    Computer Administered Safety Planning for Individuals at Risk for Suicide: Development and Usability Testing

    BACKGROUND: Safety planning is a brief intervention that has become an accepted practice in many clinical settings to help prevent suicide. Even though it is quick compared to other approaches, it frequently requires 20 min or more to complete, which can impede adoption. A self-administered, Web-based safety planning application could potentially reduce clinician time, help promote standardization and quality, and provide enhanced ability to share the created plan. OBJECTIVE: The aim of this study was to design, build, and test the usability of a Web-based, self-administered safety planning application. METHODS: We employed a user-centered software design strategy led by a multidisciplinary team. The application was tested for usability with a target sample of suicidal patients. Detailed observations, structured usability ratings, and Think Aloud procedures were used. Suicidal ideation intensity and perceived ability to cope were assessed pre-post engagement with the Web application. RESULTS: A total of 30 participants were enrolled. Usability ratings were generally strong, and all patients successfully built a safety plan. However, the completeness of the safety plan varied. The mean number of steps completed was 5.5 (SD 0.9) out of 6, with 90% (27/30) of participants completing at least 5 steps and 67% (20/30) completing all 6 steps. Some safety planning steps were viewed as inapplicable to some individuals. Some confusion in instructions led to modifications to improve understandability of each step. Ratings of suicide intensity after completion of the application were significantly lower than preratings, pre: mean 5.11 (SD 2.9) versus post: mean 4.46 (SD 3.0), t27=2.49, P=.02. Ratings of ability to cope with suicidal thoughts after completion of the application were higher than preratings, with the difference approaching statistical significance, pre: mean 5.93 (SD 2.9), post: mean 6.64 (SD 2.4), t27=-2.03, P=.05. CONCLUSIONS: We have taken the first step toward identifying the components needed to maximize usability of a self-administered, Web-based safety planning application. Results support initial consideration of the application as an adjunct to clinical contact. This allows for the clinician or other personnel to provide clarification, when needed, to help the patient build the plan, and to help review and revise the draft

    International Public Health Research Involving Interpreters: a Case Study from Bangladesh

    Background: Cross-cultural and international research are important components of public health research, but the challenges of language barriers and working with interpreters are often overlooked, particularly in the case of qualitative research. Methods: A case-study approach was used to explore experiences of working with an interpreter in Bangladesh as part of a research project investigating women's experiences of emergency obstetric care. The case study: Data from the researcher's field notes provided evidence of experiences in working with an interpreter and show how the model of interviewing was adapted over time to give a more active role to the interpreter. The advantages of a more active role were increased rapport and "flow" in interviews. The disadvantages included reduced control from the researcher's perspective. Some tensions between the researcher and interpreter remained hard to overcome, irrespective of the model used. Independent transcription and translation of the interviews also raised questions around accuracy in translation. Conclusion: The issues examined in this case study have broader implications for public health research. Further work is needed in three areas: 1) developing effective relationships with interpreters; 2) the impact of the interpreter on the research process; and 3) the accuracy of the translation and level of analysis needed in any specific public health research. Finally, this paper highlights the importance to authors of reflecting on the potential impact of translation and interpretation on the research process when disseminating their research

    GCIP water and energy budget synthesis (WEBS)

    As part of the World Climate Research Program\u27s (WCRPs) Global Energy and Water-Cycle Experiment (GEWEX) Continental-scale International Project (GCIP), a preliminary water and energy budget synthesis (WEBS) was developed for the period 1996–1999 from the “best available” observations and models. Besides this summary paper, a companion CD-ROM with more extensive discussion, figures, tables, and raw data is available to the interested researcher from the GEWEX project office, the GAPP project office, or the first author. An updated online version of the CD-ROM is also available at http://ecpc.ucsd.edu/gcip/webs.htm/. Observations cannot adequately characterize or “close” budgets since too many fundamental processes are missing. Models that properly represent the many complicated atmospheric and near-surface interactions are also required. This preliminary synthesis therefore included a representative global general circulation model, regional climate model, and a macroscale hydrologic model as well as a global reanalysis and a regional analysis. By the qualitative agreement among the models and available observations, it did appear that we now qualitatively understand water and energy budgets of the Mississippi River Basin. However, there is still much quantitative uncertainty. In that regard, there did appear to be a clear advantage to using a regional analysis over a global analysis or a regional simulation over a global simulation to describe the Mississippi River Basin water and energy budgets. There also appeared to be some advantage to using a macroscale hydrologic model for at least the surface water budgets

    Coordination and Output Attainment in Work Units Performing Non-routine Tasks: A Cross- National Study

    Based on an information-processing perspective (Galbraith 1972), a theoretical pro position is advanced which predicts that for work units performing non-routine tasks, the effect of unit coordination on output attainment is contingent on the sources from which the unit acquires information for task performance. This proposition is tested using a cross-national research design. Data from four national samples — Austria, Belgium, Hungary, and Poland — of academic research units support the proposition. The results reinforce the need for a contingency approach to the study of coordination and performance in organizations.     Hydrogen Phases on the Surface of a Strongly Magnetized Neutron Star

    The outermost layers of some neutron stars are likely to be dominated by hydrogen, as a result of fast gravitational settling of heavier elements. These layers directly mediate thermal radiation from the stars, and determine the characteristics of X-ray/EUV spectra. For a neutron star with surface temperature T\lo 10^6 K and magnetic field B\go 10^{12} G, various forms of hydrogen can be present in the envelope, including atom, poly-molecules, and condensed metal. We study the physical properties of different hydrogen phases on the surface of a strongly magnetized neutron star for a wide range of field strength BB and surface temperature TT. Depending on the values of BB and TT, the outer envelope can be either in a nondegenerate gaseous phase or in a degenerate metallic phase. For T\go 10^5 K and moderately strong magnetic field, B\lo 10^{13} G, the envelope is nondegenerate and the surface material gradually transforms into a degenerate Coulomb plasma as density increases. For higher field strength, B>>1013B>> 10^{13} G, there exists a first-order phase transition from the nondegenerate gaseous phase to the condensed metallic phase. The column density of saturated vapor above the metallic hydrogen decreases rapidly as the magnetic field increases or/and temperature decreases. Thus the thermal radiation can directly emerge from the degenerate metallic hydrogen surface. The characteristics of surface X-ray/EUV emission for different phases are discussed.     Measured Effects of Surface Cloth Impressions on Polar Backscatter and Comparison with a Reflection Grating Model

    Integrated polar backscatter has been shown to have potential applications to composites, especially for the detection of matrix cracking, delaminations, fiber waviness, fiber fracture, inclusions and porosity [1–11]. The method was attractive because it avoided several measurement limitations inherent to conventional pulse echo techniques. Polar backscatter, however, has not been without its disadvantages. It has been reported that surface texture introduces unwanted artifacts in images made using the polar backscatter method [12]. One suggested method to overcome this limitation was the use of stripable coatings, which are paints that approximately match the impedance of the composite surface and have the effect of physically “smoothing” the surface impressions away [13]. After ultrasonic testing, these paints can be removed, but this method entails additional part handling and increases the cost of production

    National Cancer Institute’s First International Workshop on the Biology, Prevention, and Treatment of Relapse after Allogeneic Hematopoietic Stem Cell Transplantation: Summary and Recommendations from the Organizing Committee

    The National Cancer Institute’s First International Workshop on the Biology, Prevention, and Treatment of Relapse after Allogeneic Hematopoietic Stem Cell Transplantation was organized and convened to identify, prioritize, and coordinate future research activities related to relapse after allogeneic hematopoietic stem cell transplantation (allo-HSCT). Each of the Workshop’s 6 Working Committees has published individual reports of ongoing basic, translational, and clinical research and recommended areas for future research related to the areas of relapse biology, epidemiology, prevention, and treatment. This document summarizes each committee’s recommendations and suggests 3 major initiatives for a coordinated research effort to address the problem of relapse after allo-HSCT: (1) to establish multicenter correlative and clinical trial networks for basic/translational, epidemiologic, and clinical research; (2) to establish a network of biorepositories for the collection of samples before and after allo-HSCT to aid in laboratory and clinical studies; and (3) to further refine, implement, and study the Workshop-proposed definitions for disease-specific response and relapse and recommendations for monitoring of minimal residual disease. These recommendations, in coordination with ongoing research initiatives and transplantation organizations, provide a research framework to rapidly and efficiently address the significant problem of relapse after allo-HSCT

    Global change, global trade, and the next wave of plant invasions

    Copyright © 2012 Ecological Society of AmericaMany non-native plants in the US have become problematic invaders of native and managed ecosystems, but a new generation of invasive species may be at our doorstep. Here, we review trends in the horticultural trade and invasion patterns of previously introduced species and show that novel species introductions from emerging horticultural trade partners are likely to rapidly increase invasion risk. At the same time, climate change and water restrictions are increasing demand for new types of species adapted to warm and dry environments. This confluence of forces could expose the US to a range of new invasive species, including many from tropical and semiarid Africa as well as the Middle East. Risk assessment strategies have proven successful elsewhere at identifying and preventing invasions, although some modifications are needed to address emerging threats. Now is the time to implement horticulture import screening measures to prevent this new wave of plant invasions.National Science Foundatio
