    Simulations of the Kelvin-Helmholtz instability driven by coronal mass ejections in the turbulent corona

    Recent high resolution AIA/SDO images show evidence of the development of the Kelvin-Helmholtz instability, as coronal mass ejections (CMEs) expand in the ambient corona. A large-scale magnetic field mostly tangential to the interface is inferred, both on the CME and on the background sides. However, the magnetic field component along the shear flow is not strong enough to quench the instability. There is also observational evidence that the ambient corona is in a turbulent regime, and therefore the criteria for the development of the instability are a-priori expected to differ from the laminar case. To study the evolution of the Kelvin-Helmholtz instability with a turbulent background, we perform three-dimensional simulations of the incompressible magnetohydrodynamic equations. The instability is driven by a velocity profile tangential to the CME-corona interface, which we simulate through a hyperbolic tangent profile. The turbulent background is generated by the application of a stationary stirring force. We compute the instability growth-rate for different values of the turbulence intensity, and find that the role of turbulence is to attenuate the growth. The fact that the Kelvin-Helmholtz instability is observed, sets an upper limit to the correlation length of the coronal background turbulence

    Course of Untreated High Blood Pressure in the Emergency Department

    <p>Introduction: No clear understanding exists about the course of a patient’s blood pressure (BP) during an emergency department (ED) visit. Prior investigations have demonstrated that BP can be reduced by removing patients from treatment areas or by placing patients supine and observing them for several hours. However, modern EDs are chaotic and noisy places where patients and their families wait for long periods in an unfamiliar environment. We sought to determine the stability of repeated BP measurements in the ED environment.</p> <p>Methods: A prospective study was performed at an urban ED. Research assistants trained and certified in BP measurement obtained sequential manual BPs and heart rates on a convenience sample of 76 patients, beginning with the patient arrival in the ED. Patients were observed through their stay for up to 2 hours, and BP was measured at 10-minute intervals. Data analysis with SAS PROC MIXED (SAS Institute, Cary, North Carolina) for regression models with correlated data determined the shape of the curve as BP changed over time. Patients were grouped on the basis of their presenting BP as normal (less than 140/90), elevated (140–160/90–100), or severely elevated (greater than 160/100) for the regression analysis.</p> <p>Results: A statistically significant downward trend in systolic and diastolic BP was observed only for those patients presenting with severely elevated BPs (ie, greater than 160/100).</p> <p>Conclusion: We demonstrate a statistically significant decline in systolic and diastolic BP over time spent in the ED only for patients with severely elevated presenting BPs.     Investigation into Reynolds Number Effects on a Biomimetic Flapping Wing

    This research investigated the behavior of a Manduca sexta inspired biomimetic wing as a function of Reynolds number by measuring the aerodynamic forces produced by varying the characteristic wing length and testing at air densities from atmospheric to near vacuum. A six degree of freedom balance was used to measure forces and moments, while high speed cameras were used to measure wing stroke angle. An in-house created graphical user interface was used to vary the voltage of the drive signal sent to the piezoelectric actuator which determined the wing stroke angle. The Air Force Institute of Technology baseline 50 mm wing was compared to wings manufactured with 55, 60, 65, and 70 mm spans, while maintaining a constant aspect ratio. Tests were conducted in a vacuum chamber at air densities between 0.5% and 100% of atmospheric pressure. Increasing the wingspan increased the wing’s weight, which reduced the first natural frequency; and did not result in an increase in vertical force over the baseline 50 mm wing. However, if the decrease in natural frequency corresponding to the increased wing span was counteracted by increasing the thickness of the joint material in the linkage mechanism, vertical force production increased over the baseline wing planform. Of the wings built with the more robust flapping mechanism, the 55 mm wing span produced 95% more vertical force at a 26% higher flapping frequency, while the 70 mm wing span produced 165% more vertical force at a 10% lower frequency than the Air Force Institute of Technology baseline wing. Negligible forces and moments were measured at vacuum, where the wing exhibited predominantly inertial motion, revealing flight forces measured in atmosphere are almost wholly limited to interaction with the surrounding air. Lastly, there was a rough correlation between Reynolds number and vertical force, indicating Reynolds number is a useful modelling parameter to predict lift and corresponding aerodynamic coefficients for a specific wing design

    The magnetic structure of a coronal X-ray bright point

    X-ray bright points are small dynamic loop structures that are observed all over the solar corona. The high spatial and temporal resolution of the TRACE instrument allows bright points to be studied in much greater detail than previously possible. This paper focuses on a specific bright point which occurred for about 20 hours on 13-14 June 1998 and examines its dynamic structure in detail. This example suggests that the mechanisms that cause bright points to form and evolve are more complex than previously thought. In this case, reconnection probably plays a major part during the formation and brightening of the loop structure. However, later on the foot points rotate injecting twist into the bright point which may cause an instability to occur with dynamic results.</p

    Anterior Cruciate Ligament Injuries in National Football League Athletes From 2010 to 2013: A Descriptive Epidemiology Study.

    BACKGROUND: There is a high incidence of anterior cruciate ligament (ACL) injuries among National Football League (NFL) athletes; however, the incidence of reinjury in this population is unknown. PURPOSE: This retrospective epidemiological study analyzed all publicly disclosed ACL tears occurring in NFL players between 2010 and 2013 to characterize injury trends and determine the incidence of reinjury. STUDY DESIGN: Descriptive epidemiological study. METHODS: A comprehensive online search identified any NFL player who had suffered an ACL injury from 2010 to 2013. Position, playing surface, activity, and date were recorded. Each player was researched for any history of previous ACL injury. The NFL games database from USA Today was used to determine the incidence of ACL injuries on artificial turf and grass fields. Databases from Pro Football Focus and Pro Football Reference were used to determine the injury rate for each position. RESULTS: NFL players suffered 219 ACL injuries between 2010 and 2013. Forty players (18.3%) had a history of previous ACL injury, with 27 (12.3%) retears and 16 (7.3%) tears contralateral to a previous ACL injury. Five players (2.28%) suffered their third ACL tear. Receivers (wide receivers and tight ends) and backs (linebackers, fullbacks, and halfbacks) had significantly greater injury risk than the rest of the NFL players, while perimeter linemen (defensive ends and offensive tackles) had significantly lower injury risk than the rest of the players. Interior linemen (offensive guards, centers, and defensive tackles) had significantly greater injury risk compared with perimeter linemen. ACL injury rates per team games played were 0.050 for grass and 0.053 for turf fields (P \u3e .05). CONCLUSION: In this retrospective epidemiological study of ACL tears in NFL players, retears and ACL tears contralateral to a previously torn ACL constituted a substantial portion (18.3%) of total ACL injuries. The significant majority of ACL injuries in players with a history of previous ACL injury were retears. Skilled offensive players and linebackers had the greatest injury risk, and significantly more ACL tears occurred among interior linemen than perimeter linemen. The month of August had the highest incidence of ACL injuries, probably because of expanded roster sizes at that point in the NFL season

    Safety of Induced Sputum Collection in Children Hospitalized With Severe or Very Severe Pneumonia.

    Background.: Induced sputum (IS) may provide diagnostic information about the etiology of pneumonia. The safety of this procedure across a heterogeneous population with severe pneumonia in low- and middle-income countries has not been described. Methods.: IS specimens were obtained as part a 7-country study of the etiology of severe and very severe pneumonia in hospitalized childrenbefore, during, and after the procedure to record oxygen requirement, oxygen saturation, respiratory rate, consciousness level, and other evidence of clinical deterioration. Criteria for IS contraindications were predefined and serious adverse events (SAEs) were reported to ethics committees and a central safety monitor. Results.: A total of 4653 IS procedures were done among 3802 children. Thirteen SAEs were reported in relation to collection of IS, or 0.34% of children with at least 1 IS specimen collected (95% confidence interval, 0.15%-0.53%). A drop in oxygen saturation that required supplemental oxygen was the most common SAE. One child died after feeding was reinitiated 2 hours after undergoing sputum induction; this death was categorized as possibly related to the procedure. Conclusions.: The overall frequency of SAEs was very low, and the nature of most SAEs was manageable, demonstrating a low-risk safety profile for IS collection even among severely ill children in low-income-country settings. Healthcare providers should monitor oxygen saturation and requirements during and after IS collection, and assess patients prior to reinitiating feeding after the IS procedure, to ensure patient safety

    Genotipos de virus papiloma humano en mujeres con alteraciones citológicas cervicales de un área con alta incidencia de cáncer cervical

    It has been well demonstrated the relationship between the infection with high-risk human papillomavirus (HPVs) genotypes and cervical cancer. In Northeastern Argentina a high incidence of this pathology has been described and therefore a high prevalence of HPV infection is expected. In order to identify HPV genotypes associated with malignant and pre-malignant cervical lesions present in the area, 53 ecto-endo cervical cell specimens obtained from women with cytohistological alterations were studied by a PCR-RFLP technique. Out of 53 patients, 34 (64.2%) were positive for HPV infection, being HPV-16 (32.3%) the most frequently found genotype, followed by HPV-58 (14.7%), -6, -18 and -45 (5.9%), -33, -52, -53, -54, -56, -66, -MM4 and -LVX100 (2.9%). Also 5 cases of infection caused by multiple genotypes were found, which corresponded to 14.7% of the positive cases. Results indicate that besides HPV-16 and -18, the most prevalent high-risk HPV genotypes worldwide, others like -45 and -58 as well as co-infection cases are frequent between women of Northeastern Argentina, and a particular attention should be paid to this circumstance because it could be an epidemiological feature of regional importance and a useful information for a future vaccination program.La relación entre la infección por los virus papiloma humanos (HPVs) de alto riesgo y el cáncer de cuello de útero ha sido bien demostrada. En el Nordeste de Argentina se observa una alta incidencia de esta patología y en consecuencia se estima una alta prevalencia de infección por HPV. A fin de identificar los genotipos de HPV presentes en el área, asociados a casos de lesiones malignas y premalignas de cuello de útero, se estudiaron 53 muestras ecto-endo cervicales de mujeres con alteraciones citohistológicas residentes permanentes de las ciudades de Resistencia y Corrientes. De las 53 pacientes estudiadas, 34 resultaron positivas para HPV (64.2%), correspondiendo la mayor frecuencia a HPV-16 (32.3%), seguido por HPV-58 (14.7%), HPV-6, -18 y -45 (5.9%), -33, -52, -53, -54, -56, -66, -MM4 y -LVX100 (2.9%). Además, se encontraron 5 casos de infecciones mixtas causadas por mas de un genotipo, lo que resulta de importancia ya que representan el 14.7% del total de los casos positivos. Los resultados demuestran que, además de HPV-16 y -18 que son los genotipos de alto riesgo de mayor prevalencia a nivel mundial, otros como el HPV-45 y -58 y los casos de infecciones múltiples son frecuentes en mujeres del nordeste argentino, lo que podría constituir un rasgo epidemiológico de importancia regional y ser de utilidad en el futuro en los programas de vacunación

    A blood-based metabolomics test to distinguish relapsing–remitting and secondary progressive multiple sclerosis: addressing practical considerations for clinical application

    The transition from relapsing–remitting multiple sclerosis (RRMS) to secondary progressive MS (SPMS) represents a huge clinical challenge. We previously demonstrated that serum metabolomics could distinguish RRMS from SPMS with high diagnostic accuracy. As differing sample-handling protocols can affect the blood metabolite profile, it is vital to understand which factors may influence the accuracy of this metabolomics-based test in a clinical setting. Herein, we aim to further validate the high accuracy of this metabolomics test and to determine if this is maintained in a ‘real-life’ clinical environment. Blood from 31 RRMS and 28 SPMS patients was subjected to different sample-handling protocols representing variations encountered in clinics. The effect of freeze–thaw cycles (0 or 1) and time to erythrocyte removal (30, 120, or 240 min) on the accuracy of the test was investigated. For test development, samples from the optimised protocol (30 min standing time, 0 freeze–thaw) were used, resulting in high diagnostic accuracy (mean ± SD, 91.0 ± 3.0%). This test remained able to discriminate RRMS and SPMS samples that had experienced additional freeze–thaw, and increased standing times of 120 and 240 min with accuracies ranging from 85.5 to 88.0%, because the top discriminatory metabolite biomarkers from the optimised protocol remained discriminatory between RRMS and SPMS despite these sample-handling variations. In conclusion, while strict sample-handling is essential for the development of metabolomics-based blood tests, the results confirmed that the RRMS vs. SPMS test is resistant to sample-handling variations and can distinguish these two MS stages in the clinics

    Polycomb group (PcG) proteins prevent the assembly of abnormal synaptonemal complex structures during meiosis

Timely expression of SC coding genes is essential for SC assembly and successful meiosis. However, SC components have an intrinsic tendency to self-organize into abnormal repetitive structures, which are not assembled between the paired homologs and whose formation is potentially deleterious for meiosis and gametogenesis. This creates an interesting conundrum, where SC genes need to be robustly expressed during meiosis, but their expression must be carefully regulated to prevent the formation of anomalous SC structures. In this manuscript, we show that the Polycomb group protein Sfmbt, the Drosophila ortholog of human MBTD1 and L3MBTL2, is required to avoid excessive expression of SC genes during prophase I. Although SC assembly is normal after Sfmbt depletion, SC disassembly is abnormal with the formation of multiple synaptonemal complexes (polycomplexes) within the oocyte. Overexpression of the SC gene corona and depletion of other Polycomb group proteins are similarly associated with polycomplex formation during SC disassembly. These polycomplexes are highly dynamic and have a well-defined periodic structure. Further confirming the importance of Sfmbt, germ line depletion of this protein is associated with significant metaphase I defects and a reduction in female fertility. Since transcription of SC genes mostly occurs during early prophase I, our results suggest a role of Sfmbt and other Polycomb group proteins in downregulating the expression of these and other early prophase I genes during later stages of meiosis.R.G.M. is supported by Portuguese national funding through Fundação para a Ciência e a Tecnologia (FCT grant refs.
