    Fibromyalgia Syndrome Care of Iraq- and Afghanistan-Deployed Veterans in Veterans Health Administration

    Little is known regarding fibromyalgia syndrome (FMS) care among Operation Iraqi Freedom/Operation Enduring Freedom/Operation New Dawn (OIF/OEF/OND) Veterans. Current recommendations include interdisciplinary, teambased combined care approaches and limited opioid use. In this study of OIF/OEF/OND Veterans who accessed Veterans Health Administration services between 2002 and 2012, we hypothesized that combined care (defined as at least 4 primary care visits/yr with visits to mental health and/or rheumatology) versus/yr only would be associated with lower risk of at least 2 opioid prescriptions 12 mo following an FMS diagnosis. Using generalized linear models with a loglink, the Poisson family, and robust standard errors, we estimated risk ratios (RRs) and 95% confidence intervals (CIs). We found that 1% of Veterans had at least 2 FMS diagnoses (International Classification of Diseases-9th Revision-Clinical Modification code 729.1) or at least 1 FMS diagnosis by rheumatology. Veterans with (vs without) FMS were more likely to be female, older, Hispanic, and never/currently married. Combined primary, mental health, and rheumatology care was associated with at least 2 opioid prescriptions (RR [95% CI] for males 2.2 [1.1–4.4] and females 2.8 [0.4–18.6]). Also, combined care was associated with at least 2 nonopioid painrelated prescriptions, a practice supported by evidence-based clinical practice guidelines. In tandem, these results provide mixed evidence of benefit of combined care for FMS. Future studies of healthcare encounter characteristics, care coordination, and benefits for Veterans with FMS are needed

    Photodetection of propagating quantum microwaves in circuit QED

    We develop the theory of a metamaterial composed of an array of discrete quantum absorbers inside a one-dimensional waveguide that implements a high-efficiency microwave photon detector. A basic design consists of a few metastable superconducting nanocircuits spread inside and coupled to a one-dimensional waveguide in a circuit QED setup. The arrival of a {\it propagating} quantum microwave field induces an irreversible change in the population of the internal levels of the absorbers, due to a selective absorption of photon excitations. This design is studied using a formal but simple quantum field theory, which allows us to evaluate the single-photon absorption efficiency for one and many absorber setups. As an example, we consider a particular design that combines a coplanar coaxial waveguide with superconducting phase qubits, a natural but not exclusive playground for experimental implementations. This work and a possible experimental realization may stimulate the possible arrival of "all-optical" quantum information processing with propagating quantum microwaves, where a microwave photodetector could play a key role.Comment: 27 pages, submitted to Physica Scripta for Nobel Symposium on "Qubits for Quantum Information", 200

    Priorities for research in child maltreatment, intimate partner violence and resilience to violence exposures:results of an international Delphi consensus development process

    BACKGROUND: Intimate partner violence (IPV) and child maltreatment (CM) are major global public health problems. The Preventing Violence Across the Lifespan (PreVAiL) Research Network, an international group of over 60 researchers and national and international knowledge-user partners in CM and IPV, sought to identify evidence-based research priorities in IPV and CM, with a focus on resilience, using a modified Delphi consensus development process. METHODS: Review of existing empirical evidence, PreVAiL documents and team discussion identified a starting list of 20 priorities in the following categories: resilience to violence exposure (RES), CM, and IPV, as well as priorities that cross-cut the content areas (CC), and others specific to research methodologies (RM) in violence research. PreVAiL members (N = 47) completed two online survey rounds, and one round of discussions via three teleconference calls to rate, rank and refine research priorities. RESULTS: Research priorities were: to examine key elements of promising or successful programmes in RES/CM/IPV to build intervention pilot work; CC: to integrate violence questions into national and international surveys, and RM: to investigate methods for collecting and collating datasets to link data and to conduct pooled, meta and sub-group analyses to identify promising interventions for particular groups. CONCLUSIONS: These evidence-based research priorities, developed by an international team of violence, gender and mental health researchers and knowledge-user partners, are of relevance for prevention and resilience-oriented research in the areas of IPV and CM

    Bonn Potential and Shell-Model Calculations for 206,205,204Pb

    The structure of the nuclei 206,205,204Pb is studied interms of shell model employing a realistic effective interaction derived from the Bonn A nucleon-nucleon potential. The energy spectra, binding energies and electromagnetic properties are calculated and compared with experiment. A very good overall agreement is obtained. This evidences the reliability of our realistic effective interaction and encourages use of modern realistic potentials in shell-model calculations for heavy-mass nuclei.Comment: 4 pages, 4 figures, submitted to Physical Review

    Taking a Proposal Seriously : Orientations to Agenda and Agency in Support Workers’ Responses to Client Proposals

    While joint decision-making is regularly launched by a proposal, it is the recipients’ responses that crucially influence the proposal outcome. This chapter examines how support workers respond to the proposals made by clients during rehabilitation group meetings at the Clubhouse. Drawing on a collection of 180 client-initiated proposal sequences, the paper describes two dilemmas that the support workers face when seeking to take client proposals “seriously.” The first concerns the meeting’s agenda and consists of a tension between providing recognition for the individual client and encouraging collective participation. The second dilemma has to do with agency and consists of a tension between focusing on the client as the originator of the proposal and avoiding treating him or her alone accountable for it.     Time-optimal CNOT between indirectly coupled qubits in a linear Ising chain

    We give analytical solutions for the time-optimal synthesis of entangling gates between indirectly coupled qubits 1 and 3 in a linear spin chain of three qubits subject to an Ising Hamiltonian interaction with equal coupling JJ plus a local magnetic field acting on the intermediate qubit. The energy available is fixed, but we relax the standard assumption of instantaneous unitary operations acting on single qubits. The time required for performing an entangling gate which is equivalent, modulo local unitary operations, to the CNOT(1,3)\mathrm{CNOT}(1, 3) between the indirectly coupled qubits 1 and 3 is T=3/2J−1T=\sqrt{3/2} J^{-1}, i.e. faster than a previous estimate based on a similar Hamiltonian and the assumption of local unitaries with zero time cost. Furthermore, performing a simple Walsh-Hadamard rotation in the Hlibert space of qubit 3 shows that the time-optimal synthesis of the CNOT±(1,3)\mathrm{CNOT}^{\pm}(1, 3) (which acts as the identity when the control qubit 1 is in the state ∣0⟩\ket{0}, while if the control qubit is in the state ∣1⟩\ket{1} the target qubit 3 is flipped as âˆŁÂ±âŸ©â†’âˆŁâˆ“âŸ©\ket{\pm}\rightarrow \ket{\mp}) also requires the same time TT.Comment: 9 pages; minor modification

    Successful Small Intestine Colonization of Adult Mice by Vibrio cholerae Requires Ketamine Anesthesia and Accessory Toxins

    Vibrio cholerae colonizes the small intestine of adult C57BL/6 mice. In this study, the physical and genetic parameters that facilitate this colonization were investigated. Successful colonization was found to depend upon anesthesia with ketamine-xylazine and neutralization of stomach acid with sodium bicarbonate, but not streptomycin treatment. A variety of common mouse strains were colonized by O1, O139, and non-O1/non-O139 strains. All combinations of mutants in the genes for hemolysin, the multifunctional, autoprocessing RTX toxin (MARTX), and hemagglutinin/protease were assessed, and it was found that hemolysin and MARTX are each sufficient for colonization after a low dose infection. Overall, this study suggests that, after intragastric inoculation, V. cholerae encounters barriers to infection including an acidic environment and an immediate immune response that is circumvented by sodium bicarbonate and the anti-inflammatory effects of ketamine-xylazine. After initial adherence in the small intestine, the bacteria are subjected to additional clearance mechanisms that are evaded by the independent toxic action of hemolysin or MARTX. Once colonization is established, it is suggested that, in humans, these now persisting bacteria initiate synthesis of the major virulence factors to cause cholera disease. This adult mouse model of intestinal V. cholerae infection, now well-characterized and fully optimized, should serve as a valuable tool for studies of pathogenesis and testing vaccine efficacy

    Parity Violation in Proton-Proton Scattering at 221 MeV

    The parity-violating longitudinal analyzing power, Az, has been measured in pp elastic scattering at an incident proton energy of 221 MeV. The result obtained is Az =(0.84 +/- 0.29 (stat.) +/- 0.17 (syst.)) x 10^{-7}. This experiment is unique in that it selects a single parity violating transition amplitude, 3P2-1D2, and consequently directly constrains the weak meson-nucleon coupling constant h^pp_rho When this result is taken together with the existing pp parity violation data, the weak meson-nucleon coupling constants h^pp_rho and h^pp_omega can, for the first time, both be determined.Comment: 8 pages RevTeX4, 3 PostScript figures.     Energetic Protons and Deuterons Emitted Following Ό⁻ Capture by ÂłHe Nuclei

    Spectra of energetic protons and deuterons emitted following negative muon capture from rest in 3He have been measured for the first time. Significant capture strength is observed at high energy transfers (mΌ- Ev \u3e60 MeV) for the two-body and three-body breakup channels, indicative of the importance of nucleon-nucleon correlations and meson exchange currents in the capture process. A simple plane wave impulse approximation calculation reproduces the proton spectrum reasonably well, but underpredicts the deuteron rate at the highest energies by a large factor
