    Generational differences in practice site selection criteria amongst primary care physicians.

    Background and Objectives: Generational differences are often viewed as shaping the overall attitudes and actions of different age cohorts. It is essential to understand the motivations and generational differences in primary care physicians for efforts to recruit, retain, and educate the future physician workforce. Determining what factors most influence different generations of primary care physicians when choosing a practice site is essential to build our future primary care system. This study examined generational differences in the factors that attracted primary care physicians to their current practice. Methods: A survey instrument was mailed to all active members of the North Carolina Medical Board who listed their primary occupation as a primary care specialty. The survey consisted of 24 demographic questions regarding personal and practice variables and a list of 21 reasons for choosing a practice location measured on a 7-point Likert type scale. A total of 975 surveys were returned and usable for the final analysis, for a return rate of 34.5%. Data were analyzed using regression and correlation procedures to determine attitudes of each generation and factors that significantly influenced responses. Results: While slight differences between generations did exist, the overall choices for choosing a site remained stable across generations. Personality of the practice, on-call responsibilities, ability to practice comprehensive care, and location were deemed the most important factors for all generations. Differences between various demographic groups and Family Medicine versus other primary care specialties were minor with very little alteration of the top ten items being seen between groups. Conclusion: This study indicated that there were few differences between generations regarding primary reasons for choosing a practice site. In addition, factors remained remarkably similar across different specialties, family situations, genders, and ethnic groups. Several of the top reasons that primary care physicians indicate are the most important for site selection were also potentially modifiable, such as on-call responsibilities, practice personality, and ability to practice comprehensive care.     EXPLORATION OF LEADERSHIP VALUE TRANMISSION IN A COLLEGE RECREATION ORGANIZATION

    This was a mixed-methods study that explored the emergence, transmission, and reception of leadership values in an American southeast college recreation organization (CRO). A structured interview was conducted with the CRO director to determine his leadership values. Questions were designed to elicit specific values held by the director that were reflective of his leadership. Results from the in-depth interview elicited themes around the values of the director, methods of transmission of values of the director, skills the director had that he considered necessary to lead, and how the director governs his values in the work-environment. Based on the director's responses, a questionnaire was created and distributed to the CRO's frontline employees (FEs) to understand if the director exhibited these values and if these values were present in the work environment. The study occurred during the height of the COVID-19 pandemic when campus was largely closed. The FE questionnaire was electronically distributed to 220 employees. The response rate was low with 48 FEs attempting the questionnaire and 22 FEs completing questions related to the CRO director's values and method of value transmission\; 23 FEs completed questions related to values present in the CRO and method of value transmission from direct supervisors. While descriptive, the results from the FE questionnaire identified four tiers of value congruence between the director and CRO. There was also a difference in the methods of transmission with FE supervisors rated higher than the director on communication via actions, language, and written communication

    Greenhouse gas emissions from milk production and consumption in the United States: A cradle-to-grave life cycle assessment circa 2008

    This article presents a cradle-to-grave analysis of the United States fluid milk supply chain greenhouse gas (GHG) emissions that are accounted from fertilizer production through consumption and disposal of milk packaging. Crop production and on-farm GHG emissions were evaluated using public data and 536 farm operation surveys. Milk processing data were collected from 50 dairy plants nationwide. Retail and consumer GHG emissions were estimated from primary data, design estimates, and publicly available data. Total GHG emissions, based primarily on 2007 to 2008 data, were 2.05 (90% confidence limits: 1.77-2.4) kg CO2e per kg milk consumed, which accounted for loss of 12% at retail and an additional 20% loss at consumption. A complementary analysis showed the entire dairy sector contributes approximately 1.9% of US GHG emissions. While the largest GHG contributors are feed production, enteric methane, and manure management; there are opportunities to reduce impacts throughout the supply chain. © 2012 Elsevier Ltd

    Greenhouse gas (GHG) emissions were evaluated from crop production through the on-farm portion of the milk supply chain for five production regions in the USA derived from publicly available data and from 536 surveys of farm operations collected from dairy operations nationwide. The production weighted national average footprint at the farm gate was 1.23 kg carbon dioxide equivalent (CO2e) per kg of fat and protein corrected milk (fat, 4%; protein 3.3%). Regional differences in GHG emissions per kg milk produced can be primarily traced to differences in production and management practices. Feed-to-milk conversion efficiency is shown to be the single most important explanatory variable, followed by choice of manure management technology. While there is no one-size-fits-all solution, GHG emissions reduction opportunities exist across the spectrum of dairy management options. However, as with all decisions, it is important to weigh potential trade-offs with other environmental and economic impacts. © 2012 Elsevier Ltd
