El objetivo es aplicar la metodología AICLE de enseñanza en la religión católica a través del desarrollo de una unidad didáctica. El proyecto incluye los principios clave para aplicar AICLE. El trabajo se ha llevado a cabo teniendo en cuenta los principios teóricos de la teoría de Krashen y el constructivismo. Este trabajo presenta y da razones para la enseñanza de la religión. Es muy importante la educación integral y de valores y la Religión Católica conecta con la dimensión transcendental del ser humano.This work includes the implementation of a teaching unit in English. The main goal of this work is to apply the methodology CLIL to teaching Catholic religion through the development of a teaching unit. The project takes into account the key principles to apply CLIL. The project was done bearing in mind the theoretical principles of Krashen theory and the Constructivism. This work presents and gives reasons for teaching religion. It is very important a comprehensible and values education. Catholic religion subject joins with the transcendental dimension of human being

    Transmural intestinal wall permeability in severe ischemia after enteral protease inhibition.

    In intestinal ischemia, inflammatory mediators in the small intestine's lumen such as food byproducts, bacteria, and digestive enzymes leak into the peritoneal space, lymph, and circulation, but the mechanisms by which the intestinal wall permeability initially increases are not well defined. We hypothesize that wall protease activity (independent of luminal proteases) and apoptosis contribute to the increased transmural permeability of the intestine's wall in an acutely ischemic small intestine. To model intestinal ischemia, the proximal jejunum to the distal ileum in the rat was excised, the lumen was rapidly flushed with saline to remove luminal contents, sectioned into equal length segments, and filled with a tracer (fluorescein) in saline, glucose, or protease inhibitors. The transmural fluorescein transport was determined over 2 hours. Villi structure and epithelial junctional proteins were analyzed. After ischemia, there was increased transmural permeability, loss of villi structure, and destruction of epithelial proteins. Supplementation with luminal glucose preserved the epithelium and significantly attenuated permeability and villi damage. Matrix metalloproteinase (MMP) inhibitors (doxycycline, GM 6001), and serine protease inhibitor (tranexamic acid) in the lumen, significantly reduced the fluorescein transport compared to saline for 90 min of ischemia. Based on these results, we tested in an in-vivo model of hemorrhagic shock (90 min 30 mmHg, 3 hours observation) for intestinal lesion formation. Single enteral interventions (saline, glucose, tranexamic acid) did not prevent intestinal lesions, while the combination of enteral glucose and tranexamic acid prevented lesion formation after hemorrhagic shock. The results suggest that apoptotic and protease mediated breakdown cause increased permeability and damage to the intestinal wall. Metabolic support in the lumen of an ischemic intestine with glucose reduces the transport from the lumen across the wall and enteral proteolytic inhibition attenuates tissue breakdown. These combined interventions ameliorate lesion formation in the small intestine after hemorrhagic shock

    Variable Renewable Energy in Modeling Climate Change Mitigation Scenarios

    This paper addresses the issue of how to account for short‐term temporal variability of renewable energy sources and power demand in long‐term climate change mitigation scenarios in energy‐economic models. An approach that captures in a stylized way the major challenges to the integration of variable renewable energy sources into power systems has been developed. As a first application this approach has been introduced to REMIND‐D, a hybrid energy‐economy model of Germany. An approximation of the residual load duration curve is implemented. The approximating function endogenously changes depending on the penetration and mix of variable renewable power. The approach can thus be used to account for variability and correlations between different sources of renewable supply and power demand within the intertemporal optimization of long‐term (energy system) investment decisions in climate change mitigation scenarios. Moreover, additional constraints are introduced to account for flexibility requirements concerning loadfollowing and ancillary services. The parameterization is validated with MICOES a highly resolved dispatch model. Model results show that significant changes are induced when the new residual load duration curve methodology is implemented. With variability, scenarios show that the German power sector is no longer fully decarbonized because natural gas combined‐cycle plants are built to complement renewable energy generation. The mitigation costs increase by about 20% compared to a model version in which variability is not taken into account

    Descriptive analysis of 179 German reports of melanoma reported to an adverse drug reaction database as a drug-related adverse effect, and comparison with melanoma cases contained in German cancer registries

    Background: Malignant melanoma (MM) is one of the most aggressive forms of skin cancer. The occurrence of MM associated with drug therapy has been described in the literature. However, there is no analysis of a substantial number of validated reports of drug-associated MM. Aim: To analyse a substantial number of validated spontaneous reports of drug-associated MM with regard to the suspected drug and the reported characteristics, and to compare these analyses with those of MM cases occurring in the general population in Germany. Methods: Spontaneous reports of MM associated with drug therapy in Germany were identified in a large adverse drug reaction database (EudraVigilance). These results were then compared with analyses of MMs in the pooled data from a population-based German cancer registry. Results: The 10 most frequently suspected drugs in the MM reports all target the immune system, with 7 of these being immunosuppressants. The median time to onset to MM diagnosis was 2.0 years. Patients with drug-associated MM were 11 years (median) younger than patients with MM in the cancer registry, and this age difference was greater for female than for male patients. Conclusions: Our results emphasize the importance of regular dermatological examinations of patients being treated with immunosuppressants. Physicians should be aware that in these patients, MM might be detected at younger ages and even within 2 years after initiating therapy.Peer Reviewe

    Enrichment of starch-based extruded cereals with chokeberry (Aronia melanocarpa) pomace: Influence of processing conditions on techno-functional and sensory related properties, dietary fibre and polyphenol content as well as in vitro digestibility

    Aiming at providing prototypes for ready-to-eat texturised (RTE) cereal products with reduced glycaemic load, starch blends with chokeberry (Aronia melanocarpa) pomace powder (CPP) rich in dietary fibre (DF) and polyphenols (PP) were extruded using a co-rotating twin-screw extruder. The CPP ratios (25%, 50%) and processing conditions applied (barrel temperature 100 °C, screw speed 200, 400, 600, 800 min−1, water content 13%, 23%) result in specific mechanical energies of 87–336 Whkg−1 and material temperatures of 111–155 °C. Extrudates containing 25% CPP still offer acceptable techno-functional and sensory related physical properties, while higher CPP ratios result in decreased expansion and cell pore size of the slightly darker and softer extrudates. The in vitro glucose release of both extruded blends is reduced by 25% and 50%, respectively. The DF contents are unaffected. As expected, anthocyanins are degraded by about 70% in both blends while phenolic acids and flavonols are fully retained. All PP are already accessible during the stomach phase of an in vitro digestion and are not changed significantly in the intestinal phases. Overall, these data substantiate, that marketable texturised RTE extruded cereals may be developed based on the results presented and on further sensory analysis

    Extrusion processing of pure chokeberry (Aronia melanocarpa) pomace: impact on dietary fiber profile and bioactive compounds

    The partial substitution of starch with dietary fiber (DF) in extruded ready-to-eat texturized (RTE) cereals has been suggested as a strategy to reduce the high glycemic index of these food products. Here, we study the impact of extrusion processing on pure chokeberry (Aronia melanocarpa) pomace powder (CPP) rich in DF and polyphenols (PP) focusing on the content and profile of the DF fractions, stability of PP, and techno-functional properties of the extrudates. Using a co-rotating twin-screw extruder, different screw speeds were applied to CPP with different water contents (cw_{w}), which resulted in specific mechanical energies (SME) in the range of 145–222 Whkg1^{-1} and material temperatures (TM_{M}) in the range of 123–155 °C. High molecular weight soluble DF contents slightly increase with increasing thermomechanical stress up to 16.1 ± 0.8 g/100 g dm as compared to CPP (11.5 ± 1.2 g/100 g dm), but total DF (TDF) contents (58.6 ± 0.8 g/100 g dm) did not change. DF structural analysis revealed extrusion-based changes in the portions of pectic polysaccharides (type I rhamnogalacturonan) in the soluble and insoluble DF fractions. Contents of thermolabile anthocyanins decrease linearly with SME and temperature from 1.80 ± 0.09 g/100 g dm in CPP to 0.24 ± 0.06 g/100 g dm (222 Whkg1_{-1}, 155 °C), but phenolic acids and flavonoids appear to be largely unaffected. Resulting techno-functional (water absorption and water solubility) and physical properties related to the sensory characteristics (expansion, hardness, and color) of pure CPP extrudates support the expectation that granulated CPP extrudates may be a suitable food ingredient rich in DF and PP

    Impact of defined thermomechanical treatment on the structure and content of dietary fiber and the stability and bioaccessibility of polyphenols of chokeberry (Aronia melanocarpa) pomace

    Dietary fiber is a potential replacement for other ingredients such as starch in reformulated extruded breakfast cereals. Analysis of chokeberry pomace powder revealed a total dietary fiber content of 57.8 ± 2 g/100 g with 76% being insoluble, 20% high molecular soluble and 4% low molecular soluble dietary fiber. The fiber polysaccharide composition was analyzed in detail by using a variety of analytical approaches. Extrusion-like processing conditions were studies in a Closed Cavity Rheometer enabling the application of defined thermal (temperature range 100–160 °C) and mechanical treatments (shear rates between 0.1 s1^{-1} and 50 s1^{-1}) to chokeberry pomace powder. Application of temperatures up to 140 °C irrespective of the mechanical treatment does not remarkably alter dietary fiber structure or content, but reduces the initial content of total polyphenols by about 40% to a final content of 3.3 ± 0.5 g/100 g including 0.63 ± 0.1 g/100 g of anthocyanins, 0.18 ± 0.02 g/100 g of phenolic acids and 0.090 ± 0.007 g/100 g of flavonols, respectively. The retained polyphenols are fully bioaccessible after in vitro digestion, and antioxidant capacity remains unchanged as compared to the untreated pomace powder. Glucose bioaccessibility remains unaffected, whereas glucose content is reduced. It is concluded that chokeberry pomace powder is a good source of dietary fiber with the potential to partially substitute starch in extruded breakfast cereals

    A qualitative study of the sources of chronic obstructive pulmonary disease-related emotional distress

    Masterthesis MScNObjective: The aim of this study is to identify the sources of illness-related emotional distress from the perspective of individuals living with mild to severe chronic obstructive pulmonary disease (COPD). Methods: A qualitative study design with purposive sampling was applied at a Swiss University Hospital. Eleven interviews were conducted with individuals who suffered from COPD. To analyze data, framework analysis was used, guided by the recently presented model of illness-related emotional distress. Results: Six main sources for COPD-related emotional distress were identified: physical symptoms, treatment, restricted mobility, restricted social participation, unpredictability of disease course and COPD as stigmatizing disease. Additionally, life events, multimorbidity and living situation were found to be sources of non-COPD-related distress. Negative emotions ranged from anger, sadness, and frustration to desperation giving rise to the desire to die. Although most patients experience emotional distress regardless of the severity of COPD, the sources of distress appear to have an individual manifestation. Discussion: There is a need for a careful assessment of emotional distress among patients with COPD at all stages of the disease to provide patient-tailored interventions

    The nursing contribution to ethical decision making

    Background: In the neonatal care units of the University Hospitals of Zurich and Bern, the nurse´s role in ethical decision-making is well established. However, nurses often reported uncertainty with regard to introducing the premature infant’s situation from the nursing perspective in ethics rounds. Aims: To empower neonatal nurses in fulfilling their role in the multiprofessional decision-making process, we performed a practice development project. On the basis of the Iowa model we developed a checklist for presenting the nursing history of premature infants in an ethically competent and responsible way. Conclusions: The ‘checklist for nursing assessment in the context of ethical decision-making’, equips nurses for their professional contribution to ethics rounds, making them better prepared to present the nursing perspective in a structured and thorough manner.
