Increased risk of chikungunya infection in travellers to Thailand during ongoing outbreak in tourist areas : cases imported to Europe and the Middle East, early 2019
We report nine travellers with confirmed chikungunya virus infection, returning from tourist areas of Thailand to Sweden, Switzerland, the United Kingdom, Romania, Israel and France, diagnosed in January and February 2019. These sentinel tourists support the intensification of chikungunya virus circulation in Thailand and highlight the potential for importation to areas at risk of local transmission
Chikungunya Virus Infection
Chikungunya virus (CHIKV) is an alphavirus transmitted by mosquitoes, mostly Aedes aegypti and Aedes albopictus. After half a century of focal outbreaks of acute febrile polyarthralgia in Africa and Asia, the disease unexpectedly spread in the past decade with large outbreaks in Africa and around the Indian Ocean and rare autochthonous transmission in temperate areas. This emergence brought new insights on its pathogenesis, notably the role of the A226V mutation that improved CHIKV fitness in Ae. albopictus and the possible CHIKV persistence in deep tissue sanctuaries for months after infection. Massive outbreaks also revealed new aspects of the acute stage: the high number of symptomatic cases, unexpected complications, mother-to-child transmission, and low lethality in debilitated patients. The follow-up of patients in epidemic areas has identified frequent, long-lasting, rheumatic disorders, including rare inflammatory joint destruction, and common chronic mood changes associated with quality-of-life impairment. Thus, the globalization of CHIKV exposes countries with Aedes mosquitoes both to brutal outbreaks of acute incapacitating episodes and endemic long-lasting disorders
Prise en charge spécialisée de manifestations rhumatismales chroniques post-chikungunya (étude rétrospective de 159 cas, Saint-Denis de la Réunion, 2006-2012)
Le chikungunya (CHIK) est un alphavirus devenu pandémique responsable de troubles ostéo-articulaires parfois durables avec un lourd retentissement médico-économique. Le spectre clinique de la phase post-CHIK (pCHIK) est riche et des recommandations de donnes protiques sont indispensables. PATIENTS et METHODE: Nous avons recueilli rétrospectivement sur 6 ans les données cliniques et thérapeutiques de patient référés à l'équipe rhumatologique de Saint Denis, La Réunion pour des douleurs persistantes après une infection CHIK confirmée. Les rhumatismes inflammatoire chroniques (RIC) de novo étaient catégorisés selon des critères validés et l'efficacité du méthotrexate (MTX) a été évaluée. RESULTATS: 159 dossiers médicaux ont été étudiées. Les troubles musculo-squelettiques (TMS) étaient adressés précocement au spécialiste et comptait surtout des pathologies mécaniques de l'épaule, des syndrome canalaires et de polyarthralgies distales bilatérales parfois oedémateuse répondant bien à une corticothérapie courte. 94 patients présentaient un RIC de novo pCHIK: 40 cas de polyarthrite rhumatoïde, 33 de spondylarthrite et 21 de polyarthrite indifférenciée; la moitié présentait des destructions osseuses. 72 RIC ont été traités par MTX avec 75% d'efficacité significativement associée à un début précoce et une intégrité osseuse à l'introduction.
