    Inferences about the conservation utility of using unmanned aerial vehicles to conduct rapid assessments for basking freshwater turtles

    Unmanned aerial vehicles (UAVs), an emerging technology, show promise in ecological research. In this comparative study, I compare UAVs to a traditional sampling method, observations using spotting scopes. UAVs have yet to be used successfully for sampling freshwater turtles; however, they have been used with mixed success for monitoring mammals and birds. Herein, I propose that the conservation utility of UAVs be formally assessed in the field prior to them being used to make adaptive conservation and management decisions. I quantitatively and qualitatively evaluate the use of UAVs using a mixed methods approach in contrast to a proven field method as a means to elucidate our basic understanding of presence-absence. Being able to successfully use UAVs for ecological surveying would provide an easy, efficient, and less invasive way to study basking turtles

    Competition and bistability of ordered undulations and undulation chaos in inclined layer convection

    Experimental and theoretical investigations of undulation patterns in high-pressure, inclined layer gas convection at a Prandtl number near unity are reported. Particular focus is given to the competition between the spatiotemporal chaotic state of undulation chaos and stationary patterns of ordered undulations. In experiments a competition and bistability between the two states is observed, with ordered undulations most prevalent at higher Rayleigh number. The spectral pattern entropy, spatial correlation lengths, and defect statistics are used to characterize the competing states. The experiments are complemented by a theoretical analysis of the Oberbeck-Boussinesq equations. The stability region of the ordered undulation as a function of their wavevectors and the Rayleigh number is obtained with Galerkin techniques. In addition, direct numerical simulations are used to investigate the spatiotemporal dynamics. In the simulations both ordered undulations and undulation chaos were observed dependent on initial conditions. Experiment and theory are found to agree well.Comment: Reduced-resolution figure

    Sunnyside Drinking Water Project: Examining Chlorinated Disinfectant By-products, Resident Perceptions and Practices, and Municipal Responses in Securing Safe Drinking Water in the Town of Sunnyside NL

    The Sunnyside drinking water project is a university-community partnership between researchers at the Environmental Policy Institute- Grenfell Campus and municipal leaders of the Town of Sunnyside. This project explores the persistent drinking water-related challenges facing Sunnyside, eastern Newfoundland and Labrador (NL), primarily through the viewpoint of local residents. These challenges include: threats to source water; effective water treatment, disinfection and distribution, including dealing with high levels of disinfectant by-products (DBPs) as a result of their current disinfection system; and developing strategies for addressing these issues. Many rural municipalities in Newfoundland and Labrador face similar issues, which present a challenge to municipalities in ensuring residents access to clean, safe drinking water. This report is structured through five main parts: 1. Introduction: Community profile, project rationale and methodology; 2. Sunnyside drinking water survey report: Analysis and discussion of the drinking water survey conducted in Sunnyside during the fall of 2014; 3. Chlorinated disinfectant by-products and drinking water in Sunnyside: Literature review and discussion of the potential health impacts posed by DBPs in publically supplied water systems, policy responses and alternative technologies; 4. Community Water Forum: Discussion of the results of the Community Water Forum presentations, which took place in Sunnyside in May 2015, and; 5. Policy recommendations: Concluding thoughts and policy recommendations. One issue that frequently arose over the course of the project is the lack of regulatory emphasis placed on federal water quality guidelines related to DBPs in NL. Arriving at an enforceable level, however, would require consideration of feasible technological solutions and/or significant increases in provincial investment into the water treatment systems of hundreds of small rural communities with high levels of DBPs. Until such time, increased public health education and support is needed for municipal leaders and the public at large. Thus, our main policy recommendation for the Government of NL is to develop and support a multi-faceted education campaign around the potential health impacts of DBPs, the role of chlorination in publicly supplied drinking water, and solutions and alternatives available in preventing and/or mitigating DBP impacts. Education (and research) is also needed on the environmental and social impacts of bottled water in the province, currently the most common drinking water source for Sunnyside residents. DBPs are a collective responsibility shared by residents, municipal and senior levels of government. Municipal governments share in the responsibility of educating their residents about the impacts of both DBPs and bottled water. Home-treatment technologies also offer an alternative for residents in reducing DBP exposure, particularly in the short-term. We are cautious in recommending this as a long-term solution due to issues of affordability and social equity, the need for proper maintenance, and difficulties finding appropriate systems. In general, we argue that it is the responsibility of public institutions (i.e. the government) to ensure that the appropriate water systems are in place to deliver safe drinking water to residents

    Intravenous remifentanil patient-controlled analgesia versus intramuscular pethidine for pain relief in labour (RESPITE): an open-label, multicentre, randomised controlled trial

    Background: Approximately a third of women receiving pethidine for labour pain subsequently require an epidural, which provides effective pain relief but increases the risk of instrumental delivery. Remifentanil patient controlled analgesia (PCA) in labour is an alternative to pethidine, but not widely utilized. We sought to determine epidural rates amongst women using remifentanil PCA compared to pethidine.Methods: We conducted a randomised, parallel, open-label trial in 14 UK maternity units. Women at term gestation, in labour with a singleton cephalic presentation, requesting opioid pain relief, were randomly assigned (1:1) to remifentanil PCA (40μg bolus with a two minute “lock-out”) or intramuscular pethidine (100mg, four-hourly, up to 400mg). Web-based or telephone randomisation minimised allocations by parity, age, ethnicity and mode of labour onset. The primary outcome was the proportion of women who received epidural analgesia after enrolment. To detect a reduction in epidural conversion from 30% to 15% with 90% power, with a 15% anticipated attrition from urgent delivery by emergency caesarean section, required 400 women. Primary analyses were unadjusted and by intention-to-treat. ISRCTN29654603.Findings: Between May 13, 2014, and Sept 2, 2016, 201 women were randomly assigned to the remifentanil PCA group and 200 to the pethidine group. One participant in the pethidine group withdrew consent, leaving 199 for analyses. The proportions of epidural conversion were 19% (39 of 201) in the remifentanil PCA group and 41% (81 of 199) in the pethidine group (risk ratio 0·48, 95% CI 0·34–0·66; p less 0·0001). There were no serious adverse events or drug reactions directly attributable to either analgesic during the study.Interpretation: Intravenous remifentanil PCA halved the proportion of epidural conversions compared with intramuscular pethidine.     Vibrio parahemolyticus septicaemia in a liver transplant patient: a case report

    <p>Abstract</p> <p>Introduction</p> <p><it>Vibrio parahemolyticus </it>is the leading cause of vibrio-associated gastroenteritis in the United States of America, usually related to poor food handling; only rarely has it been reported to cause serious infections including sepsis and soft tissue infections. In contrast, <it>Vibrio vulnificus </it>is a well-known cause of septicaemia, especially in patients with cirrhosis. We present a patient with <it>V. parahemolyticus </it>sepsis who had an orthotic liver transplant in 2007 and was on immunosuppression for chronic rejection. Clinical suspicion driven by patient presentation, travel to Gulf of Mexico and soft tissue infection resulted in early diagnosis and institution of appropriate antibiotic therapy.</p> <p>Case presentation</p> <p>A 48 year old Latin American man with a history of chronic kidney disease, orthotic liver transplant in 2007 secondary to alcoholic end stage liver disease on immunosuppressants, and chronic rejection presented to the emergency department with fever, vomiting, abdominal pain, left lower extremity swelling and fluid filled blisters after a fishing trip in the Gulf of Mexico. Samples from the blister and blood grew <it>V. parahemolyticus</it>. The patient was successfully treated with ceftriaxone and ciprofloxacin.</p> <p>Conclusion</p> <p>Febrile patients with underlying liver disease and/or immunosuppression should be interviewed regarding recent travel to a coastal area and seafood ingestion. If this history is obtained, appropriate empiric antibiotics must be chosen. Patients with liver disease and/or immunosuppresion should be counselled to avoid eating raw or undercooked molluscan shellfish. People can prevent <it>Vibrio </it>sepsis and wound infections by proper cooking of seafood and avoiding exposure of open wounds to seawater or raw shellfish products.</p

    Report completed by students enrolled in JOUR 8202: Generation and Selection of Communication Strategies, taught by Steve Wehrenberg in spring 2018.This project was completed as part of the 2017-2018 Resilient Communities Project (rcp.umn.edu) partnership with the City of Ramsey. Despite Ramsey’s rich history, it is often confused with Ramsey County (located on the opposite side of the Twin Cities metro area) or the neighboring community of Anoka. Ramsey has a central brand, but it is often not used to its full potential. Students in JOUR 8202: Generation and Selection of Communication Strategies, taught by Steve Wehrenberg, worked with City staff and used various methods to gain resident feedback on the aspects of Ramsey valued most by residents. The students used this information to develop a unique branding strategy that will not only help people distinguish Ramsey from other communities, but will also attract new residents and businesses to the city.     Optimisation of pharmacy content in clinical cancer research protocols: Experience of the United Kingdom Chemotherapy and Pharmacy Advisory Service

    Background: Clarity and accuracy of the pharmacy aspects of cancer clinical trial protocols is essential. Inconsistencies and ambiguities in such protocols have the potential to delay research and jeopardise both patient safety and collection of credible data. The Chemotherapy and Pharmacy Advisory Service was established by the UK National Cancer Research Network, currently known as National Institute for Health Research Clinical Research Network, to improve the quality of pharmacy-related content in cancer clinical research protocols. This article reports the scope of Chemotherapy and Pharmacy Advisory Service, its methodology of mandated protocol review and pharmacy-related guidance initiatives and its current impact. Methods: Over a 6-year period (2008–2013) since the inception of Chemotherapy and Pharmacy Advisory Service, cancer clinical trial protocols were reviewed by the service, prior to implementation at clinical trial sites. A customised Review Checklist was developed and used by a panel of experts to standardise the review process and report back queries and inconsistencies to chief investigators. Based on common queries, a Standard Protocol Template comprising specific guidance on drug-related content and a Pharmacy Manual Template were developed. In addition, a guidance framework was established to address ‘ad hoc’ pharmacy-related queries. The most common remarks made at protocol review have been summarised and categorised through retrospective analysis. In order to evaluate the impact of the service, chief investigators were asked to respond to queries made at protocol review and make appropriate changes to their protocols. Responses from chief investigators have been collated and acceptance rates determined. Results: A total of 176 protocols were reviewed. The median number of remarks per protocol was 26, of which 20 were deemed clinically relevant and mainly concerned the drug regimen, support medication, frequency and type of monitoring and drug supply aspects. Further analysis revealed that 62% of chief investigators responded to the review. All responses were positive with an overall acceptance rate of 89% of the proposed protocol changes. Conclusion: Review of pharmacy content of cancer clinical trial protocols is feasible and exposes many undetected clinically relevant issues that could hinder efficient trial conduct. Our service audit revealed that the majority of suggestions were effectively incorporated in the final protocols. The refinement of existing and development of new pharmacy-related guidance documents by Chemotherapy and Pharmacy Advisory Service might aid in better and safer clinical research

    Micro to nanostructural observations in neutron irradiated nuclear graphites PCEA and PCIB

    The neutron irradiation-induced structural changes in nuclear grade graphites PCEA and PCIB were investigated using scanning electron microscopy (SEM), X-ray diffraction (XRD), Raman spectroscopy, transmission electron microscopy (TEM), selected area electron diffraction (SAED) and electron energy loss spectroscopy (EELS). The graphite samples were irradiated at the Advanced Test Reactor at the Idaho National Laboratory. Received doses ranged from 1.5 to 6.8 displacements per atom and irradiation temperatures varied between 350 °C and 670 °C. XRD and Raman measurements provided evidence for irradiation induced crystallite fragmentation, with crystallite sizes reduced by 39–55%. Analysis of TEM images was used to quantify fringe length, tortuosity, and relative misorientation of planes, and indicated that neutron irradiation induced basal plane fragmentation and curvature. EELS was used to quantify the proportion of sp2 bonding and specimen density; a slight reduction in planar-sp2 content (due to the buckling basal planes and the introduction of non-six-membered rings) agreed with the observations from TEM
