    Missed opportunities for ovarian salvage in children: an 8-year review of surgically managed ovarian lesions at a tertiary pediatric surgery centre

    INTRODUCTION: The aetiology and management of ovarian pathology in children differs between antenatal and postnatal lesions. However, all lesions may present acutely due to adnexal torsion. In this setting, opportunities to preserve fertility with ovary-sparing surgery (OSS) may be missed. Some studies suggest that pediatric and adolescent gynaecology (PAG) input in care is associated with OSS. METHODS: A retrospective cohort study of children undergoing surgery for ovarian pathology at a tertiary pediatric surgery centre over an 8-year period (2011-2018). Patient factors, lesion characteristics and PAG involvement were examined for association with OSS using multivariate logistic regression. RESULTS: Thirty-five patients with ovarian pathology managed surgically were included. Ten were infants with lesions detected antenatally; all were managed by pediatric surgeons (PS) alone at median age 2 weeks (1 day-25 weeks). Twenty-five patients presented postnatally at median age 11 (0.75-15) years. In total, there were 16 cases of adnexal torsion, each managed primarily by PS. Twelve underwent oophorectomy and six (50%) of these cases had viable ovarian tissue on histology. Furthermore, two infants with large simple cysts were similarly managed by unnecessary oophorectomy based on histology. Overall rate of OSS was 46% and PAG involvement was the only factor associated with ovarian salvage. CONCLUSION: Differences in surgical management between PAGs and PS may be attributable to the different patient populations they serve. We recommend improving the knowledge of PS trainees in OSS approaches for adnexal torsion and large benign lesions

The general lack of data describing the behaviour of the infrastructure during unwanted events led to estimate their safety mainly from expert judgement. Thus the ability of the expert to predict the level of functioning of an infrastructure for a type of hazard and its intensity is crucial. An error of judgement can have very serious consequences and the production of reliable information requires the ability of the expert to report accurately the uncertainties in its estimations, as well as associated confidence. In order to meet this need, our research within Incertu project (French Ministry of Ecology funding) aims to produce relevant scientific approaches and tools for the collection and processing reliable experts'statements or combined with a confidence level in the context of uncertain information and input data

    Electrical and Magnetic Properties of Mn-Bi-Sb Alloys

    MnBi1-xSbx alloys were prepared by the conventional melt technique. The Seebeck coefficient (S), electrical resistivity (ρ), and magnetic susceptibility (c) were measured at various temperatures ranging from ~100 to 400 K. The electrical resistivity of x ÂŁ 0.15 shows both semiconducting and metallic behavior depending on temperature and Sb content, whereas samples x 3 0.2 have only semiconductor behavior in all the temperature range. The negative sign of the Seebeck coefficient increases, i.e., the positivity decreases with the increasing Sb content. The magnetic susceptibility (χ) shows that alloys undergo ferro-paramagnetic transition at a certain temperature (TC) and the TC values decrease with increasing Sb content. From thermoelectric measurements and electronic thermal conductivity calculated, it was observed that Sb doping increases the power factor (PF) and the figure of merit (ZT). Thus, Sb content plays an essential role in making these alloys applicable in the thermoelectric industry

    Enhancing the Performance of Power System under Abnormal Conditions Using Three Different FACTS Devices

    In this paper, a comparison between Flexible Alternating Current Transmission System (FACTS) devices including Static Synchronous Compensator (STATCOM), Static Synchronous Series Compensator (SSSC) and Unified Power Flow Controller (UPFC) for providing a better adaptation to changing operating conditions and improving the usage of current systems. The power system using FACTS devices is presented under different conditions such as single phase fault and three phase fault. A digital simulation using Matlab/Simulink software package is carried out to demonstrate the better performance including the voltage and the current of the presented system using FACTS that located between buses B1 and B2 under different faults types. The results obtained investigate that the presented system gives better response with FACTS as compared to not using them under abnormal conditions besides, the UPFC gives better performance of power system under several faults as compared to STATCOM or SSSC as It can absorb reactive power in a manner which significantly reduced the fault current. It is demonstrated that UPFC can reduce the peak fault current at bus B1 ‎to 63.85% of its value without ‎using FACTS devices under line to ground fault and 79.18% under three line to ‎ground fault whereas STATCOM and SSSC reduce it ‎to (75.21, 94.35%) and (75.40, 94.68%), respectively

    Trauma and Parenting Beliefs : Exploring the Ethnotheories and Socialization Goals of Palestinian Mothers

    Parenting beliefs are important to infant wellbeing, but there is a lack of research into their role in war conditions. We examined (1) how maternal traumatic experiences (war events and childhood abuse) and mental health would be associated with parenting beliefs about good childrearing practices (ethnotheories) and desirable child characteristics (socialization goals) and (2) whether parenting beliefs mediate the impact of traumatic experiences and mental health on mother-infant interaction and infant development. Palestinian mothers (N = 510) participated during pregnancy and at four and 12 months postpartum. They reported current traumatic war events, emotional and physical abuse in their own childhood and mental health (posttraumatic stress disorder, depression, and anxiety symptoms), their ethnotheories and socialization goals and the quality of dyadic mother-infant interactions and infant developmental skills (language, fine-motor, and gross-motor). High maternal exposure to traumatic war events was associated with higher autonomy-enhancing-and lower relatedness-enhancing-socialization goals. Conversely, maternal childhood abuse was associated with higher relatedness-enhancing-and lower autonomy-enhancing-socialization goals. High maternal exposure to traumatic war events was associated with high levels of positive dyadic interaction, whereas high emotional and physical abuse and mental health problems were associated with low levels of positive dyadic interaction. Parenting beliefs did not mediate the impact of maternal traumatic experiences and mental health on dyadic interaction or infant development. The nature of maternal traumatic experiences was central for the socialization goals in unique and specific ways. The findings emphasize the meaningfulness of parenting values, goals, and practices among mothers caring for their infants in war conditions.Peer reviewe

    NEW HIDING TECHNIQUE IN DIGITAL SIGNATURE BASED ON ZIGZAG TRANSFORM AND CHAOTIC MAPS

    Abstract This paper presents a novel approach to digital signature by integrating the ElGamal or Schnorr digital signature algorithms, chaotic systems, and scanning techniques. Briefly, ZZBCRP is a zigzag transformation that is used firstly to construct a permuted transaction, which technique starts from any random position and intersects in both directions, which is more complex than zigzag transform techniques. Then using ElGamal or Schnorr signature schemes based on chaotic maps. This modification aims to make private key and random number dependent on discrete chaotic maps. Even if the private key chosen is small, it is easy by using the huge amount of points in chaotic maps 2-D or 3-D to extract strong and unique key. This change complicates the relationship between the private key, public key and the transaction signature. A two-dimensional trigonometric discrete chaotic map is used that integrated Logistic-sine-cosine maps, and a three-dimensional hyperchaotic map (3-D SCC) which are based on a sine map. Our performance analysis shows that compared to schemes; this scheme not only improves the level of efficiency but also assures safety.
