Synthesis, characterization, DNA-binding and biological activity of Zn(II) complexes of sulfadiazine with different amino acids
New Zn(II) complexes (ZnII-VS, ZnII-LS, ZnII-SS, ZnII-CS and ZnII-MS) of the sulfonamide antibiotic sulfadiazine with different amino acids were prepared and fully characterized by elemental analyses, thermal analysis and IR, UV/Vis and 1H NMR spectroscopy. The IR and 1H NMR spectral data show that the ligands behave in a dibasic bidentate fashion coordinating to zinc ion. Interactions of these complexes with DNA were investigated by spectrophotometric method. Moreover, the antibacterial and antifungal activities were evaluated for five ligands and their complexes. Viral etiology, seasonality and severity of hospitalized patients with severe acute respiratory infections in the Eastern Mediterranean Region, 2007-2014.
INTRODUCTION: Little is known about the role of viral respiratory pathogens in the etiology, seasonality or severity of severe acute respiratory infections (SARI) in the Eastern Mediterranean Region. METHODS: Sentinel surveillance for SARI was conducted from December 2007 through February 2014 at 20 hospitals in Egypt, Jordan, Oman, Qatar and Yemen. Nasopharyngeal and oropharyngeal swabs were collected from hospitalized patients meeting SARI case definitions and were analyzed for infection with influenza, respiratory syncytial virus (RSV), adenovirus (AdV), human metapneumovirus (hMPV) and human parainfluenza virus types 1-3 (hPIV1-3). We analyzed surveillance data to calculate positivity rates for viral respiratory pathogens, describe the seasonality of those pathogens and determine which pathogens were responsible for more severe outcomes requiring ventilation and/or intensive care and/or resulting in death. RESULTS: At least one viral respiratory pathogen was detected in 8,753/28,508 (30.7%) samples tested for at least one pathogen and 3,497/9,315 (37.5%) of samples tested for all pathogens-influenza in 3,345/28,438 (11.8%), RSV in 3,942/24,503 (16.1%), AdV in 923/9,402 (9.8%), hMPV in 617/9,384 (6.6%), hPIV1 in 159/9,402 (1.7%), hPIV2 in 85/9,402 (0.9%) and hPIV3 in 365/9,402 (3.9%). Multiple pathogens were identified in 501/9,316 (5.4%) participants tested for all pathogens. Monthly variation, indicating seasonal differences in levels of infection, was observed for all pathogens. Participants with hMPV infections and participants less than five years of age were significantly less likely than participants not infected with hMPV and those older than five years of age, respectively, to experience a severe outcome, while participants with a pre-existing chronic disease were at increased risk of a severe outcome, compared to those with no reported pre-existing chronic disease. CONCLUSIONS: Viral respiratory pathogens are common among SARI patients in the Eastern Mediterranean Region. Ongoing surveillance is important to monitor changes in the etiology, seasonality and severity of pathogens of interest
Epidemics on contact networks: a general stochastic approach
Dynamics on networks is considered from the perspective of Markov stochastic
processes. We partially describe the state of the system through network motifs
and infer any missing data using the available information. This versatile
approach is especially well adapted for modelling spreading processes and/or
population dynamics. In particular, the generality of our systematic framework
and the fact that its assumptions are explicitly stated suggests that it could
be used as a common ground for comparing existing epidemics models too complex
for direct comparison, such as agent-based computer simulations. We provide
many examples for the special cases of susceptible-infectious-susceptible (SIS)
and susceptible-infectious-removed (SIR) dynamics (e.g., epidemics propagation)
and we observe multiple situations where accurate results may be obtained at
low computational cost. Our perspective reveals a subtle balance between the

Learning to Teach About Ideas and Evidence in Science : The Student Teacher as Change Agent
A collaborative curriculum development project was set up to address the lack of good examples of teaching about ideas and evidence and the nature of science encountered by student teachers training to teach in the age range 11-16 in schools in England. Student and teacher-mentor pairs devised, taught and evaluated novel lessons and approaches. The project design required increasing levels of critique through cycles of teaching, evaluation and revision of lessons. Data were gathered from interviews and students' reports to assess the impact of the project on student teachers and to what extent any influences survived when they gained their first teaching posts. A significant outcome was the perception of teaching shifting from the delivery of standard lessons in prescribed ways to endeavours demanding creativity and decision-making. Although school-based factors limited newly qualified teachers' chances to use new lessons and approaches and therefore act as change-agents in schools, the ability to critique curriculum materials and the recognition of the need to create space for professional dialogue were durable gains
Mucosal injury following short term tracheal intubation: a novel animal model and composite injury score
Objectives: Postintubation laryngotracheal injury is common. Assessment of histopathological changes currently requires animal models. We set about developing a viable, resource effective animal model to study these effects and to develop a detailed tissue injury score.
Methods: Six pigs were anaesthetised using a standard regimen. We intubated the tracheas using a standard endotracheal tube modified to include optical sensors. Animals were anaesthetised for a duration of two to four hours, and their lungs were ventilated using a normoxic gas mixture. Following euthanasia, the tracheas were removed and underwent histological assessment by two independent veterinary pathologists. The histological lesions, including controls, were described and quantified, and two pathologists classified tissues according to a novel injury score.
Results: Mean duration of tracheal intubation was 191 minutes (SD±41.6). In all except one animal, cuff pressures were maintained in the range of 25 – 45 cmH20. Histopathological findings in all study animals showed more extensive changes than previously described with short-term intubation. Changes were seen in all mucosal layers consistent with acute, suppurative and ulcerative tracheitis. The range of scores of the developed composite scoring system among the animals was wider than in earlier descriptions. There was a high percentage of agreement between both pathologists.
Conclusions: We have described a novel tissue injury score to assess pathological changes following short term intubation in a viable animal model. The scoring system distinguished between the test animals as well as controls and may be appropriate for continuing study of intubation injury
A qualitative study of enablers and barriers influencing the incorporation of social accountability values into organisational culture: a perspective from two medical schools
Background: Definitions of social accountability describe the obligation of medical schools to direct education,
research and service activities towards addressing the priority health concerns of the population they serve. While
such statements give some direction as to how the goal might be reached, it does not identify what factors might
facilitate or hinder its achievement. This study set out to identify and explore enablers and barriers influencing the
incorporation of social accountability values into medical schools.
Methods: Semi structured interviews of fourteen senior staff in Bar Ilan and Leeds medical schools were undertaken
following a literature review. Participants were recruited by purposive sampling in order to identify factors perceived to
play a part in the workings of each institution.
Results: Academic prestige was seen as a key barrier that was dependent on research priorities and student selection.
The role of champions was considered to be vital to tackle staff perceptions and facilitate progress. Including practical
community experience for students was felt to be a relevant way in which the curriculum could be designed through
engagement with local partners.
Conclusions: Successful adoption of social accountability values requires addressing concerns around potential negative
impacts on academic prestige and standards. Identifying and supporting credible social accountability champions to
disseminate the values throughout research and education departments in medical and other faculties is also necessary,
including mapping onto existing work streams and research agendas. Demonstrating the contribution the institution can
make to local health improvement and regional development by a consideration of its economic footprint may also be
valuable

The formation of professional identity in medical students: considerations for educators
<b>Context</b> Medical education is about more than acquiring an appropriate level of knowledge and developing relevant skills. To practice medicine students need to develop a professional identity – ways of being and relating in professional contexts.<p></p>
<b>Objectives</b> This article conceptualises the processes underlying the formation and maintenance of medical students’ professional identity drawing on concepts from social psychology.<p></p>
<b>Implications</b> A multi-dimensional model of identity and identity formation, along with the concepts of identity capital and multiple identities, are presented. The implications for educators are discussed.<p></p>
<b>Conclusions</b> Identity formation is mainly social and relational in nature. Educators, and the wider medical society, need to utilise and maximise the opportunities that exist in the various relational settings students experience. Education in its broadest sense is about the transformation of the self into new ways of thinking and relating. Helping students form, and successfully integrate their professional selves into their multiple identities, is a fundamental of medical education
How the weather affects the pain of citizen scientists using a smartphone app
Patients with chronic pain commonly believe their pain is related to the weather. Scientific evidence to support their beliefs is inconclusive, in part due to difficulties in getting a large dataset of patients frequently recording their pain symptoms during a variety of weather conditions. Smartphones allow the opportunity to collect data to overcome these difficulties. Our study Cloudy with a Chance of Pain analysed daily data from 2658 patients collected over a 15-month period. The analysis demonstrated significant yet modest relationships between pain and relative humidity, pressure and wind speed, with correlations remaining even when accounting for mood and physical activity. This research highlights how citizen-science experiments can collect large datasets on real-world populations to address long-standing health questions. These results will act as a starting point for a future system for patients to better manage their health through pain forecasts
