    The Triplex BioValsalva Prostheses To Reconstruct the Aortic Valve and the Aortic Root

    The Bentall procedure introduced in 1968 represents an undisputed cure to treat multiple pathologies involving the aortic valve and the ascending thoracic aorta. Over the years, multiple modifications have been introduced as well as a standardized approach to the operation with the goal to prevent long-term adverse events. The BioValsalva prosthesis provides a novel manner to more efficiently reconstruct the aortic valve together with the anatomy of the aortic root with the implantation of a valved conduit. This prosthesis comprises three sections: the collar supporting the valve; the skirt mimicking the Valsalva, which is suitable for the anastomoses with the coronary arteries; and the main body of the graft, which is designed to replace the ascending aorta. The BioValsalva prosthesis allows the Bentall operation to be used in patients whose aortic valve cannot be spared

    Pour que le film existe, il faut qu’il y ait « transcodage », c’est-à-dire passage d’un mode de signification à un autre. On pourrait dire que le passage du récit scénaristique au récit cinématographique se présente comme une véritable mutation où, d’un être essentiellement de mode nominatif, naîtra une créature hybride qui procède à la fois du mode nominatif et du mode représentatif. C’est ce processus de mutation qui est ici abordé, en esquissant une typologie des transformations que peut subir le récit scénaristique lors de sa réécriture sous la forme d’un récit cinématographique, puis au moyen de l’analyse comparative du récit scénaristique et du récit cinématographique du film Pouvoir intime d’Yves Simoneau (scénarisé par Pierre Curzi et Yves Simoneau).In order for a film to exist there must be a "transcoding", a passage from one signifying mode to another. One might say that the movement from screenplay version to cinematic version can be seen as a real modification in which something whose mode is essentially nominative gives birth to a hybrid creature in which the nominative mode is combined with the representational mode. This process of modification is examined here first by outlining a typology of the transformations that a screenplay version may undergo when it is recoded into a cinematic version, and then by making a comparative analysis of the screenplay and the cinematic versions of the film Pouvoir intime by Yves Simoneau (screenplay by Pierre Curzi and Yves Simoneau)

    A single center experience with the freestyle bioprosthesis : midterm results at the Québec Heart Institute

    Stentless bioprostheses show excellent early hemodynamic performance. However, longevity still remains unknown. This study reports midterm follow-up in 419 patients in which a Freestyle bioprosthesis (Medtronic Heart Valves, Minneapolis, MN) was inserted between January 1993 and January 2000 at the Quebec Heart Institute (Ste-Foy, Québec, Canada). Mean age at implantation was 68.0 +/- 8.2 years. Implantation was subcoronary in 81.9% of the patients, as a root replacement in 16.5%, and as a root inclusion in 1.7%. Mortality at 30 days was 6.2% for the whole cohort (2.8% for isolated subcoronary aortic valve replacement). Female gender, root implantation, valve sizes 19 to 21 mm, previous surgery, a history of stroke and diabetes were identified as predictors of 30-day mortality. Actuarial freedom from all death causes was 81.5% at 7 years; freedom from valve-related deaths 97.0%, and freedom from cardiac deaths 92.7%. Freedom from thromboembolic events was 86.1% at 7 years (55.1% of events were < 30 days). Freedom from endocarditis and hemorrhagic complications were respectively 98.5% and 95.6% at 7 years. Six patients required reoperations for valve explantation: 2 for endocarditis, 2 for structural dysfunction, and 2 for nonstructural dysfunction. Incidence of moderate or severe valve insufficiency at annual echocardiographic follow-up was: discharge: 0.6%; year 1: 0.7%; year 2: 1.3%; year 3: 3.3%; year 4: 3.7%; year 5: 2.6%; year 6: 0%. At 6 years after implantation, mean transvalvular gradient and effective valve orifice area were comparable to the year 1 values.     International audienceThe use of Lagrangian platforms and of Autonomous Underwater Vehicles (AUVs) in oceanography has increasedrapidly over the last decade along with the development of improved biological and chemical sensors. These vehicles provide new spatial and temporal scales for observational studies of the ocean. They offer a broad range of deployment and recovery capabilities that reduce the need of large research vessels. This is especially true for ice-covered Arctic ocean where surface navigation is only possible during the summer period. Moreover, safe underwater navigation in icy waters requires the capability of detecting sea ice on the surface (ice sheets). AUVs navigating in such conditions risk collisions, RF communication shadowing, and being trapped by ice keels. In this paper, an underwater sea-ice detection apparatus is described. The source is a polarized continuous wave (CW) diode-pumped solid-state laser (DPSS) module operating at 532 nm. The detector is composed of a polarizing beam splitter, which separates light of S and P polarization states and two photodetectors, one foreach polarized component. Since sea-ice is a strong depolarizer, the ratio P/S is an indicator of the presence or absence of sea-ice. The system is capable of detecting sea-ice at a distance of 12m. This apparatus is designed to be used by free drifting profiling floats (e.g., Argo floats), buoyancy driven vehicles (e.g., sea gliders) and propeller-driven robots (e.g., Hugin class AUV)

    The reconstruction of the right ventricular outflow tract (RVOT) system represents a considerable challenge for both manufacturers and surgeons because the patients requiring this type of devices have a very diverse set of anatomical challenges that can lead to complications and subsequent early device failures. We conducted an indepth investigation of a porcine-valve conduit explanted from a patient following an adverse event. A control device was analyzed as a reference. The rapid aging of the porcine valve in the right side of the heart together with major thrombus formation raises several questions. The difficulties encountered with materials used and also the design features of the conduits are once again highlighted. This group of patients continues to increase in number due to success in the surgical outcomes in early childhood. Therefore, there is a greater demand for an appropriate device. However, much work is still needed to achieve this goal, and the best approach to achieving success remains unanswered
