    Faktor yang tidak bisa diabaikan dalam meningkatkan kinerja pegawai adalah motivasi. Hal ini sesuai pendapat Hasibuan (1996: 45) seorang pegawai akan menghasilkan kinerja yang tinggi jika pegawai termotivasi dalam bekerja. Motivasi dapat menjadi pendorong seseorang melaksanakan suatu kegiatan guna mendapatkan hasil yang terbaik, oleh karena itulah tidak heran jika pegawai yang mempunyai motivasi kerja yang tinggi biasanya mempunyai kinerja yang tinggi pula. Faktor lain yang tidak bisa diabaikan dalam meningkatkan kinerja pegawai adalah “budaya organisasi”, semakin baik budaya organisasi yang diterapkan dalam suatu instansi maka semakin meningkat pula semangat kerja pegawai, karena pada dasarnya budaya organisasi merupakan suatu aturan yang mengikat pegawai agar bersikap dan bertindak sesuai aturan yang telah ditetapkan dan disepakati bersama. Tujuan dari penelitian ini adalah untuk mengetahui pengaruh motivasi kerja dan budaya organisasi terhadap kinerja pegawai pada Dinas Perdagangan Provinsi Lampung baik secara parsial maupun secara simultan. Adapun tekhnik pengumpulan data dalam penelitian ini adalah observasi, wawancara, kuesioner dan dokumentasi. Sampel dalam penelitian ini sebanyak 60 orang pegawai pada Dinas Perdagangan Provinsi Lampung. Dalam penelitian ini penulis menggunakan analisis data kualitatif dan kuantitatif. Secara umum hasil peneltian ini adalah sebagai berikut:. 1) Ada pengaruh yang positif antara motivasi kerja terhadap kinerja pegawai pada Dinas Perdagangan Provinsi Lampung. 2).Ada pengaruh yang positif antara budaya organisasi terhadap kinerja pegawai pada Dinas Perdagangan Provinsi Lampung 3) Motivasi kerja dan budaya organisasi secara bersama-sama berpengaruh positif terhadap kinerja pegawai pada Dinas Perdagangan Provinsi Lampung

    Safety aspects of infliximab in inflammatory bowel disease patients - A retrospective cohort study in 100 patients of a German University Hospital

    Background: Infliximab, a chimeric anti-tumour necrosis factor monoclonal antibody with potent anti-inflammatory effects, represents an effective treatment option in patients with severe inflammatory bowel disease (IBD). Serious side-effects of such an immunomodulating therapy are speculated and therefore we reviewed our clinical experience in a retrospective safety study looking upon a single cohort of 100 IBD patients from a large German University Hospital. Methods: 100 patients with severe Crohn's disease (n = 92), ulcerative colitis (n = 7) or indeterminate colitis (n = 1) treated with infliximab (5 mg/kg) from January 2000 to December 2003 were retrospectively analysed for acute and subacute adverse events by chart review. Results: Overall, infliximab therapy was generally well tolerated. No fatal complications, malignancies, autoimmune diseases, neurologic or cardiovascular complications were observed in the cohort during the study period. Overall, adverse events were observed in 10 patients: 2 patients showed an acute infusion reaction, 1 patient a serum sickness-like reaction, in 4 patients a bacterial or viral infection occurred, in 1 patient pancytopenia and 2 patients developed surgical complications. Only 6 patients with adverse events required admission to hospital. A case of tuberculosis after infliximab was not found. The lack of adverse side-effects was associated with young median age and infrequent comorbidities of the cohort. Conclusion: Regarding its strong immunomodulating capacity, infliximab appears to be an efficient and relatively safe therapeutic option for patients with severe IBD. However, the use of infliximab requires careful screening and close patient monitoring to identify patients at risk and the infrequent, but sometimes serious complications of infliximab.     Adjunctive treatment with oral AKL1, a botanical nutraceutical, in chronic obstructive pulmonary disease

    Purpose: The objective of this pilot trial was to evaluate the safety and efficacy of AKL1, a patented botanical formulation containing extracts of Picrorhiza kurroa, Ginkgo biloba, and Zingiber officinale, as add-on therapy for patients with chronic obstructive pulmonary disease (COPD) and chronic cough. Patients and methods: This randomized, double-blind, placebo-controlled trial enrolled male and female patients .18 years old with COPD and Leicester Cough Questionnaire (LCQ) score of ,18. The 10-week study period comprised a 2-week single-blind placebo run-in period followed by add-on treatment with AKL1 or placebo twice daily for 8 weeks. The primary study endpoint was the change from week 0 to week 8 in cough-related health status, as assessed by the LCQ. Results: Of 33 patients enrolled, 20 were randomized to AKL1 and 13 to placebo. Patients included 19 (58%) men and 14 (42%) women of mean (standard deviation [SD]) age of 67 (9.4) years; 15 (45%) patients were smokers and 16 (49%) were ex-smokers. The mean (SD) change from baseline in LCQ score at 8 weeks was 2.3 (4.9) in the AKL1 group and 0.6 (3.7) in the placebo group, with mean difference in change of 1.8 (95% confidence interval: -1.5 to 5.1; P=0.28). The St George’s Respiratory Questionnaire score improved substantially in the AKL1 treatment group by a mean (SD) of -7.7 (11.7) versus worsening in the placebo group (+1.5 [9.3]), with mean difference in change of -9.2 (95% confidence interval: -19.0 to 0.6; P=0.064). There were no significant differences between treatment groups in change from baseline to week 8 in other patient-reported measures, lung function, or the 6-minute walk distance. Conclusion: Further study is needed with a larger patient population and over a longer duration to better assess the effects of add-on therapy with AKL1 in COPD

    Emeishan Basalts, SW China: Reappraisal of the formation's type area stratigraphy and a discussion of its significance as a large igneous province

    The late Permian Emeishan Basalt Formation of SW China is one of Earth's LIPs (large igneous provinces), yet its basic geology remains poorly documented. Recent work on sections close to the type area in Sichuan Province enable us in part to rectify this. Descriptions of the formation and associated units at two areas, one on the lower flanks of Mt Emei and another from a series of outercrops in Ebian County, 50-70 km to the SW, are presented. The basalt pile is 180-270 m thick and in both areas comprises 12 flows that were erupted in relatively quick succession. It rests conformably upon shallow-marine limestones/lignites suggesting emplacement close to sea level. The upper half of the youngest basalt was intensively weathered, but not eroded, prior to it being conformably succeeded by complex body of rocks c. 30 m thick, that includes thin basalts, pyroclastic rocks, tuffs and organic-rich terrestrial sediments. This unit, which has previously been described as a sedimentary package, presumably because intense weathering has obscured the primary lithological fabric in key outcrops, is considered to mark the volcanic waning phase. Uppermost Permian and Triassic terrestrial sediments conformably overlie the terminal volcanic rocks. The sub-regional stratigraphy is compared, as best it can be, with that described from two sections 400 km to the SE; one section matches reasonably well, the other does not, indicating that regional correlations need to be developed carefully. The information is discussed in the context of LIP generator models; several key features of the Emeishan Basalt terrain are at odds with those commonly encountered in LIP's. The most important conclusion is that the unit marks a prematurely terminated system in which full bloodied rifting leading to the development of an ocean basin never started.published_or_final_versio

    The impact of non-severe burn injury on cardiac function and long-term cardiovascular pathology

    Severe burn injury significantly affects cardiovascular function for up to 3 years. However, whether this leads to long-term pathology is unknown. The impact of non-severe burn injury, which accounts for over 80% of admissions in developed countries, has not been investigated. Using a rodent model of non-severe burn injury with subsequent echocardiography we showed significantly increased left ventricular end systolic diameter (LVESD) and ventricular wall thickness at up to 3 months post-injury. Use of propranolol abrogated the changes in cardiac measures observed. Subsequently we investigated changes in a patient cohort with non-severe injury. Echocardiography measured at baseline and at 3 months post-injury showed increased LVESD at 3 months and significantly decreased posterior wall diameter. Finally, 32 years of Western Australian hospital records were used to investigate the incidence of cardiovascular disease admissions after burn injury. People who had experienced a burn had increased hospital admissions and length of stay for cardiovascular diseases when compared to a matched uninjured cohort. This study presents animal, patient and population data that strongly suggest non-severe burn injury has significant effects on cardiovascular function and long-term morbidity in some burn patients. Identification of patients at risk will promote better intervention and outcomes for burn patients

    Acute abdominal pain presenting as a rare appendiceal duplication: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Appendiceal duplication is a rare anomaly that can manifest as right lower quadrant pain. There are several variations described for this condition. We recommend aggressive operative management should this anatomical variation present in the presence of acute appendicitis.</p> <p>Case presentation</p> <p>We report the case of a 15-year-old African American girl who presented to our hospital with right lower quadrant pain and was subsequently found to have appendiceal duplication.</p> <p>Conclusion</p> <p>There are two categorical systems that have described and stratified appendiceal duplication. Both classification systems have been outlined and referenced in this case report. A computed tomography scan has been included to provide a visual aid to help identify true vermiform appendiceal duplication. The presence of this anatomical abnormality is not a reason for surgical intervention; however, should this be found in the setting of acute appendicitis, aggressive resection of both appendices is mandatory.</p

    Critical research gaps and translational priorities for the successful prevention and treatment of breast cancer

    INTRODUCTION Breast cancer remains a significant scientific, clinical and societal challenge. This gap analysis has reviewed and critically assessed enduring issues and new challenges emerging from recent research, and proposes strategies for translating solutions into practice. METHODS More than 100 internationally recognised specialist breast cancer scientists, clinicians and healthcare professionals collaborated to address nine thematic areas: genetics, epigenetics and epidemiology; molecular pathology and cell biology; hormonal influences and endocrine therapy; imaging, detection and screening; current/novel therapies and biomarkers; drug resistance; metastasis, angiogenesis, circulating tumour cells, cancer 'stem' cells; risk and prevention; living with and managing breast cancer and its treatment. The groups developed summary papers through an iterative process which, following further appraisal from experts and patients, were melded into this summary account. RESULTS The 10 major gaps identified were: (1) understanding the functions and contextual interactions of genetic and epigenetic changes in normal breast development and during malignant transformation; (2) how to implement sustainable lifestyle changes (diet, exercise and weight) and chemopreventive strategies; (3) the need for tailored screening approaches including clinically actionable tests; (4) enhancing knowledge of molecular drivers behind breast cancer subtypes, progression and metastasis; (5) understanding the molecular mechanisms of tumour heterogeneity, dormancy, de novo or acquired resistance and how to target key nodes in these dynamic processes; (6) developing validated markers for chemosensitivity and radiosensitivity; (7) understanding the optimal duration, sequencing and rational combinations of treatment for improved personalised therapy; (8) validating multimodality imaging biomarkers for minimally invasive diagnosis and monitoring of responses in primary and metastatic disease; (9) developing interventions and support to improve the survivorship experience; (10) a continuing need for clinical material for translational research derived from normal breast, blood, primary, relapsed, metastatic and drug-resistant cancers with expert bioinformatics support to maximise its utility. The proposed infrastructural enablers include enhanced resources to support clinically relevant in vitro and in vivo tumour models; improved access to appropriate, fully annotated clinical samples; extended biomarker discovery, validation and standardisation; and facilitated cross-discipline working. CONCLUSIONS With resources to conduct further high-quality targeted research focusing on the gaps identified, increased knowledge translating into improved clinical care should be achievable within five years

    The application of radiomics in laryngeal cancer

    Objectives: Radiomics is the conversion of medical images into quantitative high-dimensional data. Laryngeal cancer, one of the most common head and neck cancers, has risen globally by 58.7%. CT, MRI and PET are acquired during the diagnostic process providing potential data for radiomic analysis and correlation with outcomes. This review aims to examine the applications of this technique to laryngeal cancer and the future considerations for translation into clinical practice. Methods: A comprehensive systematic review-informed search of the MEDLINE and EMBASE databases was undertaken. Keywords “laryngeal cancer” OR “larynx“ OR “larynx cancer” OR “head and neck cancer” were combined with “radiomic” OR “signature” OR “machine learning” OR “artificial intelligence”. Additional articles were obtained from bibliographies using the “snowball method”. Results: The included studies (n = 15) demonstrated that radiomic features are significantly associated with various clinical outcomes (including stage, overall survival, treatment response, progression-free survival) and that predictive models incorporating radiomic features are superior to those that do not. Two studies demonstrated radiomics could improve laryngeal cancer staging whilst 12 studies affirmed its predictive capability for clinical outcomes. Conclusions: Radiomics has potential for improving multiple aspects of laryngeal cancer care; however, the heterogeneous cohorts and lack of data on laryngeal cancer exclusively inhibits firm conclusions. Large prospective well-designed studies in laryngeal cancer are required to progress this field. Furthermore, to implement radiomics into clinical practice, a unified research effort is required to standardise radiomics practice.
