclusion criteria. None of the treatment courses of these patients adhered to all the Infectious Diseases Society of America guideline recommendations. The recommendations most frequently adhered to were consultation of appropriate multidisciplinary teams (n=54, 94.7%) and performance of diagnostic imaging (n=52, 89.5%). The recommendations least frequently adhered to were diabetic foot wound classification scoring on admission (n=0, 0%), appropriate culture acquisition (n=12, 21.2%), surgical intervention when indicated (n=32, 46.2%) and appropriate empiric antibiotic selection (n=34, 59.7%). Of 56 patients, 52 (92.9%) experienced clinical cure at the end of outpatient parenteral antimicrobial therapy compared with 34 of 53 patients (64%) at 6 months after the completion of therapy. Adherence to individual guidelines was not associated with clinical outcome. Patients who experienced treatment failure were more likely to have severe diabetic foot infection or peripheral neuropathy. Conclusions Adherence to the Infectious Diseases Society of America diabetic foot infection guideline recommendations was found to be suboptimal in the present study.     6R instrumented spatial linkages for anatomical joint motion measurement—part 2: calibration

    The six-revolute-joint instrumented spatial linkage (6R ISL ) Introduction As with any measuring system, an instrumented spatial linkage (ISL) must be tested and calibrated before it can be used with confidence. This paper details the calibration technique developed for improving the measurement resolution of a 6R ISL-a serial ISL design where the links are interconnected by six revolute joints. With a 6R ISL, determination of anatomical joint position is based upon a mathematical function that approximates what is actually occurring with the physical model. The independent variables of this function are six voltages, each relating to a different linkage joint angle; the dependent variables are the six anatomical position parameters. In measuring the anatomical joint position of one bony coordinate frame relative to the other, this function contains twenty-four fixed mechanical parameters and generally twelve additional fixed electrical parameters that relate linkage joint angles to the voltages output by the devices that monitor the linkage joints, typically potentiometers The goal of the calibration procedure is to adjust the nominal mechanical and electrical parameters of the ISL to optimize the accuracy of the ISL within the calibration space. Although tolerances of the electrical devices are variable and cannot be completely calibrated out, Sommer and Miller [7] have demonstrated that the adjustment of the mechanical and electrical parameters can greatly improve positional measurement accuracy. They used a Levenberg-Marquardt algorithm to refine typically nineteen of the fixed ISL parameters. Their algorithm attempted to minimize the squared difference between known and calculated sets of three Euler angles and three orthogonal translations that related one linkage end to the other. The objective function was nonweighted and used centimeters and radians as the units in measuring translation and rotation, respectively.
