    Current antibiotic susceptibility profile of the bacteria associated with Surgical wound infections in the Buea health district in Cameroon

    Background: Most surgical wounds seen in clinical practice in the Buea Health District, Cameroon are infected prior to arrival or while they are in the hospital. Sometimes the infection necessitates a combination of local wound site measures and systemic antibiotherapy to properly manage the patient.Objective: To identify the current antibiotic susceptibility profile of the common germs that cause surgical wound infections in the Buea Health District of Cameroon.Methods: A total of 2120 specimens comprising swabs from burns, ulcers, open or post-operative wounds were collected from hospitalized patients attending health institutions in Buea. The samples were collected from different anatomic sites of the patients. Cultures were effected from the specimens and bacteria isolated from infected wounds using standard microbiological techniques. Antibiotic susceptibility of the different isolates was determined.Results: Majority (79.8%) of the wounds were infected with pathogenic bacteria. The germs globally showed multi resistant patterns to commonly used antibiotics in the study area, especially to co-trimoxazol, doxycycline, chloramphemicol, ampicilline and aztreonam. However appreciable sensitivity was noted to ofloxacillin, perflacin, and ceftriazone.Conclusion: This study has revealed ofloxacin as the only antibiotic to which all the isolated bacteria from infected wounds were sensitive in the study area.Keywords: Wound infection, Antibiotic susceptibility profile, Buea Health District, Cameroo

    Evaluation of quality of life and associated factors among HIV patients on antiretroviral therapy in North West region of Cameroon

    Background: There is evidence that Quality of Life (QoL) of People Living with HIV/AIDS (PLHIV) has a significant role in ART retention, treatment adherence, and survival. As a result, QoL is becoming increasingly important for policymakers, program implementers, and researchers. However, factors associated with QoL, in a culturally diverse country like Cameroon are unknown. Objective: We aimed to assess the QoL of PLHIV on ART and assess the extent to which physical, psychosocial, environmental, and spiritual factors drive QoL. Method: A cross-sectional study was conducted among 394 PLHIV aged >21 in North-West Cameroon from April to July 2019. Data were collected using WHO-QOL BREF questionnaire. Descriptive statistics, bivariate, and multivariate linear regression analyses were performed. Results: Majority (34.5%) of participants were in the age range of 41-50, with 73% females. The average QoL of the respondents was \u201cgood\u201d with mean score of 3.57 on 5 and 71.4% agreed to have satisfactory QoL. Bivariate regression analyses revealed that all six proposed predictors were significantly associated with QoL. Psychological factors made the greatest impact (\u3b2 = 0.213; p<0.003), followed by physical factors (\u3b2 = 0.19; p<0.001). Conclusion: PLHIV fairly agreed to have good QoL. The QoL was driven by mainly psychological and physical factors and not level of independence. However, the mean score perceptions for the investigated domains were low. Mental health services should consider these predictors when designing strategies to improve the QoL of PLHIV. While this study provides useful insights, other possible drivers of QoL among PLHIV should be investigated

    Hepatitis B, HIV, and Syphilis Seroprevalence in Pregnant Women and Blood Donors in Cameroon

    Objectives. We estimated seroprevalence and correlates of selected infections in pregnant women and blood donors in a resourcelimited setting. Methods. We performed a cross-sectional analysis of laboratory seroprevalence data from pregnant women and voluntary blood donors from facilities in Cameroon in 2014. Rapid tests were performed to detect hepatitis B surface antigen, syphilis treponemal antibodies, and HIV-1/2 antibodies. Blood donations were also tested for hepatitis C and malaria. Results. The seroprevalence rates and ranges among 7069 pregnant women were hepatitis B 4.4% (1.1-9.6%), HIV 6% (3.0-10.2%), and syphilis 1.7% (1.3-3.8%) with significant variability among the sites. Correlates of infection in pregnancy in adjusted regression models included urban residence for hepatitis B (aOR 2.9, CI 1.6-5.4) and HIV (aOR 3.5, CI 1.9-6.7). Blood donor seroprevalence rates and ranges were hepatitis B 6.8% (5.0-8.8%), HIV 2.2% (1.4-2.8%), syphilis 4% (3.3-4.5%), malaria 1.9%, and hepatitis C 1.7% (0.5-2.5%). Conclusions. Hepatitis B, HIV, and syphilis infections are common among pregnant women and blood donors in Cameroon with higher rates in urban areas. Future interventions to reduce vertical transmission should include universal screening for these infections early in pregnancy and provision of effective prevention tools including the birth dose of univalent hepatitis B vaccine

    Ten golden rules for optimal antibiotic use in hospital settings: the WARNING call to action

    Antibiotics are recognized widely for their benefits when used appropriately. However, they are often used inappropriately despite the importance of responsible use within good clinical practice. Effective antibiotic treatment is an essential component of universal healthcare, and it is a global responsibility to ensure appropriate use. Currently, pharmaceutical companies have little incentive to develop new antibiotics due to scientific, regulatory, and financial barriers, further emphasizing the importance of appropriate antibiotic use. To address this issue, the Global Alliance for Infections in Surgery established an international multidisciplinary task force of 295 experts from 115 countries with different backgrounds. The task force developed a position statement called WARNING (Worldwide Antimicrobial Resistance National/International Network Group) aimed at raising awareness of antimicrobial resistance and improving antibiotic prescribing practices worldwide. The statement outlined is 10 axioms, or “golden rules,” for the appropriate use of antibiotics that all healthcare workers should consistently adhere in clinical practice

    Bladder exstrophy – epispadias complex in a newborn: a case report and review of literature

    Background: Bladder exstrophy-epispadias complex (BEEC) is a rare congenital malformation of the genitourinary system usually associated with other malformations. Case summary: We report the case of a new born delivered vaginally at 39 weeks and 2 days of gestation with BEEC. Three prenatal ultrasound scans missed the diagnosis. Medical management was provided and the patient was discharged against medical advice 48 hours later upon refusing surgical intervention. Conclusion: The diagnosis and management of BEEC is a challenge in sub-Sahara Africa. Radiologists doing prenatal ultrasounds should check routinely to exclude foetal anomalies

    Assessment of environmental and farm business impacts of phosphorus policies in two European regions

    This paper presents analyses of the economic and environmental impact of relevant phosphorus management policy scenarios, using representative pig and poultry farms in the Emilia-Romagna region (Po River basin, Italy) and Niedersachsen region (Ems River basin, Germany) as case studies. The analysis was done by using a farm-level linear programming bioeconomic model developed for different farm and animal types-sows, fattening pigs, laying hens and broilers. The baseline for the assessed scenarios involved farms situated within the Nitrate Vulnerable Zones (NVZs) and therefore, were subject to indirect phosphorus fertilization limitations via the Nitrates Directive (ND). The analyzed phosphorus management policy scenarios included the implementation of two different balances of 8.7 kg and 4.35 kg of P per ha per year, inspired by the German Fertilizer Ordinance for phosphorus fertilization implemented in 2007 and later updated from 2017 up to 2023, respectively. We also included a more rigorous zero P balance scenario. The results of the scenario simulations, based on model assumptions, reveal that the introduction of direct phosphorus management policies for pig and poultry farms situated in high livestock dense regions can be done without causing any significant impact on farm gross margin (around 2% of reduction) for both regions and all four animal types, except for sows and broiler production in the Ems River Basin (up to 12% of reduction). Selected technologies and methods, inspired by current practices, have been analyzed for their cost efficiency to achieve the target P balance of the individual scenarios, including export of slurry out of the farm or export of separation solids. Results also highlight that the Nitrates Directive alone is not enough to handle the P issue in monogastric livestock farms

    A Community-Based Assessment of Knowledge and Practice of Breast Self-Examination and Prevalence of Breast Disease in Southwest Cameroon

    Introduction. Despite the rising trend in breast cancer incidence and mortality across Sub-Saharan Africa, there remains a critical knowledge gap about the burden and patterns of breast disease and breast cancer screening practices at the population level. This study aimed to identify socioeconomic factors associated with knowledge and practice of breast self-examination (BSE) as well as assess the prevalence of breast disease symptoms among a mixed urban-rural population of women in the Southwest region of Cameroon. Methods. We conducted a household-level community-based study in Southwest Cameroon between January and March 2017, using a three-stage cluster sampling framework. We surveyed 1287 households and collected self-reported data on 4208 female subjects, 790 of whom were household representatives. Each household representative provided information on behalf of all female household members about any ongoing breast disease symptoms. Moreover, female household representatives were questioned about their own knowledge and practice of BSE. Results. Women demonstrated low frequency of knowledge of BSE, as 25% (n=201) of household representatives reported any knowledge of BSE; and among these only 15% (n=30) practiced BSE on a monthly basis. Age (aOR: 1.04), usage of Liquid Petroleum Gas fuel, a marker of higher socioeconomic status (aOR: 1.86), and speaking English as a primary language in the household (aOR: 1.59) were significant predictors of knowledge of BSE. Eleven women reported ongoing breast disease symptoms resulting in an overall prevalence of 2.3 cases of breast disease symptoms per 1000 women. Conclusions. Socioeconomic disparities in access to health education may be a determinant of knowledge of BSE. Community-based strategies are needed to improve dissemination of breast cancer screening methods, particularly for women who face barriers to accessing care
