    Use of routinely collected national data sets for reporting on induced abortion in Australia

    Foreword The lack of national data on induced abortion in Australia represents a gap in health statistics. The AIHW’s Reproductive Health Indicators in Australia 2002 report included an indicator on induced abortions in Australia, but national data were not reported for it because data on induced abortion were not available on a routine basis Australia-wide. This report comprehensively assesses the extent to which different forms of routinely collected data can be used to quantify the incidence of induced abortion in Australia. The innovative use of data combined from hospital and non-hospital sources helps to provide a more complete picture of reproductive health in Australia, as well as providing a basis for regular reporting in the future. The compilation of the data contained in this document represents the best effort to date to provide a factual database on the incidence of induced abortion. The report does not include any analysis of the legal, social or moral issues often raised in discussion of abortion

    “Speak for Yourself”: Ovidian Women and the Suppression of Voice and Complaint in \u3ci\u3eMetamorphoses\u3c/i\u3e and \u3ci\u3eHeroides\u3c/i\u3e

    Ovid’s portrayal and attitude towards women is one that is particularly puzzling and contradictory throughout his Metamorphoses and Heroides. Recent scholarship on Ovidian literature is only divided on whether or not Ovid’s intentions within these two works were to sympathize with the Roman woman’s experience or to reinforce the lack of female representation in Roman society; however, I argue that Ovid fails to achieve empathy for the Roman woman. In Heroides, these women are pining and tragic, often meeting some terrible fate shortly after being abandoned by their suitors and putting forth a complaint. Conversely, women in Metamorphoses also issue complaints, and as a result, are raped, mutilated, and transformed. In both texts, female desires and voices are ultimately rejected or invalidated in some manner that ridicules or objectifies the woman. The combination of euphemistic language and avoidance of words like “rape,” a lack of female agency and choice, dehumanization and violence on women, as well as stereotypical presentations of female characters all point to Ovid’s ventriloquization of female voice, complaints and desires.     Activity and interactions of methane seep microorganisms assessed by parallel transcription and FISH-NanoSIMS analyses

    To characterize the activity and interactions of methanotrophic archaea (ANME) and Deltaproteobacteria at a methane-seeping mud volcano, we used two complimentary measures of microbial activity: a community-level analysis of the transcription of four genes (16S rRNA, methyl coenzyme M reductase A (mcrA), adenosine-5â€Č-phosphosulfate reductase α-subunit (aprA), dinitrogenase reductase (nifH)), and a single-cell-level analysis of anabolic activity using fluorescence in situ hybridization coupled to nanoscale secondary ion mass spectrometry (FISH-NanoSIMS). Transcript analysis revealed that members of the deltaproteobacterial groups Desulfosarcina/Desulfococcus (DSS) and Desulfobulbaceae (DSB) exhibit increased rRNA expression in incubations with methane, suggestive of ANME-coupled activity. Direct analysis of anabolic activity in DSS cells in consortia with ANME by FISH-NanoSIMS confirmed their dependence on methanotrophy, with no ^(15)NH^+_4 assimilation detected without methane. In contrast, DSS and DSB cells found physically independent of ANME (i.e., single cells) were anabolically active in incubations both with and without methane. These single cells therefore comprise an active ‘free-living’ population, and are not dependent on methane or ANME activity. We investigated the possibility of N_2 fixation by seep Deltaproteobacteria and detected nifH transcripts closely related to those of cultured diazotrophic Deltaproteobacteria. However, nifH expression was methane-dependent. ^(15)N_2 incorporation was not observed in single DSS cells, but was detected in single DSB cells. Interestingly, ^(15)N_2 incorporation in single DSB cells was methane-dependent, raising the possibility that DSB cells acquired reduced ^(15)N products from diazotrophic ANME while spatially coupled, and then subsequently dissociated. With this combined data set we address several outstanding questions in methane seep microbial ecosystems and highlight the benefit of measuring microbial activity in the context of spatial associations

    Exploring an Existing Weight Management App for Use With Adolescents and Young Adults With Spina Bifida: Usability Study

    Background: Adolescents and young adults with spina bifida (AYA-SBs) have unique user needs, given their variable and complex symptom profile. Owing to multiple barriers to prevention and intervention treatments for secondary conditions (eg, obesity), AYA-SBs may benefit from the use of behavioral intervention technologies (BITs). However, as BITs are often designed and tested with typically developing individuals, it is unclear if existing BITs may be usable for AYA-SBs. Objective: This study aimed to evaluate the usability of a high-quality, publicly available, weight management–focused mobile BIT (smartphone app) for AYA-SBs. Methods: Overall, 28 AYA-SBs attending a Young Men’s Christian Association–based summer camp completed 4 structured usability tasks using a weight management app designed for the general public called My Diet Coach (Bending Spoons). Learnability was measured by (1) time to complete task, (2) number of user errors, and (3) correct entry of data when requested by the app. Satisfaction and general usability were measured via self-reported questionnaires and qualitative feedback following interactions with the app. Results: The majority of the sample were able to complete the tasks, with increased completion rates and improved times on second attempts of the tasks (Ps Conclusions: AYA-SBs were able to learn how to complete specific tasks independently on a weight management app, but design changes consistent with previously proposed user needs were recommended. Rather than designing entirely new BITs, it may be possible to adapt existing technologies to personalize BITs for specific populations such as AYA-SBs

    Unbalanced and unbalancing acts in the Early Years Foundation Stage: a critical discourse analysis of policy-led evidence on teaching and play from the office for standards in education in England (Ofsted)

    This paper presents a critical analysis of a report by the Office for Standards in Education in England based on a survey of practitioners’ perspectives of play, focusing on children age 2–5 years in the Early Years Foundation Stage (EYFS). The report, ‘Teaching and play in the early years – a balancing act? A good practice survey to explore perceptions of teaching and play in the early years’ www.gov.uk/government/organisations/Ofsted addressed the ‘recurring myth’ that teaching and play are disconnected endeavours. Critical discourse and critical policy analysis are used to interrogate this report alongside interrelated texts, the socio-political context of production, the methods used to define policy versions of ‘good practice’ in play, and the resulting power effects within early childhood education (ECE). It is argued that this report exemplifies how the remit of Ofsted has extended to provide policy-led evidence that is based on the construction of ‘problems’ of practice, and the proposed solutions. The Ofsted version of teaching and play relies on circular discourses that reinforce the government’s standards and accountability agendas. This analysis reveals the extension from Ofsted’s remit of inspection, to a mandate for defining ‘quality’ and ‘good’ practice. These actions, relationships and processes constitute the ‘meaning-laden architectures’ [Fairclough, N., P. Graham, J. Lemke, and R. Wodak. 2004. “Introduction.” Critical Discourse Studies 1: 1–7] that connect discourse and power. Using the metaphor of a kaleidoscope, it is proposed that ‘Teaching and Play’, and related reports, represent unbalanced and unbalancing acts in which policy-led evidence, based on flawed and biased ‘research’, exerts power effects for children, families and practitioners

    High-Resolution Melting Genotyping of Enterococcus faecium Based on Multilocus Sequence Typing Derived Single Nucleotide Polymorphisms

    We have developed a single nucleotide polymorphism (SNP) nucleated high-resolution melting (HRM) technique to genotype Enterococcus faecium. Eight SNPs were derived from the E. faecium multilocus sequence typing (MLST) database and amplified fragments containing these SNPs were interrogated by HRM. We tested the HRM genotyping scheme on 85 E. faecium bloodstream isolates and compared the results with MLST, pulsed-field gel electrophoresis (PFGE) and an allele specific real-time PCR (AS kinetic PCR) SNP typing method. In silico analysis based on predicted HRM curves according to the G+C content of each fragment for all 567 sequence types (STs) in the MLST database together with empiric data from the 85 isolates demonstrated that HRM analysis resolves E. faecium into 231 “melting types” (MelTs) and provides a Simpson's Index of Diversity (D) of 0.991 with respect to MLST. This is a significant improvement on the AS kinetic PCR SNP typing scheme that resolves 61 SNP types with D of 0.95. The MelTs were concordant with the known ST of the isolates. For the 85 isolates, there were 13 PFGE patterns, 17 STs, 14 MelTs and eight SNP types. There was excellent concordance between PFGE, MLST and MelTs with Adjusted Rand Indices of PFGE to MelT 0.936 and ST to MelT 0.973. In conclusion, this HRM based method appears rapid and reproducible. The results are concordant with MLST and the MLST based population structure

    Feasibility study of portable technology for weight loss and HbA1c control in type 2 diabetes

    Background The study investigated the feasibility of conducting a future Randomised Controlled Trial (RCT) of a mobile health (mHealth) intervention for weight loss and HbA1c reduction in Type 2 Diabetes Mellitus (T2DM). Methods The intervention was a small wearable mHealth device used over 12 weeks by overweight people with T2DM with the intent to lose weight and reduce their HbA1c level. A 4 week maintenance period using the device followed. The device records physical activity level and information about food consumption, and provides motivational feedback based on energy balance. Twenty-seven participants were randomised to receive no intervention; intervention alone; or intervention plus weekly motivational support. All participants received advice on diet and exercise at the start of the study. Weight and HbA1c levels were recorded at baseline and weeks 6, 12, and 16. Qualitative interviews were conducted with participants who received the intervention to explore their experiences of using the device and involvement in the study including the training received. Results Overall the device was perceived to be well-liked, acceptable, motivational and easy to use by participants. Some logistical changes were required during the feasibility study, including shortening of the study duration and relaxation of participant inclusion criteria. Descriptive statistics of weight and HbA1c data showed promising trends of weight loss and HbA1c reduction in both intervention groups, although this should be interpreted with caution. Conclusions A number of methodological recommendations for a future RCT emerged from the current feasibility study. The mHealth device was acceptable and promising for helping individuals with T2DM to reduce their HbA1c and lose weight. Devices with similar features should be tested further in larger studies which follow these methodological recommendations

    Engineering of a calcium-ion binding site into the RC-LH1-PufX complex of Rhodobacter sphaeroides to enable ion-dependent spectral red-shifting

    The reaction centre-light harvesting 1 (RC-LH1) complex of Thermochromatium (Tch.) tepidum has a unique calcium-ion binding site that enhances thermal stability and red-shifts the absorption of LH1 from 880 nm to 915 nm in the presence of calcium-ions. The LH1 antenna of mesophilic species of phototrophic bacteria such as Rhodobacter (Rba.) sphaeroides does not possess such properties. We have engineered calcium-ion binding into the LH1 antenna of Rba. sphaeroides by progressively modifying the native LH1 polypeptides with sequences from Tch. tepidum. We show that acquisition of the C-terminal domains from LH1 α and ÎČ of Tch. tepidum is sufficient to activate calcium-ion binding and the extent of red-shifting increases with the proportion of Tch. tepidum sequence incorporated. However, full exchange of the LH1 polypeptides with those of Tch. tepidum results in misassembled core complexes. Isolated α and ÎČ polypeptides from our most successful mutant were reconstituted in vitro with BChl a to form an LH1-type complex, which was stabilised 3-fold by calcium-ions. Additionally, carotenoid specificity was changed from spheroidene found in Rba. sphaeroides to spirilloxanthin found in Tch. tepidum, with the latter enhancing in vitro formation of LH1. These data show that the C-terminal LH1 α/ÎČ domains of Tch. tepidum behave autonomously, and are able to transmit calcium-ion induced conformational changes to BChls bound to the rest of a foreign antenna complex. Thus, elements of foreign antenna complexes, such as calcium-ion binding and blue/red switching of absorption, can be ported into Rhodobacter sphaeroides using careful design processes
