    Controlled Synchronization of One Class of Nonlinear Systems under Information Constraints

    Output feedback controlled synchronization problems for a class of nonlinear unstable systems under information constraints imposed by limited capacity of the communication channel are analyzed. A binary time-varying coder-decoder scheme is described and a theoretical analysis for multi-dimensional master-slave systems represented in Lurie form (linear part plus nonlinearity depending only on measurable outputs) is provided. An output feedback control law is proposed based on the Passification Theorem. It is shown that the synchronization error exponentially tends to zero for sufficiantly high transmission rate (channel capacity). The results obtained for synchronization problem can be extended to tracking problems in a straightforward manner, if the reference signal is described by an {external} ({exogenious}) state space model. The results are applied to controlled synchronization of two chaotic Chua systems via a communication channel with limited capacity.Comment: 8 pages, 2 figure

    Empirical Evaluation of Educational Service Quality in the Current Higher Education System

    During the transition to the mixed education model during the Covid-19 pandemic, some issues have emerged in the matter of providing and supporting the quality of university education in all countries, including educational programs' relevance to the demands of the labor market, expectations of students and their families, digital transformation of the educational process, and economic stability of institution networks. The purpose of the research was to evaluate the quality of educational services in the higher education systems of Indonesia and Russia. The study systematized several essential factors for achieving educational service quality in the current higher education systems, given the limitations imposed by Covid-19. Following this objective, the study utilized a quantitative approach, adopting the SERVQUALmodel that measures the service quality across five service dimensions: tangibility, reliability, responsiveness, assurance, and empathy. The model aided the development of the questionnaire to assess the quality of the educational services of Indonesian and Russian universities. The questionnaire included 25 close-ended questions on two scales: one to measure the students' expectations regarding the quality of educational services and the other to measure the students' current perceptions. A snowball technique was used to recruit students from Indonesian and Russian universities as participants in the study. The students were asked to assess the educational services of their universities based on their initial expectations and current perceptions. Overall, the students of both countries revealed that they had high expectations from the educational services of their universities. However, the current perceptions of the Russian students regarding the quality of educational services provided by their universities are higher and more positive than the perceptions of the Indonesian students regarding the quality of educational services being delivered to them.     Features of heart remodeling in patients with chronic obstructive pulmonary disease in combination with chronic heart failure

    Goal. Clinical, structural and functional features of right and left heart remodeling in patients with chronic obstructive pulmonary disease (COPD) were studied. Materials and methods. We examined 32 patients with COPD without exacerbation. Patients were divided into 2 groups: 1 - patients with COPD in combination with chronic heart failure (CHF); 2 - patients with COPD without CHF. 19 patients showed signs of CHF. Echocardiography (EchoCG) with additional parameters, spirometry, radiography, pulse oximetry was performed. The study also assessed level of highly sensitive C-reactive protein, acid-base blood composition, brain natriuretic peptide (NTproBNP). Results. In patients with combined pathology, there was a decrease in exercise tolerance, a greater body mass index and intensity of dyspnea. The left ventricular ejection fraction over 50% observed in the majority of patients (30) in both groups, therefore the signs of heart failure are explained precisely by diastolic dysfunction of the right and left ventricles. The conclusion. The use of tissue Doppler studies allows more accurate determination of the diastolic function of the right and left ventricles. Remodeling of the right and left ventricles are interdependent processes. The study of NTproBNP and additional EchoCG parameters - allow to reveal diastolic dysfunction of both ventricles of the heart at early stages.Цель. Исследовать клинические, структурно-функциональные особенности ремоделирования правых и левых отделов сердца у больных хронической обструктивной болезнью легких (ХОБЛ) в сочетании с хронической сердечной недостаточностью (ХСН). Материалы и методы. Обследовано 32 больных ХОБЛ вне обострения. Пациенты были распределены на 2 группы: 1 - пациенты с ХОБЛ в сочетании с ХСН (19); 2 - пациенты с ХОБЛ без ХСН (13). Проводили эхокардиографию, спирометрию, рентгенографию, пульсоксиметрию. Оценивали уровень высокочувствительного С-реактивного белка, кислотно-щелочного состава крови, мозгового натрийуретического пептида (NTproBNP). Результаты. У больных с сочетанной патологией отмечались снижение толерантности к физической нагрузке, больший индекс массы тела и интенсивность одышки. Сохраненная фракция выброса левого желудочка более 50% была отмечена у большинства больных (30) в обеих группах, поэтому признаки сердечной недостаточности объясняется именно диастолической дисфункцией правого и левого желудочков. Заключение. Использование тканевого допплеровского исследования позволяет более точно определить диастолическую функцию правого и левого желудочков. Ремоделирование правого и левого желудочков являются взаимозависимыми процессами. Исследование NTproBNP и дополнительных ЭхоКГ-параметров позволяют выявить диастолическую дисфункцию правого и левого желудочков сердца на ранних стадиях

    The aim of the work – to evaluate the state of development of pediatric education in Ukraine in the beginning of the 21st century, staffing of the branch in the field of pediatrics and to develop a plan of measures to overcome the crisis phenomena in pediatric health care. The main body. The article presents data on the development of pediatric education in the early 21st century after the restoration of the educational-scientific specialty “Pediatrics” in 2017. The development of up-to-date standards for the training of pediatric doctors, based on the acquisition of future master’s degrees in pediatrics, general and professional competencies and necessary practical skills is highlighted. The current problems in the provision of pediatric health care facilities are emphasized. Conclusion. A list of measures at the sectoral and national level has been developed to restore the necessary training рediatricians for childrenʼs health care of the country and to ensure the proper methodological provision of the educational process, which may cause difficulties in providing children with quality medical care in the future.Мета роботи – оцінити стан розвитку педіатричної освіти в Україні на початку ХХІ сторіччя, кадрове забезпечення галузі за спеціальністю «Педіатрія» та розробити план заходів щодо подолання кризових явищ у педіатричній охороні здоровʼя. Основна частина. У статті наведено дані щодо розвитку педіатричної освіти на початку ХХІ сторіччя після відновлення освітньо-наукової спеціальності «Педіатрія» у 2017 р. Висвітлено розробку сучасних стандартів підготовки дитячих лікарів, що базуються на набутті майбутніми магістрами педіатрії загальних та фахових компетенцій і необхідних практичних навичок. Наголошено на теперішніх проблемах у кадровому забезпеченні педіатричних закладів охорони здоровʼя, що може викликати складнощі у забезпеченні дітей якісною медичною допомогою у майбутньому. Висновок. Розроблений перелік заходів на галузевому та загальнодержавному рівні щодо відновлення необхідних для країни обсягів підготовки дитячих лікарів та забезпечення належного методичного забезпечення навчального процесу

    The COVID-19 pandemic and its global effects on dental practice. An international survey

    Objectives A multicentre survey was designed to evaluate the impact of COVID-19 outbreak on dental practice worldwide, estimate the COVID-19 related symptoms/signs, work attitudes and behaviour and the routine use of protective measures and personal protective equipment (PPE). Methods A global survey using a standardized questionnaire with research groups from 36 countries was designed. The questionnaire was developed and pretested during April 2020 and contained three domains: 1) personal data; 2) COVID-19 positive rate and symptoms/signs presumably related to the coronavirus; 3) working conditions and PPE adopted after the outbreak. Countries’ data were grouped by the country positive rate (CPR) during the survey period and by Gross-National-Income per capita. An ordinal multinomial logistic regression model was carried out with COVID-19 self-reported rate referred by dental professionals as dependent variable to assess the association with questionnaire items. Results A total of 52,491 questionnaires were returned with a male/female ratio of 0.63. Out of the total respondents, 7,859 dental professionals (15%) reported symptoms/signs compatible with COVID-19. More than half of the sample (n=27,818; 53%) stated to use FFP2/N95 masks, while 21,558 (41.07%) used eye protection. In the bivariate analysis, CPR and N95/FFP2 were significantly associated (OR=1.80 95%CI=1.60/2.82 and OR=5.20 95%CI=1.44/18.80, respectively), while Gross-National-Income was not statistically associated with CPR (OR=1.09 95%CI=0.97/1.60). The same significant associations were observed in the multivariate analysis. Conclusions Oral health service provision has not been significantly affected by COVID-19, although access to routine dental care was reduced due to country-specific temporary lockdown periods. While the dental profession has been identified at high-risk, the reported rates of COVID-19 for dental professionals were not significantly different to those reported for the general population in each country. These findings may help to better plan oral health care for future pandemic events
