    Smoking, Cognitive Function and Mortality in a U.S. National Cohort Study

    Previous studies report that low levels cognitive function and history of smoking are associated with increased mortality risk. Elderly smokers may have increased risk of dementia, but risk in former smokers is unclear. We tested the hypotheses that the harmful effect of impaired cognitive function as related to mortality is greater in persons smoking at baseline than in others. Further, we used serum cotinine levels to assess recall bias of smoking history by cognitive function level. Data were analyzed from a longitudinal mortality follow-up study of 4,916 American men and women aged 60 years and over, examined in 1988–1994 with complete data followed an average 8.5 years. Measurements at baseline included smoking history, a short index of cognitive function (SICF), serum cotinine and socio-demographics. Death during follow-up occurred in 1,919 persons. In proportional hazards regression analysis, a significant interaction of current smoking with cognitive function was not found; but there was a significant age-smoking interaction. After adjusting for confounding by age or multiple variables, current smoking associated with over 2-fold increased mortality (hazards ratio and 95% confidence limits current versus never smoking 2.13, 1.75–2.59) and SICF with 32% reduction in mortality; top versus bottom SICF stratum 0.68, 0.53–0.88). Serum cotinine data revealed substantial recall bias of smoking history in persons with cognitive impairment. However analyses correcting for this bias did not alter the main conclusions: In a nationwide cohort of older Americans, analyses demonstrated a lower risk of death independent of confounders among those with high SICF scores and never smokers, without a significant interaction of the two

    Physical education is one of the more difficult subjects in the curriculum for generalist classroom teachers in primary schools to incorporate confidently into their teaching. In many primary schools, the generalist classroom teacher defers to a physical education specialist. This situation has both positive and negative features. In this context, this study brings together several prominent models of physical education teaching in an approach that enables the curriculum to be encountered through the interests of the children. This approach offers a generalist teacher, through appropriate professional development, a means for delivering a high-quality physical education programme, and also complements the repertoire of the specialist physical education teacher at both primary and secondary school levels.<br /

    A randomised controlled trial of a care home rehabilitation service to reduce long-term institutionalisation for elderly people

    Objectives: to evaluate the effect of a care home rehabilitation service on institutionalisation, health outcomes and service use. Design: randomised controlled trial, stratified by Barthel ADL index, social service sector and whether living alone. The intervention was a rehabilitation service based in Social Services old people's homes in Nottingham, UK. The control group received usual health and social care. Participants: 165 elderly and disabled hospitalised patients who wished to go home but were at high risk of institutionalisation (81 intervention, 84 control). Main outcome measures: institutionalisation rates, Barthel ADL index, Nottingham Extended ADL score, General Health Questionnaire (12 item version) at 3 and 12 months, Health and Social Service resource use. Results: the number of participants institutionalised was similar at 3 months (relative risk 1.04, 95% confidence intervals 0.65–1.65) and 12 months (relative risk 1.23, 95% confidence intervals 0.75–2.02). Barthel ADL Index, Nottingham Extended ADL score and General Health Questionnaire scores were similar at 3 and 12 months. The intervention group spent significantly fewer days in hospital over 3 and 12 months (mean reduction 12.1 and 27.6 days respectively, P < 0.01), but spent a mean of 36 days in a care home rehabilitation service facility. Conclusions: this service did not reduce institutionalisation, but diverted patients from the hospital to social services sector without major effects on activity levels or well-being

    Prelithification and synlithification tectonic foliation development in a clastic sedimentary sequence

    The current view regarding the timing of regionally developed penetrative tectonic fabrics in sedimentary rocks is that their development postdates lithification of those rocks. In this case, fabric development is achieved by a number of deformation mechanisms, including grain rigid body rotation, crystal-plastic deformation, and pressure solution. The latter is believed to be the primary mechanism responsible for the domainal structure of cleavage in low-grade metamorphic rocks. In this study we combine field observations with strain studies to characterize considerable (>50%) Acadian crustal shortening in a Devonian clastic sedimentary sequence from southwest Ireland. Despite these high levels of shortening there is a marked absence of the domainal cleavage structure and intraclast deformation that are expected with this level of deformation. Fabrics in these rocks are predominantly a product of rigid body rotation and repacking of extraformational clasts during deformation of a clastic sedimentary sequence before lithification was complete

    Public crises, public futures

    This article begins to map out a novel approach to analyzing contemporary contexts of public crisis, relationships between them and possibilities that these scenes hold out for politics. The article illustrates and analyses a small selection of examples of these kinds of contemporary scenes and calls for greater attention to be given to the conditions and consequences of different forms and practices of public and political mediation. In offering a three-fold typology to delineate differences between ‘abject’, ‘audience’ and ‘agentic’ publics the article begins to draw out how political and public futures may be seen as being bound up with how the potentialities, capacities and qualities that publics are imagined to have and resourced to perform. Public action and future publics are therefore analysed here in relation to different versions of contemporary crisis and the political concerns and publics these crises work to articulate, foreground and imaginatively and practically support
