    A self-directed learning intervention for radiographers rating mammographic breast density

    Purpose: Subjective methods of mammographic breast density (MBD) assessment are prone to inter-reader variability. This work aims to assess the impact of a short self-directed experiential learning intervention on radiographers’ reproducibility of MBD assessment. Method: The study used two sets of images (test and learning intervention) containing left craniocaudal and left mediolateral oblique views. The test set had MBD ratings from VolparaTM and radiologists using the fourth edition Breast Imaging and Data Systems (BI-RADS¼). Seven radiographers rated the MBD of the test set before and after a self-directed learning intervention using the percentage descriptors in the fourth edition BI-RADS¼ Atlas. The inter-reader agreement, agreement between radiographers and VolparaTM as well as radiologist, was assessed using a Weighted Kappa (Kw). Results: Overall, radiographers’ inter-reader agreement (Kw) was substantial (0.79; 95%CI: 0.70–0.87) before the intervention and almost perfect (0.84; 95%CI: 0.77–0.90) after the intervention. Before the intervention, radiographers demonstrated fair agreement with radiologists (0.24; 95%CI:-0.46–0.61) and VolparaTM (0.24; 95% CI: -0.41–0.59). A fair but slightly improved agreement was also observed between radiographers and radiologists (0.31; 95% CI: -0.33 - 0.64) as well as VolparaTM (0.28; 95% CI: -0.34- 0.61) after the intervention. Conclusion: Findings demonstrate that a short duration self-directed experiential learning intervention reduces inter-reader differences in MBD classification, but has a negligible impact on improving the agreement between inexperienced and expert readers

    Extracts of Pterocarpus osun as a histological stain for collagen fibres

    The staining ability of Pterocarpus osun extract on tissue sections was determined. 2 kg of P. osun stem was dried, milled to obtain a fine powder and a red pigment extracted from the powder with 1 L of 70% ethanol at 78°C for 24 h. The alcoholic and acidic extracts were used to stain tissue sections. Collagen fibres, red blood cells and muscles were stained in shades of reddish brown.     Effects of ethanolic and aqueous leaf extracts of Bryophyllum pinnatum on haematological parameters of normal and streptozotocin–induced diabetic rats

    The effect of the oral administration of both ethanolic and aqueous leaf extracts from Bryophyllum pinnatum (BP) on haematological parameters of normal and streptozotocin - induced (STZ) diabetic rat was investigated. Twenty-five male Wister rats were used and divided into five groups of five rats each. They were designated as (Normal Control – NC, Diabetic Control – DC, Diabetic Ethanolic Extract – DEE, Diabetic Aqueous Extract – DAE, and Normal Aqueous Extract – NAE). Groups NC and DC served as ‘control’ animals receiving food and water only. Groups DC, DEE and DAE were injected intraperitoneally with 65mg/kg body weight streptozotocin. Induction of diabetes mellitus was confirmed after 48 hours using glucose test strips. The test rats were all treated with 100mg/kgbwt ethanolic and aqueous leaf extracts of Bryophyllum pinnatum for 28days. At the end of the 28days, the rats were sacrificed and whole blood collected for Haematological assay. Results obtained showed a significant difference(P<0.05) in White Blood Cell (WBC), Red Blood Cell (RBC), Platelet (PLT), Haemoglobin (HBG), Packed Cell Volume (PCV), Mean Cell Volume (MCV), Mean Cell Haemoglobin (MCH), Mean Cell Haemoglobin Concentration (MCHC) and Lymphocytes (LYM) in treated rats with BP leaf extracts when compared to the diabetic and normal control groups

    This is the management of a 30-year-old male student, a passenger on anbsp motor cycle who had an open tibial fracture. He was resuscitated by the Accident and Emergency doctors before the Orthopeadic and Plastic surgery units were invited to take over the management. The fracture was reduced and maintained with external fixators by the Orthopeadic surgeons and an augmented fascial flap and a split thickness skin graft were used to cover the fracture by the plastic surgical team.     Investigating the Effect of Withania somnifera, Selenium and Hydrocortisone on Blood Count and Bronchial Lavage of Experimental Asthmatic BALB/c Mice

    The murine asthmatic Balb/c mouse model was used to investigate the effect of W. somnifera L., the  antioxidant selenium, W. somnifera and selenium in combination and hydrocortisone as positive control  on the number of white blood cells in blood smears and bronchial lavage smears as well as the platelet  distribution of asthmatic Balb/c mice. The mice were sensitised, nebulized and treated over a period of  43 days and blood smears were made of each individual animal and bronchial lavage was performed by  injecting 0.3ml of saline into the trachea of the mice, both on the day of termination. Light microscopy  analysis of the bronchial lavage revealed a significant decrease in the number of eosinophils counted in the  asthmatic and the different treatment groups. In the asthmatic group, numerous platelet clumps were found  distributed between white blood cells. Platelets were also found in the other treatment groups but are not  as prevalent as in the asthmatic group. Results from the blood smears showed the same trends, where cell  counts in control and hydrocortisone blood smears were decreased compared to that of the asthma group. It  is concluded that W. somnifera and W. somnifera combined with selenium significantly decreased the white  blood cells in both bronchial lavage as well as blood smears, suggesting that W. somnifera indeed has an  anti-inflammatory potential and it, in combination with an anti-oxidant like selenium, might successfully  be used in the treatment of asthma.

    Biting behaviour of Simulium damnosum complex and Onchocerca volvulus infection along the Osun River, Southwest Nigeria

    <p>Abstract</p> <p>Background</p> <p>Studies on biting behaviours and infectivity status of insect vectors are pre-requisites in understanding the epidemiology of the vector- borne diseases and planning effective control measures. A longitudinal study was carried out to investigate the transmission index of <it>Simulium damnosum </it>complex species along Osun River, South Western Nigeria. Adult flies were collected on human attractants from 07:00 to 18:00 hours for two consecutive days from February 2008 to June 2009 at three communities: Osun Eleja, Osun Ogbere and Osun Budepo. The infectivity rate was determined by dissection and Polymerase Chain Reaction amplification (PCR) of 0-150 genes of <it>Onchocerca </it>parasite using the pool screening technique.</p> <p>Results</p> <p>The results indicated that the majority of the flies collected at the three sampling points were nulliparous as they accounted for 53.90%, 57.86% and 59.58% of the flies dissected at Osun Budepo, Osun Ogbere and Osun Eleja, respectively. The parous rate was higher during the dry season than the wet season but the difference was not statistically significant (<it>p </it>< 0.05). The biting activity of the parous flies showed two peaks at Osun Budepo and three peaks at Osun Eleja and Osun Ogbere. Of the 1,472 flies dissected and 1,235 flies screened by molecular method, none was infected with <it>Onchocerca </it>parasite at the three sampling points however the annual biting rates at the three communities were higher than 1,000 considered as tolerable value for a person living in an onchocerciasis zone by Word Health Organization.</p> <p>Conclusion</p> <p>The study has provided the baseline data for further study on onchocerciasis transmission dynamics and the need to intercept man- simuliid vector contact at the study area.</p

    A giant ectopic hidradenoma papilliferum in a Niger delta region of Nigeria

    Hidradenoma papilliferum is a known example of adnexal skin tumours with apocrine differentiation. It is a rare benign tumour which tends to arise from areas with rich concentration of aporine glands such as anogenital region, vulval, perineal, axillae, and periumbilical areas. In this report, the tumour was found in the upper outer quadrant of left breast, being one of the ectopic sites for this tumour. Contrary to most reports where male preponderance was popular for ectopic hidradenoma papilliferum, the patient in this report is a 71-year-old female. Considering the location of this tumour in this report, the likely histopathological differential diagnoses such as tubular apocrine adenoma, clear cell (apocrine) adenoma, lipoma, intraductal papilloma and papillary carcinoma of the breast should be considered for exclusion. This is the first reported case of a giant ectopic hidradenoma papilliferum of the breast in a Niger Delta region of Nigeria which also highlights the role of fine needle aspiration and cytology in the diagnosis of breast lesions

    A Critical Success Factor Framework for Implementing Sustainable Innovative and Affordable Housing: A Systematic Review and Bibliometric Analysis

    The actualization of affordable housing remains a challenge. This challenge is exacerbated by the increasing societal demand for the incorporation of sustainability principles into such housing types to improve levels of occupant health and well-being whilst avouching the desired levels of affordability. Innovative technologies and practices have been described as beneficial to the effectuation of sustainable affordable housing. However, knowledge concerning the deployment of innovative technologies and practices in sustainable affordable housing (sustainable, innovative, affordable housing—SIAH) delivery remains nascent. Consequently, there is a lack of a common ontology among stakeholders concerning how to realize SIAH. This study aims to contribute toward the development of this body of knowledge through the establishment of the critical success factors (CSFs) for effective SIAH implementation. To achieve this objective, a systematic review and bibliometric analysis focusing on a juxtaposition of sustainable, innovative and affordable housing concepts was carried out based on the relevant literature. This led to the identification and clustering of CSFs for these housing concepts at individual levels and as a collective (SIAH). The findings of the study consisted of the establishment of four distinct yet interrelated facets through which SIAH can be achieved holistically, namely, housing design, house element, housing production method and housing technology. A total of 127 CSFs were found to be aligned to these facets, subsequently clustered, and conclusively used for the development of a SIAH CSF framework. The most frequently occurring CSFs with predominant interconnections were the utilization of energy-efficient systems/fittings, tenure security, a comfortable and healthy indoor environment, affordable housing price in relation to income and using water-efficient systems/fittings CSFs, and establishing the emergent SIAH CSF framework. The framework in this study is useful in the documentation of SIAH features for construction projects and further studies into SIAH CSFs

    Geographical information system and predictive risk maps of urinary schistosomiasis in Ogun State, Nigeria

    <p>Abstract</p> <p>Background</p> <p>The control of urinary schistosomiasis in Ogun State, Nigeria remains inert due to lack of reliable data on the geographical distribution of the disease and the population at risk. To help in developing a control programme, delineating areas of risk, geographical information system and remotely sensed environmental images were used to developed predictive risk maps of the probability of occurrence of the disease and quantify the risk for infection in Ogun State, Nigeria.</p> <p>Methods</p> <p>Infection data used were derived from carefully validated morbidity questionnaires among primary school children in 2001–2002, in which school children were asked among other questions if they have experienced "blood in urine" or urinary schistosomiasis. The infection data from 1,092 schools together with remotely sensed environmental data such as rainfall, vegetation, temperature, soil-types, altitude and land cover were analysis using binary logistic regression models to identify environmental features that influence the spatial distribution of the disease. The final regression equations were then used in Arc View 3.2a GIS software to generate predictive risk maps of the distribution of the disease and population at risk in the state.</p> <p>Results</p> <p>Logistic regression analysis shows that the only significant environmental variable in predicting the presence and absence of urinary schistosomiasis in any area of the State was Land Surface Temperature (LST) (B = 0.308, p = 0.013). While LST (B = -0.478, p = 0.035), rainfall (B = -0.006, p = 0.0005), ferric luvisols (B = 0.539, p = 0.274), dystric nitosols (B = 0.133, p = 0.769) and pellic vertisols (B = 1.386, p = 0.008) soils types were the final variables in the model for predicting the probability of an area having an infection prevalence equivalent to or more than 50%. The two predictive risk maps suggest that urinary schistosomiasis is widely distributed and occurring in all the Local Government Areas (LGAs) in State. The high-risk areas (≄ 50% prevalence) however, are confined to scatter foci in the north western part of the State. The model also estimated that 98.99% of schools aged children (5–14 years) are living in areas suitable for urinary schistosomiasis transmission and are at risk of infection.</p> <p>Conclusion</p> <p>The risk maps developed will hopefully be useful to the state health officials, by providing them with detailed distribution of urinary schistosomiasis, help to delineate areas for intervention, assesses population at risk thereby helping in optimizing scarce resources.</p

    Radiation protection value to the operator from augmented reality smart glasses in interventional fluoroscopy procedures using phantoms

    Introduction: Smart glasses can be adapted to display radiographic images to allow clinician’s gaze not to be directionally fixed or predetermined by computer monitor location. This study presents an analysis of eye lens dose during interventional fluoroscopy guided procedures, comparing fixed monitor positions against the use of smart glasses. Methods: Using a head phantom (simulating the clinician), thermoluminescent dosimeters and lead shielded glasses, the dose to the eye was measured for different head ‘rotations and tilts’ for: gaze directed towards the main scattering source (patient / primary beam) to represent potential gaze direction if smart glasses are used; gaze directed to a range of potential computer monitor positions. An anthropomorphic pelvis phantom was utilised to simulate the patient. Accumulated dose rates (”Gy.sˉÂč) from five 10-second exposures at 75 kV 25.2 mAs were recorded. Results: An average DAP reading of 758.84 cGy.cm2 was measured during each 10 second exposure. Whilst wearing lead shielded glasses a 6.10 – fold reduction in dose rate to the lens is possible (p&lt;0.05). Influence of the direction of gaze by the clinician demonstrated a wide range of dose rate reduction from 3.13% (p=0.16) to 143.69% (p&lt;0.05) when the clinician’s gaze was towards the main scattering source. Increased dose rate to the clinician’s eyes was received despite wearing lead shielded glasses, as the angle of gaze moved 45Âș and 90Âș from 0Âș. Conclusion: If the clinician’s gaze is directed towards the main scattering source a potential exists for reducing eye lens dose compared with fixed location computer monitors. Introduction of lead lined smart glasses into interventional radiology may lead to improvements in patient care, reducing the need for the clinician to look away from the patient to observe a radiographic image
