Getting inside acupuncture trials - Exploring intervention theory and rationale
<p>Abstract</p> <p>Background</p> <p>Acupuncture can be described as a complex intervention. In reports of clinical trials the mechanism of acupuncture (that is, the process by which change is effected) is often left unstated or not known. This is problematic in assisting understanding of how acupuncture might work and in drawing together evidence on the potential benefits of acupuncture. Our aim was to aid the identification of the assumed mechanisms underlying the acupuncture interventions in clinical trials by developing an analytical framework to differentiate two contrasting approaches to acupuncture (traditional acupuncture and Western medical acupuncture).</p> <p>Methods</p> <p>Based on the principles of realist review, an analytical framework to differentiate these two contrasting approaches was developed. In order to see how useful the framework was in uncovering the theoretical rationale, it was applied to a set of trials of acupuncture for fatigue and vasomotor symptoms, identified from a wider literature review of acupuncture and early stage breast cancer.</p> <p>Results</p> <p>When examined for the degree to which a study demonstrated adherence to a theoretical model, two of the fourteen selected studies could be considered TA, five MA, with the remaining seven not fitting into any recognisable model. When examined by symptom, five of the nine vasomotor studies, all from one group of researchers, are arguably in the MA category, and two a TA model; in contrast, none of the five fatigue studies could be classed as either MA or TA and all studies had a weak rationale for the chosen treatment for fatigue.</p> <p>Conclusion</p> <p>Our application of the framework to the selected studies suggests that it is a useful tool to help uncover the therapeutic rationale of acupuncture interventions in clinical trials, for distinguishing between TA and MA approaches and for exploring issues of model validity. English language acupuncture trials frequently fail to report enough detail relating to the intervention. We advocate using this framework to aid reporting, along with further testing and refinement of the framework.</p
Evaluating the thermal comfort performance of heating systems using a thermal manikin with human thermoregulatory control

The evaluation of the local thermal comfort and application of thermal manikins can further assist the design and selection of heating systems. This study aimed at evaluating the thermal comfort performance of different heating systems using a newly developed thermal manikin with an enhanced thermal control. The heating systems for a workstation, included a conventional radiator (convector) mounted under the window, heated floor in the occupied zone and an infrared heater mounted to the ceiling. The experiments were conducted in a test room with a façade attached to a climate chamber to simulate outdoor winter conditions. In these experiments, the supplied power for the different systems was kept constant to independently quantify the differences in their thermal comfort performance at same energy consumption. The thermal manikin was deployed in the occupied zone to evaluate the local and overall thermal comfort under each system using the equivalent temperature (Teq) approach. The thermoregulatory control used in the manikin operation is based on a model of human thermoregulation that interacts accurately with the surrounding environment through real-time measurements. The results showed that at the same energy consumption of the different systems, the variations in local thermal comfort levels were up to 1 on the comfort scale

Indoor Air Quality Monitoring (IAQ): A low-cost alternative to CO2 monitoring in comparison to an industry standard device
This paper demonstrates how low cost CO2 devices can be setup to be just as accurate as more expensive indoor CO2 device. As well as achieving a balance between sufficient accuracy in measurements and low cost other attributes such as reliability and data transmission rates has to be considered. Experiments in a naturally ventilated, mechanically ventilated and controlled environment are conducted to test the proposed device’s performance. Results found show that the device has a mean error of ±32 ppm in comparison to the industry device measurements, meeting accuracy. The device is able to measure readings consistently over consecutive days at a rate of 10 seconds per measurement meeting data transmission requirements. Furthermore it was able to monitor an enclosed room with CO 2 levels reaching over 3000 ppm, hence meeting reliability. This is followed by a discussion evaluating the effectiveness of the proposed device in terms of value to the consumer. Based on the conclusion the device is felicitous for use within an office environment from observing performance against an industry standard device.</p
