    Generation of mid-ocean eddies : the local baroclinic instability hypothesis

    Submitted in partial fulfillment of the requirements for the degree of Doctor of Philosophy at the Massachusetts Institute of Technology and the Woods Hole Oceanographic Institution August 2000The plausibility of local baroclinic instability as a generation mechanism for midocean mesoscale eddies is examined with a two-layer, quasi-geostrophic (QG) model forced by an imposed, horizontally homogeneous, vertically sheared mean flow and dissipated through bottom Ekman friction, Explanations are sought for two observed features of mid-ocean eddies: 1) substantial energy is retained in the baroclinic mode and in the associated deformation radius (Rd) scale, and 2) the ratio of eddy to mean kinetic energy is much larger than one, The tendency of QG to cascade energy into the barotropic mode and into scales larger than Rd can be counteracted when stratification is surface-trapped, for then the baroclinic mode is weakly damped, and hence enhanced, Numerical experiments are performed with both surface-trapped and uniform stratification to quantify this, Experiments with equal Ekman frictions in the two layers are also performed for purposes of contrast, Interpretation is aided with an inequality derived from the energy and enstrophy equations, The inequality forbids the simultaneous retention of substantial energy in the baroclinic mode and in scales near Rd when Ekman friction is symmetric, but points towards surface-trapped stratification and bottomtrapped friction as an environment in which both of these can be achieved, The dissertation also contains a systematic study of geostrophic turbulence forced by nonzonal flows, Narrow zonal jets emerge when shear-induced mean potential vorticity (PV) gradients are small compared to the planetary gradient (β), and energy is a strong function of the angle shear presents to the east-west direction, When shear-induced PV gradients are comparable to β, and the mean shear has a westward component, fields of monopolar vortices form and persist, Energy is asymmetric between fields of cyclones and anticyclones, Such asymmetry was commonly thought not to occur in QG, but is shown here to be introduced by the nonzonal basic state, In both jet and vortex regimes, eddy energy can be much larger than mean kinetic energy, contrary to the expectation that β stabilizes weak shear flows,My first three years here were funded by an Office of Naval Research/National Defense Science and Engineering Graduate Fellowship administered by Jeff Jarocz at the American Society for Engineering Education.     Prostatic duct adenocarcinoma with endometrioid features: report of a rare case and brief review of the literature

    We report the case of a 78-year-old male patient who presented with dysuria and lower urinary tract symptoms (LUTS). After routine blood and urine analysis, the patient underwent ultrasonography which revealed a hypertrophied and inhomogenous prostate extending to the bladder neck. Open biopsy from the bladder neck was performed. The histopathological findings were found to be consistent with prostatic duct adenocarcinoma with endometrioid features.Key Words: Prostate, duct, carcinoma, endometrioid, lower urinary tract symptoms (LUTS), dysuria

    Tuning of 2D rod-type photonic crystal cavity for optical modulation and impact sensing

    We propose a novel way of mechanical perturbation of photonic crystal cavities for on-chip applications. We utilize the equivalence of the 2D photonic crystals with perfect electric conductor (PEC) boundary conditions to the infinite height 3D counterparts for rod type photonic crystals. Designed structures are sandwiched with PEC boundaries above and below and the perturbation of the cavity structures is demonstrated by changing the height of PEC boundary. Once a defect filled with air is introduced, the metallic boundary conditions is disturbed and the effective mode permittivity changes leading to a tuned optical properties of the structures. Devices utilizing this perturbation are designed for telecom wavelengths and PEC boundaries are replaced by gold plates during implementation. For 10 nm gold plate displacement, two different cavity structures showed a 21.5 nm and 26 nm shift in the resonant wavelength. Optical modulation with a 1.3 MHz maximum modulation frequency with a maximum power consumption of 36.81 nW and impact sensing with 20 μs response time (much faster compared to the commercially available ones) are shown to be possible

    The association of calcaneal spur length and clinical and functional parameters in plantar fasciitis

    Introduction: Plantar fasciitis (PF)is the most common cause of plantar heel pain. Despite many treatment alternatives for heel spur, the association of calcaneal spur size with clinical and functional parameters is inconclusive. The objective of this study to investigate the correlation of calcaneal spur length with clinical findings and functional status documented with Foot Function Index in patients with plantar fasciitis. Methods: We performed power analysis for the sample size estimation. 87 patients with PF were scrutinized to reach the estimated patient number 75. Computer-aided linear measurements were done for spur length from tip to base in milimeters. Perceived pain intensity was evaluated by visual analog scale (VAS). Patients were asked to rate the pain experienced on a 10-cm VAS. Foot function index was applied to the patients to evaluate pain, disability and activity limitation of the patients. Results: Of the 75 participants, 24 were males (32%) and 51 were females (68%). The mean age was 47 ± 10 years (range 30-65 years). The mean calcaneal spur length was 3.86 ± 3.36 mm (range between 0 and 12.2). Calcaneal spur length was significantly correlated with age (p = 0.003), BMI (p = 0.029), symptom duration, (p = 0.001) VAS (p = 0.003), and FFI total score (p < 0.001). Discussion: Our study demonstrated that length of the calcaneal spur is significantly correlated with age, BMI, symptom duration, perceived pain, FFI pain and disability subscores, and FFI total scores. Conclusion: The size of the calcaneal spur is an important parameter correlated with pain and functional scores in PF. © 2015 IJS Publishing Group Limited

    Reconfigurable nested ring-split ring transmitarray unit cell employing the element rotation method by microfluidics

    A continuously tunable, circularly polarized X-band microfluidic transmitarray unit cell employing the element rotation method is designed and fabricated. The unit cell comprises a double layer nested ring-split ring structure realized as microfluidic channels embedded in Polydimethylsiloxane (PDMS) using soft lithography techniques. Conductive regions of the rings are formed by injecting a liquid metal (an alloy of Ga, In, and Sn), whereas the split region is air. Movement of the liquid metal together with the split around the ring provides 360° linear phase shift range in the transmitted field through the unit cell. A circularly polarized unit cell is designed to operate at 8.8 GHz, satisfying the necessary phase shifting conditions provided by the element rotation method. Unit cell prototypes are fabricated and the proposed concept is verified by the measurements using waveguide simulator method, within the frequency range of 8-10 GHz. The agreement between the simulation and measurement results is satisfactory, illustrating the viability of the approach to be used in reconfigurable antennas and antenna arrays. © 2015 IEEE

    RESCALE: Review and Simulate Climate and Catchment Responses at Burrishoole

    The climate of the Burrishoole catchment is projected to change significantly over the present century. Previous research of the catchment identified a scientific gap in knowledge in terms of understanding the implications of present and projected future changes in stream flow, water temperature, pH levels and DO concentrations on fish productivity in the catchment. To address this, a multidisciplinary team of scientists undertook an analysis of both present and likely future climate impacts on the catchment with a view to furthering the understanding of the inter-linkages between climate, climate change, and the freshwater ecosystem. The research findings outlined in the report provide climate change information at the catchment scale to assist catchment stakeholders in integrating climate change considerations into their decision-making processes. The report presents an in-depth assessment of the climate and environmental datasets from the catchment to establish if changes have occurred over the period of record. In order to assess the likely impacts of future changes in climate on the catchment, regional climate projections were developed and subsequently employed to simulate likely responses in stream flow and temperature, DOC and DO for the present century. The projected changes in both the climate and water-quality were then used to provide a basis for assessing impacts on fish growth and survival rates of salmonid and eel species in the catchment. The report provides a useful template for future studies, not just in the Burrishoole catchment but for other ecologically important catchments. The findings from the report are relevant to policy makers at the national scale; catchment managers at the regional scale; and, specifically, to stakeholders in the Burrishoole catchment, in developing adaptive responses to climate change.Funded under the Marine Research Sub-programme of the National Development Plan (2007-’13), as part of the Sea Change Strategy.Funder: Marine Institut

    Coronary artery bypass surgery in a patient with Kartagener syndrome: a case report and literature review

    Kartagener syndrome consists of congenital bronchiectasis, sinusitis, and total situs inversus in half of the patients. A patient diagnosed with Kartagener syndrome was reffered to our department due to 3-vessel coronary disease. An off-pump coronary artery bypass operation was performed using both internal thoracic arteries and a saphenous vein graft. We performed a literature review for cases with Kartagener syndrome, coronary surgery and dextrocardia. Although a few cases of dextrocardia were found in the literature, no case of Kartagener syndrome was mentioned

    Hemodynamic impact of isobaric levobupivacaine versus hyperbaric bupivacaine for subarachnoid anesthesia in patients aged 65 and older undergoing hip surgery

    BackgroundThe altered hemodynamics, and therefore the arterial hypotension is the most prevalent adverse effect after subarachnoid anesthesia. The objective of the study was to determine the exact role of local anesthetic selection underlying spinal anesthesia-induced hypotension in the elderly patient. We conducted a descriptive, observational pilot study to assess the hemodynamic impact of subarachnoid anesthesia with isobaric levobupivacaine versus hyperbaric bupivacaine for hip fracture surgery.DescriptionHundred twenty ASA status I-IV patients aged 65 and older undergoing hip fracture surgery were enrolled. The primary objective of our study was to compare hemodynamic effects based on systolic blood pressure (SBP) and dyastolic blood pressure (DBP) values, heart rate (HR) and hemoglobin (Hb) and respiratory effects based on partial oxygen saturation (SpO2%) values. The secondary objective was to assess potential adverse events with the use of levobupivacaine versus bupivacaine. Assessments were performed preoperatively, at 30 minutes into surgery, at the end of anesthesia and at 48 hours and 6 months after surgery.Among intraoperative events, the incidence of hypotension was statistically significantly higher (p <0.05) in group BUPI (38.3%) compared to group LEVO (13.3%). There was a decrease (p <0.05) in systolic blood pressure (SBP) and diastolic blood pressure (DBP) at 30 minutes intraoperatively (19% in group BUPI versus 17% in group LEVO). SpO2% increased at 30 minutes after anesthesia onset (1% in group BUPI versus 1.5% in group LEVO). Heart rate (HR) decreased at 30 minutes after anesthesia onset (5% in group BUPI versus 9% in group L). Hemoglobin (Hb) decreased from time of operating room (OR) admission to the end of anesthesia (9.3% in group BUPI versus 12.5% in group LEVO). The incidence of red blood cell (RBC) transfusion was 13.3% in group BUPI versus 31.7% in group LEVO, this difference was statistically significant. Among postoperative events, the incidence of congestive heart failure (CHF) was significantly higher in group BUPI (8,3%). At 6 months after anesthesia, no differences were found.ConclusionsGiven the hemodynamic stability and lower incidence of intraoperative hypotension observed, levobupivacaine could be the agent of choice for subarachnoid anesthesia in elderly patients
