    Satisfaction of population by activity of cultural institutions: Experience of the republic of tatarstan

    Conduction of marketing researches in sphere of culture is dictated by the branch's need of information on culture demands and preferences of population, social-economic characteristics of auditorium of these institutions, motives of their visiting or not visiting, degree of satisfaction by their activity. Today the majority of cultural organizations do not have specific information on social. demographic, economic and other signs characterizing the visitors of these institutions, on demands, preferences, remarks and requirements of population to sphere of culture, estimations of activity of theaters, museums, libraries, club and other leisure organizations. But, as the eventual target of functioning of culture and art objects and entities is the satisfaction and formation of diverse spiritual needs of people, cultural institutions have to possess a wide-scale information field on real participation of diverse social groups (segments) in cultural process. The presence of such information is one of the fundamental conditions of successful solving of economic difficulties emerged in formed contemporary market situation, connected both to perspective and with current planning of effective activity of culture's organizations. In this article the qualitative analysis of cultural institutions of the Republic of Tatarstan activity via subjective estimation of their customer's satisfaction degree is conducted. Materials of sociological research of cultural institutions services consumers (n=806), received by authors in November of 2014, allowed to detect major quantitativequalitative parameters of culture's institutions, and also to analyze the general issues in operation of these organizations in the republic.     Evaluation of the effect of the furazane fragment on the NH acidity of 4- and 5-picrylaminobenzofurazanes in DMF and DMSO

    The products of the reactions of picryl chloride with isomeric aminobenzofurazanes in DMF and DMSO were studied by means of nonaquoeus potentiometric titration. The effect of the position of the furazane fragment in 4- and 5-picrylaminobenzofurazanes on the NH acidity is considered. The electron-acceptor properties of the furazane fragment were evaluated via inclusion of the resulting data into the pK a-σ correlation for 2,4,6-trinitrodiphenylamines. On this basis, conventional Hammett σ constants were calculated for the furazane fragment located in the immediate vicinity of the N-H center and one position distant from it. © 2005 Pleiades Publishing, Inc

    Fulleretic well-defined scaffolds: Donor–fullerene alignment through metal coordination and its effect on photophysics

    Herein, we report the first example of a crystalline metal–donor–fullerene framework, in which control of the donor–fullerene mutual orientation was achieved through chemical bond formation, in particular, by metal coordination. The 13C cross‐polarization magic‐angle spinning NMR spectroscopy, X‐ray diffraction, and time‐resolved fluorescence spectroscopy were performed for comprehensive structural analysis and energy‐transfer (ET) studies of the fulleretic donor–acceptor scaffold. Furthermore, in combination with photoluminescence measurements, the theoretical calculations of the spectral overlap function, Förster radius, excitation energies, and band structure were employed to elucidate the photophysical and ET processes in the prepared fulleretic material. We envision that the well‐defined fulleretic donor–acceptor materials could contribute not only to the basic science of fullerene chemistry but would also be used towards effective development of organic photovoltaics and molecular electronics

    Ocrelizumab in treatment of primary-progressive multiple sclerosis: systematic review

    Aim. To analyze the efficacy, safety and pharmacoeconomic indicators of ocrelizumab in adult patients with primary progressive multiple sclerosis (PPMS). Methods. An information search was conducted in the databases Embase, PubMed, Cochrane and eLibrary.ru. The levels of evidence were determined in the studies. Results. Therapy with ocrelizumab compared with placebo characterized by a decrease in the rate of progression of the disease. Treatment with ocrelizumab was associated with a significant slowdown in progression compared to other drugs: rituximab, fingolimod, myelin basic protein peptide 82–98, intravenous immunoglobulin; plasmapheresis / plasma metabolism, corticosteroids, general irradiation of lymphoid tissue, and other most common adverse events: infusion reactions, nasopharyngitis, upper tract respiratory and urinary tract infections, headaches. Life years and quality-adjusted life years for patients receiving ocrelizumab were 16.11 and 3.33, compared with 15.61 and 2.75 for patients receiving better supportive care, respectively. The annual average potential impact on the budget for 1 patient with PPMS in the treatment of ocrelizumab for 5 years ranged from $ 18,300 to 44 200. Conclusions. Ocrelizumab is the only drug that has proven its clinical efficacy in the previously non-curable type of multiple sclerosis, PPC, with risk profile acceptable with respect to clinical benefits

    Extracellular Matrix in Heart Failure: Role of ADAMTS5 in Proteoglycan Remodeling

    [Abstract] Background: Remodeling of the extracellular matrix (ECM) is a hallmark of heart failure (HF). Our previous analysis of the secretome of murine cardiac fibroblasts returned ADAMTS5 (a disintegrin and metalloproteinase with thrombospondin motifs 5) as one of the most abundant proteases. ADAMTS5 cleaves chondroitin sulfate proteoglycans such as versican. The contribution of ADAMTS5 and its substrate versican to HF is unknown. Methods: Versican remodeling was assessed in mice lacking the catalytic domain of ADAMTS5 (Adamts5ΔCat). Proteomics was applied to study ECM remodeling in left ventricular samples from patients with HF, with a particular focus on the effects of common medications used for the treatment of HF. Results: Versican and versikine, an ADAMTS-specific versican cleavage product, accumulated in patients with ischemic HF. Versikine was also elevated in a porcine model of cardiac ischemia/reperfusion injury and in murine hearts after angiotensin II infusion. In Adamts5ΔCat mice, angiotensin II infusion resulted in an aggravated versican build-up and hyaluronic acid disarrangement, accompanied by reduced levels of integrin β1, filamin A, and connexin 43. Echocardiographic assessment of Adamts5ΔCat mice revealed a reduced ejection fraction and an impaired global longitudinal strain on angiotensin II infusion. Cardiac hypertrophy and collagen deposition were similar to littermate controls. In a proteomics analysis of a larger cohort of cardiac explants from patients with ischemic HF (n=65), the use of β-blockers was associated with a reduction in ECM deposition, with versican being among the most pronounced changes. Subsequent experiments in cardiac fibroblasts confirmed that β1-adrenergic receptor stimulation increased versican expression. Despite similar clinical characteristics, patients with HF treated with β-blockers had a distinct cardiac ECM profile. Conclusions: Our results in animal models and patients suggest that ADAMTS proteases are critical for versican degradation in the heart and that versican accumulation is associated with impaired cardiac function. A comprehensive characterization of the cardiac ECM in patients with ischemic HF revealed that β-blockers may have a previously unrecognized beneficial effect on cardiac chondroitin sulfate proteoglycan content.Dr Barallobre-Barreiro is a British Heart Foundation Intermediate Fellow (FS/19/33/34328).     The report reviews the options of drug supply for patients with chronic hepatitis C in the Russian Federation and elsewhere. The results of the Moscow regional program aimed at improving the quality of medical care in such patients are discussed. The proposed measures contribute to the reduced mortality rate in patients suffering from infections. The prospects of further improvements in the availability of antiviral medications for patients with chronic viral hepatitis C are outlined. В статье рассмотрены варианты лекарственного обеспечения пациентов с хроническим вирусным гепатитом С за рубежом и в Российской Федерации. Продемонстрирована эффективность работы целевой региональной программы, направленной на повышение качества оказания медицинской помощи жителям города Москвы, страдающим хроническим вирусным гепатитом С, снижение смертности от инфекционных заболеваний. Определен вектор дальнейшего перспективного развития для повышения доступности лекарственного обеспечения противовирусной терапии (ПВТ) для пациентов с хроническим вирусным гепатитом С.
