The compact, ∼1 kpc host galaxy of a quasar at a redshift of 7.1
We present Atacama Large Millimeter/submillimeter Array (ALMA) observations of the [C ii] fine-structure line and the underlying far-infrared (FIR) dust continuum emission in J1120+0641, the most distant quasar currently known (). We also present observations targeting the CO(2–1), CO(7–6), and [C i] 369 μm lines in the same source obtained at the Very Large Array and Plateau de Bure Interferometer. We find a [C ii] line flux of Jy and a continuum flux density of mJy beam−1, consistent with previous unresolved measurements. No other source is detected in continuum or [C ii] emission in the field covered by ALMA (~ 25''). At the resolution of our ALMA observations (0farcs23, or 1.2 kpc, a factor of ~70 smaller beam area compared to previous measurements), we find that the majority of the emission is very compact: a high fraction (~80%) of the total line and continuum flux is associated with a region 1–1.5 kpc in diameter. The remaining ~20% of the emission is distributed over a larger area with radius lesssim4 kpc. The [C ii] emission does not exhibit ordered motion on kiloparsec scales: applying the virial theorem yields an upper limit on the dynamical mass of the host galaxy of , only ~20 × higher than the central black hole (BH). The other targeted lines (CO(2–1), CO(7–6), and [C i]) are not detected, but the limits of the line ratios with respect to the [C ii] emission imply that the heating in the quasar host is dominated by star formation, and not by the accreting BH. The star formation rate (SFR) implied by the FIR continuum is 105–340 , with a resulting SFR surface density of ~100–350 kpc−2, well below the value for Eddington-accretion-limited star formation

Negligible microbial matground influence on pre-vegetation river functioning: Evidence from the Ediacaran-Lower Cambrian Series Rouge, France
The pre-Silurian alluvial rock record is dominated by accumulations of laterally-extensive, sheet-like sandstone strata with minimal mudrock; a depositional style frequently explained as representing fluvial processes particular to “pre-vegetation” Earth. While the sedimentological and geomorphological influence of Palaeozoic embryophytes and other higher vegetation has been commonly inferred, the influence of the non-marine microbial matgrounds that preceded them has been less well studied. The ?Ediacaran-Cambrian Series Rouge of northern France and the Channel Islands is a rare example of a predominantly alluvial succession which exhibits both pre-vegetation sedimentary motifs and evidence for the existence of terrestrial microbial mats. The latter include likely microbial sedimentary surface textures, the enigmatic matground “pseudofossils” Aristophycus and Arumberia, and probable mat fragments and mat-related microtextures preserved in argillaceous sediment. The sedimentological characteristics of the Series Rouge are described and analysed in order to assess the role of microbial influences on pre-vegetation alluvial systems. Near ubiquitously trough-cross bedded sheet-braided facies, with rarely preserved channel-forms, indicate that alluvial sedimentation was dominated by in-channel dune migration, and depositional-dip exposures reveal the periodic downstream migration of complex bar-forms. Lateral accretion elements and minor discontinuous lenses of more argillaceous material are locally present. Thus, despite the evidence for matgrounds, sedimentary architecture was essentially ‘abiotic’. Using this evidence from the Series Rouge, we argue that the surficial cohesion provided by matgrounds did not exceed thresholds for reworking by hydrodynamic processes thus having little or no effect on their preserved sedimentary architecture.Supported by Shell International Exploration and Production B.V under Research Framework agreement PT38181
The treatment of segmental tibial fractures: does patient preference differ from surgeon choice?
INTRODUCTION: Segmental tibial fractures are complex injuries with a prolonged recovery time. Current definitive treatment options include intramedullary fixation or a circular external fixator. However, there is uncertainty as to which surgical option is preferable and there are no sufficiently rigorous multi-centre trials that have answered this question. The objective of this study was to determine whether patient and surgeon opinion was permissive for a randomised controlled trial (RCT) comparing intramedullary nailing to the application of a circular external fixator. MATERIALS AND METHODS: A convenience questionnaire survey of attending surgeons was conducted during the United Kingdom's Orthopaedic Trauma Society annual meeting 2017 to determine the treatment modalities used for a segmental tibial fracture (n=63). Patient opinion was obtained from clinical patients who had been treated for a segmental tibial fracture as part of a patient and public involvement focus group with questions covering the domains of surgical preference, treatment expectations, outcome, the consent process and follow-up regime (n=5). RESULTS: Based on the surgeon survey, 39% routinely use circular frame fixation following segmental tibial fracture compared to 61% who use nail fixation. Nail fixation was reported as the treatment of choice for a closed injury in a healthy patient in 81% of surgeons, and by 86% for a patient with a closed fracture who was obese. Twenty-one percent reported that they would use a nail for an open segmental tibia fracture in diabetics who smoked, whilst 57% would opt for a nail for a closed injury with compartment syndrome, and only 27% would use a nail for an open segmental injury in a young fit sports person. The patient and public preference exercise identified that sleep, early functional outcomes and psychosocial measures of outcomes are important. CONCLUSION: We concluded that a RCT comparing definitive fixation with an intramedullary nail and a circular external fixator is justified as there remains uncertainty on the optimal surgical management for segmental tibial fractures. Furthermore, psychosocial factors and early post-operative outcomes should be reported as core outcome measures as part of such a trial
Lymphatic Filariasis Control in Tanzania: Effect of Six Rounds of Mass Drug Administration with Ivermectin and Albendazole on Infection and Transmission.
Control of lymphatic filariasis (LF) in most countries of sub-Saharan Africa is based on annual mass drug administration (MDA) with a combination of ivermectin and albendazole, in order to interrupt transmission. We present findings from a detailed study on the effect of six rounds of MDA with this drug combination as implemented by the National Lymphatic Filariasis Elimination Programme (NLFEP) in a highly endemic rural area of north-eastern Tanzania.\ud
The effect of treatment on transmission and human infection was monitored in a community- and a school-based study during an 8-year period (one pre-intervention and 7 post-intervention years) from 2003 to 2011. Before intervention, 24.5% of the community population had microfilariae (mf) in the blood, 53.3% had circulating filarial antigens (CFA) and 78.9% had specific antibodies to the recombinant filarial antigen Bm14. One year after the sixth MDA, these values had decreased considerably to 2.7%, 19.6% and 27.5%, respectively. During the same period, the CFA prevalence among new intakes of Standard 1 pupils in 10 primary schools decreased from 25.2% to 5.6%. In line with this, transmission by the three vectors (Anopheles gambiae, An. funestus and Culex quinquefasciatus) as determined by dissection declined sharply (overall vector infectivity rate by 99.3% and mean monthly transmission potential by 99.2% between pre-intervention and fifth post-intervention period). A major shift in vector species composition, from predominantly anopheline to almost exclusively culicine was observed over the years. This may be largely unrelated to the MDAs but may have important implications for the epidemiology of LF in the area. Six MDAs caused considerable decrease in all the measured indices for transmission and human infection. In spite of this, indices were still relatively high in the late period of the study, and it may take a long time to reach the recommended cut-off levels for interruption of transmission unless extra efforts are made. These should include increased engagement of the target population in the control activities, to ensure higher treatment coverage. It is expected that the recent initiative to distribute insecticide impregnated bed nets to every household in the area will also contribute towards reaching the goal of successful LF elimination
52-week efficacy and safety of telbivudine with conditional tenofovir intensification at week 24 in HBeAg-positive chronic Hepatitis B
Background and Aims: The Roadmap concept is a therapeutic framework in chronic hepatitis B for the intensification of nucleoside analogue monotherapy based on early virologic response. The efficacy and safety of this approach applied to telbivudine treatment has not been investigated.
Methods: A multinational, phase IV, single-arm open-label study (ClinicalTrials.gov ID NCT00651209) was undertaken in HBeAg-positive, nucleoside-naive adult patients with chronic hepatitis B. Patients received telbivudine (600 mg once-daily) for 24 weeks, after which those with undetectable serum HBV DNA (<300 copies/mL) continued to receive telbivudine alone while those with detectable DNA received telbivudine plus tenofovir (300 mg once-daily). Outcomes were assessed at Week 52.
Results: 105 patients commenced telbivudine monotherapy, of whom 100 were included in the efficacy analysis. Fifty-five (55%) had undetectable HBV DNA at Week 24 and continued telbivudine monotherapy; 45 (45%) received tenofovir intensification. At Week 52, the overall proportion of undetectable HBV DNA was 93% (93/100) by last-observation-carried-forward analysis (100% monotherapy group, 84% intensification group) and no virologic breakthroughs had occurred. ALT normalization occurred in 77% (87% monotherapy, 64% intensification), HBeAg clearance in 43% (65% monotherapy, 16% intensification), and HBeAg seroconversion in 39% (62% monotherapy, 11% intensification). Six patients had HBsAg clearance. Myalgia was more common in the monotherapy group (19% versus 7%). No decrease in the mean glomerular filtration rate occurred in either treatment group at Week 52.
Conclusions: Telbivudine therapy with tenofovir intensification at Week 24, where indicated by the Roadmap strategy, appears effective and well tolerated for the treatment of chronic hepatitis B.

Effect of multivitamin and multimineral supplementation on cognitive function in men and women aged 65 years and over : a randomised controlled trial
Background: Observational studies have frequently reported an association between cognitive function and nutrition in later life but randomised trials of B vitamins and antioxidant supplements have mostly found no beneficial effect. We examined the effect of daily supplementation with 11 vitamins and 5 minerals on cognitive function in older adults to assess the possibility that this could help to prevent cognitive decline. Methods: The study was carried out as part of a randomised double blind placebo controlled trial of micronutrient supplementation based in six primary care health centres in North East Scotland. 910 men and women aged 65 years and over living in the community were recruited and randomised: 456 to active treatment and 454 to placebo. The active treatment consisted of a single tablet containing eleven vitamins and five minerals in amounts ranging from 50–210 % of the UK Reference Nutrient Intake or matching placebo tablet taken daily for 12 months. Digit span forward and verbal fluency tests, which assess immediate memory and executive functioning respectively, were conducted at the start and end of the intervention period. Risk of micronutrient deficiency at baseline was assessed by a simple risk questionnaire. Results: For digit span forward there was no evidence of an effect of supplements in all participants or in sub-groups defined by age or risk of deficiency. For verbal fluency there was no evidence of a beneficial effect in the whole study population but there was weak evidence for a beneficial effect of supplementation in the two pre-specified subgroups: in those aged 75 years and over (n 290; mean difference between supplemented and placebo groups 2.8 (95% CI -0.6, 6.2) units) and in those at increased risk of micronutrient deficiency assessed by the risk questionnaire (n 260; mean difference between supplemented and placebo groups 2.5 (95% CI -1.0, 6.1) units). Conclusion: The results provide no evidence for a beneficial effect of daily multivitamin and multimineral supplements on these domains of cognitive function in community-living people over 65 years. However, the possibility of beneficial effects in older people and those at greater risk of nutritional deficiency deserves further attention.Peer reviewedPublisher PD
The role of clathrin in post-golgi trafficking in toxoplasma gondii
Apicomplexan parasites are single eukaryotic cells with a highly polarised secretory system that contains unique secretory organelles (micronemes and rhoptries) that are required for host cell invasion. In contrast, the role of the endosomal system is poorly understood in these parasites. With many typical endocytic factors missing, we speculated that endocytosis depends exclusively on a clathrin-mediated mechanism. Intriguingly, in Toxoplasma gondii we were only able to observe the endogenous clathrin heavy chain 1 (CHC1) at the Golgi, but not at the parasite surface. For the functional characterisation of Toxoplasma gondii CHC1 we generated parasite mutants conditionally expressing the dominant negative clathrin Hub fragment and demonstrate that CHC1 is essential for vesicle formation at the trans-Golgi network. Consequently, the functional ablation of CHC1 results in Golgi aberrations, a block in the biogenesis of the unique secretory microneme and rhoptry organelles, and of the pellicle. However, we found no morphological evidence for clathrin mediating endocytosis in these parasites and speculate that they remodelled their vesicular trafficking system to adapt to an intracellular lifestyle
A deep search for metals near redshift 7: The line of sight towards ULAS J1120+0641
We present a search for metal absorption line systems at the highest
redshifts to date using a deep (30h) VLT/X-Shooter spectrum of the z = 7.084
quasi-stellar object (QSO) ULAS J1120+0641. We detect seven intervening systems
at z > 5.5, with the highest-redshift system being a C IV absorber at z = 6.51.
We find tentative evidence that the mass density of C IV remains flat or
declines with redshift at z < 6, while the number density of C II systems
remains relatively flat over 5 < z < 7. These trends are broadly consistent
with models of chemical enrichment by star formation-driven winds that include
a softening of the ultraviolet background towards higher redshifts. We find a
larger number of weak ( W_rest < 0.3A ) Mg II systems over 5.9 < z < 7.0 than
predicted by a power-law fit to the number density of stronger systems. This is
consistent with trends in the number density of weak Mg II systems at z = 2.5,
and suggests that the mechanisms that create these absorbers are already in
place at z = 7. Finally, we investigate the associated narrow Si IV, C IV, and
N V absorbers located near the QSO redshift, and find that at least one
component shows evidence of partial covering of the continuum source

After having undergone surgical correction at an early age, many patients with tetralogy of Fallot develop long-term complications including progressive pulmonary regurgitation and peripheral pulmonary stenosis. A high percentage of these patients need to undergo a second operation in their adolescence or early adulthood. If simultaneous treatment of both pulmonary regurgitation and peripheral pulmonary stenosis is warranted, a complete surgical approach has several disadvantages. We describe four cases of Fallot patients with severe pulmonary regurgitation and peripheral pulmonary stenosis who were treated using a hybrid approach involving surgical implantation of a pulmonary homograft and peroperative stenting of the pulmonary artery
Interfaces: The Next NDE Challenge
Nondestructive evaluation, as practiced in the 1960’s, attempted to detect (but was often unable to characterize) the existence of defects in engineering structures. Qualitative criteria were used in the assessment of defect significance and the determination of accept/reject decisions. Advances in elasto-plastic fracture mechanics during the 1970’s focused attention upon the defect size and orientation- if these could be measured, then fracture mechanics was capable of quantitative structural integrity evaluation. The papers presented in this conference series during the 1980’s trace the considerable advances of quantitative nondestructive evaluation in satisfying this measurement need. Nowadays, for monolithic materials with well defined fracture toughness, the overconservative rejection criteria of the past are beginning to be replaced by “retirement for cause” concepts
