    What do people know about the heritability of sleep?

    Study Objectives Twin studies have provided data about the relative weight of genetic and environmental factors on sleep variables over the last few decades. However, heritability is a non-intuitive concept and it is often misunderstood even amongst the scientific community. This study aimed to analyze: (1) understanding of the meaning of heritability of insomnia; (2) the accuracy of estimations of heritability in the general population regarding three sleep traits (sleep duration, sleep quality and insomnia); (3) perceptions of the effectiveness of different treatments for insomnia depending on how the disorder is presented (i.e. having an environmental or genetic etiology) and whether the subject’s estimate of genetic influence on sleep traits impacted beliefs about the effectiveness of different treatments. Methods Participants (N = 3658) completed a survey which included: questions about general genetic knowledge; a specific question about the meaning of heritability; estimates of heritability of three different sleep traits; and the effectiveness of different treatments for insomnia depending on how the etiology of this condition was presented. Results Fewer than 25% of the participants selected the correct description of the heritability of insomnia. Almost half of the sample incorrectly believed that heritability refers to the chance of passing a disorder onto their children. We also found that participants provided different estimates for the effectiveness of different treatments depending on the presumed etiology of the disorder. Conclusion Most people do not have accurate knowledge about the concept of heritability. People’s assumptions about the etiology of a disorder may influence which treatments they consider most effective

    Slipping to the Extreme: A Mixed Method to Explain How Extreme Opinions Infiltrate Online Discussions

    Qualitative research provides methodological guidelines for observing and studying communities and cultures on online social media platforms. However, such methods demand considerable manual effort from researchers and can be overly focused and narrowed to certain online groups. This work proposes a complete solution to accelerate the qualitative analysis of problematic online speech, focusing on opinions emerging from online communities by leveraging machine learning algorithms. First, we employ qualitative methods of deep observation for understanding problematic online speech. This initial qualitative study constructs an ontology of problematic speech, which contains social media postings annotated with their underlying opinions. The qualitative study dynamically constructs the set of opinions, simultaneous with labeling the postings. Next, we use keywords to collect a large dataset from three online social media platforms (Facebook, Twitter, and Youtube). Finally, we introduce an iterative data exploration procedure to augment the dataset. It alternates between a data sampler --- which balances exploration and exploitation of unlabeled data --- the automatic labeling of the sampled data, the manual inspection by the qualitative mapping team, and, finally, the retraining of the automatic opinion classifiers. We present both qualitative and quantitative results. First, we show that our human-in-the-loop method successfully augments the initial qualitatively labeled and narrowly focused dataset and constructs a more encompassing dataset. Next, we present detailed case studies of the dynamics of problematic speech in a far-right Facebook group, exemplifying its mutation from conservative to extreme. Finally, we examine the dynamics of opinion emergence and co-occurrence, and we hint at some pathways through which extreme opinions creep into the mainstream online discourse

    Patient Preferences for Lung Cancer Treatment: A Qualitative Study Protocol Among Advanced Lung Cancer Patients

    Introduction: Lung cancer is the deadliest and most prevalent cancer worldwide. Lung cancer treatments have different characteristics and are associated with a range of benefits and side effects for patients. Such differences may raise uncertainty among drug developers, regulators, payers, and clinicians regarding the value of these treatment effects to patients. The value of conducting patient preference studies (using qualitative and/or quantitative methods) for benefits and side effects of different treatment options has been recognized by healthcare stakeholders, such as drug developers, regulators, health technology assessment bodies, and clinicians. However, evidence-based guidelines on how and when to conduct and use these studies in drug decision-making are lacking. As part of the Innovative Medicines Initiative PREFER project, we developed a protocol for a qualitative study that aims to understand which treatment characteristics are most important to lung cancer patients and to develop attributes and levels for inclusion in a subsequent quantitative preference survey. Methods: The study protocol specifies a four-phased approach: (i) a scoping literature review of published literature, (ii) four focus group discussions with stage III and IV Non-Small Cell Lung Cancer patients, (iii) two nominal group discussions with stage III and IV Non-Small Cell Lung Cancer patients, and (iv) multi-stakeholder discussions involving clinicians and preference experts. Discussion: This protocol outlines methodological and practical steps as to how qualitative research can be applied to identify and develop attributes and levels for inclusion in patient preference studies aiming to inform decisions across the drug life cycle. The results of this study are intended to inform a subsequent quantitative preference survey that assesses patient trade-offs regarding lung cancer treatment options. This protocol may assist researchers, drug developers, and decision-makers in designing qualitative studies to understand which treatment aspects are most valued by patients in drug development, regulation, and reimbursement

    Stability Enhancement in OPV: In-Situ Studies of Plasmonic Devices

    Extended Abstract Bulk heterojunction (BHJ) organic photovoltaic (OPV) devices attracted considerable research interest due to several significant characteristics, such as their flexibility, lightweight, low environmental impact and reduced cost of large-scale production. A key aspect in OPVs is improving the device long-term stability: in this contest, novel organic materials and cell architectures are developed and a significant task is played by the development of in-situ diagnostics able to detect the driving mechanisms of device degradation, considering that the optical and transport properties of the device active elements depend on the structural, morphological and interfacial characteristics. In this work we discuss our recent results on devices incorporating metallic nanoparticles (NPs) in the photoactive layer, in order to take advantage of the ability of the metallic NPs to rise the BHJ optical absorption by the excitation of Localized Surface Plasmon Resonance. Both plasmonic and reference systems are studied and a powerful approach for addressing the role of structural/morphological and interface properties of the different layers and their interfaces is used: time-resolved Energy Dispersive X-ray Reflectivity/Diffraction (EDXR/EDXD) techniques are applied jointly with in-situ atomic force microscopy (AFM).     Patient Preferences for Lung Cancer Treatments: A Study Protocol for a Preference Survey Using Discrete Choice Experiment and Swing Weighting

    Background: Advanced treatment options for non-small cell lung cancer (NSCLC) consist of immunotherapy, chemotherapy, or a combination of both. Decisions surrounding NSCLC can be considered as preference-sensitive because multiple treatments exist that vary in terms of mode of administration, treatment schedules, and benefit–risk profiles. As part of the IMI PREFER project, we developed a protocol for an online preference survey for NSCLC patients exploring differences in preferences according to patient characteristics (preference heterogeneity). Moreover, this study will evaluate and compare the use of two different preference elicitation methods, the discrete choice experiment (DCE) and the swing weighting (SW) task. Finally, the study explores how demographic (i.e., age, gender, and educational level) and clinical (i.e., cancer stage and line of treatment) information, health literacy, health locus of control, and quality of life may influence or explain patient preferences and the usefulness of a digital interactive tool in providing information on preference elicitation tasks according to patients. Methods: An online survey will be implemented with the aim to recruit 510 NSCLC patients in Belgium and Italy. Participants will be randomized 50:50 to first receive either the DCE or the SW. The survey will also collect information on participants' disease-related status, health locus of control, health literacy, quality of life, and perception of the educational tool. Discussion: This protocol outlines methodological and practical steps to quantitatively elicit and study patient preferences for NSCLC treatment alternatives. Results from this study will increase the understanding of which treatment aspects are most valued by NSCLC patients to inform decision-making in drug development, regulatory approval, and reimbursement. Methodologically, the comparison between the DCE and the SW task will be valuable to gain information on how these preference methods perform against each other in eliciting patient preferences. Overall, this protocol may assist researchers, drug developers, and decision-makers in designing quantitative patient preferences into decision-making along the medical product life cycle

    Occurrence and limited zoonotic potential of Cryptosporidium spp., Giardia duodenalis, and Balantioides coli infections in free-ranging and farmed wild ungulates in Spain

    Little information is currently available on the occurrence and molecular diversity of the enteric protozoan parasites Cryptosporidium spp., Giardia duodenalis, and Balantioides coli in wild ungulates and the role of these host species as potential sources of environmental contamination and consequent human infections. The presence of these three pathogens was investigated in eight wild ungulate species present in Spain (genera Ammotragus, Capra, Capreolus, Cervus, Dama, Ovis, Rupicapra, and Sus) by molecular methods. Faecal samples were retrospectively collected from free-ranging (n = 1058) and farmed (n = 324) wild ungulates from the five Spanish bioregions. Overall infection rates were 3.0% (42/1382; 95% CI: 2.1–3.9%) for Cryptosporidium spp., 5.4% (74/1382; 95% CI: 4.2–6.5%) for G. duodenalis, and 0.7% (9/1382; 95% CI: 0.3–1.2%) for B. coli. Cryptosporidium infection was detected in roe deer (7.5%), wild boar (7.0%) and red deer (1.5%), and G. duodenalis in southern chamois (12.9%), mouflon (10.0%), Iberian wild goat (9.0%), roe deer (7.5%), wild boar (5.6%), fallow deer (5.2%) and red deer (3.8%). Balantioides coli was only detected in wild boar (2.5%, 9/359). Sequence analyses revealed the presence of six distinct Cryptosporidium species: C. ryanae in red deer, roe deer, and wild boar; C. parvum in red deer and wild boar; C. ubiquitum in roe deer; C. scrofarum in wild boar; C. canis in roe deer; and C. suis in red deer. Zoonotic assemblages A and B were detected in wild boar and red deer, respectively. Ungulate-adapted assemblage E was identified in mouflon, red deer, and southern chamois. Attempts to genotype samples positive for B. coli failed. Sporadic infections by canine- or swine-adapted species may be indicative of potential cross-species transmission, although spurious infections cannot be ruled out. Molecular evidence gathered is consistent with parasite mild infections and limited environmental contamination with (oo)cysts. Free-ranging wild ungulate species would not presumably play a significant role as source of human infections by these pathogens. Wild ruminants do not seem to be susceptible hosts for B. coli

    Câncer de colo de útero: uma análise multifatorial, com ênfase nos métodos preventivos: Uterus cancer: a multifactorial analysis, with emphasis on preventive methods

    OBJETIVO: Estudar o câncer de colo de útero no que se refere a sua fisiopatologia, fatores de risco, epidemiológicos e desencadeantes, diagnóstico e tratamento, enfatizando os métodos preventivos primários e secundários. METODOLOGIA: Foi realizada uma revisão narrativa de literatura. As bases eletrônicas exploradas foram o Google Acadêmico, SCIELO (Scientific Electronic Library Online) e BVS (Biblioteca Virtual em Saúde), além da utilização de um acervo bibliográfico virtual. O período de abrangência incluiu artigos de 2010 a 2020. Aplicou- se os seguintes descritores: Neoplasias do Colo do Útero, exame citopatológico, Prevenção, Vacinação e HPV. RESULTADOS: Foram analisados 16 artigos que abrangeram aspectos do câncer de colo de útero, desde a fisiopatologia, epidemiologia, fatores de risco e desencadeantes, aos métodos preventivos e conduta. CONCLUSÃO: O câncer de colo de útero é uma neoplasia causada por uma infecção do vírus HPV que gera grandes impactos na vida das pacientes e que pode ser evitada com a utilização de métodos preventivos primários e secundários. Dessa maneira, o estudo apresentado é necessário para compreensão dessa patologia, a fim de preveni-la e tratá-la da melhor maneira possível. Assim, intervenções com o intuito de minimizar os efeitos dessa neoplasia devem ser realizadas, como por exemplo, nas Estratégias Saúde da Família (ESF) por meio de projetos de educação em saúde a fim de oferecer para a população local uma assistência completa

    Apolipoprotein C3 Polymorphisms, Cognitive Function and Diabetes in Caribbean Origin Hispanics

    Apolipoprotein C3 (APOC3) modulates triglyceride metabolism through inhibition of lipoprotein lipase, but is itself regulated by insulin, so that APOC3 represents a potential mechanism by which glucose metabolism may affect lipid metabolism. Unfavorable lipoprotein profiles and impaired glucose metabolism are linked to cognitive decline, and all three conditions may decrease lifespan. Associations between apolipoprotein C3 (APOC3) gene polymorphisms and impaired lipid and glucose metabolism are well-established, but potential connections between APOC3 polymorphisms, cognitive decline and diabetes deserve further attention.We examined whether APOC3 single nucleotide polymorphisms (SNPs) m482 (rs2854117) and 3u386 (rs5128) were related to cognitive measures, whether the associations between cognitive differences and genotype were related to metabolic differences, and how diabetes status affected these associations. Study subjects were Hispanics of Caribbean origin (n = 991, aged 45-74) living in the Boston metropolitan area.Cognitive and metabolic measures differed substantially by type II diabetes status. In multivariate regression models, APOC3 m482 AA subjects with diabetes exhibited lower executive function (P = 0.009), Stroop color naming score (P = 0.014) and Stroop color-word score (P = 0.022) compared to AG/GG subjects. APOC3 m482 AA subjects with diabetes exhibited significantly higher glucose (P = 0.032) and total cholesterol (P = 0.028) compared to AG/GG subjects. APOC3 3u386 GC/GG subjects with diabetes exhibited significantly higher triglyceride (P = 0.004), total cholesterol (P = 0.003) and glucose (P = 0.016) compared to CC subjects.In summary, we identified significant associations between APOC3 polymorphisms, impaired cognition and metabolic dysregulation in Caribbean Hispanics with diabetes. Further research investigating these relationships in other populations is warranted
