    A Port Accessibility Index for Mediterranean container terminals

    Using the Mediterranean as an application case, this study defines a Port Accessibility Index (PAI) useful to measure the accessibility of Mediterranean container ports. Traditionally, accessibility is measured as the quality of maritime links that connect ports together, without considering the port’s characteristics. Differently from previous studies, this paper also focuses on the internal accessibility of the port. In so doing, the PAI will combine the Liner Shipping Connectivity Index with other factors, such as, time at port of container ships, maximum quay depth and number of containers per quay crane (QC) or per yard area. The application considers the 34 main Mediterranean container ports in terms of TEUs handled in the last decade. The results can contribute to highlight and identify the potential elements that could improve the accessibility of Mediterranean container ports, providing decision-makers with a useful wealth of knowledge when carrying out management strategies. Future studies may concern the extension of the research to other geographical areas with the inclusion of additional indicators (e.g., road and/or rail accessibility)

    Toward Precision Medicine in ADHD

    Attention-Deficit Hyperactivity Disorder (ADHD) is a complex and heterogeneous neurodevelopmental condition for which curative treatments are lacking. Whilst pharmacological treatments are generally effective and safe, there is considerable inter-individual variability among patients regarding treatment response, required dose, and tolerability. Many of the non-pharmacological treatments, which are preferred to drug-treatment by some patients, either lack efficacy for core symptoms or are associated with small effect sizes. No evidence-based decision tools are currently available to allocate pharmacological or psychosocial treatments based on the patient's clinical, environmental, cognitive, genetic, or biological characteristics. We systematically reviewed potential biomarkers that may help in diagnosing ADHD and/or stratifying ADHD into more homogeneous subgroups and/or predict clinical course, treatment response, and long-term outcome across the lifespan. Most work involved exploratory studies with cognitive, actigraphic and EEG diagnostic markers to predict ADHD, along with relatively few studies exploring markers to subtype ADHD and predict response to treatment. There is a critical need for multisite prospective carefully designed experimentally controlled or observational studies to identify biomarkers that index inter-individual variability and/or predict treatment response

    The chromatin and single-cell transcriptional landscapes of CD4 T cells in inflammatory bowel disease link risk loci with a proinflammatory Th17 cell population

    IntroductionThe imbalance between Th17 and regulatory T cells in inflammatory bowel diseases (IBD) promotes intestinal epithelial cell damage. In this scenario, T helper cell lineage commitment is accompanied by dynamic changes to the chromatin that facilitate or repress gene expression. MethodsHere, we characterized the chromatin landscape and heterogeneity of intestinal and peripheral CD4 T cellsfrom IBD patients using in house ATAC-Seq and single cell RNA-Seq libraries. ResultsWe show that chromatin accessibility profiles of CD4 T cells from inflamed intestinal biopsies relate to genes associated with a network of inflammatory processes. After integrating the chromatin profiles of tissue-derived CD4 T cells and in-vitro polarized CD4 T cell subpopulations, we found that the chromatin accessibility changes of CD4 T cells were associated with a higher predominance of pathogenic Th17 cells (pTh17 cells) in inflamed biopsies. In addition, IBD risk loci in CD4 T cells were colocalized with accessible chromatin changes near pTh17-related genes, as shown in intronic STAT3 and IL23R regions enriched in areas of active intestinal inflammation. Moreover, single cell RNA-Seq analysis revealed a population of pTh17 cells that co-expresses Th1 and cytotoxic transcriptional programs associated with IBD severity. DiscussionAltogether, we show that cytotoxic pTh17 cells were specifically associated with IBD genetic variants and linked to intestinal inflammation of IBD patients

    Cold-Adapted Influenza and Recombinant Adenovirus Vaccines Induce Cross-Protective Immunity against pH1N1 Challenge in Mice

    The rapid spread of the 2009 H1N1 pandemic influenza virus (pH1N1) highlighted problems associated with relying on strain-matched vaccines. A lengthy process of strain identification, manufacture, and testing is required for current strain-matched vaccines and delays vaccine availability. Vaccines inducing immunity to conserved viral proteins could be manufactured and tested in advance and provide cross-protection against novel influenza viruses until strain-matched vaccines became available. Here we test two prototype vaccines for cross-protection against the recent pandemic virus.BALB/c and C57BL/6 mice were intranasally immunized with a single dose of cold-adapted (ca) influenza viruses from 1977 or recombinant adenoviruses (rAd) expressing 1934 nucleoprotein (NP) and consensus matrix 2 (M2) (NP+M2-rAd). Antibodies against the M2 ectodomain (M2e) were seen in NP+M2-rAd immunized BALB/c but not C57BL/6 mice, and cross-reacted with pH1N1 M2e. The ca-immunized mice did not develop antibodies against M2e. Despite sequence differences between vaccine and challenge virus NP and M2e epitopes, extensive cross-reactivity of lung T cells with pH1N1 peptides was detected following immunization. Both ca and NP+M2-rAd immunization protected BALB/c and C57BL/6 mice against challenge with a mouse-adapted pH1N1 virus.Cross-protective vaccines such as NP+M2-rAd and ca virus are effective against pH1N1 challenge within 3 weeks of immunization. Protection was not dependent on recognition of the highly variable external viral proteins and could be achieved with a single vaccine dose. The rAd vaccine was superior to the ca vaccine by certain measures, justifying continued investigation of this experimental vaccine even though ca vaccine is already available. This study highlights the potential for cross-protective vaccines as a public health option early in an influenza pandemic

    Evaluating the efficiency of Mediterranean container ports using data envelopment analysis

    Mediterranean container ports are constantly requested to face new market challenges after years of decline of their traffic in favour of a stronger leadership of Northern range ports, also favoured by the consolidation of the Arctic Route and the Belt and Road Initiative. To regain competitiveness, Mediterranean ports must undertake a path of operational, managerial and infrastructural strengthening. This study applies Data Envelopment Analysis (DEA) to 35 Mediterranean container ports in order to identify the potential key success factors on which to intervene to improve their competitiveness potential and response to the new market needs. The application uses the port throughput as output, and three inputs: yard area, number of quay cranes and distance of the port from the Suez–Gibraltar axis. The latter has been inserted in the study to evaluate whether it can affect port efficiency. Both input- and output-oriented approaches are used in order to investigate the port competitiveness through the production function (input-oriented), and the port commercial competitiveness based on the output and feasibility of its production (output-oriented). Results can provide insights for the implementation of future policies and management strategies aimed to strengthen the Mediterranean port context

    In the maritime world scenario, various challenges are affecting Mediterranean container ports, which are trying to keep high their efficiency and their competitiveness through infrastructural and managerial improvements. The identification of the priority actions requires the analysis of the productivity of each port in relation to the use of its resources. This study applies Data Envelopment Analysis (DEA) and Principal Component Analysis (PCA) in order to investigate the potential factors that can affect the efficiency of Mediterranean container ports. These methods use six input variables (yard area, berth depth, number of quay cranes, equipment, berth length and distance of the port from the Suez-Gibraltar axis) and one output variable (port throughput expressed in TEUs). The results can help to highlight the potential factors of success for Mediterranean container ports and to identify future policies and management strategies aimed towards the strengthening of the analyzed context
