    Hepatocellular carcinoma associated with pregnancy about 2 cases at the gynecological and obstetrical clinic of the Aristide Le Dantec hospital, Dakar, Senegal

    The objective of our study was to report 2 cases of hepatocellular carcinomas associated with pregnancy followed in our structure and to review the literature. Our patients were 30 and 37-year-old multi-gesture females with chronic unattended viral hepatitis B in whom the diagnosis of hepatocellular carcinoma was made in the third trimester of pregnancy at 31 weeks of amenorrhea and 4 days and at 32 weeks of amenorrhea after the incidental finding of tumor hepatomegaly on abdominal-pelvic ultrasound. The main clinical signs were jaundice and hepatomegaly and paraclinical signs were dominated by hepatic cytolysis and anemia in addition to ultrasound images. Follow-up of pregnancies revealed no particularities. A caesarean section was scheduled at 32 weeks of amenorrhea and 32 weeks of amenorrhea and 3 days allowing the birth of two preterm newborns weighing 1210 and 1500 gm with Apgar scores of 8-10/10 and 7-9/10 respectively at the fifth minute. The immediate post-operative follow-up was simple. However, the maternal-fetal prognosis was poor with the death of both patients in a multi-visceral failure table occurring respectively at 6 weeks and 3 weeks after caesarean section. The newborns had died 8 days after birth. Although rare, these two cases challenge any obstetrician to think about liver cancer in pregnant women, especially those with chronic hepatitis B. Ultrasound examination of the liver, or even better, the MRI, which is more efficient, in order to suspect early on a possible liver cancer. Indeed, early diagnosis and a thorough medical approach are essential for the treatment of HCC in pregnant patients

    Thermodynamic, thermoelectric, and magnetic properties of FeSb2: A combined first-principles and experimental study

    We analyze the thermodynamic, magnetic, and transport properties of the narrow band-gap semiconductor FeSb2 using density functional theory calculations corroborated by nuclear inelastic spectroscopy and ultrasound experiments. The vibrational properties (phonon spectrum density of states, heat capacity) and elastic constants are computed through response function calculations and are in good agreements with the measurements. The electron-phonon coupling effects are also studied. The estimations of linewidth broadening due to electron-phonon coupling along the high-symmetry directions in the first Brillouin zone are given. The linewidth broadening reaches the largest value for Fe optical modes in the vicinity of the X[0.5,0,0] point. The broadening, when compared to those obtained at the other symmetry points, differs by up to two orders of magnitude. From the Boltzmann theory applied to our electronic band structure, we investigate the electrical transport properties. It is found that a purely electronic structure description is incompatible with the record value of the Seebeck coefficient experimentally observed at T approximate to 12 K. The diamagnetic to paramagnetic crossover at a temperature around 100 K is also described from the calculation of the magnetic susceptibility, and results compare well with experiment

    Tropical Atlantic sea-surface temperatures (SSTs) maximum intraseasonal variability (ISV) and their interaction with local surface winds are investigated, applying statistical analysis to observations and to a recent coupled reanalysis over the 2000-2009 decade. Five cores of strong ISV emerge, with standard deviation reaching about 1 degrees C in frontal areas of the three main upwelling systems: equatorial, Angola-Benguela and Senegal-Mauritania (the southern side of the Canary upwelling). West of 10 degrees W along the Equator, a 20-60-day peak caused by tropical instability waves is shown to generate surface wind anomalies through the adjustment of the horizontal surface pressure gradient in addition to the modification of near-surface atmospheric stratification. East of 10 degrees W along the Equator, an intense biweekly oscillation increases the ocean and atmosphere ISV. In the two coastal upwelling fronts, intraseasonal SST anomalies resemble each other. They are shown to be influenced by coastal Kelvin waves in addition to large-scale wind forcing. Over the Angola-Benguela upwelling, coastal wind bursts controlling the SST ISV are associated with anomalously strong pressure patterns related to the Madden-Julian Oscillation, the St Helena anticyclone and the Antarctic Oscillation. In the Senegal-Mauritania upwelling, the wind anomalies mainly linked to the Azores anticyclone in the southern front during November to May appear to be connected to the Saharan heat-low in the northern front from June to September. In all five regions and as expected for such upwelling regimes, vertical oceanic mixing represents the dominant term in the mixed-layer heat budget. In the equatorial band, as found in previous studies, horizontal advection is equally important, while it appears surprisingly weak in coastal fronts. Finally, a striking result is the general lack of surface wind signal related to the SST ISV in the coastal upwellings

    Le SĂ©nĂ©gal, dans le cadre de la rĂ©habilitation des sujets dĂ©ficients moteurs, fabrique deux types de tricycles : l’un Ă  pĂ©dalier, l’autre Ă  balancier au Centre National d’Appareillage OrthopĂ©dique (CNAO) de Dakar. Cependant Ă  notre connaissance aucune Ă©tude scientifique n’a Ă©tĂ© faite pour apprĂ©cier le niveau de confort, les variations de paramètres physiologiques et mĂ©caniques internes lors de l’utilisation du tricycle par les paraplĂ©giques. C’est dans ce cadre que s’inscrit cette Ă©tude dont, l’objectif Ă©tait d’évaluer la variation des paramètres cardio-vasculaires (frĂ©quence cardiaque) et mĂ©taboliques (lactatĂ©mie) en fonction du moyen de locomotion du sujet en situation de handicap.Vingt (20) paraplĂ©giques de genre masculin avaient participĂ© Ă  l’étude. Les sujets avaient effectuĂ© un dĂ©placement aller-retour du stade Iba Mar Diop au rond point de l’UniversitĂ© Cheikh Anta Diop de Dakar (4800m), avec les tricycles Ă  balancier et Ă  pĂ©dalier du CNAO.Les rĂ©sultats avaient montrĂ© que la frĂ©quence cardiaque Ă©tait plus Ă©levĂ©e en valeur absolue lors des dĂ©placements sur le tricycle Ă  balancier, elle est Ă©gale Ă  126 bpm comparĂ©e Ă  celle relevĂ©e sur le tricycle Ă  pĂ©dalier, 124 bpm), sans qu’il y’ait une diffĂ©rence statistiquement significative entre ces diffĂ©rentes valeurs, p Ă©gale Ă  0,58. Il en est de mĂŞme que pour la lactatĂ©mie, elle n’avait prĂ©sentĂ© aucune diffĂ©rence statistiquement significative pour les deux moyens de locomotion, p est Ă©gale Ă  0,38. Les valeurs Ă©taient de 5,38 mmol. l-1 sur le tricycle Ă  pĂ©dalier et de 6,44 mmol. l-1 sur le tricycle Ă  balancier.Cette Ă©tude a montrĂ© l’intĂ©rĂŞt de l’utilisation du tricycle Ă  pĂ©dalier ou Ă  balancier fabriquĂ© au SĂ©nĂ©gal qui prĂ©sente un niveau de confort acceptable pour le dĂ©placement en vue de l’autonomie de la personne handicapĂ©e.ABSTRACTSenegal, in the framework of the rehabilitation of the motor deficient subjects, manufactures two types of tricycles: one with pedals, the other with pendulum at the National Center of Orthopedic Equipment (CNAO). However, to our knowledge, no  scientific study has been done to assess the level of comfort, variations in physiological and mechanical parameters during the use of these tricycles by paraplegic subjects. Our objective was to evaluate the variation of the cardiovascular (heart rate) and metabolic (lactatemia) parameters as a function of the means of locomotion of the subject with a disability.Twenty (20) paraplegic males participated in the study. The subjects had to make a return trip from the Iba Mar Diop stadium to the Dakar Cheikh Anta Diop roundabout (4800m), with the pendulum and pedal tricycles of the CNAO.The results showed no significant difference in absolute heart rate when moving on the tricycle, 126 bpm compared to that on the tricycle, 124 bpm, p was 0.58. Similarly no statistically significant difference was found for lactatemia: 5.38 mmol. L-1 on the pedal tricycle and 6.44 mmol. L-1 on the pendulum tricycle, 0.38. Pendulum and pedal tricycles made in Senegal present comparable level of comfort for the displacements with a view to the autonomy of the handicapped person in Senegal
