    RESUMO Neste trabalho foram utilizadas plantas de pinhão manso (Jatropha curcas L.), no mesmo estádio de desenvolvimento e sob iguais condições de manejo cultural, cultivadas em duas fazendas comerciais: uma na região norte, litorânea (Pontal do Ipiranga, Linhares) e a outra na região noroeste, montanhosa (Itarana) do Estado do Espírito Santo com o objetivo de avaliar o desempenho fotossintético in situ. Foram realizadas medidas durante um ano (maio/2010 a maio/2011) do crescimento, índice de clorofila, trocas gasosas, fluorescência transiente e modulada da clorofila a. Análises in vitro dos teores de clorofila e da atividade de enzimas antioxidantes também foram realizadas. Os resultados obtidos mostraram que as plantas cultivadas no interior do estado do Espírito Santo apresentaram maior crescimento em altura e maior eficiência real do uso da água (A/E). As plantas desta região também apresentaram menor atividade antioxidante (SOD, CAT e APX) em resposta a uma menor produção de espécies reativas de oxigênio (ROS). As plantas cultivadas no litoral, com solo de menor disponibilidade de nutrientes, apresentaram maiores teores de clorofila a e b, maiores taxas de fotossíntese líquida (A) e maiores índices de desempenho fotoquímico. Infere-se, portanto que as plantas de pinhão manso possuem estratégias adaptativas que lhes permitem viver em ambiente de condições adversas.     Barriers associated with emergency medical service activation in Italian patients with ST-segment elevation acute coronary syndromes

    Abstract Background Many ST-segment elevation acute coronary syndrome (STEACS) patients fail to activate the Emergency Medical System (EMS), with possible dramatic consequences. Prior studies focusing on barriers to EMS activation include patients with any acute coronary syndrome (ACS) without representation of southern European populations. However, barriers are influenced by the ACS type and by socio-demographic and racial factors. Purpose We aimed to investigate the barriers to EMS call for patients diagnosed for STEACS in Italy. Methods A prospective, single-center, survey-based study, including all the patients treated with primary percutaneous coronary intervention for STEACS in a tertiary hospital in northern Italy from 1st June 2018 to 31st May 2020. Results The questionnaire was filled out by 293 patients. The majority of the participants were males (74%), married (70.4%), with a high-school degree (38.4%) and with a median age of 62 years. Chest pain as a possible symptom related to a cardiovascular attack is known by most of the respondents (89%), and left arm pain/shake by 53.7% of them, whilst the other possible signs and symptoms (i.e. dyspnea, asthenia, sweating, nausea, vomiting, dizziness) were unknown to the majority of the participants. Only 191 (65.2%) of the participants activated the EMS after symptoms onset. The main reasons for not calling EMS were the perception that symptoms were not related to an important health problem (45.5%) and that a private vehicle is faster than EMS to reach the hospital (34.7%). The median time to first medical contact was 60 minutes, and it was significantly higher in the patients who did not called EMS compared to those who did (180 [60–420] mins vs 35 [15–120] mins, p<0.001). The patients who called a private doctor after symptoms onset did not called EMS more frequently than those who did not (5.9% vs 8.2%, p=0.3). Moreover, 30% of the patients who did not call the EMS would still act in the same way if a new episode occurred and the main reasons for this were that they think to be faster than EMS (57.1%) and to live close to the hospital (17.9%). Analyzing predictors of EMS activation, only prior history of cardiovascular disease has been demonstrated to be a predictor of calling the EMS in case of symptoms suspected for STEACS. Conclusions Our study, from the southern Europe, showed that a substantial percentage of patients with symptoms suspected for STEACS preferred private vehicle rather than activating the EMS. Our results highlight the need for information campaigns targeted to both the general population and medical doctors, stressing that the EMS is faster than a private vehicle to direct the patient to the right hospital and increasing the awareness of the people on the type of possible heart attack symptoms, which seem to be the most neglected issues by patients who did not call the EMS.     Differential diagnosis between constitutional delay of growth and puberty, idiopathic growth hormone deficiency and congenital hypoogonadotropic hypogonadism: a clinical challenge for the pediatric endocrinologist

    INTRODUCTION: Differential diagnosis between constitutional delay of growth and puberty (CDGP), partial growth hormone deficiency (pGHD), and congenital hypogonadotropic hypogonadism (cHH) may be difficult. All these conditions usually present with poor growth in preor peri-pubertal age and they may recur within one familial setting, constituting a highly variable, but somehow common, spectrum of pubertal delay. EVIDENCE ACQUISITION: Narrative review of the most relevant English papers published between 1981 and march 2020 using the following search terms \u201cconstitutional delay of growth and puberty\u201d, \u201ccentral hypogonadism\u201d, \u201cpriming\u201d, \u201cgrowth hormone deficiency\u201d, \u201cpituitary\u201d, \u201cpituitary Magnetic Resonance Imaging\u201d, with a special regard to the latest scientific acquisitions. EVIDENCE SYNTHESIS: CDGP is by far the most prevalent entity in boys and recurs within families. pGHD is a rare, often idiopathic and transient condition, where hypostaturism presents more severely. Specificity of pGHD diagnosis is increased by priming children before growth hormone stimulation test (GHST); pituitary MRI and genetic analysis are recommended to personalize future follow-up. Diagnosing cHH may be obvious when anosmia and eunuchoid proportions concomitate. However, cHH can either overlap with pGHD in forms of multiple pituitary hormone deficiencies (MPHD) or syndromic conditions either with CDGP in family pedigrees, so endocrine workup and genetic investigations are necessary. The use of growth charts, bone age, predictors of adult height, primed GHST and low dose sex steroids (LDSS) treatment are recommended. CONCLUSIONS: Only a step-by-step diagnostic process based on appropriate endocrine and genetic markers together with LDSS treatment can help achieving the correct diagnosis and optimizing outcomes

    This paper monitors neologisms in contemporary French in the domains of ecology and the environment (Galazzi Jullion 2008): a corpus of traditional French newspaper texts and an electronic corpus taken from Google are explored with the aim of extracting contemporary usage of of the adjective green, whose polysemy continues to multiply. According to our corpus, the semantic extension of vert (green) has now reached the domains of ecology, politics, the environment, economics, architecture, health, employment and leisure, as seen by the examples of allusions, un-fixed expressions, homophonic puns, portmanteau words, compound words and collocations of various degrees of transparency.La langue ne cesse de bouger, de s\u2019adapter au monde et \ue0 la soci\ue9t\ue9 qui changent, avec une souplesse et une dynamique extraordinaires.
