    Linking Community Service and Interprofessional Education: The Salus University Veterans Readiness Initiative

    The interdisciplinary author team has developed a model for community partnered, multisensory (hearing vision balance) outreach screening events to identify Veterans who have sensory/sensorimotor problems and link them with care while also providing an interprofessional (IPE) learning experience for students and faculty. Returning Veterans enrolled in community college to enhance employability sometimes struggle with non-specific visual, auditory and/or vestibular symptoms that interfere with academic performance. These Veterans may have experienced a traumatic brain injury (TBI) or concussion. Salus University has partnered with the Philadelphia VA Medical Center and two community colleges, to conduct an interdisciplinary outreach screening on two community college campuses in two successive years. Audiology (AuD) and Optometry (OD) student teams supervised by Salus University Audiology and Optometry faculty members administered a customized, 21-item interdisciplinary symptom-rating questionnaire and conducted screening tests of hearing, vision, balance, and tinnitus. To date, 37 students and seven faculty have participated in the screening events. All student and faculty participants attend a one-hour, pre-event training session, and students complete a post-event outcome assessment questionnaire that includes four questions about the inter-professional experience. Content analysis reveals a high frequency of positive words and phrases and self-identified learning outcomes, as well as recommendations for improvement. The students appreciate learning about a sensory system different from their intended area of practice. They also find it beneficial to observe the performance of clinical screening tests utilizing equipment different from their own. The Veterans appreciate the on-campus convenience and opportunity to work with AuD/OD student/faculty teams. We conclude that integrating IPE into a community service activity engages students, fosters meaningful, active learning, promotes confidence as healthcare professionals, and deepens student appreciation and understanding of different healthcare professions.     Truncated and Helix-Constrained Peptides with High Affinity and Specificity for the cFos Coiled-Coil of AP-1

    Protein-based therapeutics feature large interacting surfaces. Protein folding endows structural stability to localised surface epitopes, imparting high affinity and target specificity upon interactions with binding partners. However, short synthetic peptides with sequences corresponding to such protein epitopes are unstructured in water and promiscuously bind to proteins with low affinity and specificity. Here we combine structural stability and target specificity of proteins, with low cost and rapid synthesis of small molecules, towards meeting the significant challenge of binding coiled coil proteins in transcriptional regulation. By iteratively truncating a Jun-based peptide from 37 to 22 residues, strategically incorporating i-->i+4 helix-inducing constraints, and positioning unnatural amino acids, we have produced short, water-stable, alpha-helical peptides that bind cFos. A three-dimensional NMR-derived structure for one peptide (24) confirmed a highly stable alpha-helix which was resistant to proteolytic degradation in serum. These short structured peptides are entropically pre-organized for binding with high affinity and specificity to cFos, a key component of the oncogenic transcriptional regulator Activator Protein-1 (AP-1). They competitively antagonized the cJun–cFos coiled-coil interaction. Truncating a Jun-based peptide from 37 to 22 residues decreased the binding enthalpy for cJun by ~9 kcal/mol, but this was compensated by increased conformational entropy (TDS ≤ 7.5 kcal/mol). This study demonstrates that rational design of short peptides constrained by alpha-helical cyclic pentapeptide modules is able to retain parental high helicity, as well as high affinity and specificity for cFos. These are important steps towards small antagonists of the cJun-cFos interaction that mediates gene transcription in cancer and inflammatory diseases

    The ambivalence of losing weight after bariatric surgery

    This study is grounded in a phenomenological lifeworld perspective. It aims at providing rich descriptions of lived experience of the process of losing weight after obesity surgery. Two women participated in in-depth interviews four times each during the first postoperative year. Based on the women's experiences, a meaning structure—the ambivalence of losing weight after obesity surgery—was identified across the women's processes of change. This consisted of five core themes: movement and activity—freedom but new demands and old restraints; eating habits and digestion—the complexity of change; appearance—smaller, but looser; social relations—stability and change; and being oneself—vulnerability and self-assurance. These core themes changed over time in terms of dominance. The experience of ambivalence is discussed according to a phenomenological perspective of the body as lived experience

    A Passive Monitoring System in Assisted Living Facilities: 12-Month Comparative Study

    The GE QuietCare® passive monitoring system uses advanced motion sensor technology that learns the daily living patterns of senior community residents and sends alerts when certain out-of-the-ordinary events occur. This study compared falls, hospitalizations, care level changes, and resident attrition between two similar assisted living facilities where one facility adopted the QuietCare® monitoring system and the other did not over a 12-month period. Average falls per week were significantly lower in the QuietCare® facility than the control facility. There was also a trend toward fewer weekly hospitalizations in the QuietCare® facility. There was higher resident retention at the QuietCare® facility. This study provides evidence of direct benefits to both the resident and the facility for the use of QuietCare®. There was a significant reduction in the number of falls, as well as a general facility performance improvement measured by care level consistency and higher resident retention rates

    Black Girls Speak STEM: Counterstories of Informal and Formal Learning Experiences

    This study presents the interpretations and perceptions of Black girls who participated in I AM STEM – a community-based informal science, technology, engineering, and mathematics (STEM) program. Using narrative inquiry, participants generated detailed accounts of their informal and formal STEM learning experiences. Critical race methodology informed this research to portray the dynamic and complex experiences of girls of color, whose stories have historically been silenced and misrepresented. The data sources for this qualitative study included individual interviews, student reflection journals, samples of student work, and researcher memos, which were triangulated to produce six robust counterstories. Excerpts of the counterstories are presented in this article. The major findings of this research revealed that I AM STEM ignited an interest in STEM learning through field trips and direct engagement in scientific phenomena that allowed the girls to become agentic in continuing their engagement in STEM activities throughout the year. This call to awaken the voices of Black girls to speak casts light on their experiences and challenges as STEM learners ⎯ from their perspectives. The findings confirm that when credence and counterspaces are given to Black girls, they are poised to reveal their luster toward STEM learning. This study provided a space for Black girls to reflect on their STEM learning experiences, formulate new understandings, and make connections between the informal and formal learning environments within the context of their everyday lives, thus offering a more holistic approach to STEM learning that occurs across settings and over a lifetime

    Characteristics of successfully implemented telemedical applications

    <p>Abstract</p> <p>Background</p> <p>There has been an increased interest in the use of telemedical applications in clinical practice in recent years. Considerable effort has been invested in trials and experimental services. Yet, surprisingly few applications have continued beyond the research and development phase. The aim of this study is to explore characteristics of successfully implemented telemedical applications.</p> <p>Methods</p> <p>An extensive search of telemedicine literature was conducted in order to identify relevant articles. Following a defined selection process, a small number of articles were identified that described characteristics of successfully implemented telemedical applications. These articles were analysed qualitatively, drawing on central procedures from Grounded Theory (GT), including condensation and categorisation. The analysis resulted in a description of features found to be of importance for a successful implementation of telemedicine. Subsequently, these features were discussed in light of Science and Technology studies (STS) and the concept of 'social negotiation'.</p> <p>Results</p> <p>Telemedical applications introduced into routine practice are typically characterised by the following six features: 1) local service delivery problems have been clearly stated, 2) telemedicine has been seen as a benefit, 3) telemedicine has been seen as a solution to political and medical issues, 4) there was collaboration between promoters and users, 5) issues regarding organizational and technological arrangements have been addressed, and 6) the future operation of the service has been considered.</p> <p>Conclusion</p> <p>Our findings support research arguing that technologies are not fixed entities moving from invention through diffusion and into routine use. Rather, it is the interplay between technical and social factors that produces a particular outcome. The success of a technology depends on how this interplay is managed during the process of implementation.</p
