    Characteristics of severe anemia and its association with malaria in young children of the Kassena-Nankana District of northern Ghana.

    Severe anemia is thought to be the principal underlying cause of malaria death in areas of intense seasonal malaria transmission such as the Kassena-Nankana District of northern Ghana. Factors associated with severe anemia in young children, 6-24 months old, were elucidated by analyzing results of 2 malaria-associated anemia surveys (1996, 2000), separated by 4 years, but conducted in the same community and at the same seasonal time point. Age-adjusted comparison confirmed that the proportion of severely anemic children and overall mean hemoglobin (Hb) levels in the November 2000 sample were significantly improved over those of the 1996 sample (17.5 versus 26.4%, P = 0.03; Hb 7.5 versus 6.9 g/dL, P = 0.002). Weight-for-age Z-scores also indicated a significant improvement in the 2000 sample (-1.93 versus -2.20, P or = 6.0 g/dL, those with severe anemia (Hb < 6.0 g/dL) were older, more frequently parasitemic (odds ratio [OR], 1.60; 95% confidence interval [CI], 1.08-2.35), more often febrile (OR, 2.44; 95% CI, 1.71-3.48), and predominantly male (OR, 1.50; 95% CI, 1.05-2.13). An association was identified in both surveys between severe anemia and residence in the northern part of the district, but no clear link was observed in relation to irrigation. Blood transfusions, a likely surrogate index of severe anemia in young children, followed a distinct seasonal pattern. Evidence suggests that dramatic peaks and troughs of severe anemia are regular and possibly predictable events that may be used to gauge the health and survival of young children in this area

    Antibiotic Prescribing Patterns in Ghana, Uganda, Zambia and Tanzania Hospitals: Results from the Global Point Prevalence Survey (G-PPS) on Antimicrobial Use and Stewardship Interventions Implemented

    Antimicrobial resistance (AMR) remains an important global public health issue with antimicrobial misuse and overuse being one of the main drivers. The Global Point Prevalence Survey (G-PPS) of Antimicrobial Consumption and Resistance assesses the prevalence and the quality of antimicrobial prescriptions across hospitals globally. G-PPS was carried out at 17 hospitals across Ghana, Uganda, Zambia and Tanzania. The overall prevalence of antimicrobial use was 50% (30–57%), with most antibiotics prescribed belonging to the WHO ‘Access’ and ‘Watch’ categories. No ‘Reserve’ category of antibiotics was prescribed across the study sites while antimicrobials belonging to the ‘Not Recommended’ group were prescribed infrequently. Antimicrobials were most often prescribed for prophylaxis for obstetric or gynaecological surgery, making up between 12 and 18% of total prescriptions across all countries. The most prescribed therapeutic subgroup of antimicrobials was ‘Antibacterials for systemic use’. As a result of the programme, PPS data are now readily available for the first time in the hospitals, strengthening the global commitment to improved antimicrobial surveillance. Antimicrobial stewardship interventions developed included the formation of AMS committees, the provision of training and the preparation of new AMS guidelines. Other common interventions included the presentation of findings to clinicians for increased awareness, and the promotion of a multi-disciplinary approach to successful AMS programmes. Repeat PPS would be necessary to continually monitor the impact of interventions implemented. Broader participation is also encouraged to strengthen the evidence base

    Early life child micronutrient status, maternal reasoning, and a nurturing household environment have persistent influences on child cognitive development at age 5 years: Results from MAL-ED

    Background: Child cognitive development is influenced by early-life insults and protective factors. To what extent these factors have a long-term legacy on child development and hence fulfillment of cognitive potential is unknown. Objective: The aim of this study was to examine the relation between early-life factors (birth to 2 y) and cognitive development at 5 y. Methods: Observational follow-up visits were made of children at 5 y, previously enrolled in the community-based MAL-ED longitudinal cohort. The burden of enteropathogens, prevalence of illness, complementary diet intake, micronutrient status, and household and maternal factors from birth to 2 y were extensively measured and their relation with the Wechsler Preschool Primary Scales of Intelligence at 5 y was examined through use of linear regression. Results: Cognitive T-scores from 813 of 1198 (68%) children were examined and 5 variables had significant associations in multivariable models: mean child plasma transferrin receptor concentration (ÎČ: −1.81, 95% CI: −2.75, −0.86), number of years of maternal education (ÎČ: 0.27, 95% CI: 0.08, 0.45), maternal cognitive reasoning score (ÎČ: 0.09, 95% CI: 0.03, 0.15), household assets score (ÎČ: 0.64, 95% CI: 0.24, 1.04), and HOME child cleanliness factor (ÎČ: 0.60, 95% CI: 0.05, 1.15). In multivariable models, the mean rate of enteropathogen detections, burden of illness, and complementary food intakes between birth and 2 y were not significantly related to 5-y cognition. Conclusions: A nurturing home context in terms of a healthy/clean environment and household wealth, provision of adequate micronutrients, maternal education, and cognitive reasoning have a strong and persistent influence on child cognitive development. Efforts addressing aspects of poverty around micronutrient status, nurturing caregiving, and enabling home environments are likely to have lasting positive impacts on child cognitive development.publishedVersio

    The bii4africa dataset of faunal and floral population intactness estimates across Africa’s major land uses

    Sub-Saharan Africa is under-represented in global biodiversity datasets, particularly regarding the impact of land use on species’ population abundances. Drawing on recent advances in expert elicitation to ensure data consistency, 200 experts were convened using a modified-Delphi process to estimate ‘intactness scores’: the remaining proportion of an ‘intact’ reference population of a species group in a particular land use, on a scale from 0 (no remaining individuals) to 1 (same abundance as the reference) and, in rare cases, to 2 (populations that thrive in human-modified landscapes). The resulting bii4africa dataset contains intactness scores representing terrestrial vertebrates (tetrapods: ±5,400 amphibians, reptiles, birds, mammals) and vascular plants (±45,000 forbs, graminoids, trees, shrubs) in sub-Saharan Africa across the region’s major land uses (urban, cropland, rangeland, plantation, protected, etc.) and intensities (e.g., large-scale vs smallholder cropland). This dataset was co-produced as part of the Biodiversity Intactness Index for Africa Project. Additional uses include assessing ecosystem condition; rectifying geographic/taxonomic biases in global biodiversity indicators and maps; and informing the Red List of Ecosystems

    A Theoretical Examination of Contributing Factors to the Proliferation of Political Vigilantism: Evidence from Ghana.

    Political vigilantism has been a significant issue in many parts of the world, and it has undermined the rule of law and democratic processes in Ghana. Political vigilantism is understood as actions taken by individuals and/or quasi-institutions other than state officials which could either be extralegal or illegal mostly typified by violence in the political field. Ghana, in particular, has been affected by political vigilantism, and it is considered a major obstacle to political development in the Fourth Republic. This study uses an institutional performance-focused theoretical framework comprising the theories of structuration, legitimacy, institutional failure, and social learning to test the significance of the identified factors contributing to the prevalence of political vigilantism. The study employs a mainly qualitative methodology to answer the research question: “Does the theoretical framework explain the contributing factors of political vigilantism in Ghana?” The findings indicate that key state institutions have failed to fulfill their constitutional mandates and that political interference has motivated the spread of vigilante activities in Ghana. The results further suggest that a combination of factors, including unemployment, poverty, high illiteracy rates, and politicians’ desire for power, has sustained political vigilantism. Finally, the study recommends that allowing institutions such as the electoral commission and criminal justice agencies to work independently and without political influence will reduce the prevalence of political vigilantism, which has marred the beauty of Ghanaian democracy. This study contributes to our understanding of political vigilantism and its implications for theory, policy, laws, and further research. In conclusion, without institutional independence and strict adherence to the rule of law, political vigilantism will continue to be a major issue in Ghana’s political development

    Reduced egfr, elevated urine protein and low level of personal protective equipment compliance among artisanal small scale gold miners at Bibiani-Ghana: a cross-sectional study

    Abstract Background Mercury is a toxic metal with its effects on human health ranging from acute to chronic in a very short time of exposure. Artisanal and small-scale gold mining (ASGM) is the main source of direct human exposure to mercury. Aim To access the effect of mercury exposure on the renal function and level of personal protective equipment (PPE) compliance among small-scale gold miners in Bibiani District of the Western Region of Ghana Method 110 consenting male gold miners were purposively recruited for this study. A structured questionnaire was used to collect socio-demographic information from the participants. Work place assessment and interviews were conducted. Urine samples were analysed for protein; blood was analysed for mercury and creatinine. Estimated glomerular filtration rate (eGFR) was calculated using the chronic kidney disease-epidemiology collaboration (CKD-EPI) equation. Results Of the 110 participants, 61(55.5%) exceeded the occupational exposure threshold (blood mercury <5ÎŒg/L). Urine protein (41.72±68.34, P<0.0001), serum creatinine (2.24±1.19, P<0.0001) and blood mercury (18.37±10.47, P<0.0001) were significantly elevated among the exposed group compared to the non-exposed group. However, the exposed group had a significantly reduced eGFR (P<0.0001). There was a significant correlation (r=0.7338, p<0.0001) between blood mercury concentration and urine protein concentration. An increase in blood mercury correlated negatively (r = −0.8233, P<0.0001) with eGFR among the exposed group. High urine protein (P< 0.0001) and high serum creatinine (P< 0.0001) were significantly associated with increased mercury exposure. Increased mercury exposure was significantly associated with burning of amalgam (P=0.0196), sucking of excess mercury (P=0.0336), longer work duration (P=0.0314) and low educational background (P=0.0473). Conclusion Small scale miners at the Bibiani work site are exposed to excess mercury. Proteinuria and reduced eGFR is common in mine workers exposed to excess mercury. We found poor PPE compliance among the study population

    Environment, Migration and Food Security in the Upper West Region of Ghana

    Ghana has achieved dramatic improvements in national food security in recent years, but concealed in this overall progress is a considerable measure of regional unevenness, with the population living in the dry savannah regions in the north faring the worst. The Upper West Region (UWR) is the poorest region of Ghana and has long served as a reservoir of migratory labour for the southern parts of the country, but in recent years migration patterns have been both escalating and changing. Increasingly, permanent UWR migration is focusing on the more fertile lands of the Brong-Ahafo Region (BAR), where migrants are able to access farmland in different leasehold relationships. A rapid research appraisal conducted in Techiman (BAR) suggests that UWR migrants view their growing settlement in the BAR to be a long-term phenomenon. It also highlighted how land tenancy issues are central to the challenges migrant farmers face, and are largely perceived as being immutable by the farmers themselves. Nearly all new UWR migrants must begin working in sharecropping relationships for BA landlords, paying out one-third of their harvest as rent, and over time they hope to save sufficient market earnings in order to lease the land outright. Despite these rents and the high cost of transportation, this chapter suggests that evolving migration patterns from the Upper West Region (UWR) of Ghana are connected to an intensifying system of domestic “food aid” (i.e. non-market transfers) back to the region, providing a crucial means of coping with its precarious food insecurity. With environmental conditions in dry regions of Sahelian Africa projected to worsen with climate change, the agricultural capacity of the UWR is likely to deteriorate further in coming years, with migratory pressures therefore continuing to rise. In light of this, this study points towards both future research objectives in the UWR and the BAR, as well as to the implications such research could have for policy interventions and locally grounded regional initiatives
