Limitations and perceived delays for diagnosis and staging of lung cancer in Portugal: A nationwide survey analysis
Background: We aimed to identify the perception of physicians on the limitations and delays for diagnosing, staging and treatment of lung cancer in Portugal.
Methods: Portuguese physicians were invited to participate an electronic survey (Feb-Apr-2020). Descriptive statistical analyses were performed, with categorical variables reported as absolute and relative frequencies, and continuous variables with non-normal distribution as median and interquartile range (IQR). The association between categorical variables was assessed through Pearson's chi-square test. Mann-Whitney test was used to compare categorical and continuous variables (Stata v.15.0).
Results: Sixty-one physicians participated in the study (45 pulmonologists, 16 oncologists), with n = 26 exclusively assisting lung cancer patients. Most experts work in public hospitals (90.16%) in Lisbon (36.07%). During the last semester of 2019, responders performed a median of 85 (IQR 55-140) diagnoses of lung cancer. Factors preventing faster referral to the specialty included poor articulation between services (60.0%) and patients low economic/cultural level (44.26%). Obtaining National Drugs Authority authorization was one of the main reasons (75.41%) for delaying the begin of treatment. The cumulative lag-time from patients' admission until treatment ranged from 42-61 days. Experts believe that the time to diagnosis could be optimized in around 11.05 days [IQR 9.61-12.50]. Most physicians (88.52%) started treatment before biomarkers results motivated by performance status deterioration (65.57%) or high tumor burden (52.46%). Clinicians exclusively assisting lung cancer cases reported fewer delays for obtaining authorization for biomarkers analysis (p = 0.023). Higher waiting times for surgery (p = 0.001), radiotherapy (p = 0.004), immunotherapy (p = 0.003) were reported by professionals from public hospitals.
Conclusions: Physicians believe that is possible to reduce delays in all stages of lung cancer diagnosis with further efforts from multidisciplinary teams and hospital administration.his work was supported by AstraZeneca. Biological characterization of Cynara cardunculus L. Methanolic extracts: antioxidant, anti-proliferative, anti-migratory and anti-angiogenic activities
Cynara cardunculus (Cc) is a multipurpose species; beyond its use in southwestern European cuisine, it is also used for the production of solid biofuel, seed oil, biodiesel, paper pulp and cheese, as well as animal feed. In addition, Cc has a long tradition of use in folk medicine as a diuretic and liver protector. The value of this species as a source of bioactive compounds is known; however, pharmacological use would further increase its cultivation. The main goal of the current work was to evaluate the potential of Cc as source of anti-carcinogenic phytochemicals. Different methanolic extracts obtained from wild and cultivated plants were tested for antioxidant activity and effect on breast tumor cell viability. The most effective extract, both as antioxidant and inhibition of tumor cell viability, was tested for effects on angiogenesis and tumor cell migration capacity. All the extracts tested had high antioxidant activity; however, only green leaves and dry head extracts exhibit anti-proliferative activity. Green cultivated leaves (GCL) were the most effective extract both as antioxidant and inhibiting the proliferation of tumor cells; it is equally active inhibiting tumor cell migration and in vivo angiogenesis. Use of Veterinary Vaccines for Livestock as a Strategy to Control Foodborne Parasitic Diseases
Foodborne diseases (FBDs) are a major concern worldwide since they are associated with high mortality and morbidity in the human population. Among the causative agents of FBDs, Taenia solium, Echinococcus granulosus, Toxoplasma gondii, Cryptosporidium spp., and Trichinella spiralis are listed in the top global risk ranking of foodborne parasites. One common feature between them is that they affect domestic livestock, encompassing an enormous risk to global food production and human health from farm to fork, infecting animals, and people either directly or indirectly. Several approaches have been employed to control FBDs caused by parasites, including veterinary vaccines for livestock. Veterinary vaccines against foodborne parasites not only improve the animal health by controlling animal infections but also contribute to increase public health by controlling an important source of FBDs. In the present review, we discuss the advances in the development of veterinary vaccines for domestic livestock as a strategy to control foodborne parasitic diseases.Fil: Sander, Valeria Analia. MINERALIZAÇÕES DE ZINCO E CHUMBO DO DEPÓSITO SALOBRO, PORTEIRINHA (MG)
A regional prospect program carried out by CVRD since the 70´s to find out gold and base metals deposits discovered the Salobro Zn-Pb deposit among others. It is located in the Porteirinha district in the northern area of Minas Gerais state. Three units were defined in the Riacho dos Machados Group (A, B, C from bottom to top), an Arquean or Paleoproterozoic metassedimentary sequence. The geological evaluation defined 8 mt@7% (Zn+Pb) measured for a cut off of 5% and a minimum width of 2,00 m. The Riacho dos Machados Group hosts the mineralization in a metachert layer in the unit B. The mineralization is essentially stratabound. Sphalerite and galena are the ore minerals, both with coarse granulometry imposed by metamorphic processes. The iron content of sphalerite ranges from 2,4% to 7,9%. The mineralogy of the ore-gangue is composed mainly by amphiboles, quartz and magnetite. Pyrite and pyrrhotite are present disseminated in the ore and in several massive and thick bodies. In spite of being a small deposit, Salobro has favorable features for exploitation.O depósito de zinco-chumbo Salobro, localizado no município de Porteirinha (MG), foi descoberto dentro do programa regional de prospecção de ouro e metais base executado pela CVRD por mais de três décadas (desde os anos 70) na região de Riacho dos Machados, no norte do estado de Minas Gerais. O Grupo Riacho dos Machados foi individualizado na região em três unidades litoestratigráficas denominadas A, B e C da base para o topo. A avaliação geológica definiu recursos de 8 mt@7% (Zn+Pb) para um teor de corte de 5% e uma espessura mínima de 2 m. As mineralizações ocorrem na seqüência metassedimentar do Grupo Riacho dos Machados e estão hospedadas em uma camada de metacherte da unidade B. A mineralização é essencialmente do tipo stratabound. Esfalerita e galena são os minerais econômicos, ambos apresentando granulometria grossa devido ao metamorfismo regional. O teor de ferro da esfalerita varia entre 2,4% e 7,9%. A ganga do minério é composta principalmente por quartzo, anfibólio e magnetita. Pirita e pirrotita estão presentes disseminadas no minério e em vários corpos maciços e espessos. Apesar de tratar-se de um depósito de dimensões reduzidas, o Salobro apresenta características competitivas e favoráveis à sua explotação
Experiences of Female Patients Diagnosed with Rheumatoid Arthritis in a Public Hospital in Paraguay: Qualitative Study
Introducción: La artritis reumatoide es una patología caracterizada por dolor en las articulaciones, fatiga, dificultad para realizar actividades cotidianas, inflamación persistente y daño progresivo en los tejidos. Según estudios previos, la artritis reumatoide es significativamente más frecuente en mujeres que en hombres. Objetivo: Analizar las experiencias vividas por mujeres con artritis reumatoide, pacientes de un hospital público en la ciudad de Asunción durante el año 2024. Metodología: Estudio cualitativo con un diseño fenomenológico. Se entrevistó a 15 mujeres con artritis reumatoide que se atienden en un hospital público de Asunción, Paraguay. Resultados: Se identificaron tres temas centrales: el primero, impacto en la vida diaria, donde las pacientes deben adaptar su rutina diaria debido a los síntomas de la enfermedad; el segundo, manejo emocional y social, en el cual el apoyo de la familia y amigos es esencial para facilitar el manejo de la enfermedad y proporcionar soporte emocional; y el tercero, tratamiento y manejo de la enfermedad, destacando que el uso de medicamentos es crucial para controlar los síntomas y reducir la progresión de la artritis reumatoide. Conclusiones: La artritis reumatoide impacta significativamente la vida diaria, el manejo emocional y el tratamiento de las mujeres, subrayando la necesidad de un enfoque integral que incluya adaptaciones en la rutina, apoyo social constante y un uso adecuado de medicamentos para mejorar su calidad de vida.Introduction: Rheumatoid arthritis is a condition characterized by joint pain, fatigue, difficulty in performing daily activities, persistent inflammation, and progressive tissue damage. According to previous studies, rheumatoid arthritis is significantly more common in women than in men. Objective: To analyze the experiences of women with rheumatoid arthritis, patients of a public hospital in the city of Asunción during the year 2024. Methodology: A qualitative study with a phenomenological design. Fifteen women with rheumatoid arthritis who receive care at a public hospital in Asunción, Paraguay, were interviewed. Results: Three central themes were identified: first, impact on daily life, where patients must adapt their daily routines due to the symptoms of the disease; second, emotional and social management, in which the support of family and friends is essential to facilitate the management of the disease and provide emotional support; and third, treatment and disease management, highlighting that the use of medication is crucial to controlling symptoms and reducing the progression of rheumatoid arthritis. Conclusions: Rheumatoid arthritis significantly impacts daily life, emotional management, and treatment in women, underscoring the need for a comprehensive approach that includes routine adaptations, constant social support, and appropriate use of medication to improve their quality of life
MEMÓRIA E FORMAÇÃO DOCENTE: UMA ANÁLISE DO SUBPROJETO PIBID DO CAMPUS UNIVERSITÁRIO DE ABAETETUBA, ESTADO DO PARÁ.
O presente artigo tem como objetivo apresentar os resultados da pesquisa intitulada Memória docente e narrativas de resistência na Universidade, que foi desenvolvida com professores do Campus Universitário de Abaetetuba, Universidade Federal do Pará, com o objetivo central de interpretar e registrar as estratégias e dispositivos de resistência docente aos modelos impositivos de instalação da Universidade heterônoma e neoprofissional, a partir da memória docente focalizando as narrativas dos professores do Campus Universitário de Abaetetuba. Desse modo, verificamos que os professores baseiam-se na reflexão como forma de vivenciar e compreender a esfera do mundo da vida e do sistema, utilizando-se do trabalho intelectual como atividade de produção do conhecimento e da ação comunicativa para compartilhar da mesma linguagem, em uma estrutura dinâmica suscetível de inovações que possibilita a transformação da experiência humana na ação individual e coletiva
Comparison with adult-onset rheumatic diseases
Objective To compare physical disability, mental health, fatigue and health-related quality of life (HRQoL) across juvenile idiopathic arthritis (JIA) categories in adulthood and between JIA and adult-onset rheumatic diseases. Methods Cross-sectional analysis nested in a cohort of adult patients with JIA registered in the Rheumatic Diseases Portuguese Register (Reuma.pt). Physical disability (Health Assessment Questionnaire - Disability Index), mental health symptoms (Hospital Anxiety and Depression Scale), fatigue (Functional Assessment of Chronic Illness Therapy - Fatigue Scale (FACIT-F)) and HRQoL (EuroQol-5D (EQ5D) and Short Form (SF-36)) were compared across JIA categories. Patients with polyarticular JIA and enthesis-related arthritis (ERA) JIA were compared respectively to patients with rheumatoid arthritis (RA) and spondyloarthritis (SpA), matched for gender and age, adjusted for disease duration and activity. Results 585 adult patients with JIA were included. Comparison across JIA categories showed that persistent oligoarthritis and patients with ERA reported a higher score in EQ5D and SF-36 physical component when compared with other JIA categories. Polyarticular JIA reported less disability and fatigue than patients with RA (median Health Assessment Questionnaire of 0.25 vs 0.63; p<0.001 and median FACIT-F score 42 vs 40; p=0.041). Polyarticular JIA had also better scores on EQ5D and all domains of SF-36, than patients with RA. Patients with ERA reported less depression and anxiety symptoms (0% vs 14.8%; p=0.003% and 9% vs 21.3%; p=0.002) and less fatigue symptoms (45 vs 41; p=0.01) than patients with SpA. Conclusion Persistent oligoarticular JIA and ERA are the JIA categories in adulthood with better HRQoL. Overall, adult polyarticular and patients with ERA JIA have lower functional impairment and better quality-of-life than patients with RA and SpA.publishersversionpublishe
