    DAMPAK MISMATCH PENDIDIKAN-PEKERJAAN TERHADAP PENGEMBANGAN KEAHLIAN KARYAWAN DI HALMAHERA UTARA

    This study aims to determine how the impact of job-education mismatch on the development of employee skills in North Halmahera. This study used descriptive qualitative method. The method of data collection was done by means of in-depth interviews with respondents who experienced a discrepancy between education and work. The results show that the impact of the job-education mismatch is divided into 2 (two) parts, namely on competence and psychology and the impact on employee skill development. Impacts on employee competence and psychology include, employees becoming less focused, having difficulty in carrying out work, insecure and even having an impact on psychology which makes it difficult for employees to devote their expertise in the workplace as a whole. The impact on skill development requires support from the organization to support employee skill development, in the form of awards, in the form of salary increases (giving bonuses and promotions), transfers and rotations, coaching, training, mentoring and scholarships for further studies. Beside company support, work motivation is also a factor in skill development. Penelitian ini bertujuan untuk mengetahui bagaimana dampak mismatch pendidikan dan pekerjaan terhadap pengembangan keahlian karyawan di Halmahera Utara. Penelitian ini menggunakan metode deskriptif kualitatif. Metode pengumpulan data dilakukan dengan cara wawancara secara mendalam terhadap responden yang mengalami ketidaksesuaian antara pendidikan dan pekerjaan. Hasil penelitian menunjukan bahwa dampak mismatch pendidikan pekerjaan  terbagi atas 2 (dua) bagian yakni terhadap kompetensi dan psikologis dan dampak terhadap pengembangan keahlian karyawan. Dampak terhadap kompetensi dan psikologi karyawan diantaranya, karyawan menjadi kurang fokus, kesulitan dalam menjalankan pekerjaan, insecure bahkan sampai berdampak pada psikologis yang membuat karyawan sulit mencurahkan keahliannya di tempat kerja secara menyeluruh. Dampak terhadap pengembangan keahlian diperlukan dukungan dari organisasi untuk menunjang pengembangan keahlian karyawan, berupa penghargaan berupa kenaikan gaji (pemberian bonus dan promosi jabatan), mutasi dan rotasi, Coaching, Tranining, Mentoring dan beasiswa lanjut studi. Selain dukungan perusahaan, motivasi kerja karyawan menjadi penentu dalam pengembangan keahlian karyawan

    Clinical skills of veterinary students - a cross-sectional study of the self-concept and exposure to skills training in Hannover, Germany

    Background: Students of veterinary medicine should achieve basic professional competences required to practise their profession. A main focus of veterinary education is on developing clinical skills. The present study used the guidelines of the "Day-One Skills" list of European Association of Establishments for Veterinary Education (EAEVE) to create an online questionnaire for assessing the skills acquired by students at the University of Veterinary Medicine Hannover (TiHo). The theoretical and practical veterinary knowledge levels of the students and postgraduates are determined and compared. Results: In two batches, 607 people responded (response batch 1, 23.78%; response batch 2, 23.83%). From 49 defined skills, 28 are actually practised during training at the university and 21 activities are known only theoretically. Furthermore, the students showed great willingness to use simulators and models in a clinical skills lab. Conclusions: The results of this survey highlight that the opening of a clinical skills lab at the University of Veterinary Medicine Hannover and its incorporation into the study programme are ideal tools to promote practical competences and foster the motivation to learn

    “Does Short-term variation in fetal heart rate predict fetal acidaemia?” A Systematic review and meta-analysis

    Objective: To evaluate the association of short-term variation (STV) of the fetal heart rate in predicting fetal acidaemia at birth. Methods: The search strategy employed searching of electronic databases (MEDLINE, Web of Science, Scopus, and Google Scholar) and reference lists of relevant studies. Data was extracted from studies, adhering strictly to the following criteria: singleton pregnancy at ≥ 24 weeks gestation, computerised CTG (index test) and calculation of STV before delivery. The outcome measure was arterial pH assessed in cord blood obtained at birth. Results: Meta-analysis showed moderate accuracy of STV in predicting fetal acidaemia with a sensitivity of 0.57 (95% CI: 0.45 to 0.68), specificity of 0.81 (95% CI: 0.69 to 0.89), positive likelihood ratio of 3.14 (95% CI: 2.13 to 4.63) and negative likelihood ratio of 0.58, (95% CI: 0.46 to 0.72). However, in intra-uterine growth restricted fetuses, a small improvement in detecting acidaemia was observed; with a sensitivity of 0.63, (95% CI: 0.49 to 0.75) and negative likelihood ratio of 0.50, (95% CI: 0.31 to 0.80). Conclusion: STV appears to be a moderate predictor for fetal acidaemia. However, its usefulness as a stand-alone test in predicting acidaemia in clinical setting remains to be determined

    Drug-Induced Liver Injury Module for Medical Students

    Introduction No published curricula exist to introduce medical students to drug-induced liver injury (DILI). However, DILI is the most common cause of acute liver failure in the US, and drug-drug interactions are tested on the USMLE Step 1. Methods We developed an independent study module to introduce students to DILI. This module consisted of a narrated PowerPoint introduction, a journal article, and four example cases. Students completed the module independently. To evaluate the effectiveness of the activity, exam data and responses to the cases were reviewed, and end-of-course survey data were used. These responses were used to modify questions for clarity and to develop a feedback rubric. Results Mean scores on case-related questions in the module ranged from 44% to 73%. However, mean scores on test questions related to DILI ranged from 61% to 98%. It is possible that students learned from receiving feedback in the form of correct answers to the cases. On course evaluations, 52.4% of students agreed or strongly agreed that the online modules as a group (which included the DILI module) were an effective teaching method. Discussion This module introduces students to DILI and enables them to interact with valuable resources. We hope that modifications will improve the learning experience and effectiveness of the module. Going forward, we plan to collect validity evidence for the feedback rubric and develop an advanced version of the module for gastroenterology fellows

    Perspectives on Colon Cancer Screening—A Physician Panel Discussion for Preclinical Medical Students

    Introduction Colon cancer is the third most common cancer in the US, and the survival rate improves drastically with early detection. It is important for medical students to understand screening options, and to be able to effectively discuss these options with their patients. While basic information about colon cancer screening is ubiquitous in US medical school curricula, no published curricula describe teaching students the nuances of negotiating this discussion with patients and tailoring screening to individual patients' needs. Methods We developed a 90-minute session for second-year medical students as part of a gastroenterology and nutrition course. We provided a short lecture on colon cancer screening. We then had a panel of practicing gastroenterologists and a primary care physician discuss their approaches to six hypothetical cases. The students reflected in writing on what they learned from the session and on their opinions of the session format. Results Of second-year medical students, 139 attended the session and 110 submitted written reflections on the session (79% response rate). The students perceived significant gains in knowledge, communication skills, and attitudes around the discussions. Discussion This expert panel session taught medical students knowledge and communication skills related to colon cancer screening. The session could be easily implemented at any medical school, either at the preclinical or clinical level

    Evolution of vertebrate retinal photoreception

    Recent findings shed light on the steps underlying the evolution of vertebrate photoreceptors and retina. Vertebrate ciliary photoreceptors are not as wholly distinct from invertebrate rhabdomeric photoreceptors as is sometimes thought. Recent information on the phylogenies of ciliary and rhabdomeric opsins has helped in constructing the likely routes followed during evolution. Clues to the factors that led the early vertebrate retina to become invaginated can be obtained by combining recent knowledge about the origin of the pathway for dark re-isomerization of retinoids with knowledge of the inability of ciliary opsins to undergo photoreversal, along with consideration of the constraints imposed under the very low light levels in the deep ocean. Investigation of the origin of cell classes in the vertebrate retina provides support for the notion that cones, rods and bipolar cells all originated from a primordial ciliary photoreceptor, whereas ganglion cells, amacrine cells and horizontal cells all originated from rhabdomeric photoreceptors. Knowledge of the molecular differences between cones and rods, together with knowledge of the scotopic signalling pathway, provides an understanding of the evolution of rods and of the rods' retinal circuitry. Accordingly, it has been possible to propose a plausible scenario for the sequence of evolutionary steps that led to the emergence of vertebrate photoreceptors and retina

Based on the fact that organic fertilization can improve soil quality, we compared the effects of conventional and organic soil management on microbial activity and mycorrhization of seedless grape crops. We measured glomerospores number, most probable number (MPN) of propagules, richness of arbuscular mycorrhizal fungi (AMF) species, AMF root colonization, EE-BRSP production, carbon microbial biomass (C-MB), microbial respiration, fluorescein diacetate hydrolytic activity (FDA) and metabolic coefficient (qCO2). The organic management led to an increase in all variables with the exception of EE-BRSP and qCO2. Mycorrhizal colonization increased from 4.7% in conventional crops to 15.9% in organic crops. Spore number ranged from 4.1 to 12.4 per 50 g-1 soil in both management systems. The most probable number of AMF propagules increased from 79 cm-3 soil in the conventional system to 110 cm-3 soil in the organic system. Microbial carbon, CO2 emission, and FDA activity were increased by 100 to 200% in the organic crop. Thirteen species of AMF were identified, the majority in the organic cultivation system. Acaulospora excavata, Entrophospora infrequens, Glomus sp.3 and Scutellospora sp. were found only in the organically managed crop. S. gregaria was found only in the conventional crop.     Intrapulmonary Pharmacokinetics of First-line Anti-tuberculosis Drugs in Malawian Patients With Tuberculosis

    BACKGROUND: Further work is required to understand the intrapulmonary pharmacokinetics of first-line anti-tuberculosis drugs. This study aimed to describe the plasma and intrapulmonary pharmacokinetics of rifampicin, isoniazid, pyrazinamide, and ethambutol, and explore relationships with clinical treatment outcomes in patients with pulmonary tuberculosis. METHODS: Malawian adults with a first presentation of microbiologically-confirmed pulmonary tuberculosis received standard 6-month first-line therapy. Plasma and intrapulmonary samples were collected 8 and 16 weeks into treatment and drug concentrations measured in plasma, lung/airway epithelial lining fluid, and alveolar cells. Population pharmacokinetic modelling generated estimates of drug exposure (Cmax and AUC) from individual-level post-hoc Bayesian estimates of plasma and intrapulmonary pharmacokinetics. RESULTS: One-hundred-and-fifty-seven patients (58% HIV co-infected) participated. Despite standard weight-based dosing, peak plasma concentrations of first-line drugs were below therapeutic drug monitoring targets. Rifampicin concentrations were low in all three compartments. Isoniazid, pyrazinamide, and ethambutol achieved higher concentrations in epithelial lining fluid and alveolar cells than plasma. Isoniazid and pyrazinamide concentrations were 14.6 (95% CI: 11.2-18.0) and 49.8-fold (95% CI: 34.2-65.3) higher in lining fluid than plasma respectively. Ethambutol concentrations were highest in alveolar cells (alveolar cells:plasma ratio 15.0, 95% CI 11.4-18.6). Plasma or intrapulmonary pharmacokinetics did not predict clinical treatment response. CONCLUSIONS: We report differential drug concentrations between plasma and the lung. While plasma concentrations were below therapeutic monitoring targets, accumulation of drugs at the site of disease may explain the success of the first-line regimen. The low rifampicin concentrations observed in all compartments lend strong support for ongoing clinical trials of high-dose rifampicin regimens

    Ex Ante Impact Assessment of Policies Affecting Land Use, Part A: Analytical Framework

    Contemporary policy making calls for scientific support to anticipate the possible consequences of optional policy decisions on sustainable development. This paper presents an analytical framework for ex ante assessment of economic, social, and environmental impacts of policy driven land use changes that can be used as an aid to policy making. The tasks were to (1) link policy scenarios with land use change simulations, (2) link land use change simulations with environmental, social, and economic impacts through indicators, and (3) valuate the impacts in the context of sustainable development. The outcome was a basis for dialogue at the science-policy interface in the process of developing new policies on the European level that impact on land and land use. The analytical approach provides a logical thread for ex ante impact assessment within the context of sustainable development, land use multifunctionality, and land use change and it provides a thorough discussion of achievements and open challenges related to the framework. It concludes with considerations on the potential for using evidence based ex ante assessments in the process of policy development. The paper is complemented by a B-paper providing exemplary results from two applications of the framework: a financial reform scenario of the Common Agricultural Policy of the European Union, and a bioenergy policy scenario for the case of Poland (Helming et al. 2011)
